ABSTRACT Background Objective Methods Results Conclusions Tooth wear is a multifactorial process resulting in the loss of dental hard tissues. For its assessment, the tooth wear evaluation system (TWES) 2.0 has been published. Previous studies on the TWES involved well‐trained practitioners, rather than nonexperts.The first aim of this study was to investigate the reliability of the assessment of tooth wear severity on dental hard tissues, using the TWES 2.0, by nonexperts. The second aim was to investigate the reliability of assessment of wear severity on dental restorations, using a third scale.Forty‐seven dental students evaluated at least 9 of 14 possible patient cases on dental casts and intraoral photographs. Cohen's kappa (agreement with sample solution) and Fleiss' kappa (Inter‐rater reliability) were calculated.The agreements of the operators' ratings compared to the sample‐solution resulted in Cohen's kappa between 0.02 and 0.9. The agreements were 0.34 for occlusal; 0.43 for vestibular; 0.57 for oral surfaces.Inter‐rater reliability (Fleiss' kappa) was 0.35 for occlusal, 0.17 for vestibular and 0.24 for oral assessment. The inter‐rater reliability of the ratings on tooth surfaces with restorations was lower with 0.21 (occlusal), 0.14 (vestibular) and 0.39 (oral). The agreement on different restorations differed tremendously.The ability to correctly assess the cases varied considerably between individual examiners. Within the limits of this study, assessment of restorations was slightly more challenging compared to natural teeth, particularly in occlusal regions or when the restorative material is gold. Background: Prevotella bacteria are associated with inherent diseases of the oral cavity, such as periodontal disease, and systemic diseases. Oral frailty (OF) has been associated with nursing necessity and death. However, the relationship between OF and oral microbiota has not been fully clarified. Objective: This cross‐sectional study investigated the association between OF and Prevotella percentage in the oral microbiota of community‐dwelling older adults. Methods: Oral bacteria species from saliva were identified in 208 community‐dwelling older individuals aged ≥60 years in Japan. The proportion of Prevotella in the oral microbiota was classified into three tertile groups, and its relationship with each test item for OF (number of remaining teeth, masticatory performance, oral diadochokinesis, tongue pressure, difficulties eating tough foods, difficulties swallowing tea or soup, number of applicable OF judgement items, and existence of OF) was examined using ordinal logistic regression analysis. Results: The Prevotella proportions were classified into lower, middle and upper groups, comprising 70, 69 and 69 participants, respectively. The three groups showed a significant relationship between the number of remaining teeth (odds ratio [OR]: 0.946, 95% confidence interval [CI]: 0.915–0.977), masticatory performance (OR: 0.897, 95% CI: 0.844–0.953), number of applicable OF judgement items (OR: 1.477, 95% CI: 1.14–1.915), and existence of OF (OR: 4.194, 95% CI: 1.519–11.576). Conclusion: The proportion of Prevotella in oral microbiota was high in individuals with OF. Objectives: In dental tissue engineering, niche is important for maintaining stem cell function and regenerating the dental tissues. However, there is limited knowledge for the growth factors in niche to maintain the function of stem cells. In this study, we investigated the effect of IGF2, a growth factor in stem cells from apical papilla (SCAPs) niche, on differentiation and proliferation potentials of SCAPs. Materials and methods: Recombinant human IGF2 protein (rhIGF2) was used. Cell counting kit‐8 assay, Carboxyfluorescein succinimidyl ester assay, alkaline phosphatase (ALP) activity, Alizarin Red staining, quantitative calcium analysis, immunofluorescence staining and real‐time RT‐PCR were performed to investigate the cell proliferation and differentiation potentials of SCAPs. And proteomic analysis was used to identify the differential secreted proteins. Results: By ALP activity assay, we found that 5 ng/mL rhIGF2 might be the optimal concentration for treatment. Then, Alizarin Red staining, quantitative calcium analysis and osteogenesis‐related gene expression results showed that 5 ng/mL rhIGF2 could enhance the osteo‐/dentinogenic differentiation potentials in SCAPs. Immunofluorescence staining and real‐time RT‐PCR results showed that neurogenic markers were significantly induced by 5 ng/mL rhIGF2 in SCAPs. Then, CCK‐8 assay and CFSE assay results showed that 5 ng/mL rhIGF2 could enhance the cell proliferation in SCAPs. Furthermore, proteomic analysis showed that IGF2 could induce some secreted proteins which function related to the osteogenesis, neurogenesis and cell proliferation. Food perception is a multimodal sensation which implies cross‐modal interplays. Tactile, thermal, painful and kinaesthetic stimuli arising from food intake may impact on flavour perception, especially taste perception. The influence of oral somatosensory signals on food taste perception remains unclear. The aim of the present systematic review was to appraise the effects of oral mechanical, thermal, chemical and pain sensations on taste perception. In accordance with the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) statement, a search of the literature from 1968 to 2018 was conducted using MEDLINE via PubMed, EMBASE and clinical trials (PROSPERO registration reference: CRD42018100176). A total of 105 articles were included for analysis. The results from this review suggest that taste abilities and taste perception are frequently modified by food textural and chemical properties. Furthermore, saliva features, dental and prosthetic status and oral pain can also modulate taste perception. Biological and physiological phenomena underlying these results may be involved such as tastant release, tastant detection, taste transduction and central trigemino‐gustatory interplays. Summary: Background: Studies on the elderly have reported that the risk of cognitive impairment is affected by chewing difficulty. Objective: To determine whether there is a relationship between the number of pairs of antagonist teeth that come into contact when the mouth is closed (functional masticatory units [FMUs]) and the level of cognitive impairment. Methods: We conducted a cross‐sectional observational study with 502 institutionalised White individuals older than 65 years, living in the northwest of Spain and Portugal. Through a direct visual inspection, we recorded the number of FMUs. Cognitive impairment was assessed by applying the Mini‐Cognitive Examination (MCE), a test derived from the Mini‐Mental State Examination. To describe the statistical relationship between the FMUs and the MCE values, a generalised linear model (GLM) was applied. We assessed the GLM predictive capacity for detecting cognitive impairment (MCE ≤23) in a new study group consisting of 156 elderly individuals. Results: A large number of FMUs was significantly associated with a lower probability of cognitive impairment, regardless of the nature of the contact and its location (explained deviance, 30.1%). The model's discriminatory capacity for cognitive impairment based on the FMUs was "good" (0.820). The model's predictive capacity for cognitive impairment was "acceptable" (sensitivity, 0.786; positive predictive value, 0.900; accuracy, 0.729). Summary: Objective: The aim of this study was to analyse morphological changes in the upper airways in patients with anterior open bite treated with temporary anchorage devices for intrusion of upper posterior teeth. Materials and Methods: Twelve nonobese (body mass index: <25) anterior open bite patients between the ages of 19 and 44 years (mean age: 22.83 ± 8.19 years) were recruited for this study. Cephalometric radiographs and magnetic resonance imaging (MRI) scans before and after anterior bite closure without bracketing on anterior teeth were used to measure the upper airway, which was divided into retropalatal and retroglossal regions. Results: The mandibular plane angle and lower facial height were significantly reduced by intrusion of the upper posteriors and autorotation of the mandible. The retroglossal airway width (AW2) and retroglossal area (RG area) measured on cephalometric radiographs both increased significantly after treatment. Retroglossal volume increased and the retroglossal width/length ratio decreased significantly in MRI analysis. All other measurements were not significantly changed. However, no statistically significant correlations were observed between all measurements in 2D and 3D images, with the exception of the AW2 linear measurement in 2D images correlating with the AP length in MRI axial view images (r = 0.56, P = 0.0430). Conclusion: Counterclockwise rotation of the mandible after anterior open bite closed using orthodontic treatment changed the airway morphology. Summary: Background: The mandibular advancement device (MAD) is widely used for obstructive sleep apnoea (OSA) treatment, and several studies have demonstrated its effectiveness. However, no comprehensive studies have yet examined the biomechanical safety of the MAD. Objectives: The objective of this study was to analyse the biomechanical effect of different protrusion positions of a MAD on the teeth and facial bones. Methods: The posterior restorative forces due to the stretched mandibular muscles were measured by pressure sensors attached to the experimental mandibular advancement device for mandibular protrusions of 10‐70% of the maximum protrusion of the subject. A detailed three‐dimensional biomechanical model of the study subject, constructed from computed tomography scans, was used in finite element analysis, with loading conditions calculated from the measured posterior restorative forces. The outcome measures were the principal stresses on the periodontal ligaments (PDL) and cancellous bone, and the pressure at the PDL surfaces. The measurements were used to analyse the risk of the tooth movement, tooth root resorption, and bone resorption. Results: The lowest and highest restorative forces occurred at 40% and 70% of maximum protrusion, respectively. The highest risk of tooth movement occurred at the mandibular molar teeth. The mandibular second molar teeth had the highest risks of root and bone resorption. Conclusions: Mandibular advancement at 70% of maximum protrusion induces risks of tooth root resorption and bone resorption. The mandibular second molars were subjected to the highest stresses. Summary: Background: Systemic autonomic changes are well known in migraineurs. Also, masticatory disorders are reported to be associated with migraine. However, if those phenomena are interrelated, and how, is unclear. Moreover, the knowledge on the autonomic responses to masticatory stimuli in migraineurs is limited. Objective: To investigate tooth clenching‐related cardiac autonomic regulation in migraineurs. Methods: We compared maximal tooth clenching‐induced systemic autonomic responses, indicated by heart rate variability and blood pressure changes, in headache‐free migraineurs (n = 17) and control subjects (n = 22). Results: Levels of high‐frequency power, reflecting vagal activity, were lower in migraineurs at baseline but increased after tooth clenching whereas in controls they returned to baseline (P < 0.05, mixed model analysis). In multivariate regression model, the presence of migraine predicted the baseline levels of low‐ and high‐frequency power and sympathovagal balance, and the post‐test increase in high‐frequency power, with the attack frequency and side of headache as the modifiers of the measured changes in migraineurs. The painful signs of temporomandibular disorders, found in clinical oral examination, enhanced both maximal changes in RR intervals and post‐test vagal responses to tooth clenching only in migraineurs. Conclusion: The enhanced post‐clenching vagal activation may represent a marker of the augmented trigeminocardiac reflex to stimulation of trigeminal area, sensitised in migraineurs. Our results support an involvement of autonomic mechanisms in migraine pathophysiology and are interesting in terms of interactions between migraine and masticatory disorders, elucidating one potential way how masticatory disorders may aggravate migraine. Summary: Objectives: The purpose of our cross‐sectional study was to investigate the relationship between gerotranscendence and oral health‐related quality of life (OHR‐QOL). To our knowledge, this is the first study to investigate the relationship between gerotranscendence and OHR‐QOL. Methods: Participants (total n = 1969; 70‐year‐old group, n = 999; 80‐year‐old group, n = 970) were drawn from resident lists in four areas of Japan: one urban area and one rural area in each of two geographical regions. OHR‐QOL was measured using the Geriatric Oral Health Assessment Index (GOHAI). Measurement of occlusal force was performed using pressure‐sensitive sheets. Gerotranscendence was assessed with the Japanese Gerotranscendence Scale Revised (JGS‐R). We used multiple linear regression analysis to examine the relationships among OHR‐QOL and other variables. Results: Geriatric Oral Health Assessment Index score was significantly correlated with occlusal force, number of teeth, financial status and JGS‐R score on linear regression analysis. Summary: Clinicians' evaluations of older adults sometimes reveal inconsistencies between objective and subjective dental status. This study investigated which factors contribute both to good objective masticatory function (OMF) and the poor subjective masticatory function (SMF) that often becomes a clinical issue. Study participants included 635 elderly community‐dwelling Japanese adults who underwent a comprehensive geriatric health examination in 2012. SMF was assessed with a question from the Kihon Checklist on eating difficulties (poor or good). OMF was assessed by a colour‐changing gum (poor or good). Also investigated were age, sex, depressive symptoms, instrumental activities of daily living (I‐ADLs), number of people who joined the participant at dinner, grip strength, usual walking speed, number of remaining teeth, number of functional teeth and their occlusal force. The group with good OMF and good SMF, defined as group 1, and the group with good OMF but poor SMF, group 2, were compared. Logistic regression analyses confirmed that the number of remaining and functional teeth participants had was statistically unrelated to differences between OMF and SMF. Instead, differences were related to stronger depressive symptoms (OR = 1.67, CI = 1.14‐2.44), less ability to conduct I‐ADL activities (OR = 0.73, CI = 0.59‐0.91), slower usual walking speeds (OR = 0.18, CI = 0.06‐0.58) and less occlusal force (OR = 0.99, CI = 0.99‐1.00). Depressive symptoms, I‐ADLs, and physical function are shown to be significantly related to divergence between objective and subjective masticatory function in elderly Japanese. This suggests that dissociations between objective and subjective dental evaluations of elderly adults indicate need for assessment of their mental and physical function. [ABSTRACT FROM AUTHOR]

Summary: Self‐reported measures of oral health are often used to assess oral health in populations or groups, but their validity or reliability needs repeated confirmation. The objective of this cross‐sectional study was to evaluate the validity of self‐reported tooth counts and masticatory status, using data obtained from a sample of Japanese adults. A total of 2356 adults aged 40 to 75 years participated in a questionnaire survey and a clinical oral examination from 2013 through 2016. Self‐reported measures were compared with clinically measured values. For tooth counts, mean clinical and self‐reported tooth counts in all participants were 23.68 and 23.78 teeth, and no significant difference was detected. Spearman's, Pearson's and intra‐class correlation coefficients between clinical and self‐reported tooth counts were 0.771, 0.845 and 0.843, respectively. According to the Bland‐Altman analysis, the mean difference between clinical and self‐reported tooth counts was −0.098 (95% CI: −0.242, 0.047). The upper limit of agreement was 6.919 (95% CI: 6.669, 7.169), and the lower limit of agreement was −7.115 (95% CI: −7.365, −6.865). No significant fixed or proportional bias was observed. For masticatory status, the crude or age‐ and gender‐adjusted mean numbers of total teeth, posterior teeth and 3 kinds of functional tooth units significantly decreased with the deterioration of masticatory status. This study indicated that self‐reports were within an acceptable range of clinical measures. Summary: The purpose of the systematic review and meta‐analysis was to compare the performance of 3‐unit bridges on teeth with 3‐unit bridges on implants, evaluating survival of the bridges, survival of the teeth or implants, condition of the hard and soft tissues surrounding the supports, complications and patient‐reported outcome measures (PROM) after a mean observation period of at least 1 year. A literature search was conducted using a combination of the search terms: fixed partial denture and fixed dental prostheses (FDPs). An electronic search for data published until January 2017 was undertaken using the MEDLINE, EMBASE and Cochrane Library databases. Eligibility criteria included clinical human studies, either randomised or not, interventional or observational, which evaluated the results of 3‐unit FDPs on either 2 implants or 2 abutment teeth. The search identified 1686 unique references. After applying eligibility criteria, 66 articles were included in the analysis. A total of 1973 3‐unit FDPs were supported by teeth, and 765 were implant‐supported. No significant differences were found either in the survival of the supporting abutments (P = .52; 99% vs 98.7% survival per year) or in the survival of the prostheses (P = .34; 96.4% vs 97.4% survival per year). Both treatments show an almost equally low complication rate, but there is a low level of reporting of hard and soft tissue conditions and PROM. It is concluded that implant‐supported 3‐unit FDPs seem to be a reliable treatment with survival rates not significantly different from the results of teeth‐supported 3‐unit FDPs. In a previous study, we used muscle functional magnetic resonance imaging to show that the anterior movement of the occlusal point increased the activity of the superior head of the ipsilateral lateral pterygoid muscle (ipsilateral SHLP) during unilateral single-tooth clenching. The purpose of this study was to verify the hypothesis that the increased activity of the ipsilateral SHLP described above serves to antagonise the occlusal force acting on the condyle. In total, 9 healthy volunteers were requested to perform left unilateral clenching at the first molar or first premolar region for 1 minute at 20% or 40% maximum voluntary clenching force. Changes in the mean proton transverse relaxation time (∆T2) were examined from the magnetic resonance images obtained before and after each clenching act as an index of the activity in all masticatory muscles. Correlation analyses of the mean ΔT2 for each volume of interest were performed with the first molar or premolar clenches to analyse the correlation between the activities in each muscle. A statistically significant correlation was exhibited between the ipsilateral temporal and ipsilateral SHLP ( r = .651, P = .003) during first premolar clenching. However, no significant correlations were observed in the ipsilateral SHLP during first molar clenching. The results of this study suggest that the ipsilateral SHLP may contribute to the pulling of the mandibular condyle forward against the occlusal force generated by the ipsilateral temporal muscle. The aim of this study was to assess the 5-year treatment outcome of maxillary implant-retained overdentures opposed by natural antagonistic teeth. Fifty consecutive patients received maxillary overdentures supported by six dental implants. Implants were placed in the anterior region, if enough bone was present ( n = 25 patients) Implant were placed in the posterior region if implant placement in the anterior region was not possible ( n = 25 patients). Variables assessed included survival of implants, condition of hard and soft peri-implant tissues and patients' satisfaction. The five-year implant survival rate was 97·0% and 99·3%, and mean radiographic bone loss was 0·23 and 0·69 mm in the anterior and posterior group, respectively. Median scores for plaque, calculus, gingiva, bleeding and mean scores for pocket probing depth were low and stayed low. Patients' satisfaction after treatment was high in both groups. Within the limits of this 5-year study, it is concluded that six dental implants (placed in the anterior or posterior region) connected with a bar and opposed to natural antagonistic teeth result in acceptable results for clinical parameters and good outcomes for marginal bone level changes and patient satisfaction. There is limited information of the normal function of the human medial pterygoid muscle ( MPt). The aims were to determine whether (i) the MPt is active throughout horizontal jaw movements with the teeth apart and (ii) whether single motor units ( SMUs) are active during horizontal and opening-closing jaw movements. Intramuscular electrodes were placed in the right MPt of 18 participants who performed five teeth-apart tasks: (i) postural position, (ii) ipsilateral (i.e. right) jaw movement, (iii) contralateral movement, (iv) protrusive movement and (v) opening-closing movement. Movement tasks were guided by a target and were divided into BEFORE, OUT, HOLDING, RETURN and AFTER phases according to the movement trajectories recorded by a jaw tracking system. Increased EMG activity was consistently found in the OUT, HOLDING and RETURN phases of the contralateral and protrusive movement tasks. An increased RETURN phase activity in the ipsilateral task indicates an important role for the MPt in the contralateral force vector. Of the 14 SMUs active in the opening-closing task, 64% were also active in at least one horizontal task. There were tonically active SMUs at the postural jaw position in 44% of participants. These new data point to an important role for the MPt in the fine control of low forces as required for stabilisation of vertical mandibular position not only to maintain postural position, but also throughout horizontal jaw movements with the teeth apart. Studies have explored occlusal marking interpretation, repeatability and accuracy. But, when an occlusion detection product is interposed between teeth, direct tooth-tooth occlusal contact relationships are replaced by tooth-material-tooth structures. Thus, the marks cannot reflect the original contacts. This has been shown for single tooth pair contacts. The purpose of this laboratory study was to similarly examine full dentitions. A dentiform was set into Class I centric occlusion with the mandible supported by a load cell. The maxillary arch was guided by precision slides. As the weighted (~52 N) upper assembly was lowered onto and raised off the mandibular arch, the loads on the mandible were measured. With and without (control) occlusal marking material, the steps were as follows: (cleaning - control 1 - material 1) ... (cleaning - control 6 - material 6). The six materials were as follows: Accufilm I and II, Rudischhauser Thick and Thin, Hanel Articulating Silk and T-Scan. Then, the six sets of (cleaning - control - material) measurements were repeated with the mandibular assembly shifted, in turn, by 0·1 mm in the Anterior, Posterior, Right and Left directions. The five (Centric and four 0·1 mm shifted) occlusal relationships produced grossly different tooth-tooth (control) load profiles. And, in general, these controls were affected, in different ways, by the marking products. Among the five conventional products, the Rudischhausers fared the worst and the electronic T-Scan was an extreme outlier. Thus, in general, popular occlusal detection products alter the occlusal contact forces, and therefore, their markings cannot characterise the actual occlusion. [ABSTRACT FROM AUTHOR]

The tooth stress elicited by occluding contact represents critical biomechanical information about dental health during chewing. Effects of occlusal contact on tooth stress remain obscure. In this study, a mandibular first molar finite element model was built from CT images. The effects of area size, location and direction of occlusal loading on both tooth and periodontal stresses were analysed. Results showed tooth and periodontal stress had drastically different patterns. Tooth stress value was much higher than periodontal stress value under the same task. Tooth stress concentration area and its value decreased from outside to inside. The Maximum Tooth Stress ( MTS) always occurred at the loading site and a larger loading area elicited a smaller MTS value. The variation of MTS was larger when the fossa bottoms were inclined loaded than when the cusp tips were inclined loaded, larger when lingually loaded than when buccally loaded and larger when mesially loaded than when distally loaded. Distal loadings generally induced smaller Maximum Periodontal Stress ( MPS) variations than the mesial loadings. These findings indicated exposure of the rational site(s) to occlusal contact should be helpful to achieve proper tooth and periodontal stress, thus to diminish loading associated structure problems. Temporomandibular disorder ( TMD) incidences are believed to be related to parafunctional behaviours like teeth clenching. This pilot study aimed to (i) develop an automated clench-detection algorithm, and (ii) apply the algorithm to test for differences in nocturnal clenching in women with and without TMD. Subjects gave informed consent to participate. Adult women were categorised using Diagnostic Criteria for TMD according to presence/absence (+/−) of both TM joint disc placement ( DD) and chronic pain (P) into two groups (+ DD+P, − DD−P) with 12 subjects each. Surface temporalis electromyography was recorded during oral tasks performed by subjects at two laboratory sessions. The data were used to characterise muscle activity per N of bite force ( μV/N) for each subject, develop the clench-detection algorithm and test its accuracy. Ambulatory surface temporalis electromyography was self-recorded by each subject over three nights and analysed using the algorithm and bite force (N) versus muscle activity μV/N calibrations. Bonferroni-adjusted homoscedastic t-tests assessed for significant between-group differences in clenching ( P < 0·05). Sensitivity, specificity and accuracy of algorithm-detected laboratory clenches were all ≥96%. During self-recordings 95% of clenches had durations of <4 s and peak forces of <10 N in both groups. Mean clench durations were significantly longer ( P = 0·042) in + DD+P (1·9 ± 0·8 s) than − DD−P subjects (1·4 ± 0·4 s). Mean temporalis duty factors (%clench time/total recording time) were significantly larger ( P = 0·041) in + DD+P (0·47 ± 0·34%) than − DD−P (0·26 ±0·22%) subjects. Nocturnal temporalis muscle activities detected by a validated algorithm were longer per clench and recording time in + DD+P compared to − DD−P women. [ABSTRACT FROM AUTHOR]

Social media offers an accessible resource for gaining valuable insights into the social culture of bullying. The purpose of this study was to qualitatively analyse Twitter posts for common themes relating to dentofacial features, braces and bullying. Twitter's database was searched from 2010 to 2014 using keywords relevant to bullying, teeth and orthodontics. Two investigators assessed the Twitter posts, and selected those that conveyed the experiences or opinions of bullying victims. The posts were qualitatively analysed using thematic analysis. Of the 548 posts screened, 321 were included in the final sample. Four primary categories relating to 'dental-related bullying' were identified: (i) morphological features, (ii) psychological and psychosocial impact, (iii) coping mechanisms and (iv) the role of family. Bullied individuals reported a diverse range of psychological impacts and coping mechanisms. Secondary categories were also identified. Family members, for example, were found to play both a contributory and mediatory role in bullying. In summary, social media can provide new and valuable information about the causal factors and social issues associated with oral health-related bullying. Throughout lifetime, the teeth are continuously exposed to numerous chemical and physical impacts, which cause the wear of the dental hard tissues, gingival recession and other oral changes with sometimes subsequent problems. Age-related wear of tooth surfaces reduces the dental enamel thickness and exposes deeper layers of enamel, which have different physical and chemical properties than the surface enamel. Gingival recession is the main causal factor of root caries and dentine hypersensitivity. Age-related changes in dentine include the formation of secondary dentine and the reduction in tubular lumen diameter (dentine sclerosis), which lead to a reduction in the volume of the pulp chamber. In addition to the reduction in the volume of pulp chamber, changes to the dental pulp also include dental pulp calcifications. The age-related physiological changes to the teeth should be carefully distinguished from pathological changes, especially when they induce pain or a negative impact on the oral health-related quality of life ( OHRQoL) of the older individuals. The aim of this investigation was to study the clinical prosthodontic decision-making process relating to dentitions with compromised molars among Swedish general dental practitioners ( GDPs). Eleven Swedish GDPs were purposively selected, and all agreed to participate. Then, in-depth, semi-structured interviews were conducted and covered treatment considerations concerning two authentic patient cases, initially with complete dental arches, and later, a final treatment based on a shortened dental arch ( SDA) was discussed. The cases involved patients with compromised teeth situated mainly in the molar regions. One patient suffered from extensive caries and the other from severe periodontal disease. Qualitative content analysis was used to analyse the data. In the systematic analysis, two main categories were identified: holistic and functional approach. Among the interviewed GDPs, focus was put on patients' needs, background history and motivation for treatment as well as the preservation of molar support. Within the limitations of this study, the following can be concluded: keeping a dental arch with molars seems to be important to Swedish general dental practitioners. The SDA concept does not seem to have a substantial impact on the prosthodontic decision-making relating to dentitions with compromised molars. The dentist's experiences, as well as colleagues' or consulting specialist advice together with aetiological factors and the patient's individual situation, influence the decision-making more than the SDA concept. The conflicting results in the prosthetic decision-making process concerning the relevance of age and the need for molar support need further investigation, for example based on decisions made in the dentist′s own clinical practice. Previous studies have identified various factors related to masticatory performance. This study was aimed to investigate variations and impacts of factors related to masticatory performance among different occlusal support areas in general urban population in Japan. A total of 1875 Japanese subjects (mean age: 66·7 years) were included in the Suita study. Periodontal status was evaluated using the Community Periodontal Index (CPI). The number of functional teeth and occlusal support areas (OSA) were recorded, and the latter divided into three categories of perfect, decreased and lost OSA based on the Eichner Index. Masticatory performance was determined by means of test gummy jelly. For denture wearers, masticatory performance was measured with the dentures in place. The multiple linear regression analysis showed that, when controlling for other variables, masticatory performance was significantly associated with sex, number of functional teeth, maximum bite force and periodontal status in perfect OSA. Masticatory performance was significantly associated with number of functional teeth, maximum bite force and periodontal status in decreased OSA. In lost OSA, masticatory performance was significantly associated with maximum bite force. Maximum bite force was a factor significantly influencing masticatory performance that was common to all OSA groups. After controlling for possible confounding factors, the number of functional teeth and periodontal status were common factors in the perfect and decreased OSA groups, and only sex was significant in the perfect OSA group. Dental appearance may play a key role on the way we develop a first impression of another person. To test whether relatively minor changes in the lightness of tooth colour would influence the perceived social appeal (social, intellectual, psychological and relational abilities) of an unknown male and unknown female, this cross-sectional study was performed on 555 Spanish adults. The two major independent variables related to the photograph were tooth lightness (computer-derived), divided into three levels that included lightened teeth, natural teeth and darkened teeth, and the gender of the observed face. Moreover, six independent variables related to the observer were assessed (age, gender, educational level, place of residence, frequency of brushing and self-reported health status). The dependent variables were scored on five-point Likert scales designed to quantify four domains (social, intellectual, psychological and relationship competences) of the Social Appeal Scale ( SAS). Tooth lightness influences the perception of social appeal in all dimensions, as darkened smiles received significantly poorer scores than natural-colour smiles, but these were also worse than lightened smiles. A logistic regression analysis revealed that the major predictor of social appeal was tooth lightness, and for each increment in lightness (from darkened to lightened smiles), the odds ratio ( OR) of positive values being perceived increased significantly in all items (from 2·3 in Popularity to 6·9 in Happiness). A perceptible change in dental lightness is the strongest factor associated with the dental attractiveness stereotype, affecting significantly the 12 traits assessed, but mainly the Happiness, Social Relations and Academic Performance. Deciding whether to replace or preserve a compromised tooth, even with emerging trends in implant dentistry, is still a common dilemma for practitioners. This study sought to determine the 3- to more than 10-year survival rate of teeth that had undergone endodontic, periodontal and prosthodontic treatments. A total of 245 teeth in 87 patients were clinically and radiographically evaluated. All the teeth had received crown lengthening surgery by a single periodontist. Root canal therapy and prosthodontic procedures were rendered either by specialists or by experienced general dentists. Numbers of lost teeth were recorded and the criteria for hopeless teeth were defined. Survival rate was determined using the Kaplan-Meier estimator. Clinical indices including pocket depth ( PD), bleeding index ( BI), C/R ratio, position of the restoration margin relative to the gingival margin ( RM- GM) and the presence of intra-canal post were compared between different survival groups (<3, 3-5, 5-10 and >10 years) using one-way analysis of variance ( anova). Potential predictors of failure were determined using the Cox regression model. The mean ± s.d. of 3-, 5-, 10- and 13-year survival rates was 98 ± 1%, 96 ± 1·6%, 83·1 ± 4·5% and 51·9 ± 14·5%, respectively. The mean PD ( P < 0·013), as well as C/R ratio in the mesial ( P = 0·003) and distal ( P = 0·007) surfaces, was significantly higher in the >10-year-survived teeth. Bleeding index and RM- GM showed no significant differences between the groups. C/R ratio and RM- GM position appeared to be the major determinants of tooth loss. The aim of this study was to investigate risk factors with age in the long-term prognosis of autotransplantation of teeth with complete root formation at dental clinics. Participating dentists were asked to provide information on transplantations they had undertaken from 1 January 1990 to 31 December 2010. Data on a total of 708 teeth from 637 patients were collected. The data were screened to exclude patients who were under 25 or 70 years of age and over, those who were smokers or whose smoking habits were unknown, those whose transplanted teeth had incomplete root formation or multiple roots and those with fewer than 25 present teeth post-operation. The participants in this study were 71 men (74 teeth) and 100 women (107 teeth) ranging from 25 to 69 years of age. Third molars were used as donor teeth in 89·0% of the cases. The participants were divided into three age groups of 25-39, 40-54 and 55-69. Survival analysis was conducted using the Kaplan- Meier method, and a log-rank test revealed that there were no significant differences in age groups for men or women. Cox regression analysis indicated that the survival of transplanted teeth was not influenced by age. However, although not statistically significant, the clinical success rate was lower in the 55-69-year-old group than that in the younger groups. These results indicate that if suitable donor teeth are available and the conditions are right, autotransplantation is a viable treatment for missing teeth regardless of the age of the patient. Biomechanical features of occlusal contacts are important in understanding the role of the occlusion contributing to masticatory function. Cusp-fossa contact is the typical pattern of occlusion between upper and lower teeth. This includes static relations, such as that during clenching, and dynamic relations when mandibular teeth contact in function along the maxillary occlusal pathways, as during mastication. During clenching in the maximum intercuspal position (ICP), cuspal inclines may take the role of distributing the occlusal forces in multi-directions thus preventing excessive point pressures on the individual tooth involved. During chewing movement on the functional side, the mandible moves slightly from buccal through the maximum ICP to the contralateral side. The part of the chewing cycle where occlusal contacts occur and the pathways taken by the mandible with teeth in occlusal contacts are determined by the morphology of the teeth. The degree of contact is associated with the activity of the jaw muscles. To obtain repeatable static and dynamic occlusal contact information provided by the morphology of the teeth, maximum voluntary clenching and chewing movements with maximum range are needed. In conclusion, in addition to the standard occlusal concepts of centric relation/centric occlusion and group function/cuspid protection relation, biomechanics in static and dynamic cusp-fossa relationships should be included to develop an understanding of occlusal harmony which includes no interfering or deflective contacts in functional occlusal contact. This study aimed to analyse occlusal force, area and pressure for individual maxillary and mandibular teeth by assessing their occlusal-supporting abilities. Ninety-nine subjects (49 men and 50 women; average age, 24·7 ± 3·4 years; range, 20-37 years) performed maximal voluntary clenching twice for 3 s in both the intercuspal position and the intercuspal position with sliding movement from edge-to-edge occlusion, with a pressure-sensitive sheet placed between the maxillary and mandibular dental arch. Occlusal force, area and pressure of individual teeth were calculated by colour development in the pressure-sensitive sheet with special analytical equipment and software. Occlusal contact condition of individual teeth was confirmed using the intra-occlusal record. All data were analysed using unpaired Student's t-test, Kruskal- Wallis test and Scheffe's test for multiple comparisons with a significance level of P < 0·05. The occlusal pressure in the intercuspal position with sliding movement from edge-to-edge occlusion was adopted as the representative occlusal-supporting ability for each individual tooth, although there were, in part, statistically significant differences in the effects of laterality, performance and gender. Overall, the occlusal pressure increased gradually from the central incisor, peaked at the canine or first premolar and decreased sharply-and significantly ( P < 0·01)-towards the second molar. We conclude that the occlusal pressure of individual teeth can be used as an indicator of occlusal-supporting ability. This is therefore proposed as a possible suitable parameter for epidemiologic research, specifically for verifying the relationship between occlusal-supporting ability and status of residual periodontal ligament support. Oral rehabilitation restores form and function and impacts on general health. Teeth provide a discriminating sense of touch and directional specificity for occlusal perception, management of food with mastication and swallowing, and awareness of its texture and hardness. Peripheral feedback for control of jaw muscles includes the enamel-dentine-pulp complex and mechanoreceptors in the periodontal tissues. The implications of feedback from periodontal and other intra-oral mechanoreceptors as well as changes in central representation are significant for function and adaptation to oral rehabilitation. With implants, in the absence of the periodontium and periodontal mechanoreceptor feedback, fine motor control of mastication is reduced, but patients are still able to function adequately. Further, there is no significant difference in function with full-arch fixed prostheses on teeth in comparison with implants. Predictable implant outcomes depend on bone support. Optimum restoration design appears to be significant for bone remodelling and bone strains around implants with occlusal loading. Finite element analysis data confirmed load concentrations at the coronal bone around the upper section of the implant where bone loss is commonly observed clinically. Load concentration increased with steeper cusp inclination and broader occlusal table and decreased with central fossa loading and narrower occlusal table size. It is recommended that occlusal design should follow a narrow occlusal table, with central fossa loading in intercuspal contact and low cusp inclination to minimise lateral loading in function and parafunction. Summary During chewing, food is reduced in size, while saliva moistens the food and binds the masticated food into a bolus that can be easily swallowed. Characteristics of the oral system, like number of teeth, bite force and salivary flow, will influence the masticatory process. Masticatory function of healthy persons has been studied extensively the last decades. These results were used as a comparison for outcomes of various patient groups. In this review, findings from literature on masticatory function for both healthy persons and patient groups are presented. Masticatory function of patients with compromised dentition appeared to be significantly reduced when compared with the function of healthy controls. The influence of oral rehabilitation, e.g. dental restorations, implant treatment and temporomandibular disorder treatment, on masticatory function will be discussed. For instance, implant treatment was shown to have a significant positive effect on both bite force and masticatory performance. Also, patient satisfaction with an implant-retained prosthesis was high in comparison with the situation before implant treatment. The article also reviews the neuromuscular control of chewing. The jaw muscle activity needed to break solid food is largely reflexly induced. Immediate muscle response is necessary to maintain a constant chewing rhythm under varying food resistance conditions. Finally, the influence of food characteristics on the masticatory process is discussed. Dry and hard products require more chewing cycles before swallowing than moist and soft foods. More time is needed to break the food and to add enough saliva to form a cohesive bolus suitable for swallowing. [ABSTRACT FROM AUTHOR]

This study was undertaken in a young Irish population to determine the dimensions and ratios of the six maxillary anterior teeth. One hundred and nine Irish subjects (age 18-25 inclusive) had irreversible hydrocolloid impressions made of their maxillary dentition poured in type V stone. Clinical crown dimensions were measured with a digital calliper. The stone casts were digitally photographed in a standardised manner enabling calculation of various ratios between the maxillary anterior teeth. Sexual dimorphism existed for various tooth dimensions; most notably canine teeth were in the region of 0·8 mm longer and 0·6 mm wider in males. Central and lateral incisors were found to be 0·5 mm wider in males. It is, therefore, recommended that dimensional tooth guidelines should be given for each of the sexes and not on a population basis. With regard to tooth proportion ratios, no significant differences were found between genders or the left and right sides for any of the measurements or ratios measured. The digitally recorded tooth proportions were similar for both sexes, and the Golden Proportion guidelines could only be applied to the lateral incisor/central incisor widths (0·618). The aetiology of non-carious cervical lesions (NCCLs) is not well understood and still controversial. The aim of this ex-vivo study was to examine the morphological characteristics of NCCLs for clinical evidence of enamel loss above the cemento-enamel junction (CEJ) as suggested by the abfraction theory. Fifty extracted human teeth with various types of NCCLs were collected and scanned by micro computed tomography. The reconstructed three-dimensional models were evaluated from multiple aspects, including longitudinal cross-section series. The location of internal line angle and proximal exits of the lesions were evaluated in relation to the level of CEJ. The coronal margins of the lesions were inspected for evidence of enamel loss above the CEJ using the bucco-lingual longitudinal sectional images. Coronal margins of the lesions were located along and/or under the CEJ for all of the 50 samples. In most of the lesions, regardless of lesion type, the proximal exits and internal line angles were located below the CEJ. This study did not detect clinical evidence of enamel loss above the occlusal margin of NCCLs as would have been expected according to the general abfraction mechanism. [ABSTRACT FROM AUTHOR]

Computational models of interactions in the craniomandibular apparatus are used with increasing frequency to study biomechanics in normal and abnormal masticatory systems. Methods and assumptions in these models can be difficult to assess by those unfamiliar with current practices in this field; health professionals are often faced with evaluating the appropriateness, validity and significance of models which are perhaps more familiar to the engineering community. This selective review offers a foundation for assessing the strength and implications of a craniomandibular modelling study. It explores different models used in general science and engineering and focuses on current best practices in biomechanics. The problem of validation is considered at some length, because this is not always fully realisable in living subjects. Rigid-body, finite element and combined approaches are discussed, with examples of their application to basic and clinically relevant problems. Some advanced software platforms currently available for modelling craniomandibular systems are mentioned. Recent studies of the face, masticatory muscles, tongue, craniomandibular skeleton, temporomandibular joint, dentition and dental implants are reviewed, and the significance of non-linear and non-isotropic material properties is emphasised. The unique challenges in clinical application are discussed, and the review concludes by posing some questions which one might reasonably expect to find answered in plausible modelling studies of the masticatory apparatus. Patient demand for aesthetic dentistry is steadily growing. Laminates and free metal restorations have evolved in an attempt to overcome the invasiveness nature of full veneer restorations. Although many different materials have been used for making these restorations, there is no single material that fits best for all purposes. Two groups of ceramic material (Feldspathic and IPS Empress II) and one group of laboratory composite (Gradia) discs (10 discs in each group; 4 mm in diameter and 2 mm in thickness) were prepared according to the manufacturer’s instruction. The surface of ceramic discs were etched and silanized. In Gradia group, liquid primer was applied on composite surfaces. Thirty freshly extracted sound human molars and premolars were randomly divided into three groups. The enamel surface of each tooth was slightly flattened (0·3 mm) on the buccal or lingual side and then primed and cemented to the prepared discs with the aid of a dental surveyor. The finishing specimens were thermocycled between 5 °C and 55 °C for 2500 cycles and then prepared for shear bond strength testing. The resulting data were analyzed by one-wayanova and Tukey HSD test. The fractured surfaces of each specimen were inspected by means of stereomicroscope and SEM. There is significant difference between the bond strength of materials tested. The mean bond strengths obtained with Feldspathic ceramic, IPS Empress II and Gradia were 33·10 ± 4·31 MPa, 26·04 ± 7·61 MPa and 14·42 ± 5·82 MPa, respectively. The labial groove-textures of upper central incisors were defined and explored to imitate the elaborate groove-textures for aesthetic restorations. In the current study, 158 upper central incisors were selected from 79 volunteers aged from 19 to 24 and recruited from Sichuan University in Chengdu of China. The length, width, depth, combination and distribution of the horizontal and vertical grooves on the labial surface were investigated by an optical measurement method, based on the Shadow Moiré technology and Temporal Fourier analysis. Vertical grooves were confirmed to be present in 94% of the samples and horizontal grooves in 77%. Perfect symmetry was shown in the vertical grooves of the same tooth, as well as in the homonymous teeth. The majority of horizontal grooves were distributed in the proximity of the cervical fourth and the middle of the crown. Based on the combination and distribution of the grooves, eight basic labial groove-texture types of maxillary central incisors were classified. The subtle morphological characters of natural teeth could be recorded by the new method, and it could also provide the minute groove-texture for dentists and technicians to achieve an aesthetic appearance for a restoration. This study examined the association between the subjective food intake ability (FIA) using a questionnaire with 30 types of foods and the objective Mixing Ability Index (MAI) using a two-coloured wax cube. A total of 147 subjects (32 males, 115 females, mean age: 54·9 ± 20·7) were enrolled in this study. The FIA for 30 food types and the key food intake ability (KFIA) for five key food types were obtained using a self-assessed questionnaire to evaluate the subjective chewing ability. The objective chewing ability was evaluated by asking the subjects to chew a wax cube for 10 strokes. The MAI was calculated by image analysis of the chewed wax cube. The Pearson’s correlation between the subjective FIA and objective MAI was analysed according to age and number of post-canine teeth lost. The FIA for the 30 foods and five key food types showed a moderate correlation with the MAI ( r = 0·51, 0·55, respectively, P < 0·01). The KFIA of the denture wearing group ( r = 0·44, P < 0·01) and over 60-year-old group ( r = 0·49, P < 0·01) showed a significantly higher correlation with the MAI than the other groups. Although the number of sound or decayed teeth has been reported to be associated with cognitive function in elderly populations with dementia, little is known about this association in elderly populations without dementia. We evaluated this relationship, with adjustment for confounding factors, in Japanese populations of 60-year-old ( n = 270; 120 males and 150 females) and 65-year-old ( n = 123; 57 males and 66 females) individuals residing in Fukuoka Prefecture of Japan. Dental examinations were performed in all subjects, along with the Mini-mental state examination (MMSE) for assessing cognitive function. Among the total of 393 subjects, the mean MMSE score was 27·9 ± 1·9, and 391 subjects scored 24 or higher. The mean numbers of sound and decayed teeth were 12·0 ± 6·3 and 0·5 ± 1·2, respectively. Associations were found between the numbers of sound and decayed teeth and MMSE in total subjects and males, but not in females, by multiple regression analysis adjusted for gender, age, level of education, marital status, smoking, alcohol drinking, working status, systolic blood pressure and blood glucose. An association was also found between MMSE and the number of sound teeth in a logistic regression analysis. In conclusion, associations were found between normal-range cognitive function and the numbers of sound and decayed teeth, after adjustment for various confounding factors, in an elderly Japanese population. This in vivo study assessed the remaining coronal tooth structure in teeth prepared for complete and partial coverage restorations using 3D-scanning and a Tooth Restorability Index (TRI). The cuspal coverage preparation designs selected by 10 postgraduate dentists and 10 general dental practitioners were recorded in a questionnaire. Eighteen patients had molar root treatment completed at the Eastman Dental Hospital and were prescribed a coronal-radicular amalgam core and cast restoration. Each tooth was prepared in vivo by one operator for a cast restoration. Two clinical impressions were made to produce two dies: one of remaining coronal tooth structure before crown preparation and a second die of coronal tooth structure in vivo after crown preparation. For teeth prepared for partial coverage in vivo ( n = 13), a third die was prepared in vitro representing remaining tooth structure after complete coverage preparation. The three dies were of tooth structure prior to core placement. All dies ( n = 31) were scanned using a laser profilometer and the volume of remaining tooth structure calculated. Four observers scored 31 dies using the TRI. The percentage loss of coronal tooth volume following a complete instead of a partial coverage preparation varied from 3·29% to 45·23% and the mean TRI fell from 10·7 to 7·5 units. There was a strong correlation between mean TRI and scanned volume of tooth structure ( P = 0·013). Over 50% of the dentists altered their initial choice of restoration design from complete to partial coverage. Complete coverage preparations removed more tooth structure than partial coverage. More than 85% of the global population requires repair or replacement of a craniofacial structure. These defects range from simple tooth decay to radical oncologic craniofacial resection. Regeneration of oral and craniofacial tissues presents a formidable challenge that requires synthesis of basic science, clinical science and engineering technology. Identification of appropriate scaffolds, cell sources and spatial and temporal signals (the tissue engineering triad) is necessary to optimize development of a single tissue, hybrid organ or interface. Furthermore, combining the understanding of the interactions between molecules of the extracellular matrix and attached cells with an understanding of the gene expression needed to induce differentiation and tissue growth will provide the design basis for translating basic science into rationally developed components of this tissue engineering triad. Dental tissue engineers are interested in regeneration of teeth, oral mucosa, salivary glands, bone and periodontium. Many of these oral structures are hybrid tissues. For example, engineering the periodontium requires growth of alveolar bone, cementum and the periodontal ligament. Recapitulation of biological development of hybrid tissues and interfaces presents a challenge that exceeds that of engineering just a single tissue. Advances made in dental interface engineering will allow these tissues to serve as model systems for engineering other tissues or organs of the body. This review will begin by covering basic tissue engineering principles and strategic design of functional biomaterials. A structured literature review aimed to elucidate test parameters for in vitro testing of post-endodontic restorations. The literature was digitally searched using MEDLINE, EMBASE, MedPilot and an additional hand search was performed. Two independent researchers assessed the articles in relation to the defined inclusion and exclusion criteria. The literature search revealed 125 abstracts. Sixty-nine studies were included. Fifty-seven per cent of the studies investigated maxillary incisors only. The restorative stage as complex of tooth, post, core, and crown and post-and-core restored specimens without crowns were used most frequently. Fifty-nine per cent of the studies used static loading. Only 15% of the studies performed thermocycling and mechanical loading (TCML). However, the number of thermo- and load cycles varied. The cross-head speed of linear loading after TCML ranged from 0·01 to 150 mm min−1. The reviewed studies were heterogeneous in test design regarding the used test parameters. This study evaluated the effects of the short-term use of a dentifrice containing nano-sized carbonate apatite (n-CAP) on the occlusion of the dentinal tubules using a scanning electron microscope (SEM) and an image analyser in vitro. One hundred human dentine specimens were wet ground with a silicone carbide papers and etched with 6% citric acid for 1 min to allow complete opening of the dentinal tubule. Specimens showing complete opening tubules were used as the baseline. The specimens were divided randomly into five groups: G1: 0% n-CAP, G2: 5% n-CAP, G3: 10% n-CAP, G4: 20% n-CAP and G5: 10%strontium chloride (SrCl2). Five specimens from each group were brushed by applying 50, 100, 250 and 500 strokes, respectively. All the specimens were evaluated by a SEM (×3000), and the degree of occlusion of the dentinal tubules was quantified using an image analyser. The results were analysed by one-way anova and a Tukey’s test using the spss 12·0 statistical package program. The dentifrice containing 20% n-CAP for 50 strokes, which indicated 2-day use, showed the highest tubular occlusion than the other groups ( P < 0·05). Moreover, this group showed 79·5% and 77·4% less open tubular area than the baseline and 0% n-CAP group, respectively. The groups containing various concentrations of n-CAP showed significant differences in the SrCl2 group after tooth-brushing for 500 strokes, which indicated 17-day use. According to this examination of the short-term use of desensitizing dentifrices in vitro, the dentifrice containing 20% n-CAP was the most effective in occluding the dentinal tubules. Designs of removable partial dentures are suggested to affect the mobility of abutment teeth and removable partial denture (RPD) during oral functions. This study aimed to examine the effect of direct retainer and major connector designs on RPD dynamics under simulated loading. Six different Kennedy class II maxillary RPDs were fabricated on a maxillary model. These dentures involved 3 different direct retainers (wrought-wire clasp, RPA clasp, and conical crown telescopic retainer) and 2 different major connectors (Co-Cr major connector and heat-cured acrylic resin with a metal strengthener). Using an experimental model with simulated periodontal ligaments and mucosa that were fabricated using silicone impression material, three-dimensional displacements of the RPDs were measured under a simulated 30-N loading with a displacement transducer type M-3. Significant effects of “direct retainer design” on bucco-palatal displacements and “major connector” on mesio-distal displacements were revealed by 2 × 3 two-way analysis of variance of abutment teeth movements ( P < 0·001 and P = 0·002, respectively). Additionally, analysis of variance of RPD displacements revealed significant effects of “direct retainer design” on corono-apical displacements and “major connector” on mesio-distal displacements ( P = 0·001 and P = 0·028, respectively). The functional and anatomical characteristics of Down’s syndrome have direct repercussions on oral health; orofacial dysfunction results and feeding and swallowing are impaired. These problems have been described in an earlier article. Different techniques are proposed for the prevention of the development of orofacial dysfunction in Down’s syndrome. In particular, early myofunctional therapy coupled with appliance wear has been shown to be successful over the long term when multidisciplinary management is possible. Functional or conventional orthodontic treatment may be successful for older children when performed concurrently with the use of appropriate behaviour management techniques. More recently, techniques for the compensation of masticatory dysfunction in adults have been proposed, although further research is necessary to confirm their efficacy. The aim of this second article was to review techniques for the prevention, treatment and compensation of orofacial dysfunction in persons with Down’s syndrome from birth to adulthood. Human upper and lower incisors are both tilted forward in the sagittal plane. Previous theoretical and in vitro studies have investigated how proclination may help the teeth either to penetrate or to fracture food more effectively or both. We study the proclination of lower incisors in relation to efficiency and to the protection it may offer from potentially damaging torque forces. Lateral cephalographs from 57 normal human subjects were traced. In one study, a line was drawn joining the centre of the condyle to the tip of the lower incisor. The results showed the lower incisor is oriented so that it is closely parallel to the arc of a circle centred at the condyle. In another study, lines were drawn joining the tips of upper and lower incisors at different openings. Each line showed the direction of the force that must be used to bite an object held between the tips of the incisor teeth. Its direction was compared with the direction of the long axis of the lower incisor when the mandible was graphically rotated open. The results showed the long axis of the lower incisor is closest to the direction of the bite force at 12 and 15 of jaw openings (roughly 20–25 mm incisal separation). This is the opening where the maximum incisal force is normally produced. The findings suggest that to reduce the torque, lower incisors implanted or relocated during orthodontic treatment should be oriented parallel to the closing arc. [ABSTRACT FROM AUTHOR]

Restorations made on incorrectly mounted casts might require considerable intra-oral adjustments to correct the occlusion or might even necessitate a remake of the restoration. The aim of this study was to evaluate interocclusal recording materials for their ability to reproduce accurate vertical interocclusal relationships after a storage time of 1 and 48 h, respectively. A custom-made apparatus was used to simulate the maxilla and mandible. Eight interocclusal records were made in each of the following groups: G1: Aluwax (aluminium wax), G2: Beauty Pink wax (hydrocarbon wax compound), G3: Futar D, G4: Futar D Fast, G5: Futar Scan (G3-G5: vinyl polysiloxane), G6: Ramitec (polyether). The vertical discrepancies were measured by an inductive displacement transducer connected to a carrier frequency amplifier after storage of the records for two periods of 1 and 48 h. Two-wayanova was used for statistical analysis. The mean vertical discrepancies in μm (1/48 h) for G1 (31/35) and G2 (35/38) were statistically significantly higher than for the other groups G3 (8/10), G4 (11/12), G5 (6/8) and G6 (5/8) ( P ≤ 0·05). There were no statistically significant differences between the elastomers tested. The effect of storage on the vertical discrepancies was statistically significant ( P < 0·001). The jerk-cost is an index that can quantify the smoothness of various movements including human body movements. A previous study reported the usefulness of jerk-cost in the evaluation of masticatory movement, and proposed that the masticatory movement of subjects with good occlusion could be explained as a maximum smooth movement. The purpose of this study was to investigate the influence of a single prosthetic molar restoration on the smoothness of masticatory movement. Fourteen adults who visited this hospital seeking a single prosthetic restoration on a molar were selected. Each subject chewed a piece of chewing gum on the molars of the treated side before and after crown placement. Movement trajectory was recorded using the Sirognathograph Analyzing System. Normalized jerk-cost (NJC) was calculated on the closing phase of each chewing cycle and was compared before and after the crown placement. After the prosthetic restoration, NJC significantly decreased ( P < 0·05) in seven subjects with a crown placed on a lower molar, whereas significant changes were not observed in seven subjects with a crown placed on an upper molar. The study aimed to compare the dentine wear of primary and permanent human and bovine teeth because of erosion/abrasion and evaluate if bovine dentine is an appropriate substitute for human dentine in further erosion/abrasions tests. Dentine samples from deciduous molars and human third molars as well as from calves’ and cattle’s lower incisors were prepared and baseline surface profiles were recorded. Each day all samples were demineralized in 1% citric acid, tooth brushed with 100 brushing strokes with toothpaste slurry and stored in artificial saliva for the rest of the day. This cycle was run for 20 days. Afterwards, new surface profiles were recorded and dentine wear was calculated by a customized computer program. Dentine wear because of erosion/abrasion was not statistically, significantly different for human third molars and cattle’s lower incisors ( P = 0·7002). The dentine wear because of erosion/abrasion of deciduous molars and calves’ lower incisors was significantly different ( P < 0·0000). No statistically significant difference in the dentine wear of human third molars and cattle’s lower incisors was observed, so that the use of cattle’s lower incisors as substitute for adult human teeth for further investigations in erosion/abrasion studies could be accepted. The aim of this study was to examine the bond strength between acrylic teeth after various surface treatments and processing with either a heat- or microwave-polymerized denture base material. Specimens were prepared and tested according to the methods described in American National Standard/American Dental Association Specification No.15 (Synthetic Polymer Teeth) using a special assembly. Three brands of acrylic teeth were used with the following treatments: control (no treatment), grinding and grinding plus sandblasting. Treatment groups were further divided into two denture base processing subgroups: heat-polymerized and microwave-polymerized methods. Denture base processing to the acrylic teeth was performed according to the manufacturer’s recommendations. Bond strength test was performed using a universal testing machine with five specimens and each specimen is composed of six anterior teeth per experimental group. Data were analysed using a three-wayanova and the Scheffe′ F-test post hoc pair wise comparison at the 95% significance level. The mean bond strength values ranged from 33.1 ± 4.1 to 51.6 ± 2.5 MPa. The bond strength values of sandblasted surfaces were significantly higher than those of the ground surface and the control ( P < 0.05). Combined (acrylic tooth and denture base resin) cohesive failures were disclosed in all tested samples. Acrylic tooth surface pre-processed surface treatment with grinding plus sandblasting and processed with a heat-polymerized denture base provided the greatest bond strength between acrylic tooth and denture base. Tooth sensitivity is a common dental pain condition where sufferers experience brief episodes of sharp well-localized pain when their teeth are subjected innocuous stimuli such as cold, air-currents and probing with a metallic instrument. In this review, we will make no attempt to describe all the treatments that have been developed to treat tooth sensitivity. We will review the basic anatomic and physiological mechanisms responsible for sensitivity. The insights into the dental lesions responsible for tooth sensitivity, as well as the physiological processes linking stimuli and pain generation have suggested several treatments and preventive strategies. Unfortunately, many tooth sensitivity treatments fail to perform better than placebos in clinical trials that seek to assess the effect of agents on pain symptoms. In the case of the most commonly used self-applied desensitizing agent, potassium salts, the mechanism of action established by laboratory and animal models may not apply to clinical use. Thus results obtained with laboratory and animal models must be applied with care to clinical use. Clinical literature suggests that tooth sensitivity is the symptomatic manifestation of significant dental problems, such as wear and other forms of non-carious tooth structure loss. These conditions are increasing in frequency as people age, retaining their natural teeth longer. They are frequently the consequences of aggressive oral hygiene practices and diets rich in acids. Treatments directed at the underlying causes rather than the symptoms of tooth sensitivity would hinder the development of these lesions and provide researchers with objective targets for assessing therapeutic efficacy. In August 23–25, 2007, the Scandinavian Society for Prosthetic Dentistry in collaboration with the Danish Society of Oral Implantology arranged a consensus conference on the topic ‘Implants and/or teeth’. It was preceded by a workshop in which eight focused questions were raised and answered in eight review articles using a systematic approach. Twenty-eight academicians and clinicians discussed the eight review papers with the purpose to reach consensus on questions relevant for the topic. At the conference the consensus statements were presented as well as lectures based on the review articles. The objective of this systematic review was to describe the incidence of tooth and implant loss reported in long-term studies. Prospective longitudinal studies reporting on teeth or implants survival with a follow-up period of at least 10 years were considered. Papers were excluded if the drop out rate exceeded 30% or if <70% of the initial subject sample was examined at 10 years of follow-up. Seventy publications on teeth were identified as potentially relevant for the focussed question. The analysis of the abstracts yielded 37 studies eligible for full-text analysis. The inclusion criteria were met in 11 of the publications that included in all 3015 subjects. The initial search on implant studies generated 52 publications that possibly could be included. Following the evaluation of the abstracts and full-text analysis nine publications were found to fulfil the inclusion criteria. The nine studies included 476 subjects. The incidence of tooth loss among subjects with a follow-up period of 10–30 years varied from 1.3% to 5% in the majority of studies, while in two epidemiological studies on rural Chinese populations the incidences of tooth loss were 14% and 20%. The percentage of implants reported as lost during the follow-up period varied between 1% and 18%. In clinically well-maintained patients, the loss rate at teeth was lower than that at implant. Bone level changes appeared to be small at teeth as well as at implants in well-maintained patients. Comparisons of the longevity at teeth and dental implants are difficult due to heterogeneity among the studies. [ABSTRACT FROM AUTHOR]
