1. Which is the best bearing surface for primary total hip replacement? A New Zealand Joint Registry study.

Abstract

Introduction: We have investigated the revision rates of all bearing surface combinations for primary total hip replacement (THR) registered on the New Zealand Joint Registry (NZJR) to determine which coupling has been the most durable and successful over the last 16 years. Methods: There were 106,139 primary THRs registered, resulting in 4,960 revisions for any cause. We examined all-cause revision rates, reasons for revision and performed survival analyses. Results: Ceramic-on-highly cross-linked polyethylene (CoPx) had the lowest all-cause revision rate of 0.54/100-component-years (cys) (95% confidence interval 0.48 to 0.61). This was superior to all other hard-on-soft bearing combinations in unadjusted analysis. Furthermore, the age of patients receiving CoPx was significantly lower than for metal-on-polyethylene (mean 62.9; standard deviation [SD] 10.1 vs. 69.1; SD 9.6; p<0.001). Acetabular loosening was the reason for revision in 14.5% of CoPx, compared to 33% of MoP THRs (p<0.001). Metal-on-metal bearings had the highest revision rate of 1.43/100 cys and were significantly inferior to CoPx (p<0.001). Kaplan-Meier analysis and Cox regression analyses were performed and we adjusted the analyses to control for age, femoral head size, surgical approach and fixation. Conclusions: CoPx remained the most durable and successful coupling used in primary THR in New Zealand irrespective of age, gender or size of femoral head. [ABSTRACT FROM AUTHOR]

Abstract: Second-generation metal-on-metal total hip arthroplasty (THA) was introduced in the early 1990s to address osteolysis and aseptic loosening resulting from polyethylene wear. We present a comparison between the Transcend metal-on-metal and Interseal metal-on-polyethylene THAs. Thirty-seven hips with Transcend metal bearings and 36 hips with Interseal polyethylene acetabular liners but identical acetabular shells were reviewed to determine clinical performance, radiographic changes, and survivorship. Patients with higher anticipated activity levels were selected to receive the Transcend bearing. Mean follow-up time was 107.0 months for the Transcend group, and 90.4 months for the Interseal group. There were no significant differences between the Transcend and Interseal groups for mode of failure and survivorship, which is notable considering the younger and more active Transcend group. However, the Transcend group showed significantly better clinical scores, which may have been a result of the selection methods. Neither surface was differentially implicated in osteolysis, aseptic loosening, or adverse local tissue reaction (ALTR). Our study shows a favorable and comparable performance for both systems. Abstract

Adverse reactions to metal particle debris have been increasingly reported as a complication following large head metal-on-metal (MoM) hip arthroplasty. Elevated metal ion levels are a cause for concern. The aim of this study is to evaluate whether exposure to cobalt is associated with patient characteristics and symptoms of neuropathy, representing nervous system dysfunction. A cross-sectional study was conducted comparing patients with a MoM total hip arthroplasty and patients with a conventional hip arthroplasty. They received three questionnaires, one to assess neurotoxic complaints and two standardised self-administered questionnaires to identify symptoms that are suggestive of peripheral neuropathy. Current and historical data were available for whole blood cobalt levels in all patients. We analysed potential predictive factors for cobalt based on five different cut-off levels (0-2, 2-4, 4-10, 10-20, >20 µg/L). We performed 723 MoM total hip arthroplasties in 643 patients in our clinic. The response rate was 89%. Male-female ratio was 236/280, median age 63.6 years (30-72) with a mean follow-up of 50.6 months (20-86). We also sent the questionnaires to 98 patients in the control group. An increase in the incidence of reported symptoms was not consistent with higher serum cobalt levels. Female gender was the only consistent predictive factor for serum cobalt at different cobalt cut-off levels in the multivariate analysis. The study population did not show an increase in reported symptoms with elevated cobalt levels. Neurotoxic symptoms and whole blood cobalt levels did not show a consistent relationship with different dichotomised levels of cobalt exposure.
