Abstract

G A A b st ra ct s enhanced MEK/ERK activation coincided with those that inhibited mTORC2-mediated Akt phosphorylation on Ser473, suggesting a role of mTORC2. Knockdown of Rictor markedly increased baseline levels of ERK phosphorylation and treatment with NVP-BEZ235 did not produce further enhancement of ERK activation. These results imply that Rictor or mTORC2 exerts feedback inhibition of the MEK/ERK pathway in pancreatic cancer cells. Conclusion: We propose that dual PI3K/mTOR inhibitors suppress a novel negative feedback loop mediated by mTORC2 thereby leading to enhanced MEK/ERK pathway activity in pancreatic cancer cells.

Abstract

Introduction: Upper gastrointestinal (UGI) surgery is by definition high risk and frequently performed in malnourished patients with a significant incidence of sarcopenia (age-associated loss of skeletal muscle mass and function). Nutritional support is an integral component of enhanced recovery protocols (ERP) to mitigate post-operative risk, and the aim of this study was to investigate the prognostic significance of sarcopenia as defined by CT density of muscle mass in UGI cancer. Methods: One hundred and seven consecutive patients undergoing surgery for UGI cancer [median age 66 yr, 79 m, 55 esophageal (46 ACA, 9 SCC), 52 gastric cancer] were studied prospectively. The axial CT slice at the upper border of the fourth lumbar vertebra was identified and a region of interest was drawn around each psoas muscle. The mean psoas muscle CT density (PMD) was calculated within the region in Hounsfield units (HU) by the Agfa PACS system, and the greater of the two density measurements was used for analysis. Abstract

G A A b st ra ct s operating curve (AUROC) for the auto-fluorescence index ratio (AFI ratio) was 0.881 (0.810.95) and 0.91 (0.84-0.97) when utilizing the two different normal AFI areas respectively for calculating the index. The two separate readings made were highly correlated (Spearman's rho 0.82). Using a AFI ratio cut off of 1.32 the sensitivities for reading 1 and 2 were 92% (82-99) and 95% (85-99) respectively, specificities were 76% (67-80) and 62% (54-65) and likelihood ratio of a positive test was 3.8 (2.5-4.6) and 2.5 (1.9-2.8). None of the cancers and only 3 and 2 HGD's respectively were misclassified by the AFI ratio. Conclusions: The AFI ratio shows promise as a tool to improve the diagnostic capabilities of AFI endoscopy. Further prospective studies with real time determination of the ratio during video AFI endoscopy will serve to confirm its utility in clinical practice. Reference: 1] Curvers WL. Gut 2008;57:167-172
