Objective: The reconstruction of an input based on a sparse combination of signals, known as sparse coding, has found widespread use in signal processing. In this work, the combination of sparse coding with Kalman filtering is explored and its potential is shown on two use-cases., Methods: This work extends the Iterative Shrinkage and Thresholding Algorithm with a Kalman filter in the sparse domain. The resulting method may be implemented as a deep unfolded neural network and may be applied to any signal which has a sparse representation and a known or assumed relation between consecutive measurements. This method is evaluated on the use cases of noise reduction in the electrocardiogram (ECG) and the estimation of object motility., Results: For ECG denoising, the proposed method achieved an improvement in Signal-to-Noise ratio of 18.6 dB, which is comparable to state-of-the-art. In motility estimation, a correlation of 0.84 with ground truth simulations was found., Conclusion: The proposed method was shown to have advantages over sparse coding and Kalman filtering alone. Due to the low complexity and high generalizability of the proposed method, the implementation of context-specific knowledge or an extension to other applications can be readily made., Significance: The presented Kalman-ISTA algorithm is a resource-efficient method combining the promise of both sparse coding and Kalman filtering, making it well-suited for various applications.

Atrial fibrillation (AF) is a prevalent clinical arrhythmia disease and is an important cause of stroke, heart failure, and sudden death. Due to the insidious onset and no obvious clinical symptoms of AF, the status of AF diagnosis and treatment is not optimal. Early AF screening or detection is essential. Internet of Things (IoT) and artificial intelligence (AI) technologies have driven the development of wearable electrocardiograph (ECG) devices used for health monitoring, which are an effective means of AF detection. The main challenges of AF analysis using ambulatory ECG include ECG signal quality assessment to select available ECG, the robust and accurate detection of QRS complex waves to monitor heart rate, and AF identification under the interference of abnormal ECG rhythm. Through ambulatory ECG measurement and intelligent detection technology, the probability of postoperative recurrence of AF can be reduced, and personalized treatment and management of patients with AF can be realized. This work describes the status of AF monitoring technology in terms of devices, algorithms, clinical applications, and future directions.

Objective: Preterm birth is the leading cause of morbidity and mortality involving over 10% of infants. Tools for timely diagnosis of preterm birth are lacking and the underlying physiological mechanisms are unclear. The aim of the present study is to improve early assessment of pregnancy progression by combining and optimizing a large number of electrohysterography (EHG) features with a dedicated machine learning framework., Methods: A set of reported EHG features are extracted. In addition, novel cross and multichannel entropy and mutual information are employed. The optimal feature set is selected using a wrapper method according to the accuracy of the leave-one-out cross validation. An annotated database of 74 EHG recordings in women with preterm contractions was employed to test the ability of the proposed method to recognize the onset of labor and the risk of preterm birth. Difference between using the contractile segments only and the whole EHG signal was compared., Results: The proposed method produces an accuracy of 96.4% and 90.5% for labor and preterm prediction, respectively, much higher than that reported in previous studies. The best labor prediction was observed with the contraction segments and the best preterm prediction achieved with the whole EHG signal. Entropy features, particularly the newly-employed cross entropy contribute significantly to the optimal feature set for both labor and preterm prediction., Significance: Our results suggest that changes in the EHG, particularly the regularity, might manifest early in pregnancy. Single-channel and cross entropy may therefore provide relevant prognostic opportunities for pregnancy monitoring.

Objective: Filtering power line interference (PLI) from electrocardiogram (ECG) recordings can lead to significant distortions of the ECG and mask clinically relevant features in ECG waveform morphology. The objective of this study is to filter PLI from ECG recordings with minimal distortion of the ECG waveform., Methods: In this paper, we propose a fixed-lag Kalman smoother with adaptive noise estimation. The performance of this Kalman smoother in filtering PLI is compared to that of a fixed-bandwidth notch filter and several adaptive PLI filters that have been proposed in the literature. To evaluate the performance, we corrupted clean neonatal ECG recordings with various simulated PLI. Furthermore, examples are shown of filtering real PLI from an adult and a fetal ECG recording., Results: The fixed-lag Kalman smoother outperforms other PLI filters in terms of step response settling time (improvements that range from 0.1 to 1 s) and signal-to-noise ratio (improvements that range from 17 to 23 dB). Our fixed-lag Kalman smoother can be used for semi real-time applications with a limited delay of 0.4 s., Conclusion and Significance: The fixed-lag Kalman smoother presented in this study outperforms other methods for filtering PLI and leads to minimal distortion of the ECG waveform.

Capacitive electrodes are a promising alternative to the conventional adhesive electrodes for ECG measurements. They provide more comfort to the patient when integrated in everyday objects (e.g., beds or seats) for long-term monitoring. However, the application of capacitive sensors is limited by their high sensitivity to motion artifacts. For example, motion at the body-electrode interface causes variations of the coupling capacitance which, in the presence of a dc voltage across the coupling capacitor, create strong artifacts in the measurements. The origin, relevance, and reduction of this specific and important type of artifacts are studied here. An injection signal is exploited to track the variations of the coupling capacitance in real time. This information is then used by an identification scheme to estimate the artifacts and subtract them from the measurements. The method was evaluated in simulations, lab environments, and in a real-life recording on an adult's chest. For the type of artifact under study, a strong artifact reduction ranging from 40 dB for simulated data to 9 dB for a given real-life recording was achieved. The proposed method is automated, does not require any knowledge about the measurement system parameters, and provides an online estimate for the dc voltage across the coupling capacitor.

The continuous analysis of electrocardiographic (ECG) signals is complicated by morphological variability in the ECG due to movement of the heart. By aligning vectorcardiographic loops, movement-induced ECG variations can be partly corrected for. Existing methods for loop alignment can account for loop rotation, scaling, and time delays, but they lack the possibility to include a priori information on any of these transformations, and they are unreliable in case of low-quality signals, such as fetal ECG signals. The inclusion of a priori information might aid in the robustness of loop alignment and is, hence, proposed in this paper. We provide a generic Bayesian framework to derive our loop alignment method. In this framework, existing methods can be readily derived as well, as a simplification of our method. The loop alignment is evaluated by comparing its performance in loop alignment to two existing methods, for both adult and fetal ECG recordings. For the adult ECG recordings, a quantitative performance assessment shows that the developed method outperforms the existing method in terms of robustness. For the fetal ECG recordings, it is demonstrated that the developed method can be used to correct ECG signals for movement-induced morphology changes (enabling diagnostics) and that the method is capable of classifying recorded ECG signals to periods of fetal movement or rest ( 0.01). This information on fetal movement can also serve as a valuable diagnostic tool.

The ongoing trend of ECG monitoring techniques to become more ambulatory and less obtrusive generally comes at the expense of decreased signal quality. To enhance this quality, consecutive ECG complexes can be averaged triggered on the heartbeat, exploiting the quasi-periodicity of the ECG. However, this averaging constitutes a tradeoff between improvement of the SNR and loss of clinically relevant physiological signal dynamics. Using a bayesian framework, in this paper, a sequential averaging filter is developed that, in essence, adaptively varies the number of complexes included in the averaging based on the characteristics of the ECG signal. The filter has the form of an adaptive Kalman filter. The adaptive estimation of the process and measurement noise covariances is performed by maximizing the bayesian evidence function of the sequential ECG estimation and by exploiting the spatial correlation between several simultaneously recorded ECG signals, respectively. The noise covariance estimates thus obtained render the filter capable of ascribing more weight to newly arriving data when these data contain morphological variability, and of reducing this weight in cases of no morphological variability. The filter is evaluated by applying it to a variety of ECG signals. To gauge the relevance of the adaptive noise-covariance estimation, the performance of the filter is compared to that of a Kalman filter with fixed, (a posteriori) optimized noise covariance. This comparison demonstrates that, without using a priori knowledge on signal characteristics, the filter with adaptive noise estimation performs similar to the filter with optimized fixed noise covariance, favoring the adaptive filter in cases where no a priori information is available or where signal characteristics are expected to fluctuate.

For assessment of specific cardiac pathologies, vectorcardiography is generally considered superior with respect to electrocardiography. Existing vectorcardiography methods operate by calculating the vectorcardiogram (VCG) as a fixed linear combination of ECG signals. These methods, with the inverse Dower matrix method the current standard, are therefore not flexible with respect to different body compositions and geometries. Hence, they cannot be applied with accuracy on patients that do not conform to the fixed standard. Typical examples of such patients are obese patients or fetuses. For the latter category, when recording the fetal ECG from the maternal abdomen the distance of the fetal heart with respect to the electrodes is unknown. Consequently, also the signal attenuation/transformation per electrode is not known. In this paper, a Bayesian method is developed that estimates the VCG and, to some extent, also the signal attenuation in multichannel ECG recordings from either the adult 12-lead ECG or the maternal abdomen. This is done by determining for which VCG and signal attenuation the joint probability over both these variables is maximal given the observed ECG signals. The underlying joint probability distribution is determined by assuming the ECG signals to originate from scaled VCG projections and additive noise. With this method, a VCG, tailored to each specific patient, is determined. The method is compared to the inverse Dower matrix method by applying both methods on standard 12-lead ECG recordings and evaluating the performance in predicting ECG signals from the determined VCG. In addition, to model nonstandard patients, the 12-lead ECG signals are randomly scaled and, once more, the performance in predicting ECG signals from the VCG is compared between both methods. Finally, both methods are also compared on fetal ECG signals that are obtained from the maternal abdomen. For patients conforming to the standard, both methods perform similarly, with the developed method performing marginally better. For scaled ECG signals and fetal ECG signals, the developed method significantly outperforms the inverse Dower matrix method.
