Research on aging is at an important inflection point, where the insights accumulated over the last 2 decades in the basic biology of aging are poised to be translated into new interventions to promote health span and improve longevity. Progress in the basic science of aging is increasingly influencing medical practice, and the application and translation of geroscience require seamless integration of basic, translational, and clinical researchers. This includes the identification of new biomarkers, novel molecular targets as potential therapeutic agents, and translational in vivo studies to assess the potential efficacy of new interventions. To facilitate the required dialog between basic, translational, and clinical investigators, a multidisciplinary approach is essential and requires the collaborative expertise of investigators spanning molecular and cellular biology, neuroscience, physiology, animal models, physiologic and metabolic processes, pharmacology, genetics, and high-throughput drug screening approaches. In an effort to better enable the cross-talk of investigators across the broad spectrum of aging-related research disciplines, a goal of our University of Pittsburgh Claude D. Pepper Older Americans Independence Center has been to reduce the barriers to collaborative interactions by promoting a common language through team science. The culmination of these efforts will ultimately accelerate the ability to conduct first-in-human clinical trials of novel agents to extend health span and life span.

Fear of Falling (FOF) is common among community-dwelling older adults and is associated with increased fall-risk. In this cross-sectional study we examined the relationships between FOF and factors associated with fall-risk such as gait quality, cognition, and walking-confidence. Using baseline data from older adult participants in a randomized exercise trial (N=232; age 77±6; 65% females; 40% reported FOF), we quantified the following outcome measure for (1) gait quality: harmonic ratio (smoothness) and time-frequency spatiotemporal variables from triaxial accelerometry during 6 minute walk; gait speed, step-time CoV (variability) and walk-ratio (step-length/cadence) on an instrumented walkway; (2) cognition: Trails A and B (3) walking-confidence: Gait efficacy Scale. Task-specific power training (InVEST) targets leg power and mobility skills that are beneficial for treating slow gait speed for older adults. This study investigated the efficacy of a short-term InVEST training on leg power, mobility performance, and gait characteristics and further examine whether the addition of cognitive training would augment the impact on the outcomes. Mobility limited community-dwelling older Veterans age ≥65 years were recruited. Participants were randomly assigned to either InVEST training (n=10) or InVEST+cognitive training (n=11). Training occurred 3 times per week for 6 weeks. Sessions were either 70 minutes (InVest+cognitive training) or 40 minutes (InVEST) in duration. Leg power, mobility performance (Short Physical Performance Battery), and gait characteristics (gait speed, stance time, step width, swing time, step length and their variabilities under single-task, simple and complex dual-task walking conditions) were evaluated. Twenty-one men with mean age 76±7 years completed the study and 86% were of white race. Among all participants, clinically relevant and statistically significant improvements in leg power, mobility performance, and gait characteristics (gait speed, step length, stance time under all three gait conditions) were observed. There were no statistically significant or clinically relevant group differences among any of the outcomes based on cognitive training status. Short-term InVEST training led to clinically meaningful improvements in leg power, mobility performance, and gait characteristics. These findings add to the body of evidence supporting the benefits of InVEST training on mobility and do not support the contention that mixed modes of training (cognitive and physical) may augment mobility outcomes.

Test retest reliability of gait variability is less than ideal, i.e. ICC 0.013s, 2) consistent (CN, n=32) STV difference from session to session < 0.013s. Differences between groups were determined by independent t-tests and Chi-square. IN compared to CN, walked slower (0.83 vs 1.01 m/s) and was less likely to report good or better mobility (36% vs 78%) and balance (21% vs 63%), all p’s < 0.05. Overall health and vigor were similar between groups. Inconsistency in step time variability from week to week in older adults may not be due to measurement error, but may actually be an indicator of impaired mobility and balance.
