Hemisection denotes the removal of compromised root and the associated crown portion with the loss of periodontal attachment and is performed to maintain the original tooth structure and attain the fixed prosthesis. The success rate of such procedure is high. The present case report demonstrates the successful management of hemisection of 46 with occlusal rehabilitation. It was a conservative approach aiming to retain as much original tooth structure as possible against the option of extraction of the natural tooth.

Traditionally the pathological furcation involvements of a tooth lead to several problems and eventually the loss of the involved tooth was thought to be inevitable if furcation involvement happened to progress to an advanced grade II or grade III type. However, ever since the advent of the hemisection procedure it has shown to be a reliable and viable alternative to extraction in such cases. This has led to its acceptability as a routine endodontic procedure for maintenance of natural dentition. In this paper a case is presented in which hemisection was done because the tooth was grossly carious along with furcation involvement.
