Search

The environmental impact of magnetic resonance imaging (MRI) has recently come into focus. This includes its enormous demand for electricity compared to other imaging modalities and contamination of water bodies with anthropogenic gadolinium related to contrast administration. Given the pressing threat of climate change, addressing these challenges to improve the environmental sustainability of MRI is imperative. The purpose of this review is to discuss the challenges, opportunities, and the need for action to reduce the environmental impact of MRI and prepare for the effects of climate change. The approaches outlined are categorized as strategies to reduce greenhouse gas (GHG) emissions from MRI during production and use phases, approaches to reduce the environmental impact of MRI including the preservation of finite resources, and development of adaption plans to prepare for the impact of climate change. Co-benefits of these strategies are emphasized including lower GHG emission and reduced cost along with improved heath and patient satisfaction. Although MRI is energy-intensive, there are many steps that can be taken now to improve the environmental sustainability of MRI and prepare for the effects of climate change. On-going research, technical development, and collaboration with industry partners are needed to achieve further reductions in MRI-related GHG emissions and to decrease the reliance on finite resources. Cardiac MR imaging is well established for assessment of cardiovascular structure and function, myocardial scar, quantitative flow, parametric mapping, and myocardial perfusion. Despite the clear evidence supporting the use of cardiac MRI for a wide range of indications, it is underutilized clinically. Recent developments in low-field MRI technology, including modern data acquisition and image reconstruction methods, are enabling high-quality low-field imaging that may improve the cost-benefit ratio for cardiac MRI. Studies to-date confirm that low-field MRI offers high measurement concordance and consistent interpretation with clinical imaging for several routine sequences. Moreover, low-field MRI may enable specific new clinical opportunities for cardiac imaging such as imaging near metal implants, MRI-guided interventions, combined cardiopulmonary assessment, and imaging of patients with severe obesity. In this review, we discuss the recent progress in low-field cardiac MRI with a focus on technical developments and early clinical validation studies. Background: The current standard method to measure intracardiac oxygen (O 2 ) saturation is by invasive catheterization. Accurate noninvasive blood O 2 saturation by MRI could potentially reduce the duration and risk of invasive diagnostic procedures., Purpose: To noninvasively determine blood oxygen saturation in the heart with MRI and compare the accuracy with catheter measurements., Study Type: Prospective., Subjects: Thirty-two patients referred for right heart catheterization (RHC) and five healthy subjects., Field Strength/sequence: T2-prepared single-shot balanced steady-state free-precession at 1.5T., Assessment: MR signals in venous and arterial blood, hematocrit, and arterial O 2 saturation from a pulse oximeter were jointly processed to fit the Luz-Meiboom model and estimate blood O 2 saturation in the right heart. Interstudy reproducibility was evaluated in volunteers and patients. Interobserver reproducibility among three readers was assessed using data from volunteers and 10 patients. Accuracy of MR oximetry was compared to RHC in all patients., Statistical Tests: Coefficient of variation, intraclass correlation coefficient, Bland-Altman analysis, Pearson's correlation., Results: The coefficient of variation for interstudy reproducibility of O 2 saturation was 2.6% on average in volunteers and 3.2% in patients. Interobserver reproducibility among three observers yielded intraclass correlation coefficients of 0.81 and 0.87 respectively for RV and MPA O 2 saturation. O 2 saturation (y = 0.85x + 0.13, R = 0.78) and (a-v)O 2 difference (y = 0.71x + 0.90, R = 0.69) by MR and RHC were significantly correlated (N = 32, P < 0.05 in both cases) in patients. MR slightly overestimated O 2 saturation compared to RHC with 2% ± 5% bias and limits of agreement between -7% and 12%., Data Conclusion: MR oximetry is repeatable and reproducible. Purpose: To evaluate the reproducibility and repeatability of high-resolution, isotropic thoracic and abdominal aortic wall measurements, and determine the implications they have on the number of subjects necessary for future clinical trials., Materials and Methods: Using a T1-weighted three-dimensional MRI SPACE sequence, we evaluated the interobserver, intraobserver, and scan-rescan variability of isotropic thoracic and abdominal aortic wall measurements in 15 cardiovascular diseased patients and 6 normal volunteers. Main outcome analyses were intracorrelation coefficient (ICC), mean relative error (mRE), and sample size calculation at 80% power to be used to compare placebo group and treatment group means in future two-arm randomized clinical trials., Results: Excellent reliability, ICC > 0.8 (P < 0.001) and small mRE < 10% were demonstrated for the interobserver, intraobserver, and scan-rescan variability for all investigated measures: lumen area (LA), outer wall area (OWA), wall area (VWA), total wall volume (TWV), and percentage wall volume (%WV). Sample size calculation revealed slightly different sample size per treatment arm for thoracic and abdominal aorta segments (maximum number of subjects: 352 subjects for thoracic segment versus 421 subjects for abdominal segment for LA at 5% difference, and minimum of 3 thoracic versus 4 abdominal subjects needed for %WV evaluation at 25% difference)., Conclusion: Our study demonstrates the reproducibility and repeatability of SPACE aortic plaque measurements, and gives insight into the number of subjects needed for the design of therapeutic studies in aortic atherosclerosis., (© 2013 Wiley Periodicals, Inc.)

Purpose: To determine the correlation in abdominal aortic stiffness obtained using magnetic resonance elastography (MRE) (μ(MRE)) and MRI-based pulse wave velocity (PWV) shear stiffness (μ(PWV)) estimates in normal volunteers of varying age, and also to determine the correlation between μ(MRE) and μ(PWV)., Materials and Methods: In vivo aortic MRE and MRI were performed on 21 healthy volunteers with ages ranging from 18 to 65 years to obtain wave and velocity data along the long axis of the abdominal aorta. The MRE wave images were analyzed to obtain mean stiffness and the phase contrast images were analyzed to obtain PWV measurements and indirectly estimate stiffness values from the Moens-Korteweg equation., Results: Both μ(MRE) and μ(PWV) measurements increased with age, demonstrating linear correlations with R(2) values of 0.81 and 0.67, respectively. Significant difference (P ≤ 0.001) in mean μ(MRE) and μ(PWV) between young and old healthy volunteers was also observed. Furthermore, a poor linear correlation of R(2) value of 0.43 was determined between μ(MRE) and μ(PWV) in the initial pool of volunteers., Conclusion: The results of this study indicate linear correlations between μ(MRE) and μ(PWV) with normal aging of the abdominal aorta. Purpose: To compare exercise stress cardiac magnetic resonance (cardiac MR) to echocardiography in healthy volunteers with respect to adequacy of endocardial visualization and confidence of stress study interpretation., Materials and Methods: Twenty-eight healthy volunteers (age 28 ± 11 years, 15 males) underwent exercise stress echo and cardiac MR one week apart assigned randomly to one test first. Stress cardiac MR was performed using an MRI-compatible treadmill; stress echo was performed as per routine protocol. Cardiac MR and echo images were independently reviewed and scored for adequacy of endocardial visualization and confidence in interpretation of the stress study., Results: Heart rate at the time of imaging was similar between the studies. Average time from cessation of exercise to start of imaging (21 vs. 31 s, P < 0.001) and time to acquire stress images (20 vs. 51 s, P < 0.001) was shorter for cardiac MR. The number of myocardial segments adequately visualized was significantly higher by cardiac MR at rest (99.8% vs. 96.4%, P = 0.002) and stress (99.8% vs. 94.1%, P = 0.001). The proportion of subjects in whom there was high confidence in the interpretation was higher for cardiac MR than echo (96% vs. 60%, P = 0.005)., Conclusion: Exercise stress cardiac MR to assess peak exercise wall motion is feasible and can be performed at least as rapidly as stress echo., (Copyright © 2013 Wiley Periodicals, Inc.)

Purpose: To evaluate the use of a T2-weighted SPACE sequence (T2w-SPACE) to assess carotid stenosis via several methods and compare its performance with contrast-enhanced magnetic resonance angiography (ceMRA)., Materials and Methods: Fifteen patients with carotid atherosclerosis underwent dark blood (DB)-MRI using a 3D turbo spin echo with variable flip angles sequence (T2w-SPACE) and ceMRA. Images were coregistered and evaluated by two observers. Comparisons were made for luminal diameter, luminal area, degree of luminal stenosis (NASCET: North American Symptomatic Endarterectomy Trial; ECST: European Carotid Surgery Trial, and area stenosis), and vessel wall area. Degree of NASCET stenosis was clinically classified as mild (<50%), moderate (50%-69%), or severe (>69%)., Results: Excellent agreement was seen between ceMRA and T2w-SPACE and between observers for assessment of lumen diameter, lumen area, vessel wall area, and degree of NASCET stenosis (r > 0.80, P < 0.001). ECST stenosis was consistently higher than NASCET stenosis (48 ± 14% vs. 24 ± 22%, P < 0.001). Area stenosis (72 ± 2%) was significantly higher (P < 0.001) than both ESCT and NASCET stenosis., Conclusion: DB-MRI of carotid arteries using T2w-SPACE is clinically feasible. Purpose: To evaluate the feasibility of the dark blood fast spin echo (FSE) T1-weighted-Sampling Perfection with Application of optimized Contrasts using different flip angle Evolution (T1w-SPACE) sequence in assessing whole body arterial wall information from the extracranial carotids to the popliteal artery., Materials and Methods: Twenty-eight subjects were subjected to noncontrast, dark blood whole body magnetic resonance angiography (DB-WBMRA) using a T1w-SPACE sequence optimized for each of the individual stations: carotid artery, thoracic aorta, abdominal aorta, and thigh/superficial femoral artery (SFA). Image quality/vessel wall visualization and the time required to image the four stations were evaluated. Two observers checked the reproducibility of vessel wall depiction by performing quantitative measurements in registered initial and repeat studies (six subjects) of vessel wall and lumen area at 17 locations along the arterial tree., Results: In 25 of the 28 scanned subjects, dark blood arterial images acquired in approximately 1 hour total imaging time allowed whole body arterial vessel wall visualization. Quantitative measurements showed high correlation between the initial and repeat studies for each of the observers as well as high interobserver reproducibility (r > 0.95; P < 0.01)., Conclusion: DB-WBMRA using T1w-SPACE is feasible and can be performed with a high degree of reliability.

Purpose: To evaluate the feasibility of a multi-station three dimensional (3D) T1-weighted turbo spin echo (TSE) dark-blood Sampling Perfection with Application optimized Contrasts using different flip angle Evolution sequence (T1w-SPACE), to assess aorta, iliac, and superficial femoral (SFA) arteries (inflow vessels) by comparing it with a multi-station contrast enhanced MR angiography (CE-MRA) with identical resolution., Materials and Methods: A total of 6 volunteers and 14 peripheral arterial disease (PAD) patients were included in the study. Abdominal and thigh T1w-SPACE and lower leg time-resolved MRA (TR-MRA) with low dose contrast were followed by 3-station CE-MRA. Quantitative measurements of lumen area at 17 locations from T1w-SPACE and CE-MRA were obtained. Additionally, vessel wall areas at the same locations were obtained from the T1w-SPACE images., Results: Quantitative comparison of lumen areas with T1w-SPACE and CE-MRA revealed strong correlation between the two techniques and strong inter-observer agreement for each of the two imaging methods (r > 0.9; P < 0.001). Localized vessel wall area measurements obtained in PAD patients were significantly greater compared with those obtained in normal volunteers (mean difference 43.75 +/- 12.46 mm(2); P < 0.001). Stenosis severity obtained from T1w-SPACE localized measurements showed significant arterial area stenosis in PAD patients., Conclusion: T1w-SPACE imaging of inflow vessels is feasible, and in addition to CE-MRA has the ability to assess atherosclerotic plaque and vascular remodeling., ((c) 2009 Wiley-Liss, Inc.)

Magnetic susceptibility artifact has been documented with various implants and devices, which require appropriate identification with screening of patients prior to subjecting them to magnetic resonance examination. We performed cardiovascular magnetic resonance (CMR) examination of the aorta in a 24-year-old woman in the setting of repaired aortic coarctation, and found magnetic susceptibility artifact arising from the stomach in the absence of known susceptibility artifact-producing material in this region. Further history revealed that she had ingested a prenatal vitamin prior to imaging, prompting experimental analyses that led us to conclude that iron-containing vitamins may be a source of magnetic susceptibility artifact., (Copyright (c) 2006 Wiley-Liss, Inc.)

Purpose: To compare two multislice turbo spin-echo (TSE) carotid artery wall imaging techniques at 1.5 T and 3.0 T, and to investigate the feasibility of higher spatial resolution carotid artery wall imaging at 3.0 T., Materials and Methods: Multislice proton density-weighted (PDW), T2-weighted (T2W), and T1-weighted (T1W) inflow/outflow saturation band (IOSB) and rapid extended coverage double inversion-recovery (REX-DIR) TSE carotid artery wall imaging was performed on six healthy volunteers at 1.5 T and 3.0 T using time-, coverage-, and spatial resolution-matched (0.47 x 0.47 x 3 mm3) imaging protocols. To investigate whether improved signal-to-noise ratio (SNR) at 3.0 T could allow for improved spatial resolution, higher spatial resolution imaging (0.31 x 0.31 x 3 mm3) was performed at 3.0 T. Carotid artery wall SNR, carotid lumen SNR, and wall-lumen contrast-to-noise ratio (CNR) were measured., Results: Signal gain at 3.0 T relative to 1.5 T was observed for carotid artery wall SNR (223%) and wall-lumen CNR (255%) in all acquisitions (P < 0.025). IOSB and REX-DIR images were found to have different levels of SNR and CNR (P < 0.05) with IOSB values observed to be larger. Normalized to a common imaging time, the higher spatial resolution imaging at 3.0 T and the lower spatial resolution imaging at 1.5 T provided similar levels of wall-lumen CNR (P = NS)., Conclusion: Multislice carotid wall imaging at 3.0 T with IOSB and REX-DIR benefits from improved SNR and CNR relative to 1.5 T, and allows for higher spatial resolution carotid artery wall imaging., (Copyright 2006 Wiley-Liss, Inc.)

Purpose: To develop and test a delayed-enhancement imaging method for improving the contrast between myocardial infarction (MI) and blood pool., Materials and Methods: The T(2) of blood is significantly longer than that of acute or chronic MI. The proposed multi-contrast delayed-enhancement (MCODE) imaging method produces a series of images with both T(1) and T(2) weightings, which provides both excellent contrast between normal and infarcted myocardium, and between blood and MI., Results: The subendocardial border between MI and blood pool was easily discriminated in the T(2)-weighted image. The measured MI-to-blood contrast-to-noise ratio (CNR) was better in the T(2)-weighted image than in the T(1)-weighted image (22.5+/-8.7 vs. 2.9+/-3.1, mean+/-SD, N=11, P<0.001, for True FISP, and 19.4+/-10.8 vs. 3.9+/-2.3, N=11, P<0.001, for Turbo FLASH)., Conclusion: The MCODE method provides a significant improvement in the ability to easily discriminate subendocardial MI by providing a T(2)-weighted image with high contrast between blood and MI. Purpose: To evaluate the consistency and inversion time (TI) independence of phase-sensitive reconstruction (PSIR) delayed-enhancement (DE) MRI in a clinical setting., Materials and Methods: Mid-ventricular short-axis DE images were acquired in 25 patients using three TIs: 1) optimized to null viable myocardium, 2) 50 msec less than optimal TI, and 3) 50 msec greater than optimal TI. At each TI, images were acquired with PSIR and without magIR. In each image, percent scar was computed as the ratio of nonviable to total pixels in the left ventricle (LV)., Results: In the magIR images, percent scar was 23% +/- 15% (optimal), 11% +/- 11% (short), and 22% +/- 15% (long). In PSIR images, percent scar was 25% +/- 15% (optimal), 22% +/- 15% (short), and 22% +/- 14% (long). Percent scar was significantly underestimated in magIR images with short TI, but no statistically significant difference in percent scar was observed at the optimal or long TIs., Conclusion: DE-MRI is a robust imaging technique for clinical use. PSIR provided consistent image quality independently of TI, at least over the range of TIs evaluated in this study. However, neither image quality nor scar appearance in the PSIR images was significantly different from that in the magIR images when TI was at or above the null point of viable myocardium.

Purpose: To compare signal-to-noise ratio (SNR), contrast-to-noise (CNR) ratio, and diagnostic accuracy of a newly developed saturation recovery (SR)-TrueFISP-two-dimensional (2D) sequence with an SR-TurboFLASH-2D sequence., Materials and Methods: In seven healthy subjects and nine patients with coronary artery disease (CAD), contrast-enhanced perfusion imaging (with Gd-DTPA) was performed with SR-TrueFISP and SR-TurboFLASH sequences. Hypoperfused areas were assessed qualitatively (scale = 0-4). Furthermore, SNR and CNR were calculated and semiquantitative perfusion parameters were determined from signal intensity (SI) time curves. Standard of reference for patient studies was single-photon emission computer tomography (SPECT) and angiography., Results: The perception of perfusion deficits was superior in TrueFISP images (2.6 +/- 1.0) than in TurboFLASH (1.4 +/- 0.6) (P < 0.001). Phantom measurements yielded increased SNR (143 +/- 34%) and CNR (158 +/- 64%) values for TrueFISP. In patient/volunteer studies SNR was 61% to 100% higher and signal enhancement was 110% to 115% higher with TrueFISP than with TurboFLASH. Qualitative and semiquantitative assessment of perfusion defects yielded higher sensitivities for detection of perfusion defects with TrueFISP (68% to 78%) than with TurboFLASH (44% to 59%)., Conclusion: SR-TrueFISP-2D perfusion imaging provides superior SNR and CNR than TurboFLASH imaging. Moreover, the dynamic range of SIs was found to be higher with TrueFISP, resulting in an increased sensitivity for detection of perfusion defects., (Copyright 2004 Wiley-Liss, Inc.)

Purpose: To evaluate the accuracy of segmented k-space magnetic resonance phase velocity mapping (PVM) in quantifying aortic blood flow from through-plane velocity measurements., Materials and Methods: Two segmented PVM schemes were evaluated, one with seven lines per segment (seg-7) and one with nine lines per segment (seg-9), in twenty patients with cardiovascular disease. A non-segmented (non-seg) PVM acquisition was also performed to provide the reference data., Results: There was agreement between the aortic flow curves acquired with segmented and non-segmented PVM. The calculated systolic and total flow volume per cycle from the seg-7 and the seg-9 scans correlated and agreed with the flow volumes from the non-seg scans (differences < 5%). Sign tests showed that there were no statistically significant differences (P-values > 0.05) between the segmented and the non-segmented PVM measurements [corrected]. Seg-9, which was the fastest among the three sequences, provided adequate spatial and temporal resolution (> 10 phases per cycle)., Conclusion: Segmented k-space PVM shows great clinical potential in blood flow quantification., (Copyright 2002 Wiley-Liss, Inc.)

Purpose: To test the accuracy of a high performance true fast imaging with steady-state precession (TrueFISP) pulse sequence for the assessment of left ventricular (LV) mass in a large animal model on 1.5-T scanners., Materials and Methods: We imaged dogs (N = 10) on a clinical 1.5-T clinical scanner using electrocardiogram (ECG)-gated TrueFISP. In all animals, contiguous segmented k-space cine images were acquired from base to apex (in-plane resolution 1 x 1 mm(2), slice thickness 5 mm, TR = 4.8 msec, TE = 1.6 msec) during repeated breath-holds. In nine of the 10 animals, single-shot images gated to end-diastole were also acquired from base to apex in a single breath-hold (in-plane resolution 1 x 1 mm(2), slice thickness 5 mm, TR = 3.2 msec, TE = 1.6 msec). After imaging, animals were killed, the left ventricle was isolated, and the true mass of the left ventricle (free wall and septum) was determined. Independently, two observers blinded to the post-mortem results computed LV masses based on analysis of the magnetic resonance (MR) images., Results: Comparison of the computed LV mass using TrueFISP to the actual mass showed excellent agreement. Cine-systole was the most accurate technique (mass = 98.6% +/- 4.5% actual, bias = 1.2 +/- 3.4 g) followed by cine-diastole (mass = 97.9% +/- 5.3% actual, bias = 1.8 +/- 4.1 g) and single shot (mass = 94.7% +/- 7.9% actual, bias = 4.2 +/- 6.3 g). Inter- and intra-observer variabilities were low (5.8% +/- 7.1% and 0.4% +/- 4.8%, respectively)., Conclusion: We conclude that TrueFISP imaging is an accurate, rapid method to determine ventricular mass. In single-shot mode, TrueFISP requires only one breath-hold to estimate the mass of the heart within 6% of the actual value, whereas the segmented k-space implementation measured LV mass to within 3% of the true value., (Copyright 2002 Wiley-Liss, Inc.)

Purpose: To minimize contrast agent dosage for intra-arterial (IA) contrast-enhanced magnetic resonance angiography (CE-MRA) by examining the effects of encoding order (elliptical vs. sequential) and injection duration (100% to 30% of the acquisition time)., Materials and Methods: Catheter-based IA gadolinium (Gd) injections were performed in an arterial flow phantom. Blood flow rates, injection rates, and injection durations were systematically varied. Signal-to-noise (SNR) measurements were obtained in the aorta, renal artery, and common iliac artery., Results: No significant SNR losses were observed for any of the vessels with 75% injection duration, or for the aorta and iliac artery with 50% injection duration. Excellent images of all vessels were obtained at 50% injection duration. There was no significant SNR difference between encoding schemes., Conclusion: Contrast agent dosage can be substantially reduced without loss of SNR by limiting injection to part of the imaging acquisition time., (Copyright 2002 Wiley-Liss, Inc.)
