Introduction: To highlight an alternative method of treatment for femoral fractures associated with vascular injury, allowing both fracture fixation and vascular repair from the same surgical approach.Design: Retrospective case series.Methods: Twelve patients with arterial injury as a consequence of femoral fracture were treated by internal fixation. There were five femoral diaphysis, seven distal femoral including supracondylar and intercondylar fractures. Four patients had injury to the popliteal artery, whereas eight to the superficial femoral artery. Seven patients had a nerve injury. Medial femoral approach was used both for the fixation of the fracture and vascular repair. Fracture fixation was done by plate-screw in all patients, whereas vascular injuries were treated using saphenous vein interposition graft in ten patients and end-to-end repair in two patients. Nerve injuries were not dealt by exploration during the vascular repair.Results: All of the limbs were ultimately survived. Clinical and radiological union was observed in all patients at the last follow-up radiographs. Of the 4 total sciatic nerve palsies, 2 had developed total, one had tibial nerve recovery. One patient had no motor recovery. Of the 3 peroneal nerve palsies, 1 patient had recovery, 2 patients had no motor improvement.Conclusions: A midlateral approach is traditionally used for fractures of femur when plates and screws are used for fixation. In association with vascular injury, this procedure requires two separate incisions. The added soft tissue disruption associated with open reduction and internal fixation by lateral incision can be reduced with the use of single medial approach, which allows bony stabilization under direct visualization of the repaired vessels. Introduction: The purpose of this study was to evaluate the feasibility of the anteromedial minimally invasive plate osteosynthesis (MIPO) approach for distal third humeral shaft fractures and identify neurovascular structures at risk with this approach.Methods: Twenty cadaveric arms were fixed with 12-hole precontoured narrow locking compression plates (LCP) with the anteromedial approach using MIPO technique. The proximal approach was done between the biceps and deltoid muscle directly to the bone. The distal approach involved elevating the brachialis from medial intermuscular septum. The plate was inserted beneath the brachialis tunnel from distal to proximal. Three locking screws were fixed at each end through incisions and the rest of screws were inserted percutaneously. The arms were then dissected to identify damage to or direct contact between the screws and brachial artery (BA), median nerve (MN), musculocutaneous nerve (MCN), and radial nerve (RN). The distances from the screws to structures at risk, humeral length, and length of three distal screws in mediolateral (ML) direction were measured.Results: The average humeral length was 28.97 cm. The average danger zone for the BA and MN were 20.47%-62.66% of the humeral length from the lateral epicondyle, and 20.47%-75.02% for the MCN. The ulnar nerve was not endangered by this approach as it lies posteromedially to the humerus. The danger zone for the RN averaged 27.07%-43.74%, and the most dangerous screw that either penetrated or touched the nerve was at the fifth hole, which lay at 33.14% of the humeral length. The average length of three distal screws in ML direction were 41.4, 25.0 and 22.5 mm.Conclusions: The anteromedial MIPO approach can be performed through the internervous plane beneath the brachialis muscle without exposing any nerves or causing any muscle splitting with a 12-hole plate. Both proximal and distal screw insertion must be done with direct exposure. Insertion of percutaneous screws in the middle part of the plate between the two incisions is not possible. This approach could be an alternative for extra-articular distal third humeral shaft fractures which provides less invasive surgical dissection, allows the use of longer distal screws, and achieves better cosmesis. Introduction: Minimally invasive plate osteosynthesis (MIPO) on the medial side of the femur appears to be a dangerous procedure due to possible femoral artery injury.Objective: This study aims to determine the feasibility of applying MIPO of the femur via the medial approach, and to determine the anatomical relationship and structures at risk between the artery and the implant using computed tomography angiography.Materials and Methods: A descriptive study of ten fresh cadavers was done. Two separate incisions were made, creating a submuscular tunnel close to the medial side of the femur. An 11- or 13-hole LCP lateral proximal tibial plate (5.0mm) was inserted through the distal incision into the submuscular tunnel and fixed. A CT angiogram with 3D reconstruction was made to determine the distance from and location of the plate relative to the femoral artery and surgical dissection was done to identify the structures at risk.Results: No disruptions of superficial or deep femoral arteries were found. The closest distances from the superficial femoral artery and deep femoral artery to the plate were 8.3-27.2mm (average 16.3mm) (99% CI: 12.7-19.9) at the level 3 and 4.5-20.0mm (average 8.6mm) (99% CI: 6.4-10.9) at the level 2 in the proximal part of femur, respectively. The location where the SFA crossed the anterior cortex of the femur in the sagittal plane was 9.7-36.0% of the femoral length (average 20.1%) (99% CI: 15.0-25.3%) and the posterior cortex of the femur was 24.7-55.3% of the femoral length (average 40.8%) (99% CI: 35.0-46.7%). The location where the DFA crossed the anterior cortex of the femur in the sagittal plane was 7.9-25.3% of the femoral length (average 13.4%) (99% CI: 10.6-16.3%) and where it crossed the posterior cortex of the femur was 21.7-39.4% of the femoral length (average 31.2%) (99% CI: 27.1-33.3%).Conclusion: MIPO of the femur via medial approach is a feasible option for treatment of femoral fractures when the lateral approach is contraindicated. Abstract: Objective: In 1981, Moore introduced a new classification for dislocation-type fractures caused by high-energy mechanisms. The most common medial Moore-type fractures are entire condyle fractures with the avulsion of the median eminence (tibial anterior cruciate ligament (ACL) insertion). They are usually associated with a posterolateral depression of the tibial plateau and an injury of the lateral menisco-tibial capsule. This uniform injury of the knee is increasingly observed in the recent years after skiing injuries due to the high-speed carving technique. This uprising technique uses shorter skis with more sidecut allowing much higher curve speeds and increases the forces on the knee joint. The aim of this study was to describe the injury pattern, our developed operative approach for reconstruction and outcome. Methods: A total of 28 patients with 29 postero-medial fracture dislocation of the proximal tibia from 2001 until 2009 were analysed. Clinical and radiological follow-up was performed after 4 years on average (1 year in minimum). Evaluation criteria included the Lysholm score for everyday knee function and the Tegner score evaluating the activity level. All fractures were stabilised post primarily. The surgical main approach was medial. First, the exposure of the entire medial condyle fracture was performed following the fracture line to the articular border. The posterolateral impaction was addressed directly through the main fracture gap from anteromedial to posterolateral. Small plateau fragments were removed, larger fragments reduced and preliminarily fixed with separate K-wire(s). The postero-medial part of the condyle was then prepared for main reduction and application of a buttress T-plate in a postero-medial position, preserving the pes anserinus and medial collateral ligament. In addition, a parapatellar medial mini-arthrotomy through the same main medial approach was performed. Through this mini-arthrotomy, the avulsed anterior eminence with attached distal ACL is retained by a transosseous suture back to the tibia. Results: We could evaluate 26 patients (93%); two patients were lost to follow-up due to foreign residence. Median age was 51 years (20–77 years). The fractures were treated post primarily at an average of 4 days; in 18 cases a two-staged procedure with initial knee-spanning external fixator was used. All fractures healed without secondary displacement or infection. As many as 25 patients showed none to moderate osteoarthritis after a median of 4 years. One patient showed a severe osteoarthritis after 8 years. All patients judge the clinical result as good to excellent. The Lysholm score reached 95 (75–100) of maximal 100 points and the Tegner activity score 5 (3–7) of maximal 10 points (competitive sports). The patients achieved a median flexion of 135° (100–145°). Conclusion: In our view, it is crucial to recognise the different components of the injury in the typical postero-medial fracture dislocation of the proximal tibia. The described larger medial approach for this type of medial fracture dislocation allows repairing most of the injured aspects of the tibial head, namely the medial condyle with postero-medial buttressing, the distal insertion of the ACL and the posterolateral impaction of the plateau.
