Abstract: Introduction. Early diagnosis of acute inflammatory demyelinating polyneuropathy (AIDP) is of fundamental importance for the timely administration of therapy. At early stages of this condition, conventionally used electrophysiological diagnostic techniques are not sensitive enough. The objective of this work was to study H-reflex as a potential tool for early diagnosis of AIDP in children. Materials and methods. A total of 57 children were examined: 20 healthy children (7-14 years; mean age 12 years) and 37 patients diagnosed with AIDP (8−13 years; mean age 11 years). Electroneuromyography (ENMG) was performed on the 3d-7th day after the first symptoms onset. Nerve conduction velocity for motor fibers, amplitude of M-response during stimulation of n. tibialis, n. ulnaris and n. medianus, as well as latency and threshold of M-response and H-reflex during stimulation of m. soleus, were evaluated. Results. No significant intergroup differences in amplitudes of motor responses and the nerve conduction velocity were recorded, while the residual latency of M-response was significantly higher in the AIDP group. In individuals of the control group, the H-reflex was recorded in 100% of cases, while being recorded only in 2 (5.4%) patients in the AIDP group. In the latter two patients, the study was performed at the earliest stage (3d day) after the onset of the first symptoms. Conclusions. In pediatric patients with AIDP, on the 3d-7th day after the first symptoms onset, H-reflex was absent in 94.6% of cases. Evaluation of the H-reflex at the early stage of AIDP in children can be used as an additional diagnostic criterion. [ABSTRACT FROM AUTHOR]

Electroneuromyography (ENMG) was used to assess the effectiveness of a short combined neurorehabilitation course (temporal epidural stimulation of the spinal cord combined with robotic kinesiotherapy) in the restorative treatment of patients with traumatic spinal cord disease. Before and after the combined instrumental neurorehabilitation course (the duration was from two to three weeks), 75 patients suffering from consequences of the spinal cord injury were tested. Global and stimulation ( H-reflex, M-response) electromyography methods were used. On the basis of the ENMG data, the indices of sensorimotor deficit (ISD) and their postrehabilitation trends were calculated. The ENMG signs of regression of sensorimotor deficit in the lower extremities were observed in 46.6% of the cases; those in the upper extremities (if the cervical spine was damaged) were found in 78.6% of the cases. The stabilizing effect of the neurorehabilitation technique used was observed in an average of 24.0% of the cases. In 18.8% of the cases, the use of the combined neurorehabilitation technique was ineffective. The indications for the use of the combined neurorehabilitation course series should include the ENMG signs of the partial preservation of the corticospinal tract conduction and a positive ISD trend after completion of each course. To study the effect of local temporary ischemia of the ulnar nerve on the motor nerve conduction velocity, electroneuromyography was conducted at the level of the forearm in 26 children of the comparison group, 16 children with acute viral meningitis (VM), 11 children 14 days after clinical manifestations of VM, and 11 children with Guillain-Barré syndrome (GBS) in the catamnesis. The group of children with GBS in the catamnesis exhibited a less pronounced reactivity of nerve conduction (RNC) on the 10th minute of ischemia (by 50%, p < 0.00001) than that in the comparison group. A lower RNC (by 29%) compared to the control was recorded in children with acute VM. The resistance to ischemia of motor axons in immune-mediated neuropathies and acute infections may be caused by cytokine-mediated channelopathies. [ABSTRACT FROM AUTHOR]
