Abstract

This article discusses the management of splenic arterial bleeding as an intraprocedural complication during pseudocyst drainage using LAMS (lumen-apposing metal stents). The case study involves a 56-year-old male with a pancreatic pseudocyst who experienced abdominal pain. During the LAMS release, significant hemorrhagic content and actively bleeding vascular structure were observed. An urgent angio-CT scan revealed active arterial bleeding from the splenic artery, leading to radioembolization. The article emphasizes the rarity and life-threatening nature of this complication, highlighting the importance of performing this technique exclusively in reference centers. Abstract

This article discusses the development of an artificial intelligence (AI) algorithm for the evaluation of pancreatic cystic lesions (PCLs) during endoscopic ultrasound (EUS). PCLs are increasingly common, and distinguishing between mucinous PCLs (M-PCLs) and non-mucinous PCLs (NM-PCLs) is important for patient management. The study used a multicenter approach, involving two tertiary centers in Portugal and Spain, and developed a convolutional neural network (CNN) based on 42 EUS exams. The CNN achieved high levels of accuracy, sensitivity, and specificity in distinguishing between M-PCLs and NM-PCLs. Abstract

This article discusses the use of artificial intelligence (AI) algorithms applied to digital single-operator cholangioscopy (D-SOC) for the diagnosis and characterization of biliary strictures. The study involved D-SOC exams from three centers in Portugal, Spain, and the United States. The AI model achieved an overall accuracy of 94.1% in detecting malignant biliary strictures, with a sensitivity of 93.5% and a specificity of 94.8%. The model also showed good performance in detecting morphological features associated with malignancy. Abstract

This article discusses the development of a deep learning model for the automatic differentiation of solid pancreatic lesions using endoscopic ultrasound (EUS). The model was trained using a dataset of 27,756 images from 107 EUS exams conducted in two specialized centers. Abstract

Introduccion: La enteropatia por AINE de intestino delgado es una entidad de creciente importancia que cada vez se diagnostica con mas frecuencia en pacientes con hemorragia digestiva de origen oscuro (HDOO), sin conocerse su incidencia ni factores de riesgo. Objetivo: Analizar los factores de riesgo de HDOO en pacientes con consumo cronico de AINE. Material y Metodo: Se han incluido 37 pacientes con enteropatia por AINE detectada por capsula endoscopica entre los anos 2008 – 2015 en nuestro centro, que presentaban artrosis (n= 27,72.9%), artritis reumatoide (n= 5,13.5%) y otros (n= 5,13.5%). Se seleccionaron dos controles por caso, de forma randomizada, entre los pacientes consecutivos con las mismas patologias de base y consumo cronico de AINES que consultaron en el Servicio de Reumatologia entre enero y marzo del 2015. Tres controles fueron excluidos por antecedentes de HDOO. Se realizo un analisis multivariante mediante una regresion logistica binaria para establecer los factores asociados al riesgo de hemorragia. Resultados: Un total de 108 pacientes (37 casos con HDOO y 71 controles sin HDOO) fueron incluidos. Ambos grupos fueron similares en sexo femenino (67.6% vs. 61.4%, p= 0.57) y consumo de inhibidores de la bomba de protones (IBP, 71.9% vs. 55.5%, p= 0.121). En analisis multivariante, la diabetes [OR: 3.25. (IC 95%: 1.05 – 10.08), p = 0.041], la enfermedad coronaria [OR: 6.23, (IC 95%: 1.46 – 26.5), p = 0.013] y el consumo de aspirina [OR: 17.02, (IC 95%: 4.28 – 67.63), p < 0.001] se demostraron factores de riesgo independientes para HDOO. La hipertension arterial (p = 0.87), asi como el consumo de clopidogrel (p = 0.7), anticoagulantes orales (p = 0.29) e IBP (p = 0.85) no se relacionaron con un mayor riesgo de sangrado. Conclusiones: Los pacientes consumidores cronicos de AINE tienen mayor riesgo de desarrollar un cuadro de HDOO secundaria si son diabeticos, presentan enfermedad coronaria y/o consumo de aspirina. Por tanto, su indicacion debe realizarse con cautela en estos casos para prevenir el riesgo de hemorragia por enteropatia de intestino delgado.
