Nasal sprays, which work on the Venturi principle, have the disadvantage of suck-back which makes them unhygienic for use in more than one patient. In Wrexham we have developed a disposable nasal spray system, which is safe and would be of benefit to other ENT departments.

We present the fourth reported case of metastases of adenocarcinoma from the prostate to the larynx. The patient was a 64-year-old male who presented with stridor from a smooth subglottic mass six months after he had been diagnosed as having adenocarcinoma of the prostate with bone secondaries. We have reviewed the mechanisms by which such a tumour might metastasize to the larynx and we feel that ossification in the laryngeal cartilages may be important in the development of such metastases.

A case of oat cell carcinoma of the epiglottis eleven years after radiotherapy to the larynx for squamous cell carcinoma of the vocal cords is presented. There is a 13.5 per cent incidence of a second neoplasm in these cases. This is a very aggressive tumour and a combination of surgery, radiotherapy and multi-drug chemotherapy appears to offer the best hope of cure. The two-year survival rate is only 8.5 per cent.
