The St. Jude Medical (SJM) prosthesis (St. Jude Medical, St. Paul, MN) has a long record of safety and very low incidence (0.06%) of mechanical failure. While failure of a mechanical valve is rare, most reports occur years after the initial surgery, usually involving an obstruction preventing the valve from fully closing. The authors present a case of immediate intraoperative mechanical mitral valve failure in which a robotically inserted valve did not open upon weaning from cardiopulmonary bypass (CPB).
