Objectives: To investigate the effects of orthodontic non-extraction treatment with or without headgear on the position of and the space available for upper third molars in growing children with class II malocclusions., Materials and Methods: The sample consisted of pre- and post-treatment panoramic radiographs and lateral cephalograms of 294 class II orthodontic patients; 160 were treated with headgear and 134 were treated without headgear. The space available for the upper third molar was measured on the lateral cephalogram as the distance from pterygoid vertical (PTV) to the distal surface of the upper first molar crown (PTV-M1). Angulation, vertical position and tooth development stage of the upper third molars were evaluated on panoramic radiographs. All measurements were evaluated statistically., Results: In both groups PTV-M1 increased, but the increase in PTV-M1 was significantly higher for patients treated without headgear. A linear model for repeated measures revealed that this difference was still significant after correction for age, gender and molar occlusion. Further, there is no evidence that the change in angulation, vertical position and development stage of the upper third molars during orthodontic treatment is influenced by headgear therapy., Conclusion: This study indicates that the use of headgear in growing patients significantly affects the space available for upper third molars. However, orthodontic treatment with headgear does not influence the angulation, vertical position and development stage of upper third molars. It is therefore important to always take into account third molars during treatment planning., (© The Author 2017. The aim of the present study was to measure the dental arches in order to assess the potential influence of third molars on lower incisor crowding. Changes in dental arch dimensions were assessed in 47 patients (36 females, 11 males) three years following either removal of the third molars or deciding to leave them in situ. A dental pantomogram (DPT) was taken at the start of the study and plaster study models were obtained at both the beginning and end of the observation period. The study models were used to measure crowding, and to prepare photocopies for linear measurements of the dental arch (length and width). Using the DPT, the ratio of retromolar space to the width of the crown of the third molar was calculated using the method originally described by Olive and Basford and later modified by Ganss (Ganss ratio). The results showed that the measurements of crowding and arch length and width had changed in 12 lower and two upper dental arches. The relationship between these results and the Ganss ratio was statistically significant. Calculation of the Ganss ratio may therefore assist investigations into the development of dental arch crowding and also help determine the indications for third molar removal.

The aim of this study was to investigate the differences between two groups of patients with either erupted or impacted mandibular third molars before and after orthodontic treatment. All patients were treated with Begg appliances following extraction of the four first premolars. The erupted group consisted of 14 subjects and the impacted group 13 subjects. Lateral cephalometric films, peri-apical radiographs, orthopantomographs and orthodontic casts of each subject were taken before and after treatment. Thirteen measurements were carried out on the lateral cephalometric radiographs. The results revealed slightly more vertical growth and a smaller mandibular arc angle in the erupted group at pre-treatment, and also a more upright lower third molar position and slightly greater distance between the distal point of the lower second molars and the centre of ramus (Xi) in the same group. In the course of treatment, mandibular third molars seemed less likely to erupt when the condyle grew vertically. The lesser resorption on the anterior border of the ramus might have played a part in this finding. In addition, a greater mesial inclination of the mandibular third molars might be an indication of the tendency for these teeth to be impacted in the present study.
