Objectives To assess imaging features of primary renal sarcomas in order to better discriminate them from non-sarcoma renal tumors. Methods Adult patients diagnosed with renal sarcomas from 1995 to 2018 were included from 11 European tertiary referral centers (Germany, Belgium, Turkey). Renal sarcomas were 1:4 compared to patients with non-sarcoma renal tumors. CT/MRI findings were assessed using 21 predefined imaging features. A random forest model was trained to predict “renal sarcoma vs. non-sarcoma renal tumors” based on demographics and imaging features. Results n = 34 renal sarcomas were included and compared to n = 136 non-sarcoma renal tumors. Renal sarcomas manifested in younger patients (median 55 vs. 67 years, p < 0.01) and were more complex (high RENAL score complexity 79.4% vs. 25.7%, p < 0.01). Renal sarcomas were larger (median diameter 108 vs. 43 mm, p < 0.01) with irregular shape and ill-defined margins, and more frequently demonstrated invasion of the renal vein or inferior vena cava, tumor necrosis, direct invasion of adjacent organs, and contact to renal artery or vein, compared to non-sarcoma renal tumors (p < 0.05, each). The random forest algorithm yielded a median AUC = 93.8% to predict renal sarcoma histology, with sensitivity, specificity, and positive predictive value of 90.4%, 76.5%, and 93.9%, respectively. Tumor diameter and RENAL score were the most relevant imaging features for renal sarcoma identification. Conclusion Renal sarcomas are rare tumors commonly manifesting as large masses in young patients. A random forest model using demographics and imaging features shows good diagnostic accuracy for discrimination of renal sarcomas from non-sarcoma renal tumors, which might aid in clinical decision-making. The aim of this study was to compare optical colonoscopy to fecal-tagging-based MR colonography in a screening population in terms of comfort and acceptance ratings as well as for future preferences as colorectal cancer screening examinations. Two hundred eighty-four asymptomatic patients (mean age 59 years) underwent MRC and OC within 4 weeks. While MRC was based on a fecal tagging technique, OC was performed after bowel cleansing. For OC, sedatives and analgesics were used. Patients evaluated both modalities and certain aspects of the examination according to a 10-point-scale with higher scores denoting a worse experience. Furthermore, preferences for future examinations were evaluated. No significant difference was noted for the overall acceptance of OC (mean value 3.0) and MRC (mean value 3.4). For MRC, the placement of the rectal tube was rated as the most unpleasant part, whereas bowel purgation was regarded most inconvenient for OC. Patients aged 55 years and older perceived most aspects less unpleasant than younger patients. Of the patients, 46% preferred MRC for future screening examinations (OC: 44%). OC and MRC have comparable general acceptance levels in a screening population. Especially for patients declining endoscopy as a screening method MRC may evolve as an attractive alternative.

The aim of this study was to investigate the feasibility of 7-T contrast-enhanced MR imaging of the female pelvis.Ten healthy female volunteers were examined on a 7-T whole-body MR system utilising a custom-built eight-channel transmit/receive radiofrequency body coil. The examination protocol included (1) T1-weighted fat-saturated 2D spoiled gradient echo (FLASH), (2) dynamic T1-weighted fat-saturated 3D FLASH, and (3) T2-weighted TSE sequences. For qualitative image analysis pelvic anatomy, uterine zonal anatomy and image impairment due to artefacts was assessed using a five-point scale. For quantitative analysis contrast ratios between the junctional zone and myometrium were obtained for T2-weighted MRI.Two-dimensional FLASH MRI offered the best overall image quality (meancontrast-enhanced 4.9) and highest tissue contrast (meancontrast-enhanced 4.7). T2-weighted TSE imaging provided a moderate to high conspicuity of the uterine zonal anatomy with mean scores ranging from 3.5 for endometrium to 4.65 for myometrium. Overall image impairment was rated strongest for T2-weighted MRI (2.9) and least for 2D FLASH MRI (mean 4.2).This study demonstrated the feasibility of 7-T T1-weighted MRI of the female pelvis and current constraints associated with T2-weighted MRI.• Dynamic contrast-enhanced female pelvis MR imaging at 7 T is feasible. • Unenhanced T1-weighted MRI offers inherent hyperintense delineation of pelvic arterial vasculature. • Two-dimensional FLASH MRI provided best overall image quality and least artefact impairment.

Multi-station contrast-enhanced magnetic resonance angiography (MRA) is considered as the imaging investigation of first choice in patients suffering from peripheral arterial occlusive disease. In order to overcome venous overlay and to gain dynamic flow information as provided by digital subtraction angiography (DSA), we developed a triple injection protocol for high-resolution MRA of the entire peripheral vascular system, applying time-resolved (TR) four-dimensional (4D) MRA sequences.Ten patients underwent three-station TR-MRA of the pelvis and lower extremities with DSA as reference standard. Both investigations were compared concerning stenosis on a segment-by-segment basis. Furthermore, 28 consecutive patients underwent the same MR-only imaging protocol. All images were evaluated concerning image quality (1 = non-diagnostic, 4 = excellent), venous overlay (from none up to substantial) and time to venous enhancement (very early/early/normal/late).Three-station TR-MRA proved feasible and was comparable with DSA in 282 vessel segments, with underestimation grade of stenosis in four segments and overestimation in four segments, respectively. In 32/38 patients no venous overlay was noted; in six patients there was mild venous overlay. Image quality was rated excellent or good in most cases.TR-MRA provides morphological and functional information without any timing issues due to optimal arterial enhancement at high spatial resolution without venous overlay.

To investigate the feasibility of 7T MR imaging of the kidneys utilising a custom-built 8-channel transmit/receive radiofrequency body coil.In vivo unenhanced MR was performed in 8 healthy volunteers on a 7T whole-body MR system. After B(0) shimming the following sequences were obtained: 1) 2D and 3D spoiled gradient-echo sequences (FLASH, VIBE), 2) T1-weighted 2D in and opposed phase 3) True-FISP imaging and 4) a T2-weighted turbo spin echo (TSE) sequence. Visual evaluation of the overall image quality was performed by two radiologists.Renal MRI at 7T was feasible in all eight subjects. Best image quality was found using T1-weighted gradient echo MRI, providing high anatomical details and excellent conspicuity of the non-enhanced vasculature. With successful shimming, B(1) signal voids could be effectively reduced and/or shifted out of the region of interest in most sequence types. However, T2-weighted TSE imaging remained challenging and strongly impaired because of signal heterogeneities in three volunteers.The results demonstrate the feasibility and diagnostic potential of dedicated 7T renal imaging. Further optimisation of imaging sequences and dedicated RF coil concepts are expected to improve the acquisition quality and ultimately provide high clinical diagnostic value.

Sufficient luminal distension is mandatory for small bowel imaging. However, patients often are unable to ingest volumes of currently applied oral contrast compounds. The aim of this study was to evaluate if administration of low doses of an oral contrast agent with high-osmolarity leads to sufficient and diagnostic bowel distension. Six healthy volunteers ingested at different occasions 150, 300 and 450 ml of a commercially available oral contrast agent (Banana Smoothie Readi-Cat, E-Z-EM; 194 mOsmol/l). Two-dimensional TrueFISP data sets were acquired in 5-min intervals up to 45 min after contrast ingestion. Small bowel distension was quantified using a visual five-grade ranking (5 = very good distension, 1 = collapsed bowel). Results were statistically compared using a Wilcoxon-Rank test. Ingestion of 450 ml and 300 ml resulted in a significantly better distension than 150 ml. The all-over average distension value for 450 ml amounted to 3.4 (300 ml: 3.0, 150 ml: 2.3) and diagnostic bowel distension could be found throughout the small intestine. Even 45 min after ingestion of 450 ml the jejunum and ileum could be reliably analyzed. Small bowel imaging with low doses of contrast leads to diagnostic distension values in healthy subjects when a high-osmolarity substance is applied. These findings may help to further refine small bowel MRI techniques, but need to be confirmed in patients with small bowel disorders.
