1. Trajectory correction enables free-running chemical shift encoded imaging for accurate cardiac proton-density fat fraction quantification at 3T

Abstract

ABSTRACT: Background: Metabolic diseases can negatively alter epicardial fat accumulation and composition, which can be probed using quantitative cardiac chemical shift encoded (CSE) cardiovascular magnetic resonance (CMR) by mapping proton-density fat fraction (PDFF). To obtain motion-resolved high-resolution PDFF maps, we proposed a free-running cardiac CSE-CMR framework at 3T. To employ faster bipolar readout gradients, a correction for gradient imperfections was added using the gradient impulse response function (GIRF) and evaluated on intermediate images and PDFF quantification. Methods: Ten minutes free-running cardiac 3D radial CSE-CMR acquisitions were compared in vitro and in vivo at 3T. Monopolar and bipolar readout gradient schemes provided 8 echoes (TE1/ΔTE = 1.16/1.96 ms) and 13 echoes (TE1/ΔTE = 1.12/1.07 ms), respectively. Bipolar-gradient free-running cardiac fat and water images and PDFF maps were reconstructed with or without GIRF correction. PDFF values were evaluated in silico, in vitro on a fat/water phantom, and in vivo in 10 healthy volunteers and 3 diabetic patients. Results: In monopolar mode, fat-water swaps were demonstrated in silico and confirmed in vitro. Using bipolar readout gradients, PDFF quantification was reliable and accurate with GIRF correction with a mean bias of 0.03% in silico and 0.36% in vitro while it suffered from artifacts without correction, leading to a PDFF bias of 4.9% in vitro and swaps in vivo. Using bipolar readout gradients, in vivo PDFF of epicardial adipose tissue was significantly lower compared to subcutaneous fat (80.4 ± 7.1% vs 92.5 ± 4.3%, P < 0.0001). Conclusions: Aiming for an accurate PDFF quantification, high-resolution free-running cardiac CSE-MRI imaging proved to benefit from bipolar echoes with k-space trajectory correction at 3T. This free-breathing acquisition framework enables to investigate epicardial adipose tissue PDFF in metabolic diseases.

Abstract

Abstract Background Heart failure- (HF) and arrhythmia-related complications are the main causes of morbidity and mortality in patients with nonischemic dilated cardiomyopathy (NIDCM). Cardiovascular magnetic resonance (CMR) imaging is a noninvasive tool for risk stratification based on fibrosis assessment. Diffuse interstitial fibrosis in NIDCM may be a limitation for fibrosis assessment through late gadolinium enhancement (LGE), which might be overcome through quantitative T1 and extracellular volume (ECV) assessment. T1 and ECV prognostic value for arrhythmia-related events remain poorly investigated. We asked whether T1 and ECV have a prognostic value in NIDCM patients. Methods This prospective multicenter study analyzed 225 patients with NIDCM confirmed by CMR who were followed up for 2 years. CMR evaluation included LGE, native T1 mapping and ECV values. The primary endpoint was the occurrence of a major adverse cardiovascular event (MACE) which was divided in two groups: HF-related events and arrhythmia-related events. Optimal cutoffs for prediction of MACE occurrence were calculated for all CMR quantitative values. Results Fifty-eight patients (26%) developed a MACE during follow-up, 42 patients (19%) with HF-related events and 16 patients (7%) arrhythmia-related events. T1 Z-score (p = 0.008) and global ECV (p = 0.001) were associated with HF-related events occurrence, in addition to left ventricular ejection fraction (p 32.1% (optimal cutoff) remained the only CMR independent predictor of HF-related events occurrence (HR 2.15 [1.14–4.07], p = 0.018). In the arrhythmia-related events group, patients had increased native T1 Z-score and ECV values, with both T1 Z-score > 4.2 and ECV > 30.5% (optimal cutoffs) being independent predictors of arrhythmia-related events occurrence (respectively, HR 2.86 [1.06–7.68], p = 0.037 and HR 2.72 [1.01–7.36], p = 0.049). Conclusions ECV was the sole independent predictive factor for both HF- and arrhythmia-related events in NIDCM patients. Native T1 was also an independent predictor in arrhythmia-related events occurrence. The addition of ECV and more importantly native T1 in the decision-making algorithm may improve arrhythmia risk stratification in NIDCM patients. Trial registration NCT02352129. Abstract

Abstract Background Single-voxel proton cardiovascular magnetic resonance spectroscopy (1H-CMRS) benefits from 3 T to detect metabolic abnormalities with the quantification of intramyocardial fatty acids (FA) and creatine (Cr). Conventional point resolved spectroscopy (PRESS) sequence remains the preferred choice for CMRS, despite its chemical shift displacement error (CSDE) at high field (≥ 3 T). Alternative candidate sequences are the semi-adiabatic Localization by Adiabatic SElective Refocusing (sLASER) recommended for brain and musculoskeletal applications and the localized stimulated echo acquisition mode (STEAM). In this study, we aim to compare these three single-voxel 1H-CMRS techniques: PRESS, sLASER and STEAM for reproducible quantification of myocardial FA and Cr at 3 T. Sequences are compared both using breath-hold (BH) and free-breathing (FB) acquisitions. Methods CMRS accuracy and theoretical CSDE were verified on a purposely-designed fat–water phantom. FA and Cr CMRS data quality and reliability were evaluated in the interventricular septum of 10 healthy subjects, comparing repeated BH and free-breathing with retrospective gating. Results Measured FA/W ratio deviated from expected phantom ratio due to CSDE with all sequences. sLASER supplied the lowest bias (10%, vs -28% and 27% for PRESS and STEAM). In vivo, PRESS provided the highest signal-to-noise ratio (SNR) in FB scans (27.5 for Cr and 103.2 for FA). Nevertheless, a linear regression analysis between the two BH showed a better correlation between myocardial Cr content measured with sLASER compared to PRESS (r = 0.46; p = 0.03 vs. r = 0.35; p = 0.07) and similar slopes of regression lines for FA measurements (r = 0.94; p

Abstract

Background 4D Phase-Contrast MRI ("4D Flow”) has become a choice modality for assessment of flow patterns in complex vasculatures, such as congenital heart diseases. For thoracic imaging, the technique relies on ECG-triggering and an imaging navigator to compensate for breathing motion [Markl-JMRI2007]. The acquisition of the navigator requires switching off the 4D flow acquisition, thus breaking the steady-state and missing a phase in the cardiac cycle. We propose to use double gating from ECG and a pressure-driven belly-belt to monitor breathing motion while maintaining continuous steady-state.

Abstract

Background Late gadolinium enhancement (LGE) cardiac MRI is the clinical gold standard for non-invasive characterization of myocardial scar [1]. However, up to 75% of patients who may benefit from LGE MRI have preexisting implanted cardiac devices such as implantable cardioverter defibrillators (ICD) and pacemakers (PM) [2]. The presence of an ICD produces hyper-intensity (HI) image artifacts in LGE (Figure 1) and can prevent assessment of myocardial scar. We recently proposed a wideband LGE MRI technique that removes these artifacts in ICD patients [3,4]. In this abstract, we present our two-center experience of using this wideband LGE sequence on a cohort of patients with ICDs who were referred to cardiac MRI.

Abstract

Background To predict the type and extent of CMR artifacts caused by commonly used pediatric trans-catheter devices at 1.5 T and 3 T as an aid to clinical planning and patient screening. Methods Eleven commonly used interventional, catheter-based devices including stents, septal occluders, vascular plugs and embolization coils made from either stainless steel or nitinol were evaluated ex-vivo at both 1.5T and 3T. Pulse sequences and protocols commonly used for cardiovascular magnetic resonance (CMR) were evaluated, including 3D high-resolution MR angiography (MRA), time-resolved MRA, 2D balanced-SSFP cine and 2D phase-contrast gradient echo imaging (GRE). We defined the signal void amplification factor (F) as the ratio of signal void dimension to true device dimension. F1 and F2 were measured in the long axis and short axes respectively of the device. We defined F3 as the maximum extent of the off-resonance dark band artifact on SSFP measured in the B0direction. The effects of field strength, sequence type, orientation, flip angle and phase encode direction were tested. Abstract

Diabetes mellitus may alter cardiac structure and function independently of underlying coronary artery disease or hypertension. This so called “diabetic cardiomyopathy” is associated with myocardial fibrosis and is characterized by a long and silent phase of progressive left ventricular (LV) remodeling before the occurrence of contractile dysfunction and heart failure symptoms. In the preclinical stage, with normal LV ejection fraction, new echocardiographic techniques reported subtle myocardial dysfunction with a decrease in systolic strain. Recently, myocardial T1 mapping has been proposed to detect interstitial fibrosis early in the disease course.

Abstract

Background Conventional breath-held gadolinium-based contrastenhanced MR angiography (CE-MRA) provides excellent definition of extra-cardiac anatomy, but does not provide diagnostic image quality for intra-cardiac anatomy because it is not gated to ECG due to time constraints associated with breath-holding and the need to capture the first-pass of gadolinium. To address this issue, we propose a 4D non-breath-held multiphase steady-state imaging sequence (MUSIC) using ferumoxytol, which is an FDA approved iron-oxide particle for treating irondeficiency anemia, as an intravascular contrast agent and demonstrate its feasibility in children with congenital heart disease (CHD).

Abstract

Background Late gadolinium enhancement (LGE) cardiac MRI is the clinical gold standard for non-invasive assessment of myocardial viability and plays an important role in guiding catheter ablation of ventricular tachycardia (VT). The majority of VT patients have implanted cardiac devices such as implantable cardioverter defibrillators (ICDs). The presence of ICDs gives rise to strong off-resonance within the myocardium. This produces hyper-intensity (HI) artifacts in LGE, which can mask scar tissue, compromising the diagnostic value of LGE. Recent studies show that HI artifacts can be eliminated by using a wideband inversion recovery (IR) pulse in the LGE sequence. However, the current wideband LGE is a 2D sequence, which limits spatial resolution, especially slice thickness (8 mm). This is problematic for using LGE to guide catheter ablation of VT. High resolution LGE is feasible using a 3D LGE sequence. However, no prior studies have explored 3D LGE under the influence of strong off-resonance imposed by ICDs.

Background Golden angle radial reordering (GA) has been applied in many abdominal and neurological imaging applications to allow for retrospective choice of temporal resolution by providing a near-uniform k-space sampling within any image reconstruction time window [1]. However, its application in cardiac imaging is limited because the ECG-gated acquisition, which is required in most cases, breaks a single reconstruction window into several temporally isolated k-space data so that the k-space coverage may not be as uniform as GA without ECG gating (Fig. 1a) [2]. Therefore, we sought to investigate the image artifacts caused by applying GA to ECG-gated cardiac imaging and propose a segmented GA method to address this issue.

Abstract

Background Cardiac motion self-gating is a technique where MRI signal is used to derive motion triggers instead of ECG, which might be problematic in high B0 field or cases where ECG is not accessible (e.g. fetal cardiac imaging). However, the performance of existing cardiac self-gating approaches have not yet enabled clinical utility. We propose and evaluate here a novel cardiac self-gating strategy that potentially improves the trigger detection accuracy and reliability.

Abstract

Background Cardiac motion presents a major challenge in diffusion weighted MRI (DWI), often leading to large signal dropouts that necessitate repeated measurements (Pai, V.M., MRM 2011). While cardiac DWI is generally ECG gated to apply diffusion weighting during peak-systole or enddiastole, these intervals can be short and difficult to pinpoint, resulting in poor sequence reproducibility. Recent improvements in gradient hardware provide larger maximum gradients than current systems (Gmax=80mT/m), which can substantially reduce the temporal footprint of diffusion preparation and make cardiac DWI more robust to bulk motion.
