This discussion describes an interesting aspect of ongoing research in Japan. Some neurological insults, e.g., alcohol withdrawal, can produce both delirium and dissociated sleep-wake states, and such conditions may involve both generalized and focal brain dysfunction. Although none of the papers in this issue focuses implicitly on changes in sleep in delirium, these comments represent an interesting contribution.</fn>
