Objectives: In lung cancer, one of the main limitations for the optimal integration of the biological and anatomical information derived from Positron Emission Tomography (PET) and Computed Tomography (CT) is the time and expertise required for the evaluation of the different respiratory phases. In this study, we present two open-source models able to automatically segment lung tumors on PET and CT, with and without motion compensation., Materials and Methods: This study involved time-bin gated (4D) and non-gated (3D) PET/CT images from two prospective lung cancer cohorts (Trials 108237 and 108472) and one retrospective. For model construction, the ground truth (GT) was defined by consensus of two experts, and the nnU-Net with 5-fold cross-validation was applied to 560 4D-images for PET and 100 3D-images for CT. The test sets included 270 4D- images and 19 3D-images for PET and 80 4D-images and 27 3D-images for CT, recruited at 10 different centres., Results: In the performance evaluation with the multicentre test sets, the Dice Similarity Coefficients (DSC) obtained for our PET model were DSC(4D-PET) = 0.74 ± 0.06, improving 19% relative to the DSC between experts and DSC(3D-PET) = 0.82 ± 0.11. The performance for CT was DSC(4D-CT) = 0.61 ± 0.28 and DSC(3D-CT) = 0.63 ± 0.34, improving 4% and 15% relative to DSC between experts., Conclusions: Performance evaluation demonstrated that the automatic segmentation models have the potential to achieve accuracy comparable to manual segmentation and thus hold promise for clinical application. The resulting models can be freely downloaded and employed to support the integration of 3D- or 4D- PET/CT and to facilitate the evaluation of its impact on lung cancer clinical practice., Clinical Relevance Statement: We provide two open-source nnU-Net models for the automatic segmentation of lung tumors on PET/CT to facilitate the optimal integration of biological and anatomical information in clinical practice. The models have superior performance compared to the variability observed in manual segmentations by the different experts for images with and without motion compensation, allowing to take advantage in the clinical practice of the more accurate and robust 4D-quantification., Key Points: Lung tumor segmentation on PET/CT imaging is limited by respiratory motion and manual delineation is time consuming and suffer from inter- and intra-variability. Background: We explored perceptions and preferences regarding the conversion of in-person to virtual conferences as necessitated by travel and in-person meeting restrictions., Methods: A 16-question online survey to assess preferences regarding virtual conferences during the COVID-19 pandemic and future perspectives on this subject was disseminated internationally online between June and August 2020., Findings: A total of 508 responses were received from 73 countries. The largest number of responses came from Italy and the USA. The majority of respondents had already attended a virtual conference (80%) and would like to attend future virtual meetings (97%). The ideal duration of such an event was 2-3 days (42%). The preferred time format was a 2-4-h session (43%). Most respondents also noted that they would like a significant fee reduction and the possibility to attend a conference partly in-person and partly online. Respondents indicated educational sessions as the most valuable sections of virtual meetings. The reported positive factor of the virtual meeting format is the ability to re-watch lectures on demand. On the other hand, the absence of networking and human contact was recognized as a significant loss. In the future, people expressed a preference to attend conferences in person for networking purposes, but only in safer conditions., Conclusions: Respondents appreciated the opportunity to attend the main radiological congresses online and found it a good opportunity to stay updated without having to travel. However, in general, they would prefer these conferences to be structured differently. Background and Objective: The systematic collection of medical images combined with imaging biomarkers and patient non-imaging data is the core concept of imaging biobanks, a key element for fuelling the development of modern precision medicine. Our purpose is to review the existing image repositories fulfilling the criteria for imaging biobanks., Methods: Pubmed, Scopus and Web of Science were searched for articles published in English from January 2010 to July 2021 using a combination of the terms: "imaging" AND "biobanks" and "imaging" AND "repository". Moreover, the Community Research and Development Information Service (CORDIS) database ( https://cordis.europa.eu/projects ) was searched using the terms: "imaging" AND "biobanks", also including collections, projects, project deliverables, project publications and programmes., Results: Of 9272 articles retrieved, only 54 related to biobanks containing imaging data were finally selected, of which 33 were disease-oriented (61.1%) and 21 population-based (38.9%). Most imaging biobanks were European (26/54, 48.1%), followed by American biobanks (20/54, 37.0%). Among disease-oriented biobanks, the majority were focused on neurodegenerative (9/33, 27.3%) and oncological diseases (9/33, 27.3%). The number of patients enrolled ranged from 240 to 3,370,929, and the imaging modality most frequently involved was MRI (40/54, 74.1%), followed by CT (20/54, 37.0%), PET (13/54, 24.1%), and ultrasound (12/54, 22.2%). Most biobanks (38/54, 70.4%) were accessible under request., Conclusions: Imaging biobanks can serve as a platform for collecting, sharing and analysing medical images integrated with imaging biomarkers, biological and clinical data. Objectives: Vascular characteristics of tumour and peritumoral volumes of high-grade gliomas change with treatment. This work evaluates the variations of T2*-weighted perfusion parameters as overall survival (OS) predictors., Methods: Forty-five patients with histologically confirmed high-grade astrocytoma (8 grade III and 37 grade IV) were included. All patients underwent pre- and post-treatment T2*-weighted contrast-enhanced magnetic resonance (MR) imaging. Tumour, peritumoral and control volumes were segmented. Relative variations of cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT), K trans-T2* , k ep-T2* , v e-T2* and v p-T2* were calculated. Differences regarding tumour grade and surgical resection extension were evaluated with ANOVA tests. For each parameter, two groups were defined by non-supervised clusterisation. Survival analysis were performed on these groups., Results: For the tumour region, the 90th percentile increase or stagnation of CBV was associated with shorter survival, while a decrease related to longer survival (393 ± 189 vs 594 ± 294 days; log-rank p = 0.019; Cox hazard-ratio, 2.31; 95% confidence interval [CI], 1.12-4.74). K trans-T2* showed similar results (414 ± 177 vs 553 ± 312 days; log-rank p = 0.037; hazard-ratio, 2.19; 95% CI, 1.03-4.65). The peritumoral area values showed no relationship with OS., Conclusions: Post-treatment variations of the highest CBV and K trans-T2* values in the tumour volume are predictive factors of OS in patients with high-grade gliomas., Key Points: • Vascular characteristics of high-grade glioma tumour and peritumoral regions change with treatment. • Quantitative assessment of MRI perfusion provides valuable information regarding tumour aggressiveness. • Quantitative T2*-weighted perfusion parameters can help to predict overall survival. • Post-treatment variations of CBV and K trans-T2 values are predictive factors of OS. • Increased values may justify treatment intensification in these patients.

Objective: The efficiency of endovascular liver gene transfer in pigs is evaluated by comparing two models of retrograde catheterization: single lobe catheterization with portal inflow (open procedure) versus whole liver isolation with portal and inferior vena cava blockage (close procedure)., Methods: Percutaneous endovascular catheterization was performed in pigs. Open procedure (n = 3): 8Fr balloon catheter placement in a suprahepatic branch through the jugular vein. Closed procedure (n = 3): simultaneous catheterization of the intrahepatic portal vein (transhepatic catheterization, 10Fr balloon catheter), the supra- and infrahepatic cava veins (8Fr balloon catheters through the jugular and femoral veins). In both models, 200 ml of hAAT DNA solution (20 μg/ml) were retrogradely injected at 20 ml/s. Tissue samples (8 per liver) were obtained 14 days later and the exogenous DNA, RNA and protein per cell were quantified. Blood samples were collected periodically for transaminase determination in all the animals., Results: The open procedure achieved lower (approx. 1000-fold) DNA delivery, resulting in a significantly lower (p < 0.001) gene transcription (> 100-fold). The closed model also achieved a higher translation index, although differences were smaller (p < 0.001)., Conclusions: Portal inflow blockage increased the delivery, transcription and translation indexes, significantly improving the final procedure efficacy when compared with an open procedure., Key Points: Endovascular hydrodynamic pig liver gene transfer: open procedure versus closed procedure. Open procedure resulted in much lower DNA delivery than closed model. Open procedure reached significantly lower gene transcription index. Translation index with closed model was higher than with the open.

Risks associated to ionising radiation from medical imaging techniques have focused the attention of the medical society and general population. This risk is aimed to determine the probability that a tumour is induced as a result of a computed tomography (CT) examination since it makes nowadays the biggest contribution to the collective dose. Several models of cancer induction have been reported in the literature, with diametrically different implications. This article reviews those models, focusing on the ones used by the scientific community to estimate CT detriments. Current estimates of the probability that a CT examination induces cancer are reported, highlighting its low magnitude (near the background level) and large sources of uncertainty. From this objective review, it is concluded that epidemiological data with more accurate dosimetric estimates are needed. Prediction of the number of tumours that will be induced in population exposed to ionising radiation should be avoided or, if given, it should be accompanied by a realistic evaluation of its uncertainty and of the advantages of CTs. Otherwise they may have a negative impact in both the medical community and the patients. Reducing doses even more is not justified if that compromises clinical image quality in a necessary investigation. Key Points • Predictions of radiation-induced cancer should be discussed alongside benefits of imaging. • Estimates of induced cancers have noticeable uncertainties that should always be highlighted. • There is controversy about the acceptance of the linear no-threshold model. • Estimated extra risks of cancer are close to the background level. • Patients should not be alarmed by potential cancer induction by CT examinations.

Objectives: To evaluate the quantitative parameters obtained from dynamic MR T2*-weighted images as predictors of survival taking into consideration the biasing effects of other survival-related covariates., Methods: Thirty-nine patients (60 ± 14 years; survival 267 ± 191 days) with high-grade gliomas (8 grade III, 31 grade IV) were retrospectively included in the study. Additional data incorporated Karnofsky performance scale, tumour resection extension after surgery and type of treatment. Dynamic T2*-weighted MRI was acquired before treatment. Tumour curves were extracted for each voxel, and several quantitative parameters were obtained from the whole tumour volume and the 10 % maximum values. Additional image covariates included the presence of necrosis, single or multiple lesions, and tumour and oedema volumes. The relationship between quantitative parameters and survival was assessed using clusterisation techniques and the log-rank method. Cox regression analysis was used to evaluate each parameter's predictive value., Results: Only the mean of the 10 % maximum values of the transfer coefficient showed an independent relationship with patient survival (log-rank chi-squared test <0.001, Cox regression P = 0.015), with higher values corresponding to lower survival rates., Conclusions: High maximum transfer coefficient values show an independent statistical relationship with low survival in high-grade glioma patients. This imaging biomarker can be used as a predictor of prognosis.

Normal and degenerated cartilages have different magnetic resonance (MR) capillary permeability (K(trans)) and interstitial interchangeable volume (v(e)). Our hypothesis was that glucosamine sulfate treatment modifies these neovascularity abnormalities in osteoarthritis. Sixteen patients with patella degeneration, randomly distributed into glucosamine or control groups, underwent two 1.5-Tesla dynamic contrast-enhanced MR imaging studies (treatment initiation and after 6 months). The pain visual analog scale (VAS) and American Knee Society (AKS) score were used. A two-compartment pharmacokinetic model was used. Percentages of variations (postreatment-pretreatment/pretreatment) were compared (t-test for independent data). In the glucosamine group, pain and functional outcomes statistically improved (VAS: 7.3 +/- 1.1 to 3.6 +/- 1.3, p < 0.001; AKS: 18.6 +/- 6.9 to 42.9 +/- 2.7, p < 0.01). Glucosamine significantly increased K(trans) at 6 months (-54.4 +/- 21.2% vs 126.7 +/- 56.9%, p < 0.001, control vs glucosamine). In conclusion, glucosamine sulfate decreases pain while improving functional outcome in patients with cartilage degeneration. Glucosamine sulfate increases K(trans), allowing its proposal as a surrogate imaging biomarker after 6 months of treatment.

The purpose of this report was to determine the rate at which abstracts orally presented at the European Congress of Radiology (ECR) 2001 were published in 2001-2005 Medline-indexed journals and to compare publication rates and factors with presentations at the ECR in two different periods (2001 and 2000). Absolute and relative publication rates (APR, RPR) and different publication-related factors were analysed. From 991 abstracts originating from 52 countries, 449 articles (APR 45%) were subsequently published in 125 journals, most frequently in European Radiology (n=79, 18%). Country of origin statistically (p<0.0001) influences subsequent publication of the abstract, with Germany having the highest number of presentations (n=300) and derived articles (n=175, RPR 58%) whereas Sweden had the highest RPR (82%). Interventional and physics studies had the highest RPR (59% and 58%, respectively). The ECR meeting has a very high and stable APR (ECR 2001: 45% vs ECR 2000: 47%), and the journal European Radiology had the larger number of related publications (18% RPR following ECR 2001 compared with 14% from ECR 2000). Germany had the highest number of presentations and publications for both meetings. The highest RPR for ECR 2001 was found in interventional and physics studies whereas chest and cardiac studies had the highest RPR for ECR 2000.

To determine the rate at which abstracts orally presented at the ECR 2000 were published between 2000-2004, and to identify predictive factors of publication and differences between abstracts and subsequently published papers. Specific search profiles were devised to retrieve items from the Medline database. From 1020 abstracts originating from 39 countries, 479 articles (publication rate 47%) were subsequently published in 139 Medline-indexed journals, most frequently in European Radiology (14%). Country of origin statistically (P<0.0001) influences the subsequent publication of the abstract, Germany having the highest number of presentations (n=343) and derived articles (publication rate 54%). Abstracts presented by authors from the USA (n=21) had the highest publication rate (76%). Most papers were published within the first 3 years after the meeting, as original articles and in English-language journals. Both the study sample size and the first author frequently changed. Chest and cardiac studies had the highest publication rates (56%, both). In summary, abstracts presented at the ECR 2000 had a high publication rate in Medline-indexed journals. Country of origin and subspecialty of presentation appeared to influence subsequent full publication. More articles were published in European Radiology than in other journal.

Mangafodipir trisodium (Mn-DPDP, Teslascan) is a well-tolerated liver contrast agent. Although the enhancement characteristics of the cirrhotic liver after Mangafodipir trisodium administration have been studied, at present there is no published data on the impact that cirrhosis might have on the safety and efficacy profiles of this agent. Our objective is to evaluate by means of a retrospective comparison the safety and efficacy of Mangafodipir trisodium in patients with underlying cirrhosis who were examined for suspicion of focal liver lesions. A total of 923 patients received Mangafodipir trisodium (5 micromol/kg) in 11 prospective randomized European clinical trials. Adverse events and discomfort were recorded and graded in all patients. The efficacy analyses were performed on the subsets consisting of 617 patients with independent lesion counts (detection), and on the subset with 399 patients with independent and onsite final lesion diagnosis (characterization). Of the 399 patients, 149 had histologic confirmation. One hundred eighty of 923 patients (19.5%) had cirrhosis. There were no main differences between cirrhotic and non-cirrhotic patients. Adverse events were observed in 64 patients (6.9%), 6.7% in the cirrhotic group and 7.0% in the non-cirrhotic group, a non-significant difference. Adverse events in most patients were mild or moderate. The presence and intensity of the events did not differ between groups. Discomfort was recorded in 79 patients (8.6%), equally distributed in cirrhotic (6.1%) and non-cirrhotic (9.2%) patients. Regarding lesion count, significantly more lesions were found in the post- than in the precontrast images in both the cirrhotic and non-cirrhotic groups ( p<0.0001). This increase was not influenced by the presence of liver cirrhosis ( p=0.94). Lesion characterization was significantly improved in cirrhotic patients after administration of Mangafodipir trisodium ( p=0.002) but not in non-cirrhotic patients ( p=0.13). Mangafodipir trisodium is a safe and well-tolerated useful contrast agent in patients with cirrhosis.

The objective of this study was to evaluate the diagnostic value of fat-suppressed T2-weighted (FS-T2) images or short tau inversion recovery (STIR) imaging in distinguishing lipoma from lipoma-like subtype of well-differentiated liposarcoma. Spin-echo T1-weighted and STIR or fat-suppression T2-weighted sequences were performed in 60 lipomas and 32 lipoma-like well-differentiated liposarcomas, histologically proven, looking for thick septa or nodules in T1-weighted images and linear, nodular, or amorphous hyperintensities on FS-T2/STIR sequences. Fourteen lipomas (23.3%) showed thick septa and/or nodules on T1, whereas on FS-T2 or STIR sequences only seven (11.7%) displayed hyperintense nodules and/or septa. All well-differentiated liposarcomas contained these signs on FS-T2 or STIR sequences. The presence of hyperintense septa or nodules in a predominantly lipomatous tumor on FS-T2/STIR sequences helps to differentiate malignant tumors from lipomas. Employing the presence of hyperintense nodules and/or septa as criteria of malignancy specificity was 76.6% and sensitivity 100%. Overdiagnoses of well-differentiated liposarcoma can occur due to the presence of non-lipomatous areas within lipomas.

In abdominal MR imaging, ghost artifacts from noncyclic bowel movements can reduce the quality of the images. Although pharmacologic suppression of motion is effective, no study has being conducted to analyze the influence of drug motion suppression on fast breath-hold 1.5-T examinations of the upper abdomen. A prospective, randomized, double-blind trial was conducted in 50 patients. Patients were randomly distributed into two groups: The control group received only an oral solution, whereas the other group received the oral solution plus a subcutaneous injection of 20 mg of butylscopolamine 10 min before the MR examination. Breath-hold T1-weighted gradient-recalled-echo (GRE) MR images were obtained in a 1.5-T superconductive unit. Quantitative image analysis was performed with region-of-interest (ROI) measurements of the signal intensity of the liver and in background air anterior and lateral to the patient. A qualitative analysis of the subjective quality of the T1-weighted images was also done, and the adverse reactions were registered. The groups were homogeneous regarding age, gender, and weight distribution. No significant differences in the signal intensity of the liver and in the incoherent noise measurements were found between the two groups. Gastrointestinal noise showed significant differences between groups, with lower values for the butylscopolamine group compared with the control group. There was also a statistically significant difference in the image quality between groups, and optimal studies were only found in the butylscopolamine group, where most patients had a good-quality evaluation. Regarding adverse events, there were non-significant differences between groups. In conclusion, administration of an antiperistaltic agent to reduce the movements of the gastrointestinal tract diminishes the motion artifacts generated on MR imaging of the abdomen, even at high field strength and with fast imaging sequences. Images of the upper abdomen obtained after pharmacologic suppression of the gastrointestinal movement are of significantly superior quality.

Our objective was to study the coexistence of lower thoracic-spine disc changes in patients with low back pain using a large field of view (FOV) in lumbar spine MR imaging. One hundred fifty patients with low back pain were referred to an MR examination. All patients were studied with a large FOV (27 cm), covering from the coccyx to at least the body of T11. Discs were coded as normal, protrusion, and extrusion (either epiphyseal or intervertebral). The relationship between disc disease and level was established with the Pearson chi(2) test. The T11-12 was the most commonly affected level of the lower thoracic spine with 58 disc cases rated as abnormal. Abnormalities of T11-12 and T12-L1 discs were significantly related only to L1-L2 disease ( p=0.001 and p=0.004, respectively) but unrelated to other disc disease, patient's gender, and age. No correlation was found between other discs. Magnetic resonance imaging of the lumbar spine can detect a great amount of lower thoracic disease, although its clinical significance remains unknown. A statistically significant relation was found within the thoracolumbar junctional region (T11-L2), reflecting common pathoanatomical changes. The absence of relation with lower lumbar spine discs is probably due to differences in their pathomechanisms.

Our objectives were to analyze different semiological patterns in craniopharyngiomas studied with CT and MR sequences. Retrospective study of 26 patients with confirmed craniopharyngiomas. All cases were examined with CT and MR imaging using a variety of pulse sequences (spin echo, inversion recovery, gradient echo in-phase and opposed-phase). The analyzed component patterns were classified as solid, calcium, proteic-like, cerebrospinal fluid (CSF)-like, hematic-like, and fatty patterns. The different patterns were related by means of contingency tables and the Fisher exact test and also to epidemiological findings and tumor size. A solid pole was detected in all patients, whereas a cystic component was present in 92.3% of the cases. Calcification was visualized in 65.3%, proteic-like in 53.8%, CSF-like in 23%, hematic-like in 19.2%, and fatty component in 15.3%. There were no statistical associations between patterns, with the exception that in no case did CSF-like and proteic-like patterns coexist ( P=0.004). Tumor size was related to components. Hematic-like (17.0+/-18.9 vs 3.9+/-2.6 mm, non-present vs present) and CSF-like (16.9+/-19.6 vs 6.5+/-4.0 mm) patterns were observed most frequently in smaller tumors, whereas larger tumors usually had proteic-like (5.9+/-5.4 vs 21.1+/-21.0 mm) and calcified (4.6+/-1.9 vs 19.1+/-19.9 mm) patterns. Computed tomography and a combination of different MR images frequently allow the detection of different semiological patterns in these tumors. Semiological patterns were correlated only to tumor size.

Self-citation, considered as the number of times a paper cites other papers in the same journal, is an important criteria of journal quality. Our objective is to evaluate the self-citation in the official journal of the Spanish Society of Radiology (Radiología), and to compare it with the European Radiology and Radiology journals. Papers published in Radiología, European Radiology, and Radiology during 1997 and 1998 were analyzed. The Self Citation Index, considered as the ratio between self-references and total number of references per article, for the journals Radiología (SCIR), European Radiology (SCIER), and Radiology (SCIRY), were obtained and expressed as percentages. Also, the number of references to Radiología in European Radiology and Radiology papers were calculated. Stratification of the index per thematic area and article type was also performed. Mean SCIR, SCIER, and SCIRY values were compared with the ANOVA and the Student-Newman-Keuls tests. The self-citation index was statistically higher in Radiology (23.2%; p<0.0001) than in Radiología (1.8%) and European Radiology (0.8%). There were no statistically significant differences between SCIR and SCIER indexes ( p=0.25). In the stratification per thematic areas and article type, self-citation in Radiology was statistically higher ( p<0.0001), with the only exception of "Radioprotection" area ( p=0.2), to SCIR and SCIER. Although there were no statistically significant differences, by thematic areas SCIR was always larger than SCIER, with the only exception of the "Genitourinary imaging" area, and by article type SCIR also went greater to SCIER, except in review articles. Radiología, The Spanish official radiological journal, although not included in Index Medicus and its database Medline, had a larger number of self-citing than European Radiology in the period 1997-1998.

The aim of this study was to assess the accuracy of MR in the diagnosis of synovialisation of the anterior cruciate ligament (ACL) compared with arthroscopy. One hundred and forty-nine patients were examined with MR imaging and arthroscopy of the knee. The MR sign used to consider a synovialised ACL consisted of hypointense fibrillar tracts, disrupted and wavily, in its expected course. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV), comparison of proportions (McNemar test) and Kappa values for agreement between MR imaging and arthroscopy were calculated. Of the 133 (89.3 %) ligaments without synovialisation at arthroscopy, 130 accorded with the MR results. Of the 16 (10.7 %) synovialised ligaments, 13 accorded with the MR results. Three false-positive and three false-negative MR diagnoses were identified. The agreement between both techniques was excellent (Kappa = 0.79; p = 0.000), without differences (McNemar test; p = 1). Sensitivity was 0.81, specificity 0.98, PPV 0.98 and NPV 0.81. Magnetic resonance imaging is highly reliability for synovialisation diagnosis. The imaging sign used to diagnose synovialised ACL (hypointense comma-like tracts in its expected course) is reliable. As this reparative process can simulate an intact ligament, knowledge of this sign is important in diagnosing synovialisation of ACL tears so as not to confuse it with normal ACL.

The aim of this case report is to describe the appearance on magnetic resonance imaging (MRI) of an incidentally found adenomatoid tumor of the adrenal gland, and to evaluate the utility of MRI in characterizing this type of tumor. The appearance of the tumor was nonspecific on T1-weighted in-phase, opposed-phase, and T2-weighted images, as well as its behavior after paramagnetic contrast administration, outlining the differential diagnosis among carcinoma, metastatic tumors, and pheochromocytoma. After surgery, the pathologic diagnosis was adenomatoid benign tumor of mesothelial origin. Although MRI enables the characterization of most benign lesions of the adrenal gland, the appearance of other lesions is nonspecific. In our case, MRI did not assist in preoperative diagnosis, guiding us towards a diagnosis of malignancy.

Osteoid osteoma (OO) is a benign skeletal neoplasm. Twenty-eight patients with proven OO were studied with MRI regarding soft tissue involvement which was diagnosed when high proton-density and T2-weighted signal intensity and low signal intensity on T1-weighted images were found close to bone. Most tumors were located in the femur and tibia; 6 cases diaphyseal, 12 metaphyso-diaphyseal, and 10 epiphyseal. In relation to the cortex, 15 were located centrally or in its outer margin. Soft tissue involvement was found in 15 patients (53.6 %). A statistical relationship was found between soft tissue involvement and the tumor's location with regard to the cortex, being more frequent in peripherally located tumors. Therefore, soft tissue involvement is a frequent finding in peripherally located OO.

Objectives: Iopentol (Nycomed Imaging AS, Oslo, Norway) and iopromide (Schering AG, Berlin, Germany) are low-osmolar, non-ionic, iodinated contrast media (CM) used in abdominal CT examinations. The intravenous safety profile and radiological efficacy of iopentol and iopromide were studied in 518 patients. Specifically, frequency of adverse events (AEs), subjective change in quality of diagnostic information, and quantitative enhancement characteristics were compared., Materials and Methods: A prospective, double-blind, randomized, multicentre, parallel-group study was conducted at 8 hospitals. Patients received 100 ml of either iopentol 300 mg I/ml or iopromide 300 mg I/ml., Results: The incidence of patients with AEs was statistically significantly lower in the iopentol group compared to the iopromide group (2.3% vs. 8.9%, p < 0.001). Discomfort was frequent in both groups (44.8% vs. 49.4%, p = 0.33), sensation of heat and warmth being most common. Overall, diagnostic information was similar in both groups. Both CM gave high percentages of examinations rated as optimal (87.1% vs. 90.5%, p = 0.34) and in which diagnostic confidence was increased (87.5% vs. 91.1%, p = 0.22). No significant differences between the two CM were found concerning quantitative enhancement characteristics., Conclusions: In this study iopentol was significantly safer than iopromide for contrast enhanced CT examination of the abdomen. Radiological efficacy was similar with both CM.

There is no doubt that radiologists play an increasingly important role in the detection of focal liver lesions, and in the evaluation of persistent or recurrent malignant disease after treatment. The characterization of focal liver lesions depends on the clinical integration of the information generated by different radiologic techniques. Most often, MR imaging is quite effective in liver tumor characterization. Our purpose is to provide an overview of the MR characteristics of the most commonly encountered liver tumors and their differential diagnosis, with the most accepted reliable proof of the lesion type in a clinical environment. Radiologists must know the level of confidence they can reach in a given diagnosis, and the influence that this diagnosis has in the patient's management. With this knowledge, radiologists decide whether to leave or follow-up the lesion, perform tissue samples, or carry out therapeutic procedures.

Nodular regenerative hyperplasia (NRH) of the liver is a condition characterized by multiple monoacinar regenerative nodules in the absence of fibrous septa. When these nodules become confluent they may be seen with sonography or CT. The appearance of these pseudotumoral pattern of NRH has been scarcely described with MRI. We present the imaging findings of five patients with NRH and a pseudotumoral form at sonography. Sonography depicted hyperechoic lesions in four patients and hypoechoic lesions in another. Computed tomography showed hypodense lesions with little contrast enhancement in two patients. Three patients showed subtle focal liver lesions on MRI: isointense in one, mildly hypointense in another, and minimally hyperintense in a patient with siderosis. The dynamic behavior at MRI was similar to the normal liver parenchyma. Hyperechoic lesions on sonography or hypodense lesions on CT, barely or not seen on MRI, can be indicative of NRH in an appropriate clinical setting.

Magnetic resonance (MR) field strength is one of the key aspects to consider when purchasing MR equipment. Other aspects include the gradient system, coil design, computer and pulse sequence availability, purchase cost, local reimbursement policies, and current opinion within the medical community. Our objective here is to evaluate the decision-influencing aspects of the MR market, with a focus on some specific areas such as high resolution studies, examination times, special techniques, instrumentation, open design magnets, costs and reimbursement policies, academic and industrial interests, contrast media, clinical efficacy, and finally, clinicians' preferences. Certainly the advantage of high-field is a higher signal-to-noise ratio and improved resolution. With a high-field unit, higher spatial resolution images and higher temporal resolution images can be obtained. Typical imaging times needed to produce clinically diagnostic images are about 3 times longer at 0.1 T than at 1.0 or 1.5 T. High-field-related advanced techniques, such as functional imaging, spectroscopy and microscopy, may become clinically useful in the near future. As long as there is an unlimited demand for MR examinations, it appears financially profitable to run a high-field system, despite the associated higher costs. However, if demand for MR becomes saturated, low-field systems will cause less financial strain on the reimbursement organisation and service provider. Recent emphasis on cost containment, the development of interventional techniques, the increased use of MR for patients in intensive care and operating suites, the deployment of magnets in office suites, and the development of new magnet configurations, all favour the supplementary use of low-field systems. Hence, MR units of all field strengths have a role in radiology.
