5. Microbiote et inhibiteurs du checkpoint immunitaire.

Résumé: L'immunothérapie anticancéreuse est en plein essor grâce au développement des inhibiteurs du checkpoint immunitaire. Ces anticorps monoclonaux ciblant notamment le CTLA-4 et le PD-1 permettent de stimuler l'immunité antitumorale de l'hôte. Ils ont d'abord transformé le pronostic du mélanome métastatique et de certains cancers broncho-pulmonaires à petites cellules. Ces bénéfices s'étendent progressivement à de nombreux autres organes et notamment aux cancers du tractus gastro-intestinal. Bien que leur efficacité réside dans leur effet immunologique, la contribution du microbiote intestinal est désormais indéniable, jouant un rôle sur l'efficacité et la tolérance des inhibiteurs du checkpoint immunitaire. En effet, certains patients peuvent développer des effets indésirables immuno-médiés et notamment des entérocolites qui miment une maladie inflammatoire chronique de l'intestin. Cet article décrit plus particulièrement les liens entre microbiote intestinal et inhibiteurs du checkpoint immunitaire. Cancer immunotherapy use and more specifically checkpoint inhibitor is currently rising based on dramatic improvement of prognosis of many cancers such as metastatic melanoma and some subtypes of lung cancer. Checkpoint inhibitors are now implemented in many other cancers including those of the gastrointestinal tract. The contribution of intestinal microbiome has been suggested through many studies allowing some microbial fingerprints to predict efficacy and safety. Indeed, some patients may experience immune-related adverse events notably enterocolitis mimicking inflammatory bowel disease. 6. Prise en charge des sténoses œsophagiennes réfractaires.

Résumé: Les sténoses œsophagiennes bénignes sont fréquentes en pathologie digestive et sont à l'origine d'une dysphagie avec altération de la qualité de vie. Le traitement par dilatation endoscopique est la référence, avec souvent plusieurs sessions nécessaires de dilatation pour lever l'obstacle (trois à cinq). Les sténoses œsophagiennes réfractaires sont celles qui récidivent après cinq dilatations. La prise en charge de ces sténoses réfractaires est basée sur l'optimisation du traitement par inhibiteurs de la pompe à protons (IPP), les dilatations itératives, et des thérapies endoscopiques adjuvantes comme l'injection de corticoïdes dans la sténose pour les sténoses inflammatoires, les incisions radiaires dans les sténoses courtes ou encore la pose d'une prothèse œsophagienne couverte pendant quatre à six semaines, dont l'effet est souvent transitoire. Malgré de nombreuses études, la prise en charge des sténoses œsophagiennes réfractaires reste un problème important de prise en charge pour les praticiens et les patients. Nous faisons ici le point sur les données disponibles de la littérature pour la prise en charge des sténoses œsophagiennes réfractaires bénignes. Benign esophageal strictures are common in digestive diseases and led to dysphagia with an impaired quality of life. Treatment by endoscopic dilation is the reference, often with several sessions of dilation to remove the obstruction (three to five sessions). Refractory strictures are those that recur after five dilations. The management of these refractory strictures is based on the optimization of PPI treatment, iterative dilations, and adjunctive endoscopic therapies such as corticosteroid injections into the stricture for inflammatory strictures, radial incisions in short strictures, or covered esophageal prosthesis placement for four to six weeks. Despite numerous studies, the management of refractory stricture remains an important issue for physicians and patients. Résumé: Les endoprothèses digestives œsophagiennes, gastroduodénales et colorectales sont couramment utilisées dans la prise en charge des patients présentant une sténose maligne obstruant la lumière digestive de façon symptomatique. Dans les situations palliatives les plus communes de sténose tumorale symptomatique œsophagienne, gastroduodénale ou colique, la faisabilité, la faible morbidité et les bons résultats symptomatiques à court terme de ces dispositifs médicaux ne sont plus à démontrer. Les endoprothèses digestives sont clairement indiquées dans les situations palliatives où l'objectif principal est d'améliorer la qualité de vie, mais ils ne sont généralement pas recommandés dans les stratégies à visée curative des cancers obstructifs du tube digestif. Cette mise au point a pour but d'exposer les indications de prothèses digestives dans le traitement palliatif des cancers du tube digestif. Esophageal, gastroduodenal and colorectal stents are commonly used in the management of patients with malignant stenosis obstructing the digestive system. In palliative situations of symptomatic esophageal, gastroduodenal or malignant colonic symptomatic stenosis, the feasibility, low morbidity and good short-term symptomatic results of these medical devices are now well established. Gastrointestinal stents are clearly indicated in palliative situations where the main objective is to improve the quality of life of the patients but they are not generally recommended in the curative strategies of obstructive cancers of the digestive tract. Résumé: La dénutrition est un problème fréquent chez les patients atteints de maladies inflammatoires chroniques de l'intestin (MICI). Celle-ci est souvent associée à la sarcopénie, définie comme une réduction de la masse et de la fonction musculaire. La sarcopénie est peu évaluée dans les MICI mais est probablement fréquente, avec une prévalence estimée à 52 % et 37 % pour la maladie de Crohn et la rectocolite hémorragique respectivement. Elle est indispensable à évaluer dans la prise en charge des MICI, notamment pour son impact en termes de qualité de vie et de morbi-mortalité. L'objectif de cette revue est de faire un état des lieux de la sarcopénie dans les MICI, de comprendre les mécanismes conduisant à la sarcopénie, d'évaluer son impact pronostic et de définir une prise en charge adaptée. Undernutrition is a common problem in patients with chronic inflammatory bowel disease (IBD). It is often associated with sarcopenia, defined as a reduction in muscle mass and function. Sarcopenia is poorly evaluated in IBD but is probably frequent, with an estimated prevalence of 52% and 37% for Crohn's disease and ulcerative colitis respectively. It is essential to evaluate it in the management of IBD, especially for its impact in terms of quality of life, morbidity and mortality. The objective of this review is to assess the current state of sarcopenia in IBD, to understand the mechanisms leading to sarcopenia, to evaluate its prognostic impact and to define an adapted management. Résumé: Le cannabis, drogue illicite la plus consommée en France, affecte de nombreux processus hépato-gastro-intestinaux en raison de ses actions sur le système endocannabinoïde. Celui-ci est composé des cannabinoïdes, substances issues de la plante Cannabis sativa , dont les trois principales sont le tétracannabinol (THC), le cannabidiol (CBD) et le cannabinol (CBN) ; et de deux principaux récepteurs : le CB1 et le CB2. Le récepteur CB1 pourrait avoir un rôle pro-fibrogénique et le récepteur CB2 pourrait avoir des effets anti-fibrogéniques. Le CBD, qui n'est lié à aucun des deux récepteurs, pourrait également avoir un effet sur la fibrose hépatique et également sur la stéatose hépatique en modulant les réponses inflammatoires et les réactions oxydatives. Réduire ou supprimer l'accumulation de la fibrose et de la stéatose hépatique représente un objectif important de la prise en charge des maladies chroniques du foie, quand nous savons que l'hépatopathie stéatosique métabolique est la première cause d'hépatopathie chronique dans le monde et qu'il n'existe aucun traitement curatif pour ralentir/supprimer la fibrose hépatique et la stéatose hépatique. Par ailleurs, avec la légalisation du cannabis dans de nombreux pays, il est important de discuter de la place du cannabis chez les personnes en attente de transplantation hépatique et également les effets secondaires du cannabis en post-greffe hépatique. Cannabis, the most widely consumed illicit drug in France, affects many hepato-gastrointestinal processes through its actions on the endocannabinoid system. This system is composed of cannabinoids, substances from the Cannabis Sativa plant, the three main ones being tetracannabinol (THC), cannabidiol (CBD) and cannabinol (CBN), and two main receptors: CB1 and CB2. The CB1 receptor may have a pro-fibrogenic role and the CB2 receptor may have anti-fibrogenic effects. CBD, which is not bound to either receptor, could also have an effect on liver fibrosis and also on hepatic steatosis by modulating inflammatory and oxidative responses. Reducing or suppressing the accumulation of hepatic fibrosis and steatosis is an important goal in the management of chronic liver disease, as we know that metabolic steatotic liver disease is the leading cause of chronic liver disease worldwide and that there is currently no curative treatment available to slow/suppress hepatic fibrosis and steatosis. Furthermore, with the legalisation of cannabis in many countries, it is important to discuss the place of cannabis in people awaiting liver transplantation and also the side effects of cannabis in post liver transplantation. 19. Polyposes digestives et syndromes de prédisposition au cancer colorectal. Quelle prise en charge en 2021 ?

Résumé: La grande majorité des cancers colorectaux (CCR) se développent dans un contexte sporadique. Cependant, on estime qu'environ 35 % des CCR ont une composante familiale et que 2 à 5 % surviennent dans le cadre d'une prédisposition génétique. Parmi cette population à très haut risque de CCR, on distingue deux profils de prédisposition. Les syndromes dits « polyposiques » et ceux dits « non polyposiques », essentiellement représentés par le syndrome de Lynch. La particularité de ces syndromes est de présenter un spectre carcinologique large, touchant à la fois le côlon mais aussi d'autres organes digestifs ou extra-digestifs (endomètre, ovaire...). L'identification et la prise en charge de ces syndromes nécessite une approche multidisciplinaire, dont la pierre angulaire repose sur la consultation d'oncogénétique accessible via l'un des 17 réseaux régionaux ou inter-régionaux du dispositif d'oncogénétique (INCa 2012). Ces réseaux permettent d'organiser et de coordonner le suivi, en élaborant des plans personnalisés de suivi (PPS) propres à chaque syndrome. L'endoscopie digestive y joue un rôle central en permettant d'assurer à la fois un rôle de dépistage et de prévention des cancers. Dans cette mise au point, nous aborderons les dernières recommandations de prise en charge, en détaillant les algorithmes et les critères de qualité syndrome par syndrome. Most of colorectal cancers (CRC) develop in a sporadic context. However, it is estimated that around 35% of CRCs have a familial component and that 2 to 5% occur as part of a genetic predisposition. Among this population, we can distinguish between two phenoptypes: The so-called "polyposis" syndromes and those called "non-polyposis" syndromes, mainly represented by the Lynch syndrome. The particularity of these syndromes is that they present a broad carcinological spectrum, affecting both the colon but also other digestive or extra digestive organs (endometrium, ovary, etc.). The identification and management of these syndromes requires a multidisciplinary approach, the cornerstone of which is based on consultation of oncogenetics accessible via one of the 17 regional or inter-regional networks of the oncogenetics system (INCa 2012). These networks make it possible to organize and coordinate manadgment, by developing specific personalized follow-up plans (PPS). Digestive endoscopy plays a central role in this, ensuring both a cancer screening and prevention role. In this update, we will discuss the latest management recommendations, syndrome by syndrome. 20. Reflux gastro-œsophagien : que faire en cas d'échec des inhibiteurs de la pompe à protons ?

Résumé: Si les inhibiteurs de la pompe à protons représentent le traitement empirique de première intention en cas de symptômes évocateurs de reflux gastro-œsophagien, 30 à 40 % des patients rapportent un échec symptomatique total ou partiel du traitement antisécrétoire. Après vérification de l'observance thérapeutique et optimisation du traitement médical, il est essentiel de pratiquer des explorations endoscopiques et fonctionnelles dont le but est : 1) de démontrer la présence d'un RGO pathologique en l'absence de tout traitement, et 2) en cas de RGO avéré, de prouver que les symptômes persistants sont bien en rapport avec un RGO mal contrôlé par le traitement médical (définition du RGO réfractaire). Les critères diagnostiques du RGO et du RGO réfractaire ont fait l'objet de plusieurs consensus d'experts internationaux. Un authentique RGO réfractaire va relever essentiellement d'une prise en charge chirurgicale (fundoplicature), en l'absence d'alternative médicale ou instrumentale actuellement disponible en France. En l'absence de RGO pathologique démontré, une prise en charge spécifique sera proposée en cas de symptômes atypiques/extradigestifs, alors qu'en cas de pyrosis ou douleurs thoraciques fonctionnels les traitements modulateurs de la sensibilité viscérale doivent être privilégiés. If PPIs represent the first line therapy of gastro-esophageal reflux symptoms, 30 to 40% of patients fail to achieve adequate symptom relief. Physicians have to check for treatment compliance, optimize anti-secretory therapy, and then perform endoscopic and functional investigations to: 1) demonstrate the presence of baseline GERD off therapy, and 2) in case of proven GERD, demonstrate that persisting symptoms are related to ongoing pathological GERD on therapy (refractory GERD). Diagnostic criteria of GERD and refractory GERD have been proposed by international experts consensus. A patient with true refractory GERD should be proposed a surgical approach (fundoplication) since medical and endoscopic alternatives are scarce, at least in France. If no GERD is present, atypical/extra-esophageal symptoms should be referred for specialized investigations. 21. Traitement du reflux gastro-œsophagien par les inhibiteurs de la pompe à protons : les recommandations et la pratique.

Résumé: Les inhibiteurs de la pompe à protons (IPP) sont le traitement de référence du reflux gastro-œsophagien (RGO). Ils sont indiqués pour la cicatrisation de l'œsophagite, pour la prévention des récidives et pour le soulagement des symptômes. La posologie varie en fonction de l'indication. Le traitement est instauré sans examen complémentaire préalable en cas de symptômes typiques de reflux sans signe d'alarme. Au contraire, des explorations complémentaires sont nécessaires avant la mise en route d'un traitement par IPP, en cas de signes d'alarme ou de symptômes atypiques pour confirmer le diagnostic de RGO. Le test thérapeutique aux IPP n'est pas recommandé. En cas de traitement au long cours, l'indication doit être réévaluée périodiquement et la posologie minimale efficace cherchée. Il n'y a pas d'argument pour privilégier un IPP plutôt qu'un autre. Dans certains cas bien sélectionnés, une augmentation de la posologie (hors autorisation de mise sur le marché) peut se discuter. Proton pump inhibitors (PPI) are the cornerstone of gastro-esophageal reflux disease (GERD) treatment. Their indications are esophagitis healing, recurrence prevention and symptom relief. Treatment dose and duration are dependent on the indication. PPI are prescribed as first line treatment without previous examination in patients with typical GERD symptoms without warning signs. Complementary examinations are required in patients with warning signs or atypical symptoms to confirm diagnosis before PPI treatment. PPI therapeutic test is not recommended. In case of long-term treatment, the indication should be periodically re-evaluated and the minimal efficient dose should be prescribed. There is no argument to choose one PPI rather an other one. Résumé: La maladie hémorroïdaire est une maladie fréquente ayant une expression clinique variée. Ses symptômes ne sont pas spécifiques et l'examen clinique est essentiel afin d'éliminer d'autres diagnostics différentiels. De plus, il n'existe pas de parallélisme entre l'importance de la maladie anatomique et les symptômes hémorroïdaires. Plusieurs classifications ont été proposées afin d'évaluer la sévérité de la maladie hémorroïdaire, guider le traitement et apprécier son efficacité. La plus connue et utilisée est celle de Goligher mais ne permet d'évaluer que le prolapsus. Plusieurs autres scores plus composites prenant en compte la maladie hémorroïdaire dans sa globalité (saignement, douleur, qualité de vie, etc.) sont récemment disponibles mais leur utilisation reste limitée au quotidien. De ce fait, la classification idéale qui vise à guider de manière précise le traitement n'existe pas encore. Haemorrhoidal disease is a very frequent pathology with a varied clinical presentation. Its symptoms are not specific and the clinical exam is essential in order to rule out other differential diagnoses. Moreover, there is no parallelism between the importance of the anatomical pathology and the haemorrhoidal symptoms. Several classifications have been proposed to evaluate the severity of the haemorrhoidal disease, guide its treatment and evaluate its efficacy. The most commonly used is the Goligher's Classification which only evaluates prolapse. Other scores are more composite and take in consideration this disease in its globality (bleeding, pain, quality of life, etc.). They were recently available and their use remains limited. Résumé: L'incontinence anale (IA) est un enjeu de santé publique du fait de sa prévalence élevée, entre 5 % et 17 % selon la définition-même de l'incontinence, de l'impact significatif sur la qualité de vie et du coût économique élevé de sa prise en charge. Les troubles du transit sont des facteurs de risque majeurs d'IA dont la cause est le plus souvent multifactorielle. Ainsi, la prise en charge des patients doit être globale et pluridisciplinaire. Elle repose en premier lieu sur la correction des troubles du transit et sur la rééducation ano-périnéale. En cas d'échec de ce traitement de première ligne, des examens complémentaires sont demandés afin d'orienter la prise en charge. Le traitement de deuxième ligne repose surtout sur la correction chirurgicale d'un trouble de la statique rectale, la réparation sphinctérienne en cas de lésion du sphincter anal externe et plus rarement sur les techniques d'irrigation colique. La place de la réparation sphinctérienne est le plus souvent limitée aux déchirures périnéales obstétricales de haut grade. Elle tend à être remplacée par la neuromodulation des racines sacrées qui a révolutionné depuis une dizaine d'années la prise en charge des patients souffrant d'IA sévère. Ses indications ne cessent de s'élargir. Toutefois, on estime que seul un patient sur deux éligible au traitement en bénéficie à long terme. Ce qui soulève le problème de la prise en charge des patients en échec, pour lesquels le dernier recours est la colostomie. D'autres traitements sont à l'étude. Il s'agit principalement de la radiofréquence anale, des agents de comblements ou encore des cellules souches autologues. Les premiers résultats, encourageants, doivent être confirmés avant de pouvoir recommander ces nouveaux traitements. Anal incontinence (IA) is a public health issue because of its high prevalence, between 5 and 17% according to the definition of incontinence, the significant impact on the quality of life and the high economic cost of treatments. Transit disorders are major risk factors for AI whose etiology is most often multifactorial. The management of AI must be global and multidisciplinary. It is based primarily on the regulation of transit and stool consistency and perineal rehabilitation. In case of failure of first line treatment, further explorations will be requested to guide second line treatment. Second-line treatment is mainly based on the surgical correction of a rectal static disorder, sphincter repair in case of external anal sphincter injury and more rarely on colonic irrigation techniques. Sphincter repair is limited to high-grade perineal tears after childbirth. It tends to be replaced by sacral nerve stimulation, which has revolutionized the management of patients with severe AI. There are various indications for sacral nerve modulation. However, it is estimated that only 1 out of 2 patients eligible for treatment benefits in the long term. Now, question is how to treat patients who failed, for whom currently the last hope is colostomy. Other treatments are under study. It is mainly anal radiofrequency, bulking agents and autologous stem cells. The first positive results must be confirmed before being able to recommend these treatments. [ABSTRACT FROM AUTHOR]
