This special section represents a collection of papers on recruitment and retention of couples from underrepresented backgrounds in couple intervention research. Research shows that couples from underrepresented backgrounds tend to be missing from intervention research. This gap is concerning; conclusions about the effectiveness of these interventions are not being drawn from diverse and representative samples and it may be that scholars are inadvertently creating inappropriate and inaccessible services for these couples. Recruiting and retaining these couples require specialized efforts and attention. In this summary paper, we describe (a) the origins of this special section, (b) the existing research on recruitment and retention in couple intervention research, (c) an overview of the papers in this special section, and (d) future recommendations and directions for this aspect of methodology in couple research. This collection of papers elevates the need to involve community members from the beginning, reduce barriers to access, and create recruitment materials and a service delivery environment that is specific for the target population. The psychotherapy field has a long history of integration to improve treatment effectiveness. One type, assimilative integration, offers innovative opportunities to family therapy to incorporate the clinical and research contributions of different approaches. This paper contributes to the literature on integration by exploring how Eye Movement Desensitization and Reprocessing (EMDR) can be assimilated into Attachment‐Based Family Therapy (ABFT) for youth in residential psychiatric treatment. ABFT seeks to improve attachment security to parental figures by asking adolescents and young adults to discuss attachment ruptures. This process, specially designed for patients with internalizing disorders, can provoke anxiety, particularly for a patient population that commonly has a history of trauma and adverse life experiences. EMDR is a first‐line therapy for post‐traumatic stress disorder and has the potential to be effective in treating various other mental health conditions. When patient emotional withdrawal or dysregulation due to breaches in trust between child and parent emerges in ABFT, EMDR may help desensitize associated memories and bolster the family therapy process. The authors provide an overview of both models, and a detailed case study as an example. The paper concludes with a discussion on implications, integration efforts, and limitations. Attachment theory and the science of emotion provide a strong foundation for intervention at the family system level. Four therapeutic models in particular, Attachment‐Based Family Therapy, Emotion‐Focused Family Therapy, Dyadic Developmental Psychotherapy, and Emotionally Focused Family Therapy, demonstrate how a broad and accurate view of attachment relationships and emotion can be utilized to effectively intervene for a variety of presenting problems in a relational and empathic way for all involved. This paper continues a conversation that began at the Summit for Attachment and Emotion in Family Therapy in 2021 and aims to foster openness, collaboration, and affirmation between four different models of family therapy with shared theoretical roots. The presenters at the Summit and the authors of this paper view similarities across these models as validating and differences as opportunities to serve more families in unique ways, learning from one another's creativity to promote healing within families in the most effective and efficient ways possible. The paper frames the value of attachment theory and emotion science for family therapy, discusses the importance of learning from a variety of models with shared theoretical roots, presents brief summaries of the four models presented at the Summit, compares the models for similarities and complementarities, and shares highlights from each of the presenters from the Summit. An editorial is presented on ethical relational human being and as an ethical professional whose work focuses on relational life. Topics include papers offering a probing discussion firmly anchored in the relevant literature about their specific focus, interwoven with their observations from clinical practice; and enormous literature on relational and systemic therapies and related methods of intervention.

This paper will explore the role of the therapist's values in couple therapy and the challenge these values pose to therapeutic neutrality. It will illuminate how the therapist's values shape what is considered healthy and unhealthy, functional and dysfunctional, and hence frame the problem the couple therapist seeks to treat. Values have particular relevance for couple treatment because the conversations the couple therapist facilitates may unwittingly privilege one partner's needs and wishes over the other's, creating the potential for therapeutic misalliances. This paper will explore common values and their impact on treatment, including the values of monogamy, gender equity, and talk intimacy (emotional expression as a path to intimacy). It will conclude by discussing how couple therapists can avoid partisanship with one partner and maintain a balanced approach in their couple work. [ABSTRACT FROM AUTHOR]

In this clinical paper, the focus is on the use of questionnaires in family therapy practice. Psychotherapy research has indicated that the quality of the therapeutic relationship is the most robust predictor of therapeutic change. While the therapeutic relationship is even more important in family therapy than in individual therapy, it is also more complex. As will be illustrated in this paper, questionnaires can be useful in clinical practice to deal with the complex alliances in family therapy. In this paper, we will introduce two questionnaires: the Worries Questionnaire and the Dialogical Feedback Questionnaire. As illustrated in detailed case stories, we use these questionnaires in our practice as conversational tools rather than measuring instruments. Research on the gendered division of household work in western political economies often neglects its emotional dimensions. This conceptual paper draws on concepts of emotion work and feminist care ethics to explore gendered and intersecting divisions of emotions and emotional work in couple relationships and their implications for couple therapy. Although emotion work has been studied in workplace settings, less attention has been paid to inequalities in the privatized realm of interpersonal relationships, including romantic and filial ones. Women and feminine partners' culturally presumed expertise in emotions typically positions them as carrying primary responsibility for managing emotions in intimate relationships. Couple therapy is an important site of interaction that can both support and, potentially, disrupt the invisibility and gendering of emotion work in intimate relationships, thus shedding light on recurring patterns of women's subordination and exploitation. A lack of diversity exists in couples research with an overrepresentation of white, middle to upper income couples. Additionally, researchers often do not reflect the study sample, particularly when studying underrepresented minority and historically marginalized (URM‐HM) populations. Emancipatory research practices focus on language, processes, and practices to ensure that researchers and the research they conduct honor and contribute to the empowerment of the URM‐HM research participants. Therefore, in this paper, we discuss five critical considerations with recommendations for emancipatory research practices for including couples from URM‐HM populations. The aim is to provide a framework for researchers to critically reflect on their work with URM‐HM populations. The research practices include: (a) research positionality and reflexivity; (b) understanding of the population being served; (c) power and empowerment; (d) accountability, voice, and participant engagement; and (e) research that benefits URM‐HM populations and challenges the systems that perpetuate inequities. Additionally, we present practical strategies to implement these five considerations based on our own experiences conducting community‐effectiveness studies with low‐income and diverse couples. Scholars have spent the last four decades expanding the theoretical understanding of parental racial socialization—or parent–child communication about race. What is largely absent from existing conceptualizations, however, is a consideration for how the practice manifests in Multiracial families. The interracial structure of Multiracial families complicates racial socialization in ways that are not being captured in empirical research due to the overreliance on universal frameworks. It is imperative that we close this theoretical gap as the proportion of Multiracial families in the United States is expanding at record rates. Accordingly, we present the Multiracial‐Black Socialization Model (MRB‐SM) in this paper. The MRB‐SM is designed explicitly for Multiracial‐Black families, one of the fastest‐growing sub‐groups of Multiracial families. The model builds upon the strengths of universal socialization theories by outlining the content, process, and context components of racial socialization in Multiracial‐Black families. Relationship interventions, including healthy relationship education, couple therapy, and dyadic approaches to treating mental and physical health issues, hold promise for promoting relationship and individual health among sexual and gender minority (SGM) populations. Because SGM couples live within a context of societal stigma against their minority identities and relationships, they are likely to be best served by targeted, culturally sensitive relationship interventions that are affirming, free of hetero‐ and cis‐normativity, and address the unique stigma‐based challenges that they face. Therefore, a key goal for the field today is to conduct research evaluating and refining newly developed relationship interventions designed specifically for SGM couples. In this paper, we offer recommendations for effectively recruiting and retaining large, diverse samples of SGM couples for clinical trials of tailored relationship interventions, grounded in guidelines for psychological practice and conducting research with SGM populations. Throughout, we offer examples and lessons learned from our experiences conducting clinical trials of tailored SGM relationship education programs. We encourage the use of recruitment and retention strategies that involve members of the target SGM community from the outset, are informed by knowledge about SGM individuals and relationships, use currently preferred language for individual identities and relationships, attend to issues of confidentiality regarding sexual/gender identity or relationship involvement, and adhere to the norms of the particular community and recruitment venue. This paper asserts that whiteness is a major ideological framework that is subtlety interwoven into our institutional structures and is a major organizing principle in the workplace. Efforts to increase racial equity, inclusion, and belongingness within the workplace and throughout society at large are ineffectual and virtually impossible without addressing the deleterious effects of whiteness. Addressing these pervasive subtleties in the workplace requires more than changing the complexion of an organization; instead, it also involves devoting ample attention to promoting widespread cultural change by being intentional about dismantling whiteness. Papers in the Special Section on Racial Disparities in Health Care stemmed from. the 60th Anniversary of Family Process Conference, The Heart of the Matter: Systemic Imperatives to Address Health Disparities and Racism in the Time of COVID, which took place in Washington, DC in September 2021. Of the 12 presenters at the conference, these four were asked to recreate their talks into articles. They address key issues that help to explain health disparities in people of color, particularly African Americans, in the United States, as well as suggest innovations to clinical interventions and health care delivery systems to better serve people who have suffered adversity from the racial inequities in the American system. This study explores the experience of promoting inclusion and anti‐racism work as either performance or deep work in the couple and family therapy (CFT) field through narrative qualitative analysis. While performance‐based approaches focus on meeting external expectations and diversity quotas, deep work involves critical self‐reflection, ongoing learning, and a commitment to addressing systemic inequalities. The paper prioritizes how deep inclusion assists with developing new approaches to creating meaningful and lasting change in teaching, research, and clinical work in the CFT field. Some professionals in the field include educators, researchers, and therapists who are engaged in anti‐racism work, a recognition of the impact of systemic racism on family dynamics and therapeutic interventions, and a commitment to centering the voices and experiences of marginalized individuals and communities. Implications present a need for ongoing education, training, and support for professionals in the CFT field and other family science and family mental health‐related professions. This study also identifies limitations and future directions for research in promoting inclusion and anti‐racism work in family science and family mental health‐related fields. It is essential to promote inclusion and anti‐racism as deep work to create more inclusive and equitable teaching, research, and therapeutic environments that value the experiences of all individuals and communities. Challenges include resistance from shaking systems and making ourselves and others vulnerable with uncomfortable and continuous conversations. Our primary goal is to contribute to and inspire dialogue about the perspectives CFTs and other mental health‐related professionals are taking in the relationship to inclusion and anti‐racism work. In response to highly publicized instances of overt racial injustice, there has been a recent resurgence of interest and commitment to identifying processes through which anti‐racist behaviors develop among White individuals. One particularly important context in which anti‐racist behaviors can develop is within families and as a result of childrearing. Theories of anti‐racism typically neglect the role of families and family science research typically neglects a focus on anti‐racist parenting outcomes. To further research and applied work on fostering anti‐racism within White families, this paper introduces a new integrative model called routes to effective anti‐racist parenting (REAP). The model draws on theories from various fields, including family science and social psychology, and uses a metaphor of nurturing a plant to explain the nuanced, multi‐faceted approaches to anti‐racist parenting. The model incorporates factors related to the "pot" (i.e., fundamental values and structure necessary to contain more specific anti‐racist skills and behaviors), "soil" (i.e., characteristics that define anti‐racist commitment), "seeds" (i.e., direct transmission of anti‐racism skills), and "environment" (i.e., influential external factors). Finally, we describe the intended benefits that can be reaped from this intentional approach to anti‐racist parenting. The REAP model contributes to the family science literature by providing an empirically grounded theoretical model describing the roles that parents can play in children's anti‐racist development. Power dynamics, generally defined as the patterns of partners enacting or resisting influence, are inherent in all relationships. Power structures and processes play a role in people's perceptions of themselves and others, their feelings and emotions, and both their implicit and explicit behaviors. As such, understanding power dynamics is crucial for fully conceptualizing and intervening within relationships. Although power was not always given high priority in many of the early systemic family therapy models, that has changed over the years, with scholars working to address how power is manifested in relationships, how power imbalances affect relationships, and how power can be addressed more explicitly in treatment. Nonetheless, there is much additional work needed to ensure that systemic therapists have an appropriate depth of understanding regarding power dynamics to fully recognize their manifestations in relationships and then intervene appropriately. To help in these efforts, this paper aims to synthesize relational power research into a more complete description of what power is and how it is enacted in couple relationships. To do this, we introduce relevant perspectives of power not fully integrated with family therapy theories. Overall, we provide a brief history of power‐oriented research in the fields of family therapy, outline couple research regarding the sources of power that can inform therapeutic case conceptualizations and interventions, describe how power is addressed in specific couple therapy models, and highlight some important clinical applications that can help systemic therapists more fully address power. Distressed couples often become polarized and caught up in power struggles, with competing claims and perspectives. When escalated, partners may become reactive and unkind. The competitive‐individualistic worldview of the Euro‐American culture feeds polarization between partners. This article explores relational views of the self and relational ethics developed in philosophy, psychology, feminist theory, neurobiology, and couple and family therapy that counter this individualistic view. A major focus is on the ways in which partners impact each other's identity and well‐being for better or worse, and the ethical responsibility this entails. The discussion addresses couple relational ethics within the larger sociocultural context that privileges power and competition. The paper offers applications to couple relationships and couple therapy, with interventions to promote mutual responsibility, fairness, dialogue, and care—helping partners to identify their higher values and interact with greater intentionality. Romantic relationships are more satisfying and fulfilling when power is balanced relatively equally between partners (Leonhardt et al., Journal of Family Psychology, 34, 2020, and 1). Yet, few couples therapy models explicitly outline how to confront relational power issues (Knudson‐Martin & Huenergardt, 2015, Socio‐emotional relationship therapy: Bridging emotion, societal context, and couple interaction, Springer). Emotionally Focused Therapy (EFT; Johnson, 2020, The practice of emotionally focused couple therapy, Routledge) is a well‐established, evidence‐based therapy modality that many therapists use with couple clients, yet despite its effectiveness, it does not provide direction for explicitly addressing and treating power differentials in couple relationships. In this paper, we explore the integration of EFT with Socio‐emotional Relationship Therapy (SERT), a model overlay that acknowledges the impact of social discourse on enactments of power in intimate couple relationships. We first address the importance of understanding power in couple relationships, addressing power in couples therapy, and provide a brief overview of SERT and EFT. We then introduce an integration of the models intended to help therapists balance power, increase connection, and secure attachment bonds between romantic partners. [ABSTRACT FROM AUTHOR]

Our clinical experience in psychoanalytic family therapy with families where one member has anorexic symptoms has shown that the therapy space is often invaded by the deathly dimension, by an absence of family historicity, and by a lack of autonomy. These different elements appear as “voids,” missing pieces of a family puzzle, and reflect a psychic container damaged by the weight of inherited intergenerational trauma. Rather than disappear, these elements are passed down from one generation to the next, their effects weakening the current group whose psychic envelope develops “holes” and becomes “elastic.” This paper will focus on the changes in this psychic container, which shift according to the rhythm of family functioning, oscillating between activation of the deathly toxic function within the group, on the one hand, and tension between the isomorphic and homomorphic mode, on the other. We will show how this clinical identification around the quality of the psychic envelope and its changes is valuable for family therapy. [ABSTRACT FROM AUTHOR]

This paper describes "Turning back the clock" (TBC)—an innovative strategy addressing unacceptable or coercive youth behavior based on nonviolent principles, inspired by the nonviolent resistance movement (NVR)—also called connecting authority or caring authority (CA) approaches to guidance and supervision of parents and other adults. Variants of NVR/CA have been evaluated as effective in RCT and pre–post designs. TBC has not been evaluated regarding its effectiveness but shows promising usability in case studies. The aim of this description of the TBC strategy is to encourage development and testing of its usability on large scale to improve it and pave the way for effectiveness evaluations. The core of TBC is to create possibilities for improving behavior without delay by negotiating the social timeline narrative. This allows improvement through reenactment of events immediately after having said or done something unfortunate or unacceptable instead of waiting for the next comparable situation. Adults introduce the strategy by modeling it before youths are encouraged to improve their own misbehavior immediately without waiting for a later opportunity. Finally, adults declare that a set of unacceptable behaviors will be considered disqualifying to any request or demand, but that attempting again as if it had not happened can be a possibility: Using the TBC strategy. This declaration is intended to increase youth's interest in using TBC themselves, and with successful use reduce escalation of conflicts into coercion and threats. [ABSTRACT FROM AUTHOR]

The transition to parenthood (TP) interview and coparenting capacity (CC) coding scheme is a mixed method approach to evaluating readiness for the interpersonal challenges of parenthood. This paper focuses on the validation of the TP–CC system with a diverse sample of 140 young expectant father/mother pairs. The TP interview is designed to assist expectant parents in expressing their thoughts and feelings about parenthood and coparenting and the CC coding scheme is designed to assess a new parent's capacity for expressing fondness, acceptance, growth, cohesion, and commitment in their relationship with their coparenting partner. Convergent validation of the TP–CC system involved measuring self‐ and partner‐reported relationship quality, relationship security, and observed warmth and hostility during the pregnancy. Predictive validation focused on the same set of variables, measured at the 6‐month postbirth follow‐up. Results supported the convergent validity of the TP–CC system for mothers and fathers, with higher‐specific CC scores correlating with higher relationship quality, relationship security and warmth scores, as well as lower hostility scores. Results partially supported predictive validity, with fathers' total CC scores predicting fathers' interpersonal hostility and mothers' relationship quality, relationship security, hostility, and warmth at follow‐up. Consistently positive and significant correlations between both mothers and fathers prebirth and postbirth CC scores provide evidence for the test–retest reliability of the TP–CC system. Generally, findings demonstrate the potential utility of the TP–CC system for evaluating coparenting readiness across the transition to parenthood. [ABSTRACT FROM AUTHOR]

An introduction is presented in which the author discusses articles within the issue on topics including evidence-based practice of couple and family therapy and the representativeness of samples in couple therapy and couple relationship education.

It is important to understand the need to have directions and goals in order to organize a therapeutic path. After having considered the common aspects of strategic therapies, the authors—both representing the Milan School of Boscolo and Cecchin—explain the unavoidable use of a strategic attitude and its evolution, from the adherence to the Palo Alto model, to Tomm's (1987) proposal, to considering strategizing the fourth guideline of the Milan Approach. We then discuss the utilization of strategizing in the present times. Does it make sense to speak of directive or nondirective psychotherapists now a days? The answer is that if we take a second‐order positioning—which is the attitude that distinguishes therapy from a common chat among friends—we are inevitably and contemporarily both directive and nondirective. Open Dialogue is a collaborative approach to mental health care emphasizing integrated services and a dialogical psychotherapy approach. Open Dialogue training programs eschew traditional didactic teaching of technical therapeutic skills in favor of more experiential learning processes. It is unclear how these training programs affect trainees and shape their perspectives on Open Dialogue. Our aim was to follow up a group of Australian Open Dialogue trainees and explore their perspectives on learning processes and psychotherapeutic practice. We utilized a prospective focus group design with data from audio‐recorded focus groups convened before (n = 2) and after (n = 3) participants completing an advanced Open Dialogue training program. Data were subjected to reflective thematic analysis. We identified the theme "Extending possibilities by holding ideas lightly," which represented a universal principle that participants applied to multiple aspects of their practice, for example, favoring multiple perspectives and approaches to therapy, including those other than Open Dialogue. This theme had two sub‐themes: (1) "Allowing intimacy by being aware of personal biographies" and (2) "Learning by joining others," which reflected an increased willingness by participants to reflect on and share their inner experiences and an emphasis on joint experiential exercises in the training program. "Extending possibilities by holding ideas lightly" facilitated a means of incorporating a dialogical perspective into existing practices thus avoiding the potential barriers to a wholesale implementation of Open Dialogue. Findings indicated that the participants were not learning how to practice a therapeutic technique or propositional knowledge, but were socialized into a dialogical way of being. Drawing on decades of research in family systems, coparenting, and developmental science, we present a clinical approach to address unmet service needs in children's mental health. Specifically, we describe Lausanne Family Play – Brief Intervention (LFP‐B) – a manualized family systems approach providing a caregiver‐caregiver‐child therapy (and sibling/s, when applicable). The LFP‐B is ultra‐brief, typically delivered in as few as three sessions (two assessment sessions followed by a video feedback session), with the aim of reducing children's mental health symptomatology by enhancing the coparenting relationship. We review literature on systemic family therapies and provide a rationale for including coparents and children in child mental health care. We then provide a rationale for using behavioral observations and video feedback in treatment, drawing on research in related family‐based treatments (e.g., parent–child therapies). Lower income couples tend to report more difficulty sustaining high‐quality intimate relationships. As a result, policy initiatives have been enacted to fund relationship education (RE) programs that aim to increase lower income couples' relationship satisfaction. Generally, these programs demonstrate small, albeit statistically significant improvements in mean levels of relationship functioning. It is critical, however, to understand if RE programming influences the developmental course of intimate relationships, and if this influence depends on couples' initial levels of concerns about their relationships. Using dyadic group‐based modeling and three waves of data from 6034 couples in the Supporting Healthy Marriages project, a randomized control trial of RE, we categorized couples into four relationship concern groups (No Relational Concerns, Both Relationally Concerned, Men's Relational Concerns, and Women's Relational Concerns) and explored how these groups moderated the long‐term efficacy of RE programming. Results indicated that RE was associated with different developmental trajectories of satisfaction, but RE effects differed for men and women. Specifically, random assignment into RE was associated with men maintaining high levels of relationship satisfaction, whereas women's satisfaction decreased over time. These effects were not moderated by initial relationship concerns. The association between RE and relationship satisfaction trajectories was small in magnitude, suggesting that more comprehensive services are needed to strengthen lower income couples' intimate relationships. For some adolescent gamers, playing online games may become problematic, impairing functioning in personal, family, and other life domains. Parental and family factors are known to influence the odds that adolescents may develop problematic gaming (PG), negative parenting and conflictual family dynamics increasing the risk, whereas positive parenting and developmentally supportive family dynamics protecting against PG. This suggests that a treatment for adolescent PG should not only address the gaming behaviors and personal characteristics of the youth, but also the parental and family domains. An established research‐supported treatment meeting these requirements is multidimensional family therapy (MDFT), which we adapted for use as adolescent PG treatment. We report here on one adaptation, applying in‐session gaming. In‐session demonstration of the "problem behavior" is feasible and informative in PG. In the opening stage of therapy, we use in‐session gaming to establish an alliance between the therapist and the youth. By inviting them to play games, the therapist demonstrates that they are taken seriously, thus boosting treatment motivation. Later in treatment, gaming is introduced in family sessions, offering useful opportunities to intervene in family members' perspectives and interactional patterns revealed in vivo as the youth plays the game. These sessions can trigger strong emotions and reactions from the parents and youth and give rise to maladaptive transactions between the family members, thus offering ways to facilitate new discussions and experiences of each other. One of the most serious psychosocial problems worldwide is substance abuse because of its repercussions not only on the physical and psychological health of the abuser but also on their relational functioning. Among the well‐established therapeutic approaches for the treatment of substance abuse is family therapy, which, in addition to influencing personal variables, promotes changes in family dynamics. The main objective of this study is to review the scientific literature published from 2010 to the present on the efficacy and effectiveness shown by family‐based treatment approaches for substance use problems both in adolescent and adult samples. In addition, the effect on secondary variables such as family functioning and behavioral problems is evaluated. The empirical evidence accumulated in the last decade and reviewed in the present study indicates that the incorporation of family members in the treatment of substance abuse produces benefits by diminishing consumption and improving family functioning. The integration of theories and techniques is part of family therapists' daily practice, raising the need to understand which resources are used in this professional's clinical actions. Our aim is to reflect on the use of theoretical and technical resources by family therapists in their daily practice. We developed an inquiry process inspired by collaborative action research and social constructionism. Couple and family therapy sessions were conducted by two family therapists, and we developed subsequent dialogs with them focused on describing theoretical and technical inspirations for their actions. Next, we named four categories of actions in this process: exploratory actions, actions for the construction of relational visions, actions for the construction of communication, and actions for family orientation. The therapists‐participants referred to many theoretical and technical resources that inspired their practice. For them, the choice of using one or another resource was based on the demands of the interactive moment, which involves their relational and embodied responses to what was experienced in the sessions. We argue that epistemological coherence is not something that guides the therapist's practice a priori, but something that can be achieved by talking about and reflecting on their actions. This article can contribute to clinical practice and training in family therapy by presenting an interesting reflexive process that can be useful to enhance practice in couple and family therapy (FAPESP, Process n. 2015/21316–1).
