Purpose: To compare the magnitude and time course of nearwork-induced transient myopia (NITM) in preadolescent Hong Kong Chinese myopes and emmetropes., Method: Forty-five Hong Kong Chinese children, 35 myopes and 10 emmetropes aged 6 to 12 years (median, 7.5), monocularly viewed a letter target through a Badal lens for 5 minutes at either 5.00- or 2.50-D accommodative demand, followed by 3 minutes of viewing the equivalent target at optical infinity. Accommodative responses were measured continuously with a modified, infrared, objective open-field autorefractor. Accommodative responses were also measured for a countercondition: viewing of a letter target for 5 minutes at optical infinity, followed by 3 minutes of viewing the target at a 5.00-D accommodative demand. The results were compared with tonic accommodation and both subject and family history of refractive error., Results: Retinal-blur-driven NITM was significantly greater in Hong Kong Chinese children with myopic vision than in the emmetropes after both near tasks, but showed no significant dose effect. The NITM was still evident 3 minutes after viewing the 5.00-D near task for 5 minutes. The magnitude of NITM correlated with the accommodative drift after viewing a distant target for more than 4 minutes, but was unrelated to the subjects' or family history of refractive error., Conclusions: In a preadolescent ethnic population with known predisposition to myopia, there is a significant posttask blur-driven accommodative NITM, which is sustained for longer than has previously been found in white adults.

Purpose: To determine whether the use of progressive addition spectacle lenses reduced the progression of myopia, over a 2-year period, in Hong Kong children between the ages of 7 and 10.5 years., Methods: A clinical trial was carried out to compare the progression in myopia in a treatment group of 138 (121 retained) subjects wearing progressive lenses (PAL; add +1.50 D) and in a control group of 160 (133 retained) subjects wearing single vision lenses (SV). The research design was masked with random allocation to groups. Primary measurements outcomes were spherical equivalent refractive error and axial length (both measured using a cycloplegic agent)., Results: There were no statistically significant differences between the PAL and the SV groups for of any of the baseline outcome measures. After 2 years there had been statistically significant increases in myopia and axial length in both groups; however, there was no difference in the increases that occurred between the two groups., Conclusions: The research design used resulted in matched treatment and control groups. There was no evidence that progression of myopia was retarded by wearing progressive addition lenses, either in terms of refractive error or axial length.

Purpose: To investigate the effects of age on transient vernier visual evoked potential (VEP) and vernier acuity estimated by extrapolation., Methods: Transient vernier VEPs were examined in normal subjects aged 20 to 75 years. Vernier offsets were presented for the first 350 msec of the stimulus period, and the segments were then realigned in the following 400 msec. The six vernier offsets used were 20, 40, 60, 80, 100, and 140 seconds of arc. Averaging for each offset setting produced vernier VEP waveforms, for which amplitude and latency of visual evoked response and background electroencephalographic (EEG) noise level were determined. Extrapolation of the function relating signal-to-noise ratio and log vernier offset to a ratio of 1.0 resulted in an estimate of vernier acuity., Results: Amplitude of vernier VEP waveforms was significantly reduced in subjects more than 60 years of age, and the latency to the first negative peak was progressively prolonged with increasing age. There was no statistically significant change in electroencephalographic (EEG) noise with advancing age. VEP vernier acuity was significantly degraded in the 61- to 75-year age group. These results are parallel to recent psychophysical findings that alignment performance is worse in older persons than in younger ones., Conclusions: The present findings provide the first electrophysiological evidence of age-related cortical degeneration associated with vernier processing. Reduced neural activity probably contributes to the loss of vernier acuity with advancing age. Also provided are the first normative data for subjects of different ages for vernier VEP and VEP vernier acuity. Moreover, the present study has demonstrated that vernier VEP is sensitive to neural changes and therefore may be applied in clinical situations to evaluate the integrity of the visual system.

Purpose: To investigate the variation of visual evoked potential (VEP) function at different eccentricities of the visual field in esotropic amblyopes and anisometropic amblyopes., Methods: Data from 5 esotropic amblyopic eyes, 6 anisometropic amblyopic eyes, and 45 control eyes were analyzed. A VERIS system was used to generate a stimulus matrix containing 61 hexagons on a computer monitor. Each hexagon of the display contained a number of small black and white hexagonal patches that reversed in polarity during stimulation according to a pseudorandom binary m-sequence. The VERIS system extracted the local responses by cross-correlating the input and output signals. The latencies and amplitudes of the responses from the central 8.6 degrees of arc in the visual field were analyzed., Results: In esotropic amblyopia, the multifocal VEP latency is prolonged, and the amplitude is reduced in the central region of the visual field. The mean amplitude is significantly smaller, and the mean latency is significantly longer in the temporal visual field than in the nasal visual field. In anisometropic amblyopia, latencies are markedly prolonged, and the amplitudes of multifocal VEP are attenuated in the central region of the visual field, and these effects are lessened in the periphery., Conclusions: The results are in agreement with psychophysical studies reporting a greater foveal deficit in amblyopia and a greater visual loss in the temporal field than in the nasal field in esotropic amblyopia.
