Abstract

Nodules, or bumps, located under the skin on the forehead or scalp are common. Nodules can be cysts or tumors, and they have a wide variety of different names, such as dermoid tumor, epidermal cyst, cephalocele, hemangioma, meningioma, and lipoma. Dermoid tumors are the most common type of skull tumor that occurs during childhood, and epidermal cysts are most common in adulthood. Nodules that appear on the forehead or scalp may be benign and located on top of the skull, or they may extend down into the bone and membranes that surround the brain. There are several different factors that increase the risk of having a nodule that extends into the skull or membranes of the brain. These include the location of the nodule (in the center of the forehead or scalp), the presence of the nodule at birth or its appearance during early childhood, head injury or trauma, prior head surgery, a family history of nerve abnormalities, and the consistency of the nodule (if it is filled with fluid). In a study of 70 children who had nodules on the forehead or scalp, 37 percent had nodules that extended into the bone or brain membranes. Most nodules that appear later in life are on top of the skull and do not penetrate the bone. Nodules that appear later in life, are firm and rubbery, are not located on the midline of the forehead or scalp, and occur in people with no history of head injury or surgery, can generally be surgically removed in a doctor's office. Patients who have nodules that are likely to penetrate the bone need to have X-rays and/or CT scans (computed tomography) for an accurate diagnosis, and surgical removal must be performed in a hospital by a neurosurgeon.
