Many middle-aged and older adults participate in running to maintain their health and fitness; however, some have to stop running due to osteoarthritis-attributed knee pain. It was unclear whether gait biomechanics and knee physical findings differ between those who can and cannot run. What are the gait and knee physical findings of patients with knee osteoarthritis who remain capable of running in comparison to those who are not capable of running? This was a cross-sectional study, which recruited 23 patients over the age of 40 who had been diagnosed with knee osteoarthritis. Their knee joint ranges of motion and muscle strength, knee pain, and the maximum gait speed (walk as fast as possible) were measured. Knee alignment was calculated from X-ray images, and the knee joint extension angle and adduction moment during a self-selected gait speed were determined using motion analysis. Participants were divided into two groups—those able to run (n=11) and those unable to run (n=12). The measured and calculated outcomes were compared between groups, and logistic regression analyses of significantly different outcomes were performed. There were significant group differences in the maximum knee extension angle during stance phase (p = 0.027), maximum gait speed during the 10-m walk test (p = 0.014), knee pain during gait (p = 0.039) and medial proximal tibial angle by X-ray (p = 0.035). Logistic regression analyses revealed that the maximum knee extension angle during stance phase (OR: 1.44, 95%CI: 1.06¬1.94, p = 0.02) was a significant factor. Although changes in the gait biomechanics during pregnancy are known to occur, it is unclear whether they return to their original state after childbirth. The objective of the current systematic review is to summarize existing literature reporting the biomechanical characteristics of gait in postpartum women and to identify knowledge gaps that calls for future investigations in this population. Relevant articles were selected using MEDLINE (PubMed), Scopus (Elsevier), Embase (Elsevier), and Web of Science (Clarivate Analytics). Articles were included if they met the following criteria: They must (1) include postpartum women, (2) report measures related to three-dimensional (3D) kinematics and/or kinetics, (3) have been published after 1990, and (4) have been published in English. The current systematic review included a total of 14 articles, five of which compare the data from postpartum women with nulliparous women or pre-pregnancy data. We observed that gait biomechanics changes of postpartum women could persist for more than 5 years after the childbirth. In patients with medial knee osteoarthritis (OA), medial meniscus extrusion during gait is aggravated by mechanical stress, such as knee adduction moment (KAM). Conversely, the decrease in the range of knee rotation during stance phase is also one of the important issues in early knee OA, whereas the correlation between medial meniscus extrusion and knee rotation during gait are unclear. To investigate the correlation between increase in medial meniscus extrusion and range of knee rotation during gait in patients with early- and late-stage of knee OA. Forty patients with medial knee OA were enrolled and divided into early- and late-OA group by Kellgren-Lawrence grading scale. During gait tasks, the extent of medial meniscus extrusion was measured using ultrasonography and kinetic/kinematic data were measured using three-dimensional motion analysis system. The correlation between medial meniscus extrusion and the range of knee rotation or KAM were evaluated in the overall, early-, and late- OA groups. A significant negative correlation was observed between an increase in medial meniscus extrusion and range of knee rotation angle in early-OA group only. However, an increase in medial meniscus extrusion significantly correlated with the second KAM peak in the overall and early-OA groups. Decreasing an external hip adduction moment (HAM) impulse during stance is important to prevent the progression of hip osteoarthritis. A hip adduction angle (HAA) during walking influences the HAM impulse. Although a wider step-width (WS) gait is a gait modification to decrease a peak HAM, no study has reported the HAM impulse and HAA. We investigated whether the HAA influences the peak HAM and HAM impulse during WS gait. Twenty-six healthy young adults walked with normal step-width (NS) and WS comfortably. They were not instructed about hip adduction motion during gait, and the peak HAM, HAM impulse, HAA, and other gait parameters were evaluated using a 3D motion capture system. The participants were divided into two groups according to the HAA size during WS gait. The percentage reduction of HAM variables (the WS condition relative to the NS condition) and other gait parameters were compared between the groups. No difference in gait parameters was found between the groups. The percentage reduction of the HAM impulse in participants with smaller HAA was significantly higher than that in participants with larger HAA (14.5 % vs. 1.6 %, p < 0.01). Also, during normal step-width gait, the large HAA group showed a significantly larger HAA compared to the small HAA group (about 3°). Participants with smaller HAA could decrease the HAM impulse more effectively during WS gait compared with those with larger HAA. Thus, the HAA would influence the HAM impulse reduction effect on the WS gait. Background: Increased hip adduction and internal rotation can lead to excessive patellofemoral joint stress and contribute to patellofemoral pain development. The gluteus maximus acts as a hip extensor, abductor, and external rotator. Improving hip extensor use by increasing one's forward trunk lean in the sagittal plane may improve frontal and transverse plane hip kinematics during stair ascent.Research Question: Does increasing forward trunk lean during stair ascent affect peak hip adduction and internal rotation?Methods: Twenty asymptomatic females performed five stair ascent trials (96 steps/min) on an instrumented stair using their self-selected and forward trunk lean postures. Three-dimensional kinematics (200 Hz) and kinetics (2000 Hz) were recorded during the stance phase of stair ascent. Biomechanical dependent variables were calculated during the stance phase of stair ascent and included peak forward trunk lean, hip flexion, hip adduction, hip internal rotation angles, and the average hip extensor moment.Results: During the forward trunk lean condition, decreases were observed for peak hip adduction (MD = 2.8˚; 95% CI = 1.9, 3.8; p < 0.001) and peak hip internal rotation (MD = 1.1˚; 95% CI = 0.1, 2.2; p = 0.04). In contrast, increases were observed during the forward trunk lean condition for the peak forward trunk lean angle (MD = -34.7˚; 95% CI = -39.1, -30.3; p < 0.001), average hip extensor moment (MD = -0.5 N·m/kg; 95% CI = -0.5, -0.4; p < 0.001), and stance time duration (MD = -0.02 s; 95% CI = -0.04, 0.00; p = 0.017).Significance: Increasing forward trunk lean and hip extensor use during stair ascent decreased peak hip adduction and internal rotation in asymptomatic females. Background: Gait speed is recognized to correlate to knee kinematic alterations. Clinically, patients with knee diseases tend to walk slowly compared to healthy controls. Hence, gait speed may serve as a confounding factor in the kinematic characteristics of patients during gait compared to healthy controls.Research Question: Whether and how gait speed affects six degrees of freedom (6DOF) knee kinematics remains unclear. The current study was designed to explore whether and how decreased gait speeds affect 6DOF knee kinematics.Methods: Thirty subjects (15 males and 15 females) were recruited for this study. A three-dimensional gait analysis system was used to assess the 6DOF knee kinematics of subjects at gait speeds of 4.0 km/h, 3.5 km/h, 3.0 km/h, 2.5 km/h, 2.0 km/h, 1.5 km/h, and 1.0 km/h. Kinematics of gait cycle (GC) were assessed at all gait speed levels.Results: Decreased adduction angle (0.5-3.2 °, p < 0.05), increased external rotation (0.6-3.3 °, p < 0.05) and decreased flexion angle (1.5-17.4 °, p < 0.05) were found during most GC as gait speed level decreased. Greater anterior tibial translation (0.9-2.6 mm, p < 0.05), greater proximal translation (0.4-2.4 mm, p < 0.05) and decreased lateral tibial translation (0.5-3.0 mm, p < 0.05) were found during most GC as gait speed level decreased. Gender was also found to have great effects on 6DOF knee kinematics (p < 0.05). Interactions between gender and gait speed were also found (p < 0.05).Significance: Our findings suggest that additional attention should be paid when dealing with kinematic comparisons of GC between controls and patients with significantly different gait speeds or genders than controls. Kinematic alterations induced by gait speed may raise concern for patients with knee diseases who struggle to walk faster than their normal speed. Background: Radiographic factors estimate the state of the static knee joint, and it is questionable how well these parameters reflect the dynamic knee condition. The external knee adduction moment (KAM) during gait is known to be a kinetic variable contributing to osteoarthritis progression. This study aims to investigate the effects of static radiographic parameters on the dynamic KAM during gait.Methods: Overall, 123 patients (mean age, 65.7 years; standard deviation, 8.1 years; 34 men and 89 women) were included. Seven radiographic parameters including the mechanical tibiofemoral angle (mTFA), Kellgren-Lawrence grade, and ankle joint line orientation (AJLO) were measured on radiographs, and the maximum KAM and KAM-time integral in the stance phase were obtained using three-dimensional gait analysis. The correlation and multiple regression analyses were performed for identifying significant radiographic measurements associated with the KAM.Results: Most of the radiographic measurements correlated with the maximum KAM and KAM-time integral. As a result of multiple regression analysis, the mTFA (p < 0.001) and AJLO (p = 0.003) were identified as significant factors associated with the KAM-time integral (R2 = 0.450); the mTFA (p < 0.001) and AJLO (p = 0.003) were identified as a significant factor associated with the maximum KAM (R2 = 0.352) in multiple regression analysis. The discriminant validity of KAM was highest at varus 5.7 degree of the mTFA and 7.5 degree of the AJLO.Significance: The mTFA and AJLO were significantly associated with the KAM. However, to be used as a surgical indication for corrective osteotomy, a longitudinal study is needed to validate whether the mTFA and AJLO values directly cause osteoarthritis progression as we have suggested.Level Of Evidence: III. Background: It is common practice to align transfemoral prosthetic sockets in adduction, due to the physiologic, adducted femoral alignment in unimpaired legs. An adducted femoral and socket alignment helps tightening hip abductors to stabilize the pelvis and reduce pelvic and trunk related compensatory movements.Research Question: How do different socket adduction conditions (SAC) of transfemoral sockets affect pelvic and trunk stabilization during level ground walking in the frontal plane?Methods: Seven persons with transfemoral amputation with medium residual limb length participated in this study. The prosthetic alignment in the sagittal plane was performed according to established recommendations. SAC varied (0°, 3°, 6°, 9°). Kinematic and kinetic parameters were recorded in a gait laboratory with a 12-camera optoelectronic system and two piezoelectric force plates embedded in a 12-m walkway. The measurements were performed during level ground walking with self-selected comfortable gait speed.Results: In the frontal plane, nearly all investigated kinematic and kinetic parameters showed a strong correlation with the SAC. The pelvis was raised on the contralateral side throughout the gait cycle with increasing SAC. During the prosthetic side stance phase, the mean shoulder obliquity and mean lateral trunk lean to the prosthetic side tended to be reduced with increased SAC. Prosthetic side hip abduction moment decreased with increasing SAC.Significance: The results confirm that transfemoral SAC contributes to pelvic stabilization and reduced compensatory movements of the pelvis and trunk. Transfemoral SAC of 6 ± 1° for bench alignment seems adequate for amputees with medium residual limb length. However, the optimum value for the individual patient may differ slightly. Background: Patients with hip osteoarthritis (OA) exhibit an increased step width (SW) during walking before and up to 2 years after total hip arthroplasty. Wider SW is associated with a reduction in the external knee adduction moment (KAM), but there is a lack of research regarding the effect of SW on the hip adduction moment (HAM).Research Question: Is a wider SW an effective compensatory mechanism to reduce the hip joint loading? We hypothesized that (1) an increased SW reduces, (2) a decreased SW increases the KAM/HAM, and (3) secondary kinematic gait changes have an effect on the KAM/HAM.Methods: Twenty healthy individuals (24.0 ± 2.5 years of age) underwent instrumented gait analyses with 4 different subject-specific SW modifications (habitual, halved, double, and triple SW). The resulting external KAMs and HAMs were compared using statistical parametric mapping (SPM).Results: Post hoc testing demonstrated significantly lower HAM for both the double (p < 0.001, 15-31 % and 61-98 % of the stance phase) and the triple SW (p < 0.001, 1-36 % and 58-98 %) compared to the habitual SW. The extent of the reduction at the first and second peak was comparable for HAM (15-25 % reduction) and less pronounced at the first peak of KAM (9-11 % reduction) compared to the second peak of KAM (19-28 % reduction). In contrast, halving the SW did not lead to a significant change in KAM or HAM compared to the habitual SW (p > 0.009).Significance: An increase in SW is an effective and simple gait mechanism to reduce the frontal plane knee and hip joint moments. However, hypothesis 2 could not be confirmed, as halving the SW did not cause a significant change in KAM or HAM. Background: Patellofemoral pain (PFP) is the most common running-related injury. Altered hip and knee kinematics and increases in weekly distance and running pace are often associated with PFP development and exacerbation.Research Question: Are altered movements and training load characteristics (weekly distance and running pace) relate to pain intensity or physical function level in runners with PFP?Methods: Forty recreational runners with PFP (20 males and 20 females) participated in this cross-sectional observational study. Three-dimensional hip and knee kinematics were quantified during the stance phase of running. Weekly distance was defined as the average weekly kilometers of running and running pace as the average pace of the activity measured as minutes per kilometer. A visual analogue scale was used to evaluate worst knee pain during the last week. The anterior knee pain scale (AKPS) was used to evaluate knee functional score. A Pearson correlation matrix was used to investigate the association between each dependent variable (worst pain in the last week and AKPS score) and the independent variables (knee and hip kinematics, weekly distance and running pace).Results: There was no significantly correlation between kinematic variables, pain and functional score for both males and females separately and combined. Weekly distance (km/week) was found to positively correlate to pain intensity (r = 0.452; p < 0.05) in females with PFP. A simple linear regression revealed that weekly distance was significant predictor emerged of pain in females with PFP. Females exhibited significantly greater peak hip adduction and hip adduction ROM than the males and males had significantly greater running pace compared to females.Significance: Weekly distance should be considered in the clinical context during rehabilitation of PFP in females runners aiming at pain reduction. Background: Soldiers that suffer a service-related knee musculoskeletal injury routinely develop joint osteoarthritis. Knee osteoarthritis is a substantial and costly problem among soldiers, yet it is unknown how body borne load and duration of walking impact knee adduction biomechanics linked to progression and severity of osteoarthritis.Research Question: This study determined the adaptations in magnitude and variability of knee adduction joint angle (KAA) and moment (KAM) during prolonged walking with body borne load.Methods: Thirteen recreationally active participants had knee biomechanics quantified while walking over-ground for 60-min at 1.3 m/s with three body borne loads (0, 15, and 30 kg). Magnitude and variability of KAA and KAM measures were quantified and submitted to a RM ANOVA to test the main effect and interactions between load (0, 15 and 30 kg) and time (0, 15, 30, 45 and 60 min).Results: Body borne load increased peak KAM (p < 0.001), whereas time increased peak and range of KAA (both: p < 0.001). Specifically, peak KAM increased with each addition of body borne load (all: p < 0.025), and peak and range of KAA increased after 30 min of walking (both: p < 0.040). Neither body borne load, nor time had a significant effect on KAA or KAM variability (both: p > 0.05).Significance: Prolonged walking with heavy body borne load increased knee adduction biomechanics related to osteoarthritis. Adding heavy body borne load increased in peak KAM whereas duration of walking increased KAA, knee biomechanics that may increase loading of the medial knee joint compartment and risk of OA at the joint. Background: Studies have found that toe-in gait reduced the peak knee adduction moment (KAM) during early stance, while toe-out gait reduced the peak KAM during late stance. However, some other studies found that toe-in or toe-out gait could reduce the KAM throughout stance phase. There is still a divergence of opinion on the use of toe-in or toe-out gait for reducing the KAM.Research Question: This study aimed to investigate whether static foot posture affected participants' biomechanical responses to three self-selected foot progression angles (FPA): neutral, toe-out and toe-in.Methods: Twenty-seven healthy participants were recruited for this FPA gait modification experiment and classified into three groups: neutral (n = 8), supination (n = 9) and pronation (n = 10), based on the Foot Posture Index (FPI). The kinematic and kinetic data were recorded with Vicon motion capture system and three force plates. The knee adduction moment and ankle eversion moment were calculated using an inverse dynamics model. The effect of the FPA modification on the knee loading parameters was analysed by the Friedman non-parametric test.Results: The KAM results in the neutral group showed that the toe-in gait modification reduced the first peak of the KAM (KAM1), while the KAM1 was increased in the supination group. The effect of the FPA modification on the KAM1 did not reach significance in the pronation group. The toe-out gait modification reduced the second peak (KAM2) regardless of the static posture.Significance: Different static foot postures were correlated with different peak KAM during the early stance phase due to FPA modification. Background: The goal of valgus knee brace treatment is to reduce medial knee joint loading during walking, often indicated by external knee adduction moment (KAM) measures. However, existing healthy-subjects studies have been equivocal in demonstrating KAM reduction with valgus knee bracing.Research Question: What are the immediate effects of valgus bracing at different tension levels on KAM during walking at a controlled speed and does body height modify the brace-KAM associations?Methods: Data from 32 knee-healthy participants were analysed in this randomized crossover trial. Participants performed walking trials at controlled speed (1.3 ± 0.065 m/s) both with and without an Ossür Unloader One® brace. During the bracing condition, valgus tension was incrementally increased, from zero tension to normal tension and to maximum tolerable tension.Results: Valgus bracing minimally increased knee flexion at heel-strike (P < 0.001) in a dose-dependent manner and minimally reduced gait velocity (∼0.015m/s) across all tension levels. Valgus bracing, overall, did not significantly reduce the various KAM measures. However, brace use at maximal tension was associated with a 0.04Nm/kg (9.2 %) increase in first peak KAM amongst participants with a body height of 1.75 m and a 0.03Nm/kg (7.6 %) decrease in first peak KAM amongst participants with a body height of 1.55 m.Significance: Valgus bracing did not reduce the various KAM measures during walking; however, body height may play a moderating role. Given knee brace sizes vary more in circumference than length, this result may be due to the ratio between effective moment arm length relative to limb length. A deeper understanding of the potential neuro-biomechanical effects of valgus knee bracing and how these effects are potentially modified by body height may be critical to the design of effective knee braces. Background: Previous studies have reported good test-retest reliability for peak knee adduction moment (KAM) during walking. However, reliability of other KAM measurements has not been established.Research Question: What is the test-retest reliability of peak KAM, KAM impulse, and KAM loading rate measurements during walking in knee-healthy individuals?Methods: Data from 32 knee-healthy participants were analysed in this test-retest reliability study. Various KAM measurements were reported for two sessions with kinematic and kinetic data obtained from a motion capture system synchronised with force plates, with a median of 1 week between sessions.Results: For all KAM measures, intra-class correlation coefficients were above 0.90 and their lower bound 95 % confidence limits exceeded 0.81. However, absolute measurement variability differed across measures, with normalized SEM (8 %-15 %), normalized MDC95 (20 %-40 %), intra-session MAD (10 %-18 %), and inter-session MAD (12 %-22 %) varying over a 2-fold range. Overall and first peak KAM, KAM impulse over 50 % stance, and KAM loading rate (15 frame window) showed ≤10 % and ≤15 % intra- and inter-session MAD, respectively.Significance: This study provided previously undefined test-retest reliability estimates for various KAM measures during walking. Background: Military personnel are required to run while carrying heavy body-borne loads, which is suggested to increase their risk of tibial stress fracture. Research has retrospectively identified biomechanical variables associated with a history of tibial stress fracture in runners, however, the effect that load carriage has on these variables remains unknown.Research Question: What are the effects of load carriage on running biomechanical variables associated with a history of tibial stress fracture?Methods: Twenty-one women ran at 3.0 m/s on an instrumented treadmill in four load carriage conditions: 0, 4.5, 11.3, and 22.7 kg. Motion capture and ground reaction force data were collected. Dependent variables included average loading rate, peak absolute free moment, peak hip adduction, peak rearfoot eversion, and stride frequency. Linear mixed models were used to asses the effect of load carriage and body mass on dependent variables.Results: A load x body mass interaction was observed for stride frequency only (p = 0.017). Stride frequency increased with load carriage of 22.7-kg, but lighter participants illustrated a greater change than heavier participants. Average loading rate (p < 0.001) and peak free moment (p = 0.015) were greater in the 22.7-kg condition, while peak rearfoot eversion (p ≤ 0.023) was greater in the 11.3- and 22.7-kg conditions, compared to the unloaded condition. Load carriage did not affect peak hip adduction (p = 0.67).Significance: Participants adapted to heavy load carriage by increasing stride frequency. This was especially evident in lighter participants who increased stride frequency to a greater extent than heavier participants. Despite this adaptation, running with load carriage of ≥11.3-kg increased variables associated with a history of tibial stress fracture, which may be indicative of elevated stress fracture risk. However, the lack of concomitant change amongst variables as a function of load carriage may highlight the difficulty in assessing injury risk from a single measure of running biomechanics. Background: Negotiating stairs is an important activity of daily living that is also associated with large loads on the knee joint. In medial compartment knee osteoarthritis, the knee adduction moment during level walking is considered a marker for disease severity. It could be argued that the discriminative capability of this parameter is even better if tested in a strenuous stair negotiation task.Research Question: What is the relation with knee osteoarthritis on the knee adduction moment during the stance phase of both stair ascent and descent in patients with and without obesity?Methods: This case control study included 22 lean controls, 16 lean knee osteoarthritis patients, and 14 obese knee osteoarthritis patients. All subjects ascended and descended a two-step staircase at a self-selected, comfortable speed. Three-dimensional motion analysis was performed to evaluate the knee adduction moment during stair negotiation.Results: Obese knee osteoarthritis patients show a prolonged stance time together with a more flattened knee adduction moment curve during stair ascent. Normalized knee adduction moment impulse, as well as the first and second peaks were not different between groups. During stair descent, a similar increase in stance time was found for both osteoarthritis groups.Significance: The absence of a significant effect of groups on the normalized knee adduction moment during stair negotiation may be explained by a lower ambulatory speed in the obese knee osteoarthritis group, that effectively lowers vertical ground reaction force. Background: In treating patients with limitations of hip motion, it is generally assumed that correction of bony morphology will provide the patient with the joint motion required to resume their activities. However, the positions of impingement and the specific excursions of joint motion required by each sport may vary. This systematic review aimed to define the envelope of active hip joint motion for participation in different sporting, recreational, and daily-living activities.Methods: The EMBASE, PubMed, Google Scholar, and Cochrane databases were searched to identify studies that reported kinematics of the hip in sporting, recreational, and daily-living activities. Inclusion criteria were (1) peer-reviewed articles reporting hip kinematics in a certain type of activity, and (2) presented in English. To synthesize the kinematic data, the peak values of kinematic components (i.e. flexion/extension, abduction/adduction, and internal/external rotation) during an activity, as well as the concurrent values in a certain phase of the activity were collected from each study.Results: A total of 67 studies met the inclusion criteria, involving 32 different types of activities. Seventeen activities required at least one component of supra-physiologic hip motion, however, there were eight different combinations of flexion/extension, abduction/adduction, and internal/external rotation observed. Specifically, three activities (sex, sitting cross-legged, and grand ecart lateral of ballet dancing) required simultaneous extreme degrees of all three components, five activities (arabesque, developpe devant right, and developpe a la seconde right of ballet dancing, picking up something, and taekwondo) required high degrees of two components, most commonly hip abduction combined with flexion or internal rotation.Significance: This review highlighted that many activities place supraphysiologic demands on hip joint motion, however, the kinematic components affected differ dramatically with the specific activity. This suggests that the demands of each patient's individual activities must be assessed before recommending or planning treatment rather than assuming that a fixed value of "normal" hip motion is applicable to all. Background: Stepping is critical for responding to perturbations, whether externally induced or self-initiated. Falls post-stroke is equally likely to happen from either mechanism. The objective of the study was, to examine lateral stepping performance during waist-pull induced reactive steps and voluntary choice reaction time steps in chronic stroke and controls.Methods: In this cross-sectional study participants with chronic stroke (N = 10) and age- and gender-matched controls (N = 10) performed reactive and voluntary lateral steps. Step initiation time, global step length, step clearance, and step velocity were calculated. Other measures for reactive step included, Balance tolerance limit (perturbation magnitude when recovery transitioned from single to multiple steps), and step type. The Community Balance & Mobility Scale, and hip abductor and adductor isokinetic asymmetry torque ratio were assessed.Results: The paretic and non-paretic leg were combined since step characteristics did not differ. Step (voluntary vs. reactive) by group (stroke vs. controls) was significant for step initiation time. The stroke group took longer initiating a voluntary step (P = 0.004). Reactive and voluntary steps were executed slower (P = 0.041), with a reduced step length (P = 0.028) by the stroke group. The stroke group had a lower balance tolerance limit (P = 0.01) and took reactive medial steps more frequently (P = 0.001). The Community Balance & Mobility Scale (P > 0.001), and hip abductor and adductor asymmetry torque ratio (P > 0.001; P = 0.015) was reduced in the stroke group.Significance: Our findings indicate individuals post-stroke are slower initiating and executing reactive and voluntary steps. Background: Osteoarthritis (OA) of the knee joint is a common disease accompanied by pain and impaired mobility. Despite some recent concerns on the lack of correlation between the medial load and the knee adduction moment (KAM), KAM is routinely considered as a surrogate measure of medial load and hence a marker where its reduction is the main focus of preventive and treatment interventions.Research Question: Determine the relative sensitivity of the tibiofemoral medial-lateral contact load partitioning to changes in the knee adduction angle (KAA) versus KAM.Methods: Using a lower extremity hybrid musculoskeletal (MS) model driven by gait kinematics and kinetics, we compute here in asymptomatic subjects the sensitivity of the knee joint biomechanical response (muscle and ligament forces) in general and medial/lateral load partitioning in particular to the relative changes in the reported KAA versus changes in reported KAM (both by one standard deviation).Results: As KAA increased (at constant KAM), so did the passive moment resistance of the knee joint which as a result and at all stance periods substantially reduced forces in lateral hamstrings while increasing those in medial hamstrings. At 25% and 75% stance as two highly loaded periods of gait, the drop in KAA (from + SD to -SD while at constant KAM) drastically reduced the medial contact force by 44% and 30% and the medial over lateral contact load and area ratios by 92% and 79% as well as 64% and 51%, respectively. In contrast, the equivalent alterations in KAM (by ± SD at constant KAA) had lower and less consistent effects (<7%) showing much smaller sensitivity to changes in KAM alone. Ligament forces altered at various stance periods with inconsistent trends; peak values of 418 N in the anterior cruciate ligament (90% carried by the posterolateral bundle) and 1056 N in the patellar tendon were computed both at 25% stance and minimum KAA.Significance: These findings indicate a poor correlation between KAM and tibiofemoral load distribution suggesting instead that KAA and knee alignment should be in focus as the primary marker of knee joint load partitioning and associated prevention and treatment interventions. Background: Pathological movement patterns are characterized by abnormal kinematics, kinetics and muscle activations that alter the distribution of muscle forces during walking.Aim: The objective of this study was to identify what compensatory strategy is evident in muscle force distribution in patients with drop-foot, in response to weakness in the dorsiflexor muscles.Methods: A sample of 10 patients with drop-foot were evaluated by a computerized gait analysis system and compared to a group of 10 healthy subjects. Muscle-actuated simulations of normal and drop-foot walking were performed using OpenSim software. A musculoskeletal model with 43 muscles acting on one lower extremity was used in order to perform the simulations. In order to evaluate the difference between muscle force curves in the healthy and the drop-foot populations, an integrals of each muscle curve were computed.Results: The group of patients with drop-foot exhibited an increased force integral for all muscle groups, except for the ankle evertors. The highest increases were observed for hip adductors (112%), hip extensors (88%), knee and hip flexors (83% and 50%, respectively) and for the plantarflexor (47%). These results were mainly influenced by the following muscles: flexor digitorum and hallucius, tibialis posterior and semitendinosus. The force integral for these muscles increased by more than 200% in the drop-foot group as compared to the control group. In addition, significant changes (>100%) were noted for the posterior thigh muscle group (semitendinosus, biceps femoris long and short head), which are responsible for bending the knee joint and straightening the hip joint.Conclusions: It was proved that the loss in muscle force in individual muscle groups of the ankle joint are compensated for by the increased force and activity in other muscles acting on this joint and another muscles in neighbouring joints. Background: Lateral wedge insoles (LWIs) are non-surgical interventions used in medial knee osteoarthritis (KOA) aiming at restoring correct joint biomechanics. However, the mechanical efficacy of LWIs, based on modulation of the external knee adduction moment, is partially proved and high variability in response to these devices was observed.Research Question: The principal aim of the study was to employ subject-specific musculoskeletal models to investigate the immediate effect of LWIs on the medial compressive force (MCF) in a population with medial KOA and varus alignment.Methods: Fifteen adults (8 healthy controls age 56±3.4, BMI 25.2±2.2, hip-knee-ankle angle -1.3±2.3; and 7 KOA participants age 62±6.6, BMI 31.7±3.9, hip-knee-ankle angle 6.3±2) were recruited. Subject-specific LWIs were designed in CAD based on shape capture of the foot and manufactured via 3D printing. The required degree of heel post was added to the orthotic shell to create insoles with 0°, 5° and 10° of lateral wedge. Gait data were collected for each condition and a musculoskeletal model implemented in the Anybody Modeling System estimated the CFs normalised per bodyweight. The effect of the LWIs with respect to the baseline on the peak and the impulse of the MCF were tested with a Wilcoxon non-parametric test for paired samples.Results: For the KOA group, LWIs did not reduce significantly the impulse and the peak of the MCF. No dose-response trend according to the degree of wedging was observed. A high inter-subject variability was found: the impulse of the MCF varied between -12%, +10%, the peak between -5%, +7%. Moreover, LWIs had no consistent effect on shifting the load from the medial to the lateral compartment.Significance: Subject-specific response to LWIs in a cohort of medial KOA patients was observed. Background: Prosthetic alignment is used to optimize prosthetic functioning and comfort. Spatio-temporal and kinematic gait parameters are generally observed to guide this process. However, they have been shown to be influenced by compensations, which reduces their sensitivity to changes in alignment. Alternatively, the use of moments working at the base of the prosthetic socket, external socket reaction moments (ESRM), has been proposed to quantify prosthetic alignment.Research Question: To investigate if a predetermined kinetic alignment criterion, 0Nm averaged over the stance phase, can be used to fine-tune prosthetic alignment.Methods: 10 transtibial amputees were included in this intervention study. Firstly, their prostheses were aligned using conventional alignment procedures. Kinetic parameters and Socket Comfort Score (SCS) were measured in this initial alignment (IA) condition. Subsequently, the coronal plane ESRM during gait was presented to the prosthetist in real time using a Gait Real-time Analysis Interactive Lab. The prosthetist iteratively adapted the prosthetic alignment towards a predetermined average ESRM during the stance phase of 0 Nm. At the Final Alignment (FA), kinetic parameters and SCS were measured again and a paired sample t-test was performed to compare ESRMs and SCSs between alignments.Results: A significant (p < 0.001) change was found in the absolute coronal plane ESRM (mean ± SD) from IA (|0.104| ± 0.058 Nm/kg) to FA (|0.012| ± 0.015 Nm/kg). In addition a significant (p < 0.001) change of the external coronal adduction knee moments was observed from IA (-0,127 ± 0.079 Nm/kg) to FA (-0.055 ± 0.089 Nm/kg), however this change was more variable among participants. On average, no significant (p = 0.37) change in the SCS was observed.Significance: While this study shows the potential of quantifying and guiding alignment with the assistance of kinetic criteria, it also suggests that a sole reliance on the ESRM as a single alignment criterion might be too simple. Background Valgus knee braces have been reported to reduce the external knee adduction moment during walking. However, mechanistic investigations into the effects of valgus bracing on medial compartment contact forces using electromyogram-driven neuromusculoskeletal models are limited. Research question What are the immediate effects of valgus bracing on medial tibiofemoral contact forces and muscular loading of the tibiofemoral joint? Methods Sixteen (9 male) healthy adults (27.7 ± 4.4 years) performed 20 over-ground walking trials at self-selected speed both with and without an Ossür Unloader One® brace. Assessment order (i.e., with or without brace) was randomised and counterbalanced to prevent order effects. While walking, three-dimensional lower-body motion, ground reaction forces, and surface electromyograms from eight lower-limb muscles were acquired. These data were used to calibrate an electromyogram-driven neuromusculoskeletal model of muscle and tibiofemoral contact forces (N), from which muscle and external load contributions (%) to those contact forces were determined. Results Although walking with the brace resulted in no significant changes in peak tibiofemoral contact forces at the group-level, individual responses were variable and non-uniform. At the group-level, wearing the brace resulted in a 2.35% (95% CI 0.46-4.24; p = 0.02) greater relative contribution of muscle to lateral compartment contact loading (54.2 ± 11.1%) compared to not wearing the brace (51.8 ± 12.1%) (p < 0.05). Average relative contributions of muscle and external loads to medial compartment loading were comparable between brace and no brace conditions (p ≥ 0.05). Significance Wearing a valgus knee brace did not immediately reduce peak tibiofemoral contact forces in healthy adults during normal walking. It appears this population may modulate muscle activation patterns to support brace-generated valgus moments, thereby maintaining normal walking knee moments and tibiofemoral contact forces. Future investigations are warranted to better understand effects of valgus knee brace in people with medial knee osteoarthritis using an electromyogram-driven neuromusculoskeletal model. Background: Individuals with unilateral lower limb loss are at increased risk for developing knee osteoarthritis in their contralateral limb. The mechanisms underlying this phenomenon are unknown, but large or unusual loads on the limb are thought to contribute to osteoarthritis development. Yet, to our knowledge, there have been no longitudinal assessments of knee joint kinetics to assist with identifying the origin or progression of such loads.Research Question: This study aimed to examine knee joint kinetics of individuals with lower limb loss as a function of time from independent ambulation.Methods: Eight male Service Members with unilateral lower limb loss (3 transfemoral/5 transtibial) completed gait analyses, walking at self-selected speed and cadence, at 0, 2, and 6 months following initial independent ambulation.Results: Although there was a significant time effect on stride length (p = 0.047), there were no pairwise differences (all p ≥ 0.152). Additionally, there was not a significant effect of time on the peak (p = 0.666), loading rate (p = 0.336), or impulse (p = 0.992) of knee adduction moment (KAM), peak knee flexion moment (KFM) (p = 0.128), or the peak (p = 0.485) or loading rate (p = 0.130) of vertical ground reaction force (VGRF).Significance: The results of the current study demonstrate that major features of knee joint loading do not change over the first 6 months of independently walking with a prosthesis. The magnitude of these loads are similar to loads observed in individuals with lower limb loss further from injury/initial ambulation, but the present results do not imply that no changes occur after 6 months. Background: Lateral wedge insoles (LWI) were proposed to treat medial knee osteoarthritis through reductions of the ambulatory knee adduction moment (KAM). Limited attention was however paid to the LWI length, resulting in unclear understanding of its effect on KAM reductions. The knee flexion moment (KFM) was also shown to be important in knee osteoarthritis, but little is known about the effect of LWI length on it.Research Question: This study aimed to compare the KAM and KFM of healthy subjects walking with four different lengths of LWI, explicitly without LWI and with LWI below the hindfoot (HF), below the hindfoot and forefoot (HF + FF) and below the hindfoot, forefoot and hallux (HF + FF + HX) segments.Methods: Nineteen healthy participants (63% male; 24 ± 3 years old) walked in an instrumented gait lab with LWI of four different lengths. Repeated one-way ANOVAs and post-hoc t-tests were used to compare knee kinetics among LWI lengths.Results: The peak value of the KAM during the first half of stance and the KAM impulse differed with respect to the LWI length (p < 0.001). A length of at least HF + FF, but not necessarily longer, was needed to decrease both KAM parameters compared to walking without LWI. The LWI length had no effect on the peak value of the KFM during the first half of stance (p = 0.86).Significance: The results in this study could contribute to better selections of LWI for medial knee osteoarthritis and suggested that the length of the LWI could be a critical factor that should be considered in future research. Background: Knee adduction moment (KAM) is often used as a surrogate marker of knee contact force (KCF) during walking. Previous studies have reported potential benefits to reduce KAM in patients with knee osteoarthritis (OA) by foot progression angle adjustment. However, KAM is an external moment and it does not consider any muscle contribution to the joint loading, which should pose a greater influence in running than walking.Research Question: This study used a computational model to compare KAM and KCF between runners with and without knee OA during running. In addition, we evaluated the KAM and KCF when runners adjusted to an out-toe running style.Methods: Kinematic, kinetic, and lower limb EMG data were collected from 9 runners with knee OA and 10 healthy counterparts. They were asked to run at their usual speed with standard shoes on an instrumented treadmill.Results: We found no significant difference in the KAM during running between OA and the healthy group (p > 0.376). However, runners with knee OA exhibited a greater total KCF than the healthy counterparts (p < 0.041). We did not observe any reduction in KAM after foot progression angle adjustment (p > 0.346). Surprisingly, an increase in the longitudinal KCF and total KCF were found with adjustment of foot progression angle (p < 0.046).Significance: Unlike the findings reported by the previous walking trials, our findings do not support the notion that foot progression angle adjustment would lead to a lower joint loading during running. Background: Rigid attachment systems are one of the methods used to compensate for soft tissue artifact (STA) inherent in joint motion analyses.Research Question: The goal of this study was to quantify STA of an exoskeleton design to reduce STA at the knee, and to assess the accuracy of 3D knee kinematics recorded with the exoskeleton in non-obese and obese subjects during quasi-static weight-bearing squatting activity using biplane radiography.Methods: Nine non-obese and eight obese subjects were recruited. The exoskeleton was calibrated on each subject before they performed a quasistatic squatting activity in the EOS® imaging system. 3D models of exoskeleton markers and knee bones were reconstructed from EOS® radiographs; they served to quantify STA and to evaluate differences between the markers and bones knee kinematics during the squatting activity.Results: The results showed that STA observed at the femur was larger in non-obese subjects than in obese subjects in frontal rotation (p = 0.004), axial rotation (p = 0.000), medio-lateral displacement (p = 0.000) and antero-posterior displacement (p = 0.019), while STA observed at the tibia was lower in non-obese subjects than in obese subjects for the three rotations (p < 0.05) and medio-lateral displacement (p = 0.015). Differences between the markers and bones knee kinematics increased with knee flexion and were similar in both groups, except for abduction-adduction: 4.9° for non-obese subjects against 2.3° for obese subjects (p = 0.011).Significance: This study demonstrated that STA at the femur and its impact on knee abduction-adduction using a specific exoskeleton were greater among non-obese subjects than obese subjects, which is encouraging for future biomechanical studies on pathologies such as osteoarthritis. Background: Decreasing the daily cumulative hip moments in the frontal and sagittal planes may lower the risk of hip osteoarthritis. Therefore, it may be important to evaluate factors that affect hip moment impulse during gait.Research Question: It is unclear what factors affect hip moment impulse during gait. This systematic review aimed to evaluate different factors that affect hip moment impulse during gait in healthy adults and patients with hip osteoarthritis.Methods: Four databases (Scopus, ScienceDirect, PubMed, and PEDro) were searched up to August 2017 to identify studies that examined hip moment impulse during gait. Data extracted for analysis included the sample size, age, height, body mass, type of intervention, and main findings.Results: After screening, 10 of the 975 studies identified were included in our analysis. Several factors, including a rocker bottom shoe, FitFlop™ sandals, ankle push-off, posture, stride length, body-weight unloading, a rollator, walking poles, and a knee brace, were reviewed. The main findings were as follows: increasing ankle push-off decreased both the hip flexion and extension moment impulses; body-weight unloading decreased both the hip extension and adduction moment impulses; the FitFlop™ sandal increased the sum of the hip flexion and extension moment impulses; long strides increased the hip extension moment impulse; and the use of a knee brace increased hip flexion moment impulse. Of note, none of the eligible studies included patients with hip osteoarthritis.Significance: The hip moment impulses can be modified by person-specific factors (ankle push-off and long strides) and external factors (body-weight unloading and use of the FitFlop™ sandals and a knee brace). Objective: To test the hypothesis that toe-in gait (TI) will further reduce first peak (Knee Adduction Moment) KAM and decrease balance when combined with a knee brace (KB) and laterally wedged insoles (LWI) in medial knee osteoarthritis (kOA) patients.Participants: Twenty patients with bilateral symptomatic medial kOA.Interventions: 4-point leverage-based KB, full-length LWI with 5° inclination and toe-in gait (TI).Main Outcome Measures: First and second peak knee adduction moment (fKAM and sKAM respectively), balance and pain.Methods: The fKAM and sKAM were determined from 3-dimensional gait analysis with six randomized conditions: (1) N (without any intervention), (2) KB, (3) KB + TI, (4) LWI, (5) LWI + TI, (6) KB + LWI + TI. Balance was assessed by Biodex Balance System using three stability settings, (i) Static (ii) Moderate dynamic setting for fall risk (FR12) and (iii) High dynamic setting for fall risk (FR8).Results: The reduction in fKAM and sKAM was greatest (19.75% and 12%) when TI was combined with KB and LWI respectively. No change in balance was observed when TI combined with KB, and LWI and when used concurrently with both the orthosis at static and FR12 conditions. Significant balance reduction was found at FR8 for KB + TI (22.22%), and KB + LWI + TI (35.71%). Pain increased significantly for KB (258%), KB + TI (305%), LWI + TI (210%) and KB + LWI + TI (316%). LWI showed no effect on pain.Conclusions: There is a synergistic effect of TI when combined with KB and LWI concurrently in sKAM reduction. The aim of this study was to analyze individual muscle contributions to knee angular acceleration using a musculoskeletal simulation analysis and evaluate knee extension mechanics in the early stance phase in patients with knee osteoarthritis (OA). The subjects comprised 15 patients with medial knee OA and 14 healthy elderly individuals. All participants underwent gait performance test using 8 infrared cameras and two force plates to measure the kinetic and kinematic data. The simulation was driven by 92 Hill-type muscle-tendon units of the lower extremities and a trunk with 23° of freedom. We analyzed each muscle contribution to knee angular acceleration in the 5%-15% and 15%-25% periods of the stance phase (% SP) using an induced acceleration analysis. We compared accelerations by individual muscles between the two groups using an analysis of covariance for controlling gait speed. Patients with knee OA had a significantly lesser knee extension acceleration by the vasti muscles and higher knee acceleration by hip adductors than those in controls in 5-15% SP. In addition, knee OA resulted in significantly lesser knee extension acceleration by the vasti muscles in 15-25% SP. These results indicate that patients with knee OA have decreased dependency on the vasti muscles to control knee movements during early stance phase. In clinical gait analysis, it is challenging to acquire usable force plate data for a patient in a limited amount of time. The aim of this study was to compare three measurement protocols, to investigate if any one of them was more time-efficient than the others at collecting kinetic data. Three conditions were compared for 15 orthopaedic patients: 1) approaching the force plate with four steps, 2) approaching the force plate with six steps, and 3) approaching the force plate with four steps while stepping on a target one step before the first force plate. Then, the following characteristics were analysed: the rate of usable force plate steps, the spatio-temporal parameters, the full-body gait kinematics, and the lower body kinetics. For the condition with four steps and targeting, the rate of usable force plate steps was highest: 84% (6.8 usable trials out of 8.1 trials on average per patient). Left hip adduction and rotation, right shoulder flexion, and total left hip power were the gait parameters with statistically significant differences between the four and six step approach. Left cadence, right step time, left thorax lateroflexion, left shoulder abduction, total right knee power, hip rotation, thorax tilt, and head tilt on both sides were statistically different between the four step approach with targeting and without targeting. None of the differences in gait parameters (except for head tilt) were of clinical relevance. Therefore, approaching the force plate with four steps and stepping on a foot-sized target one step prior to stepping on the force plate increases the rate of usable kinetic data. The external knee adduction moment has been identified as a key biomarker in biomechanics research, with associations with this variable and degenerative diseases such as knee osteoarthritis. Heterogeneity in participant characteristics and the protocols used to measure this variable may however complicate its interpretation. Previous reviews have focused on interventions or did not control for potential moderator variables in their analysis. In this meta-regression analysis, we aimed to determine the influence of factors including the cohort type, footwear, and walking speed on the measurement of knee adduction moment. We performed a systematic review of the literature, identifying articles that used the Plug-in-Gait inverse dynamics model to calculate the knee adduction moment during level walking, and used a mixed effect model to determine the effect of the previously described factors on the measurement. Results for 861 individuals were described in 19 articles. Walking speed had the largest influence on knee adduction moment (p<0.001), and participants with medial knee osteoarthritis had an increased knee adduction moment (p=0.008) compared to healthy subjects. Footwear was found to have a significant overall effect (p=0.024). Participants tested barefoot or wearing their own shoes had lower adduction moments than those tested in footwear provided by the researchers. Overall, the moderators accounted for 60% of the heterogeneity in the results. These results support the hypothesis that an increased knee adduction moment is associated with medial compartment knee osteoarthritis, and that footwear choice can influence the results. Lateral wedge insoles (LWIs) are prescribed for patients with medial knee osteoarthritis to reduce the external knee adduction moment (KAM). However, the biomechanical effects of LWIs are limited in some patients. The purpose of this study was to investigate whether the biomechanical effects of LWIs depend on individual foot alignment and to examine the relationship between change in KAM and changes in foot and ankle biomechanics when wearing LWIs. Twenty-one patients participated in this study. They were categorized into normal or abnormal foot groups based on the foot posture index (FPI). All patients were requested to perform a normal gait under barefoot and LWI conditions. A three-dimensional motion analysis system was used to record 1st and 2nd KAM, knee adduction angular impulse (KAAI), center of pressure displacement, and knee-ground reaction force lever arm. Furthermore, the foot and ankle frontal plane kinematic parameters were evaluated. The 1st KAM was significantly reduced under the LWI condition compared to that under the barefoot condition in the normal foot group. In contrast, there was no significant difference in 1st KAM between both conditions in the abnormal foot group. Decreased rear foot eversion strongly correlated with reduction in the 1st KAM in the normal foot group. These findings suggested that it is helpful to assess individual foot alignment to ensure adequate insole treatment for patients with medial knee osteoarthritis and that decreased rear foot eversion during the early stance phase is significantly involved in the reduction of 1st KAM when wearing LWIs with normal feet. Background: Dynamic and static varus alignment, both, have been reported as risk factors associated with structural progression of knee osteoarthritis. However the association of none of the static and dynamic alignment with structural, clinical, and functional progression associated with knee osteoarthritis has not been assessed yet in a longitudinal study.Methods: Forty-seven women with early and established medial knee osteoarthritis were evaluated. Static and dynamic alignment as well as MRI detected structural features, clinical, and functional characteristics of patients were assessed at baseline and at 2 years follow-up. Associations between baseline static and dynamic alignment with structural, functional, and clinical characteristics at the time of entry, as well as the changes over 2 years were evaluated.Findings: Both static and dynamic varus alignment at baseline were significantly associated with osteoarthritis related tibio-femoral joint structural abnormalities detected on MRI, at the time of entry. Only the magnitude of varus thrust at baseline was predictive of the changes in the presence of meniscal maceration over two years. None of the static or dynamic measures of knee joint alignment were associated with clinical characteristics associated with medial knee osteoarthritis.Interpretation: The key finding of this study is that both frontal plane dynamic and static alignment, are associated with structural abnormalities in patients with medial knee osteoarthritis. We conducted a meta-analysis to analyze how high tibial osteotomy (HTO) changes gait and focused on the following questions: (1) How does HTO change basic gait variables? (2) How does HTO change the gait variables in the knee joint? Twelve articles were included in the final analysis. A total of 383 knees was evaluated. There were 237 open wedge (OW) and 143 closed wedge (CW) HTOs. There were 4 level II studies and 8 level III studies. All studies included gait analysis and compared pre- and postoperative values. One study compared CWHTO and unicompartmental knee arthroplasty (UKA), and another study compared CWHTO and OWHTO. Five studies compared gait variables with those of healthy controls. One study compared operated limb gait variables with those in the non-operated limb. Gait speed, stride length, knee adduction moment, and lateral thrust were major variables assessed in 2 or more studies. Walking speed increased and stride length was increased or similar after HTO compared to the preoperative value in basic gait variables. Knee adduction moment and lateral thrust were decreased after HTO compared to the preoperative knee joint gait variables. Change in co-contraction of the medial side muscle after surgery differed depending on the degree of frontal plane alignment. The relationship between change in knee adduction moment and change in mechanical axis angle was controversial. Based on our systematic review and meta-analysis, walking speed and stride length increased after HTO. This study investigated in three-dimensional space, firstly whether the aquatic medium and secondly ageing, had any effect on the lower limb's joint angles during aquatic-based gait. Three-dimensional joint kinematics of the lower limb of 51 healthy male participants [25 young group (24.6±4.9 years, 172.1±5.5cm, 69.8±10.3kg) and 26 older group (58.5±5.1 years, 167.9±5.1cm, 70.8±12.1kg)] were quantified during land and shallow water walking. Participants walked at their self-selected comfortable speed in both mediums. The results suggested that the properties of water - hydrodynamic drag, and buoyancy - affected the gait kinematics for both groups. Both age groups used more of their hip flexion in the aquatic environment to help them propel forward instead of using the ankle plantarflexion. The effect of age during the aquatic-based gait was identified in ankle adduction angle and knee abduction/adduction angle at initial contact. Only the older group elicited a significantly smaller ankle adduction angle during the aquatic-based gait when compared to the land-based gait. Only the young group elicited a significantly larger knee abduction/adduction angle at initial contact during the aquatic-based gait when compared to the land-based gait. These findings can facilitate professionals in the area of aquatic rehabilitation to better customise aquatic-based walking exercise programmes to suit their client's specific needs. The relationship between age and gait characteristics in people with and without medial compartment osteoarthritis (OA) remains unclear. We aimed to characterize this relationship and to relate biomechanical and structural parameters in a subset of OA patients. Twenty five participants with diagnosed unilateral medial knee OA and 84 healthy participants, with no known knee pathology were recruited. 3D motion capture was used to analyse sagittal and coronal plane gait parameters while participants walked at a comfortable speed. Participants were categorized according to age (18-30, 31-59 and 60+ years), and those with and without OA were compared between and within age groups. In a subset of OA patients, clinically available Computed Tomography images were used to assess joint structure. Differences in coronal plane kinematics at the hip and knee were noted in participants with OA particularly those who were older compared with our healthy controls, as well as increased knee moments. Knee adduction moment correlated with structural parameters in the subset of OA patients. Increased knee moments and altered kinematics were observed in older participants presenting with OA only, which seem to be related to morphological changes in the joint due to OA, as opposed to being related to the initial cause of medial knee OA. Objective: This study, firstly, investigates the effect of using an anatomical versus a functional axis of rotation (FAR) on knee adduction moment (KAM) in healthy subjects and patients with knee osteoarthritis (KOA). Secondly, this study reports KAM for models with FAR calculated using weight-bearing and non-weight-bearing motion.Design: Three musculoskeletal models were created using OpenSim with different knee axis of rotation (AR): transepicondylar axis (TEA); FAR calculated based on SARA algorithm using a weight-bearing motion (wFAR) and a non-weight-bearing motion (nwFAR). KAM were calculated during gait in fifty-nine subjects (n=20 healthy, n=16 early OA, n=23 established OA) for all models and groups.Results: Significant differences between the three groups in the first peak KAM were found when TEA was used (p=0.038). However, these differences were no longer present when using FAR. In subjects with established OA, KAMs were significantly reduced when using nwFAR compared to TEA models but also compared to wFAR models.Conclusion: The presence of excessive KAM in subjects with established KOA showed to be dependent on the definition of the AR: anatomical versus functional. Therefore, caution should be accounted when comparing KAM in different studies on KOA patients. In patients with end-stage knee OA where increased passive knee laxity is likely to exist, the use of weight-bearing motions should be considered to avoid increased variability in the location and orientation of a FAR obtained from activities with only limited joint loading. Objective: To quantify the magnitude of global rearfoot motion, in particular, rearfoot adduction and to investigate its relationship to tibial rotation.Design: One hundred and four participants ran barefoot on an Ethylene Vinyl Acetate (EVA) foam. Global range of motion values for the shank, rearfoot and medial metatarsal segment as well as foot motion within the transverse plane were determined using an optoelectric motion capture system. Relationships between parameters were assessed using partial correlation analysis.Results: Global rearfoot adduction amounts to 6.1° (±2.7). Furthermore global rearfoot adduction and rearfoot eversion were significantly related to internal tibial rotation (partial correlation: r=0.37, p<0.001 and r=-0.24, p=0.015, respectively). Furthermore, a strong relationship between rearfoot adduction and transverse within foot motion (r=-0.65, p<0.001) was found.Conclusion: Next to rearfoot eversion, rearfoot adduction may be also important to the understanding of ankle joint coupling. Objective: To determine (1) if Medial Thrust or Trunk Lean reduces the knee adduction moment (EKAM) the most during gait in patients with medial knee osteoarthritis, (2) if the best overall strategy is the most effective for each patient and (3) if these strategies affect ankle and hip kinetics.Design: Thirty patients with symptomatic medial knee osteoarthritis underwent 3-dimensional gait analysis. Participants received verbal instructions on two gait strategies (Trunk Lean and Medial Thrust) in randomized order after comfortable walking was recorded. The peaks and impulse of the EKAM and strategy-specific kinematic and kinetic variables were calculated for all conditions.Results: Early stance EKAM peak was significantly reduced during Medial Thrust (-29%). During Trunk Lean, early and late stance EKAM peak and EKAM impulse reduced significantly (38%, 21% and -25%, respectively). In 79% of the subjects, the Trunk Lean condition was significantly more effective in reducing EKAM peak than Medial Thrust. Peak ankle dorsi and plantar flexion, knee flexion and hip extension and adduction moments were not significantly increased.Conclusions: Medial Thrust and Trunk Lean reduced the EKAM during gait in patients with knee osteoarthritis. Individual selection of the most effective gait modification strategy seems vital to optimally reduce dynamic knee loading during gait. Wedged insoles are believed to be of clinical benefit to individuals with knee osteoarthritis by reducing the knee adduction moment (KAM) during gait. However, previous clinical trials have not specifically controlled for KAM reduction at baseline, thus it is unknown if reduced KAMs actually confer a clinical benefit. Forty-eight participants with medial knee osteoarthritis were randomly assigned to either a control group where no footwear intervention was given, or a wedged insole group where KAM reduction was confirmed at baseline. KAMs, Knee Injury and Osteoarthritis Outcome Score (KOOS) and Physical Activity Scale for the Elderly (PASE) scores were measured at baseline. KOOS and PASE surveys were re-administered at three months follow-up. The wedged insole group did not experience a statistically significant or clinically meaningful change in KOOS pain over three months (p=0.173). Furthermore, there was no association between change in KAM magnitude and change in KOOS pain over three months within the wedged insole group (R2=0.02, p=0.595). Improvement in KOOS pain for the wedged insole group was associated with worse baseline pain, and a change in PASE score over the three month study (R2=0.57, p=0.007). As an exploratory comparison, there was no significant difference in change in KOOS pain (p=0.49) between the insole and control group over three months. Medial opening wedge high tibial osteotomy is a surgical procedure intended to redistribute loads on the knee in patients with medial compartment knee osteoarthritis (OA). The surgery may affect moments in multiple planes during ambulation, with potential beneficial or detrimental effects on joint loads. The objective of this study was to investigate three-dimensional external knee moments before and after medial opening wedge high tibial osteotomy during level walking and during stair ascent. Fourteen patients with varus alignment and osteoarthritis primarily affecting the medial compartment of the tibiofemoral joint were assessed. Three-dimensional motion analyses during level walking and stair ascent was evaluated using inverse dynamics before, 6 and 12 months after surgery. Mean changes at 12 months suggested decreases in the peak knee adduction, flexion and internal rotation moments, with standardized response means ranging from 0.15 to 2.54. These decreases were observed despite increases in speed. Changes in alignment were associated with changes in the adduction and internal rotation moments, but not the flexion moment. Both pre- and postoperatively, the peak knee adduction moment was significantly lower ( p = 0.001) during stair ascent than during level walking, while the flexion and internal rotation moments were significantly higher ( p < 0.01). There were no changes in the knee moments on the non-surgical limb. Medial opening wedge high tibial osteotomy is associated with sustained (12 months) changes in knee moments in all three planes of motion during ambulation, suggesting substantial alterations of the loads on the knee during ambulation. External knee adduction moment has been studied as a surrogate for medial knee contact force. However, it is not known whether adduction moment is a rational measure for predicting medial knee contact force. The aim of this study was to investigate the correlation between knee adduction moment and medial knee contact force in older people, using musculo-skeletal simulation analysis. One hundred and twenty-two healthy older subjects participated in this study. Knee moment and medial knee contact force were calculated based on inverse dynamics analysis of normal walking. Muscle force and joint reaction force were used to determine the medial knee contact force during stance phase. The results showed that the maximum medial knee contact force was moderately correlated to the maximum knee adduction (r=0.59) as well as the maximum extension moment (r=0.60). The first peak of medial knee contact force had a significant strong correlation with the first peak of adduction moment and a moderate correlation with the maximum flexion moment. The second peak of medial knee contact force had a significant moderate correlation with both the second peak of adduction and the maximum extension moment. These results implied that the maximum adduction moment value could be used, to some extent, as a measure of the maximum medial knee contact force. [ABSTRACT FROM AUTHOR]
