Objectives: To determine the association between rapid cognitive decline and burden of comorbidities as assessed using the Charlson Comorbidity Index in individuals aged 65 and older with Alzheimer's disease (AD)., Design: Retrospective cohort study., Setting: Memory clinic at the University Hospital of Nantes., Participants: Individuals aged 65 and older with AD (n=170)., Measurements: Subjects were followed for 1 year. Rapid cognitive decline was defined as a decrease of 3 or more points on the Mini-Mental State Examination per 12-month period. Variables studied were the Charlson Comorbidity Index (measure of comorbidity burden), age, sex, AD stage, type of residence (living at home or not), presence of caregiver, functional abilities (Lawton and Katz scales), risk of malnutrition or depression, and intercurrent events (hospitalization or initiating home care)., Results: Rapid cognitive decline at 1-year follow-up occurred in 65 subjects (38.2%). In fully adjusted logistic regression analysis, Charlson Comorbidity Index was significantly associated with rapid cognitive decline (odds ratio (OR)=1.30, P=.03). Moderate stage of AD (OR=2.07, P=.04) and living at home (OR=4.17, P=.04) were also associated with rapid cognitive decline., Conclusion: Comorbidity burden was associated with rapid cognitive decline in subjects with AD., (© 2015, Copyright the Authors Journal compilation © 2015, The American Geriatrics Society.)

Objectives: To determine whether dual task-related changes in walking speed were associated with recurrent falls in frail older adults., Design: Twelve-month prospective cohort study., Setting: Thirteen senior housing facilities., Participants: Two hundred thirteen subjects (mean age 84.4+/-5.5)., Measurements: Usual and dual-tasking walking speeds (m/s) were calculated on a 10-m straight walkway at baseline. Information on incident falls during the follow-up year was collected monthly, and participants were divided into three groups based on the occurrence of falls (0, 1, and >or=2). Recurrent falls were defined as two or more falls during the 12-month follow-up period., Results: Twenty subjects (9.4%) were classified as recurrent fallers. The occurrence of recurrent falls was associated with age (crude odds ratio (OR)=1.11, P=.02), number of drugs (crude OR=1.28, P=.002), and walking speed under both walking conditions (crude OR=0.96, P=.002 for usual walking and crude OR=0.60, P=.005 for walking while counting backward). Multiple Poisson regression showed that only walking speed while dual tasking and number of drugs were associated with incident falls (incident rate ratio (IRR)=0.84, P=.045 and IRR=1.10, P=.004)., Conclusion: Slower walking speed while counting backward was associated with recurrent falls, suggesting that changes in gait performance while dual tasking might be an inexpensive way of identifying frail older adults prone to falling.

Objectives: To determine the protein requirements of elderly hospitalized patients., Design: Cross-sectional evaluation of nitrogen balance., Setting: Short-stay geriatric ward or rehabilitation care unit., Participants: Thirty-six elderly hospitalized patients (aged 65-99) admitted to short-stay and rehabilitation care units., Measurements: Resting energy expenditure and nitrogen balance were determined under usual and spontaneous energy and protein intake after subjects were clinically stable (3-5 days after admission). All items consumed over a 3-day period were weighed to determine energy and protein intake., Results: Energy (23.5+/-6.3 kcal/kg per day) and protein (0.99+/-0.24 g/kg per day) intake were similar in men and women, and nitrogen balance was neutral (0.37+/-2.6 g/day; P=.41 vs a neutral nitrogen balance, i.e., 0 g/d). Half of the patients had a positive nitrogen balance. Plasma C-reactive protein, renal function, nutritional status, and initial diagnosis had no influence on nitrogen balance. In contrast, energy and protein intakes correlated positively with nitrogen balance. Linear regression analysis suggested that an elderly hospitalized patient with an energy intake of 1.31 times resting energy expenditure or greater appears to require a minimum protein intake of 1.06+/-0.28 g/kg per day., Conclusion: Mean protein intake to reach a neutral nitrogen balance in elderly hospitalized patients is 1.06+/-0.28 g/kg per day, which is higher than current recommendations for healthy elderly people. Safe protein intake (that would be adequate to ensure that 95% of patients remain in positive nitrogen balance) is difficult to establish.

Objectives: To estimate energy intake and energy expenditure (EE) in elderly hospitalized patients recovering from an acute illness., Design: Cross-sectional evaluation of the disparity between energy intake and expenditure., Participants: Ninety geriatric patients (mean age+/-standard deviation 79.7+/-7.5) admitted to acute care or rehabilitation units., Measurements: Patients' energy intake and resting EE (REE) were measured over a 3-day period. Blood samples were taken to determine C-reactive protein (CRP), creatinine, and albumin concentrations and to check renal function., Results: Energy intake was higher than REE by a factor of 1.29, but it was lower than the energy requirement. Energy intake, adjusted for differences in body weight, was independent of sex, highest in those who were malnourished (defined as a body mass index (BMI) <21), and lowest in patients who scored poorly on the Mini-Mental State Examination. Energy intake and REE were independent of plasma CRP, creatinine, and albumin concentrations, as well as the initial diagnosis. REE was similar in men and women, at 18.8 kcal/kg per day. REE was 21.4 kcal/kg per day in patients with a BMI of 21 or less and 18.4 kcal/kg per day in those with a BMI greater than 21 kg/m2. The Harris-Benedict equation accurately predicted mean REE., Conclusion: The mean REE of the geriatric patients studied was 18.8 kcal/kg per day, whereas energy intake was just sufficient to cover minimal requirements. Thus, hospitalized elderly patients are likely to benefit from higher calorie intake.

Objectives: To estimate energy requirements in diseased elderly patients with pressure ulcers (PUs)., Design: Open, case-control study., Setting: University Hospital of Angers (France)., Participants: Twenty-nine patients with PUs (Norton index risk=14.3+/-3.3) and 27 controls hospitalized for various diseases (Norton=13.9+/-3.3)., Measurements: Energy requirements were estimated using measured resting metabolic rate (RMR) and multiplied by 1.26 and 1.5 to range between minimal World Health Organization (WHO) requirements and those of adults undergoing light physical activity. Energy intakes were estimated using 3-day food weight records. Measured RMR was compared with the prediction equations of Harris-Benedict, WHO, and Schofield., Results: Measured RMR did not differ between the two groups (P=.48), and was not related to grade or size of the PUs. The WHO equation (82 kcal/d, P=.006) and the Schofield formula (57 kcal/d, P=.05) slightly underestimated calculated RMR, but the Harris-Benedict equation (40 kcal/d, P=.13) accurately estimated it. Energy requirements therefore ranged between 1,536+/-340 kcal/d and 1,828+/-405 kcal/d, (25-30 kcal/kg body weight per day). Energy intake was lower than energy requirements by 176 to 479 kcal/d., Conclusion: Diseased elderly patients with PUs do not have greater energy expenditure, with their requirements suggested to range between 25 and 30 kcal/kg body weight per day. Malnutrition within this population is most likely the result of low energy intake.
