Objectives: to investigate whether baseline mineral distribution modulates the ability of silver diammine fluoride (SDF) to remineralize and stain enamel caries lesions., Methods: This laboratory study followed a 3 [treatment: SDF/fluoride varnish (FV)/deionized water (DIW)] ×3 [lesion protocol: methylcellulose (MeC)/hydroxyethylcellulose (HEC)/Carbopol 907 (C907)] factorial design. Lesions were created in bovine enamel specimens (n = 20). Treatments were applied and lesions remineralized in artificial saliva. Digital transverse microradiography (TMR-D) was used to analyze lesions. Lesion color was monitored spectrophotometrically. The effects of lesion protocol and treatment on changes in lesion depth (ΔLD), mineral loss (ΔΔZ), maximum mineral density at the surface zone (ΔSZ max ), and color changes related to remineralization (ΔL* remin ) were analyzed using two-way ANOVA., Results: The treatment×lesion protocol interaction was significant for ΔΔZ (p < 0.01) and ΔL* remin (p < 0.01), however not for ΔLD (p = 0.23) or ΔSZ max (p = 0.91). There were no differences in ΔΔZ between treatments in HEC and C907 lesions. However, DIW resulted in more remineralization than both SDF (p < 0.01) and FV (p = 0.01) in MeC lesions. Considering changes from lesion baseline after remineralization in MeC lesions, SDF treatment resulted in the highest mineral gain in the surface zone. However, DIW revealed the highest mineral gain after remineralization in the lesion body. SDF stained lesions with the intensity increasing after remineralization in C907 lesions, whereas staining decreased in MeC and HEC lesions., Conclusion: High fluoride treatments can interfere with continuous remineralization of caries lesions due to partial arrest. Baseline lesion mineral distribution affects SDF's ability to enhance remineralization and the staining caused by SDF., Clinical Significance: SDF is being used to arrest active caries lesions extending into dentin and to treat dentin hypersensitivity. This study shed light on SDF's effect on an isolated process in dental caries only, remineralization. Objectives: Although there is some consensus that carious lesions in early stages (non-cavitated) could be treated using sealants, neither the type of materials nor their use in lesions with localized enamel breakdown (microcavitated) has been reported To compare the efficacy of resin or glass ionomer (GI) sealants in arresting microcavitated carious lesions (ICDAS 3) in first permanent molars., Materials and Methods: A double-blinded randomized controlled clinical trial was conducted in 41 healthy 6 to 11-year-old children. At the baseline examination, each subject had at least one carious lesion classified as ICDAS 3 on the first permanent molar. One hundred fifty-one lesions were randomized into: Group 1: resin sealants (76 lesions) and Group 2: GI sealant (75 lesions). Carious lesion progression was assessed clinically and radiographically. Progression and retention failure were the outcomes used for group comparisons at p-value<0.05., Results: After a two-year follow-up, only one lesion progressed to ICDAS 5, without statistically significant differences between the groups (χ 2 (1) = 0.90, p = 0.53). Radiographically, 100 lesions (98%) were arrested and 2 (2%) showed radiographic progression, without differences between groups (χ 2 (1) = 0.93, p = 0.93). At 2 years, complete retention was observed in 77% of the resin-based and in 83% of the GI sealants, without statistical differences between type of sealant (χ 2 (1) = 0.71, p = 0.48). The multilevel mixed model demonstrated that location and type of sealant did not affect retention rates (χ 2 (1) = 24,98, p < 0.001)., Conclusion: Sealing ICDAS 3 carious lesions in permanent molars appears to be effective in arresting lesions after a two-year follow-up. Clinicaltrials.gov: RCTICDAS3/2015., Clinical Significance: Minimally invasive approaches for carious lesion management have been promoted. Using sealants for the treatment of microcavitated lesions (ICDAS 3) appears predictable in the routine practice, without predilection for resin or glass ionomer materials. Objective: To investigate the remineralisation of chitosan pre-treated enamel white spot lesions (WSLs) by bioglass in the presence of the pellicle layer., Methods: 50 artificial enamel white spot lesions were created by acidic gel. Two lesions were used to investigate the formation of the pellicle layer by treating with human whole saliva for 3 min. 48 lesions were assigned to 6 experimental groups (n = 8): (1) bioactive glass slurry, (2) bioactive glass containing polyacrylic acid (BG + PAA) slurry, (3) chitosan pre-treated WSLs with BG slurry (CS-BG), (4) chitosan pre-treated WSLs with BG + PAA slurry (CS-BG + PAA), (5) "standard" remineralisation solution (RS) and (6) de-ionised water (negative control, NC). Remineralisation was carried out using a pH-cycling model for 7 days. Before each treatment using remineralising agents, 3-min pellicle was formed on lesions' surfaces. Mineral content changes, surface and subsurface microhardness and ultrastructure were evaluated by Raman intensity mapping, Knoop microhardness and scanning electron microscopy, respectively. Data were statistically analysed using one-way ANOVA with Tukey's test (p < 0.05 is considered as significant)., Results: Despite the heterogeneously formed pellicle layer, all groups showed an increase in surface mineral content after pH-cycling. Chitosan pre-treatment enhanced the subsurface remineralisation of WSLs using bioglass as both pre-treated groups showed greater surface and subsurface microhardness compared to NC. CS-BG exhibited denser subsurface structure than BG, while in CS-BG + PAA the crystals were bigger in size but resemble more enamel-like compared to BG + PAA as shown in SEM observations. Remineralisation of RS was limited to the surface as no significant subsurface changes of mechanical properties and structure were found., Conclusions: Chitosan pre-treatment can enhance WSL remineralisation with bioglass biomaterials when a short-term salivary pellicle is present. A further investigation using a long-term pH-cycling model with mature pellicle is suggested with regards to clinical application., Clinical Significance: Chitosan pre-treatment has the potential in clinical application to remineralise subsurface lesions to achieve lesion consolidation., (Copyright © 2018 Elsevier Ltd. Objectives: When planning primary oral health care services the cost implications of adopting new intervention practices are important, especially in resource-strapped countries. Although on a trajectory to be phased-out, amalgam remains the standard of care in many countries., Methods: Adopting a government perspective, this study compared the costs of performing amalgam and ART/high-viscosity glass-ionomer cement (HVGIC) restorations and the consequences of failed restorations over 3 years in suburban Brasilia, Brazil. Cost data were collected prospectively; cost estimates were developed for the study sample and a projection of 1000 single- and 1000 multiple-surface restorations per group. Probabilistic sensitivity analysis was conducted in TreeAge Pro., Results: Results were mixed. For single-surface restorations, ART/HVGIC will cost US$51 per failure prevented, while for multiple-surface restorations, ART/HVGIC was cost-effective with a savings of US$11 compared to amalgam. Probabilistic sensitivity analysis (Monte Carlo simulation) predicted amalgam would be cost-effective 49.2% of the time compared to HVGIC at 50.6% of the time at a willingness to pay threshold of US$237 per failure prevented. Personnel accounted for more than half the cost burden for both methods; instruments and supplies accounted for about one third. The per restoration cost to replace amalgam with HVGIC ranges from US$1 to a savings of US$0.84., Conclusion: Replacing amalgam with a high-viscosity glass-ionomer as part of the ART method comes at a minimal increase in cost for governments. Increasing the number of restorations seems to diminish the cost burden., Clinical Significance: ART/HVGIC could be considered a viable alternative to amalgam in primary teeth., (Copyright © 2018 Elsevier Ltd. Objectives: The aim of this in vitro study was to evaluate three treatment modalities [infiltrant resin (IR), micro-filled infiltrant resin (MFIR), infiltrant-sealant-combination (ISC)] regarding both their abilities to penetrate lesions differing in ICDAS-codes and to fill fissures and cavities., Materials and Methods: Extracted human molars (n=90) showing fissure caries lesions with and without cavitations were etched with 15% hydrochloric acid (HCl) that was mixed with abrasives and a 15% HCl-solution (1:1). The etching gel was rubbed for 30s within the fissure and, if eligible, within the cavity using a brush. After this pretreatment an infiltrant (Icon; DMG; IR) or an infiltrant mixed with microfillers (MFIR) was applied. ISC included the application of an infiltrant followed by a fissure sealant (Helioseal; Ivoclar Vivadent) From each tooth slices showing a non-cavitated (based on ICDAS-2) or cavitated lesion part (based on ICDAS-3/5) were prepared. Lesion (LA) and penetration areas (PA) as well as the completeness of fissure and cavity filling were analyzed using dual staining and confocal laser scanning microscopy., Results: Percentage penetration (PP) was calculated as 100×PA/LA. PP [median (25th/75th)] did not differ significantly between IR [95 (86/100)%], MFIR [93 (62/100)%] or ISC [89 (67/97)%] (p>0.05; Kruskal-Wallis test). All three materials filled about 90% of the dimensions of fissures and cavities (p>0.05; Kruskal-Wallis test)., Conclusion: It can be concluded that MFIR seems to be suitable to fill fissures and cavities like a fissure sealant and that it penetrates fissure caries lesions similarly deep as the conventional infiltrant after an experimental etching regime., Clinical Significance: The MFIR seems to combine advantages of the fissure sealing and the caries infiltration procedure., (Copyright © 2017 Elsevier Ltd. Objective: To compare the 18-month retention rates of composite restorations in non-carious cervical lesions [NCCLs] bonded with a self-etch adhesive with and without preliminary conditioning with EDTA., Methods: Forty-eight patients with two similar-sized NCCL were selected and randomly allocated to one of two groups. Two calibrated operators placed 96 restorations with a one-step self-etch adhesive (Adper Easy One, 3M ESPE). Half of the restorations were placed according to the manufacturer's instructions while, for the other half, the surfaces of the lesions were conditioned with 17% EDTA for 2 min prior to adhesive application. Two blinded and independent examiners evaluated the restorations at baseline, 6, 12, and 18 months, according to the FDI criteria. The comparison between groups in each period was conducted with the Fisher's exact test, and the performance of each group at the different periods was evaluated by McNemar's test (α=0.05)., Results: After 18 months, significantly higher retention rates (95% CI) were observed for the EDTA group (95.5 [84.9-98.7]) than the control group (79.6% [65.5-88.9]) (p=0.02). Significant deterioration of the marginal adaptation and marginal discoloration were observed for both groups over the 18-month evaluation., Conclusions: The preliminary conditioning with EDTA before application of a one-step self-etch adhesive significantly improved the retention rates of composite restorations in cervical lesions., Clinical Significance: Conditioning with EDTA is an alternative that improves the 18-month retention rate of cervical restorations bonded with a self-etch adhesive., (Copyright © 2015 Elsevier Ltd. Objectives: Biofilm acids contribute to secondary caries, which is a main reason for dental restoration failures. The objectives of this study were to: (1) develop a protein-repellent and antibacterial composite, and (2) investigate the effects of combining 2-methacryloyloxyethyl phosphorylcholine (MPC) with quaternary ammonium dimethylaminohexadecyl methacrylate (DMAHDM) on composite mechanical properties and biofilm response for the first time., Methods: MPC, DMAHDM and glass particles were mixed into a dental resin composite. Mechanical properties were measured in three-point flexure. Protein adsorption onto the composites was measured by a micro bicinchoninic acid method. A human saliva microcosm model was used to grow biofilms on composites. Colony-forming unit (CFU) counts, live/dead assay, metabolic activity, and lactic acid production of biofilms were determined., Results: Incorporation of 3% MPC and 1.5% DMAHDM into composite achieved protein-repellent and antibacterial capabilities without compromising the mechanical properties. Composite with 3% MPC+1.5% DMAHDM had protein adsorption that was 1/10 that of a commercial composite (p<0.05). The composite with 3% MPC+1.5% DMAHDM had much greater reduction in biofilm growth than using MPC or DMAHDM alone (p<0.05). Biofilm CFU counts on composite with 3% MPC+1.5% DMAHDM were more than three orders of magnitude lower than that of commercial control., Conclusions: Dental composite with a combination of strong protein-repellent and antibacterial capabilities was developed for the first time. Composite containing MPC and DMAHDM greatly reduced biofilm growth and lactic acid production, without compromising mechanical properties of the composite., Clinical Significance: Novel composite with MPC and DMAHDM greatly reduced biofilm activity and is promising to inhibit secondary caries. Objective: To evaluate the potential of bio-active glass (BAG) powder and BAG containing polyacrylic acid (PAA-BAG) to remineralise enamel white spot lesions (WSL)., Methods: 32 human enamel samples with artificial WSLs were assigned to 4 experimental groups (n=8); (a) BAG slurry, (b) PAA-BAG slurry, (c) "standardised" remineralisation solution (positive control) and (d) de-ionised water (negative control). Mechanical properties of enamel were assessed using surface and cross-section Knoop microhardness. Micro-Raman spectroscopy in StreamLine™ scan mode was used to scan lesion cross-sections. The intensity of the Raman phosphate peak at 959 cm(-1) was fitted and measured producing depth profiles analysed using a double-step fitting function. A further 20 samples (n=5) were used to obtain 3D images of surfaces using non-contact white light profilometry permitting measurement of lesion step height in relation to the sound enamel reference level, and to scan the lesion surface using scanning electron microscopy (SEM). Data were analysed statistically using one-way ANOVA with Tukey's HSD post-hoc tests., Results: BAG, PAA-BAG and the remineralisation solution exhibited statistically significantly higher surface and cross-section Knoop microhardness compared to the negative control. Micro-Raman spectroscopy detected significantly higher phosphate content within the treated groups compared to the negative control group. Lesions' depth was not significantly reduced. SEM images revealed mineral depositions, with different sizes and shapes, within BAG, PAA-BAG and the positive control groups., Conclusion: BAG and PAA-BAG surface treatments enhance enamel WSL remineralisation, assessed by the resultant improved mechanical properties, higher phosphate content and morphological changes within the artificial lesions., (Copyright © 2013 Elsevier Ltd. Objectives: To evaluate the bond capacities of four self-etching resin cements and the self-etching adhesives of the same manufacturer when used to cement bone fragments and compare them with a well-known N-butyl-2-cyanoacrylate bone adhesive., Methods: 125 cylindrical bone specimens from pig mandibular ramus bone were prepared using terphane burs and cemented to the corticals of 125 other specimens obtained from pig mandibular body bone using the following bond systems: Group A: Adper PLP/Relyx; group B: Optibond/Maxcem; group C: Hystoacryl; group D: AdheSE/Multilink; group E: G-Bond/G-Cem. Shear bond strength was measured 15 min after cement application using a universal testing machine., Results: Shear bond strength results: group A 2.54+/-0.23 MPa; group B 4.83+/-0.4 MPa; group C 2.90+/-0.24 MPa; group D 2.10+/-0.17 MPa; group E 4.22+/-0.24 MPa. Values for shear bond strength were significantly greater for group B and E compared to groups A, C and D (p<0.005, test Mann-Whitney). SEM images showed the presence of a hybrid layer similar to that formed by these bond systems when used on dentine., Conclusion: Within the limitations of an in vitro investigation, results show that self-etching resin cements together with self-etching adhesives may be used for cementing bone fragments., (Copyright 2010 Elsevier Ltd. Objectives: The role of fluoride in preventing erosion and abrasion on dentine remains unclear. The aim of this study was to investigate the effect of repeated applications of a 0.05% fluoride mouthwash and an adhesive on wear of dentine using stainless steel discs reference points and a laser profilometer., Methods: Small stainless steel discs were bonded to 60 caries free extracted molars which had previously been sectioned horizontally. The teeth were equally divided and subjected to four modalities in an erosion and abrasion laboratory model. Step heights were measured from the metal discs using a non-contacting laser profilometer at 100, 500, 1000, 2500 and 5000 cycles., Results: The results after 5000 cycles showed that dentine coated with a dentine adhesive (0.015 microm, S.D. 0.090) had statistically less wear than fluoride (0.127 microm, S.D. 0.150), abrasion and erosion (0.182 microm, S.D. 0.150) or abrasion only (0.096 microm, S.D. 0.090) (p<0.001)., Conclusion: In conclusion dentine surfaces coated with a resin-based adhesive provided more protection against erosion and abrasion than a fluoride mouthrinse. There were no statistical differences between the modalities for cycles less than 5000.
