Background and Aim of the Study: Human homografts are frequently used to establish an anatomic continuity between the right ventricular outflow tract (RVOT) and the pulmonary artery. Their limited availability, especially in small sizes, has encouraged the use of alternative strategies, such as size-reduced bicuspid homografts. The study aim was to analyze the follow up of patients who had received a standard tricuspid or size-reduced bicuspid homograft in the RVOT position, and to investigate modifications of the patients' Z-scores over the years., Methods: A consecutive series of 107 patients aged < or = 16 years, who underwent RVOT repair between 1989 and 2010 to treat tetralogy of Fallot (ToF), was retrospectively reviewed. Of these patients, 17 received a size-reduced bicuspid pulmonary homograft, while 90 received a standard tricuspid homograft. The mean follow up periods were 10.5 years (range: 0.02-21.4 years) for the whole study population, and 11.8 years and 3.4 years, respectively, for the tricuspid and size-reduced bicuspid homograft groups., Results: Freedom from mortality at 10 years was 95 +/- 3%. During the observation period, 27 patients (31%) in the tricuspid homograft group and two (125) in the size-reduced bicuspid group presented with graft failure. According to the multivariable analysis, the only independent predictor of graft failure was patient age (hazards ratio 0.86). The 17 patients who had received a size-reduced bicuspid homograft were then age-matched to an equal-sized population of tricuspid homograft patients. A comparative analysis of the time-weighted average of the Z-scores for these tricuspid and size-reduced bicuspid homograft subgroups during the follow up period failed to identify any statistical difference (p = 0.5)., Conclusion: In terms of Z-score evolution, size-reduced bicuspid homografts offer results which are comparable to those achieved with tricuspid homografts.

Background and Aim of the Study: The study aim was to evaluate the very long-term patient survival, clinical and hemodynamic status after aortic valve replacement (AVR) with the Edwards PRIMA model 2500 stentless porcine aortic valve., Methods: A cohort of 50 patients was prospectively followed between September 1991 and November 2006, with clinical evaluation and echocardiography. The mean patient age at implantation was 72.2 +/- 5.2 years, and the mean valve size 24 mm (range: 19-29 mm). Most patients were in NYHA class II or III before valve implantation., Results: The early mortality was 4% (n=2). Early morbidity included two revisions for bleeding, and thromboembolic events in four patients. A total of 41 patients died during the follow up period; death was cardiac-related or sudden in 19 patients (40%) and undefined in seven (15%). Prosthetic valve endocarditis was diagnosed in five patients, and late thromboembolic events were reported in nine. Bioprosthesis explantation was performed in four patients because of severe structural valve deterioration (SVD). After 10 years, the aortic regurgitation (AR) was grade 1+ or 2+ in 12 of the 17 survivors (71%), and grade > or = 3+ in three (18%); at this time the effective orifice area was 1.81 +/- 0.59 cm2. After 12 years, the aortic regurgitation was grade 1+ or 2+ in two of seven survivors (28%), and grade > or = 3+ in four (57%). Survival of the patients did not differ significantly from that of an age-matched Belgian population., Conclusion: Patient survival was comparable with that of an age-matched population after AVR with the Edwards PRIMA model 2500 valve. SVD was mainly characterized by the development of AR.

Endogenous ochronosis, a rare inherited disease of tyrosine metabolism, is caused by a deficiency of the enzyme, homogentisic acid oxidase, and may lead to cardiovascular involvement seen most frequently as aortic valve stenosis. We report the case of a patient with generalized ochronosis who developed cardiovascular symptoms due to aortic valve stenosis and who underwent aortic valve replacement.

Background and Aim of the Study: The Ross operation has become very popular during the last decade. However little is known about the cellular behaviour of a normally functioning pulmonary autograft., Methods: This case report deals with a 14-year-old female who died from a non-valve-related cause 17 months after a Ross-Konno operation using a cryop-reserved viable pulmonary homograft for the right outflow tract. Comparison is made between the homologous and autologous pulmonary valves by macroscopic description, histology and immunohistochemistry., Results: The autograft kept its cellular population-except for the dendritic cells which have disappeared, and developed a jet lesion on the ventricular aspect of one cusp as a likely adaptation to a transvalvular gradient. The homograft was extensively devitalized, its cusps being partially covered with a fibrous sheath of recipient origin; few inflammatory cells, consisting of macrophages and rare T lymphocytes were present., Conclusions: The most puzzling observation, which needs confirmation, is the selective disappearance of the dendritic cells from the viable autograft. It is disappointing that a viable cryopreserved homograft valve has devitalized in the midterm. This phenomenon seems to result from a clinically silent immune reactions.

Background and Aims of the Study: The study aim was to assess the durability of the Carpentier-Edwards PERIMOUNT pericardial bioprosthesis in the mitral position., Methods: This seven-center retrospective clinical study involved the follow up of 333 patients who underwent isolated mitral valve replacement and 102 patients who underwent double (mitral and aortic) valve replacement with the PERIMOUNT pericardial valve between 1984 and 1989. Mean patient age at implant was 60.7 +/- 11.6 years; 41.1% were males. The most common etiology was rheumatic heart disease (53.9%) and the most common mechanism mitral insufficiency. All patients but six were followed for an average of 7.2 +/- 3.6 years after surgery; total follow up was 3071.7 patient-years (pt-yr)., Results: The total operative mortality rate was 7.6%; this included a valve-related mortality rate of 0.2%. The late mortality rate was 5.2% per pt-yr, of which 1.4% per pt-yr was considered valve-related. At 11 years, the overall actuarial survival rate was 53.3 +/- 2.8%, and actuarial freedom from valve-related death 83.9 +/- 2.6%. At follow up, 80% of patients were in NYHA class I or II, and 74% showed improvement. Twenty-eight incidences of deterioration requiring explant were observed. At 11 years the actuarial freedom from explant due to structural failure was 84.9 +/- 3.1%. Rates of structural failure decreased with age: the actuarial freedom from explant due to structural failure was 78.1 +/- 4.8% for patients aged < or = 60 years, 89.4 +/- 4.4% for those aged 61-70 years, and 100% for those aged over 71 years., Conclusion: The Carpentier-Edwards PERIMOUNT pericardial bioprosthesis is a reliable choice when a tissue valve is required, especially in patients over 60 years of age.

Background of the Study: Between February 1987 and December 1996, 187 children and young adults underwent right ventricular outflow tract (RVOT) reconstruction with aortic or pulmonary homografts., Methods: Patients were allocated to four groups according to preoperative diagnosis: RVOT obstructions with ventriculo-arterial (VA) concordance (n = 90), RVOT obstructions with VA discordance (n = 26), truncus arteriosus (n = 19) and RVOT reconstruction after the Ross procedure (n = 52). RVOT reconstruction was a reoperation in 49.7% of cases. A pulmonary homograft was used in preference (87% in concordant, 90% in Ross, 79% in truncus, and 50% in discordant groups)., Results: Five patients died after homograft repair (hospital mortality rate 2.7%). Mean follow up was 34 +/- 27 months. Four patients died during subsequent follow up; hence actuarial survival rate was 93 +/- 2% at 60 months. All other patients are currently in NYHA class I or II. Nine patients underwent reoperation because of homograft dysfunction. Overall survival was 90 +/- 3% at 60 months., Conclusions: Uni- and multivariate analysis identified young age at correction, the use of aortic homografts, corrections in patients with VA discordance, and longer aortic cross-clamp time as independent predictors of homograft failure.

Background and Aims of the Study: To determine the short-term results of the Bicarbon (Sorin Biomedica, Saluggia, Italy) heart valve prosthesis at our institution., Methods: Between November 1991 and December 1995, 256 patients (mean age 59.9 +/- 9.8 years) underwent valve replacement with a Bicarbon valve prosthesis. The mean ejection fraction was 60 +/- 14.8% and the mean left ventricular end-diastolic pressure 15.9 +/- 8.7 mmHg. Redo (n = 61) and combined procedures (n = 72) were included in this study; mixed types of prostheses were excluded. The procedures were divided into groups: aortic valve replacement (n = 163), mitral valve replacement (n = 60) and aortic-mitral (double) valve replacement (n = 33). Kaplan-Meier survival analysis was performed and predictors for survival were tested univariately and multivariately., Results: The mean follow up was 30 +/- 25.1 months with a total of 7,696 patient-months (641.3 patient-years) and was 100% complete. The Kaplan-Meier survival rate at 48 months was 87 +/- 2%. The valve-related morbidity expressed as linearized incidence rate (% per patient-year) was: anticoagulation bleeding 2.3 +/- 0.6; thromboembolic event 1.0 +/- 0.4; periprosthetic leak 0.6 +/- 0.3; and endocarditis 0.3 +/- 0.2. Univariate significant factors for worse survival were: higher preoperative New York Heart Association (NYHA) functional class, valve implantation site, presence of associated operation, increasing age, decreasing ejection fraction, longer aortic cross-clamp time, and longer cardiopulmonary bypass time. In multivariate analysis longer aortic cross-clamp time and higher preoperative NYHA classification were significant independent predictors., Conclusions: These good short-term results after Bicarbon valve replacement are encouraging and comparable with those obtained with other bileaflet mechanical heart valves. However, further follow up is mandatory.

Background and Aims of the Study: In vivo hemodynamic assessment of bileaflet aortic valve prostheses using standardized echocardiography is still uncommon; hence, adequate comparison of valve types can rarely be made. We compared the postoperative hemodynamics of St. Jude Standard valves (SJS) with those of Sorin Bicarbon valves (BC) implanted in the aortic position, using pulsed, continuous and color Doppler echocardiography., Methods: The examination was performed four months after aortic valve prosthesis implantation in 76 patients (39 SJS valves, 37 BC valves). Valve sizes varied from 19 mm to 25 mm. Maximal and mean instantaneous pressure gradients were measured by Doppler echocardiography. Effective valve orifice area (EOA) was calculated and prosthetic valve regurgitation was assessed by color Doppler flow imaging., Results: At valve sizes of 21 mm, 23 mm and 25 mm, SJS valves had a significantly lower EOAs than BC valves (p < 0.05). However, for a given nominal size, BC valves are larger, i.e. they have a larger anatomic (AOA) and geometric orifice area (GOA) than SJS valves. Consequently, BC valves were implanted in patients with a larger left ventricular outflow tract (p < 0.05). When EOA is related to the corresponding AOA, BC valves still show a larger EOA than SJS valves (p < 0.05). Prosthetic valve regurgitation is low in both valve types., Conclusions: (a) Nominal valve size is not always a good basis for comparison of hemodynamic profiles between valve types. (b) Using the relationship between EOA and AOA, the hemodynamic profile of BC valves in the aortic position is shown to be superior to that of SJS valves.

A case of aortic and mitral valve disease associated with ergotamine abuse is reported. A 34-year-old woman presented with dyspnea after a long standing history of peripheral vascular pathology associated with ergotamine abuse. Cardiac dysfunction was rapidly progressive. At surgery, nodular thickening of both valves and fusion of the posterior leaflet of the mitral valve were seen. Microscopy revealed proliferation of myofibroblasts with little destruction of normal valvular elements. The similarity to lesions in carcinoid heart disease and in methysergide-associated valvular disease suggests direct stimulation of myofibroblast growth by serotonin agonist effect.

The surgical relief of complex multilevel left ventricular outflow tract (LVOT) obstruction remains a challenging surgical problem. We present a new operation which combines the concepts of aortoventriculoplasty, extended aortic root replacement and the use of a pulmonary autograft. Sixteen patients underwent this operation: 11 patients after previous attempts to relieve diffuse subvalvular stenosis and five patients who presented excessive gradients over an outgrown aortic valve prosthesis. All patients, except one survived the operation. One patient developed massive right ventricular infarction and was transplanted after five days. Another patient developed complete heart block and transient tricuspid regurgitation after a septal infarction. One patient remained in congestive heart failure and died suddenly after 17 months. All other patients are in NYHA class I after a mean follow up of 21 +/- 12 months. One patient developed progressive dilatation of the neo-aortic root and was reoperated four years after initial surgery. All other patients showed laminar flow in the LVOT and excellent function of the autograft and homograft valve at follow up. This operation might present a durable or even a definitive solution in the management of these complex left ventricular outflow tract obstructions.

Total of 123 patients (mean age: 63.8 +/- 7.3 years, (male/female 65/35 percent) underwent mitral valve surgery combined with coronary artery bypass grafting during a seven year period. Preoperatively 12% of them belonged to NYHA functional class II, 54% to class III, 29% to class IV and 3% was operated under emergency conditions. The mitral valve lesion was most frequently either ischaemic (45%) or rheumatic (33%) in origin. Left ventricle function was moderately decreased in 18% and severely damaged in 3% of the patients as documented by preoperative ventriculography. Coronary surgery was performed in all cases with an average number of distal anastomosis of 2.2 +/- 1.1 per patient. The hospital mortality was 13%. Risk factors for early and late mortality were determined by univariate and multivariate analysis. Advanced preoperative functional class and decreased left ventricular function or ischaemic etiology were identified as significant risk factors for early mortality. The patients were followed for an average of 33 +/- 25 months. The majority of them experienced significant functional improvement postoperatively with 69% belonging to NYHA class I or II. The late survival for the 107 hospital survivors was 94.7% at one year, and 84.7% at five years, respectively. Late survival was independently determined by preoperative functional class or previous myocardial infarction. Freedom from ischemic and valve related complications at five years was 95% and 71.2% respectively. 58.2% of the hospital survivors were in functional class I or II and free of any valve related or ischemic complications at the end of the fifth follow up year.(ABSTRACT TRUNCATED AT 250 WORDS)

A prospective randomized study, comparing the clinical results between two groups of patients undergoing valve replacement with the Monostrut (MS) and Medtronic-Hall (MH) heart valve prosthesis was carried out between January 1985 and December 1988. A MS valve was implanted in 205, the MH in 198 patients selected randomly. Statistical analysis showed a perfect match between the two groups regarding preoperative and operative variables, including implantation site. All patients were given life-long anticoagulant therapy after operation. Follow up was 5.06 years (median) with a total of 1971.1 patient-years (pty) and 96.6% complete. The two groups showed no significant difference in the incidence of the following postoperative events: 30-day mortality, late mortality, reoperation, anticoagulant related bleeding, paravalvular leak, prosthetic valve endocarditis and thromboembolism (TE). Multivariate analysis indicated a borderline increased hazard of TE (2.1 +/- 0.4%/pty versus 0.7 +/- 0.2%/pty) (p = 0.0087, Bonferoni corrected significance) in the MS group. It is concluded that long term outcome is excellent with these two tilting disc valves. The marginally lower incidence of thromboembolism, however, favors the Medtronic Hall prosthesis.

Between May 1st 1985 and December 31st 1992, 283 Carpentier-Edwards pericardial valves were implanted in 260 patients in the aortic (n = 196), mitral (n = 41) and both aortic and mitral (n = 23) positions at the Gasthuisberg University Hospital in Leuven, Belgium. Patients undergoing tricuspid valve replacement or mixed replacement with another type of prosthesis were excluded from this study. The mean age was 70 +/- 7 years, there were 121 males and 139 females. The mean follow up was 40.55 months, the total follow up experience 10543 months (878.6 patients years). Hospital mortality was 10.4%, and was not significantly related to the position of the valve: 17.3% +/- 7.88% (n = 23) for double valve replacement, 10.2% +/- 2.16% (n = 196) for aortic valve replacement and 7.3% +/- 4.06% (n = 41) for mitral valve replacement. Hospital mortality was 14.1% +/- 3.27% for those with and 7.48% +/- 2.5% for those without concomitant coronary surgery (p = NS). Survival at 92 months was 63% +/- 6% and was not significantly related to the position of the valve. Not a single patient needed to be reoperated because of primary tissue failure. We conclude that the mid-term durability of this valve is excellent and consider the Carpentier-Edwards pericardial valve as the stented bioprosthesis of choice both in the aortic and mitral positions for the elderly. Because of the older age of our study population and the medium term length of follow up, we were unable to draw any conclusions concerning the incidence of calcific degeneration of this valve.
