Background: The purpose of this investigation is to evaluate the T(1ρ) relaxation times of articular cartilage surrounding focal defects in the tibiofemoral joint.Methods: Quantitative cartilage assessment was performed using 3T MRI with T(1ρ) mapping in 19 healthy individuals and 44 OA patients. Sagittal T2-weighted fast spin echo (FSE) images were acquired for lesion assessment. Differences were determined using analysis of variance (ANOVA).Results: Cartilage lesions were found in 37% of controls, and 93% of OA patients. Meniscal tears were found in 16% of controls and 57% of OA patients. We observed no difference in T(1ρ) relaxation times when comparing cartilage immediately surrounding a focal defect, and the remaining cartilage within that compartment. The medial femoral condyle (MFC) had the highest incidence of cartilage defects. MFC and medial meniscus posterior horn T(1ρ) were higher in subjects having multiple focal lesions (p = 0.048, pb 0.001 respectively) and extensive full thickness lesions (p = 0.009, pb 0.001 respectively) compared to subjects with no MFC defects. Significant elevations in T(1ρ) of the adjacent compartment (medial tibia) and medial meniscus were observed in subjects with MFC lesions.Conclusion: Increased relaxation times in the involved compartment as well as the adjacent compartment and associated meniscus underscore the interdependence of these structures at bearing load. However, no differences in cartilage composition immediately surrounding a defect were noted. Finally, an association was observed between cartilage defects and meniscal damage in advanced disease.Clinical Relevance: Cartilage defects were not associated with degeneration in the immediately adjacent cartilage. Study Design: Case-control., Objective: To examine whether patients with patellar tendinopathy (PT) display greater patellar mobility and different lower body kinematics than patients without PT., Background: PT is a common overuse condition of the patellar tendon that can cause pain and impair function. Subjects with overuse knee problems display different hip and knee functional mechanics, specifically valgus collapse. Patellar hypermobility has not been specifically studied as a possible risk factor for PT., Methods: 11 patients with PT and 11 controls without PT, age 18 to 40, were studied. Using a patellofemoral arthrometer (PFA), maximal lateral and medial patellar displacement was measured. 3-D motion analysis was performed to determine lower extremity joint motions during single-leg step down and drop vertical jump tests., Results: Patients with PT had significantly increased lateral patellar mobility compared to controls (12.21 ± 3.33 mm vs. 9.19 ± 1.92 mm, P = .017). PT patients showed significantly greater peak hip adduction with both drop vertical jump (2.7° ± 6.3° vs. -5.6° ± 4.2°; P = .003) and step down (17.0° ± 3.8° vs. 12.5° ± 4.4°, P = .024). PT patients demonstrated increased peak ankle external rotation with drop vertical jump (-21.1° ± 5.9° vs. -14.8° ± 5.5°, P = .023) and step down (-15.6° ± 5.5° vs. -9.0° ± 6.0°, P = .017)., Conclusions: Patients with PT exhibit increased lateral patellar mobility, hip adduction, and ankle external rotation. Background: This study aimed to utilize voxel-based relaxometry (VBR) to examine local correlations between patellofemoral joint (PFJ) stress during gait and PFJ cartilage relaxation times., Methods: Eighty-three subjects with and without PFJ osteoarthritis (OA) underwent knee magnetic resonance (MR) images using fast spin-echo, T1ρ and T2 relaxation time sequences. Patellar and trochlear cartilage relaxation times were computed for each voxel. Peak PFJ stress was computed during the stance phase from three-dimensional gait analysis. Statistical Parametric Mapping was used to perform VBR analyses. Pearson partial correlations were used to evaluate the associations between peak PFJ stress and cartilage relaxation times while controlling for age, sex, and body mass index., Results: A higher percentage of the trochlear cartilage (15.9-29.1%) showed significant positive correlations between PFJ stress and T1ρ and T2 than the patellar cartilage (7.4-13.6%). Average correlation coefficient (R) of the voxels showing significant positive correlations ranged from 0.27 to 0.29. Subcompartment analysis revealed a higher percentage of lateral compartment cartilage (trochlea: 30.2-34.7%, patella: 8.1-14.8%) showed significant correlations between peak PFJ stress and T1ρ and T2 than the medial compartment cartilage (trochlea: 7.1-27.2%, patella: 5.5-5.9%). Subgroup analysis showed that larger percentages of PFJ cartilage demonstrated significant positive correlations with PFJ stress in subjects with PFJ OA than those without PFJ OA., Conclusions: The findings of this study suggest that peak PFJ stress has a greater influence on the biochemical composition of the trochlear than the patellar cartilage, and the lateral than the medial PFJ compartment., (Copyright © 2018 Elsevier B.V. Background: Patellar maltracking is a leading cause of patellofemoral pain syndrome (PFPS). The aim of this study was to determine the inter- and intra-rater reliability of a semi-automated program for magnetic resonance imaging (MRI) based patellofemoral kinematics., Methods: Sixteen subjects (10 with PFPS [mean age 32.3; SD 5.2; eight females] and six controls without PFPS 19 [mean age 28.6; SD 2.8; three females]) participated in the study. One set of T2-weighted, fat-saturated fast spin-echo (FSE) MRIs were acquired from each subject in full extension and 30° of knee flexion. MRI including axial T1ρ relaxation time mapping sequences was also performed on each knee. Following image acquisitions, regions of interest for kinematic MRI, and patellar and trochlear cartilage were segmented and quantified with in-house designed spline- based MATLAB semi-automated software., Results: Intraclass Correlations Coefficients (ICC) of calculated kinematic parameters were good to excellent, ICC > 0.8 in patellar flexion, rotation, tilt, and translation (anterior -posterior, medial -lateral, and superior -inferior), and contact area translation. Only patellar tilt in the flexed position and motion from extended to flexed state was significantly different between PFPS and control patients (p=0.002 and p=0.006, respectively). No significant correlations were identified between patellofemoral kinematics and contact area with T1ρ relaxation times., Conclusions: A semi-automated, spline-based kinematic MRI technique for patellofemoral kinematic and contact area quantification is highly reproducible with the potential to help better understand the role of patellofemoral maltracking in PFPS and other knee disorders., Level of Evidence: Level IV., (Published by Elsevier B.V.)

Background: The objective of this pilot study was to evaluate cartilage T1ρ and T2 relaxation times and knee mechanics during walking and drop-landing for individuals with anterior cruciate ligament reconstruction (ACL-R)., Methods: Nine patients (6 men and 3 women, age 35.8 ± 5.4 years, BMI 23.5 ± 2.5 kg/m(2)) participated 1.5 ± 0.8 years after single-bundle two-tunnel ACL reconstruction. Peak knee adduction moment (KAM), flexion moment (KFM), extension moment (KEM), and peak varus were calculated from kinematic and kinetic data obtained during walking and drop-landing tasks. T1ρ and T2 times were calculated for medial femur (MF), and medial tibia (MT) cartilage and compared between subjects with low KAM and high KAM. Biomechanical variables were compared between limbs., Results: The high KAM group had higher T1ρ for MT (p=0.01), central MT (p=0.05), posterior MF (p=0.04), posterior MT (p=0.01); and higher T2 for MT (p=0.02), MF (p=0.05), posterior MF (p=0.002) and posterior MT (p=0.01). During walking, ACL-R knees had greater flexion at initial contact (p=0.04), and lower KEM (p=0.02). During drop-landing, the ACL-R knees had lower KAM (p=0.03) and KFM (p=0.002)., Conclusion: Patients with ACL-R who have higher KAM during walking had elevated MR relaxation times in the medial knee compartments. These data suggest that those individuals who have undergone ACL-R and have higher frontal plane loading, may be at a greater risk of knee osteoarthritis., (Copyright © 2014 Elsevier B.V. Purpose: Altered kinematics following ACL-reconstruction may be a cause of post-traumatic osteoarthritis. T(1ρ) MRI is a technique that detects early cartilage matrix degeneration. Our study aimed to evaluate kinematics following ACL-reconstruction, cartilage health (using T(1ρ) MRI), and assess whether altered kinematics following ACL-reconstruction are associated with early cartilage degeneration., Methods: Eleven patients (average age: 33 ± 9 years) underwent 3T MRI 18 ± 5 months following ACL-reconstruction. Images were obtained at extension and 30° flexion under simulated loading (125 N). Tibial rotation (TR) and anterior tibial translation (ATT) between flexion and extension, and T(1ρ) relaxation times of the knee cartilage were analyzed. Cartilage was divided into five compartments: medial and lateral femoral condyles (MFC/LFC), medial and lateral tibias (MT/LT), and patella. A sub-analysis of the femoral weight-bearing (wb) regions was also performed. Patients were categorized as having "abnormal" or "restored" ATT and TR, and T(1ρ) percentage increase was compared between these two groups of patients., Results: As a group, there were no significant differences between ACL-reconstructed and contralateral knee kinematics, however, there were individual variations. T(1ρ) relaxation times of the MFC and MFC-wb region were elevated (p ≤ 0.05) in the ACL-reconstructed knees compared to the uninjured contralateral knees. There were increases (p ≤ 0.05) in the MFC-wb, MT, patella and overall average cartilage T(1ρ) values of the "abnormal" ATT group compared to "restored" ATT group. The percentage increase in the T(1ρ) relaxation time in the MFC-wb cartilage approached significance (p=0.08) in the "abnormal" versus "restored" TR patients., Conclusions: Abnormal kinematics following ACL-reconstruction appear to lead to cartilage degeneration, particularly in the medial compartment., (Copyright © 2011 Elsevier B.V.
