We conducted a prospective study to evaluate the vertebrobasilar system in adults with and without obstructive sleep apnea (OSA). Our study population was made up of 48 patients with OSA and 21 healthy volunteers who served as controls; the OSA patients were subdivided into one group with mild or moderate OSA (n = 22) and another with severe OSA (n = 26). Each participant underwent Doppler ultrasonography three times to measure the diameter of the vertebral artery, the peak systolic velocity (PSV), the resistive index (RI), and the vertebral artery flow volume; the mean of the three measurements was calculated for each patient, for the OSA and control groups, and for various subgroups. No significant differences in vessel diameter, PSV, or RI were seen among any of the subgroups. Overall, the vertebral artery flow volume was slightly, but not significantly, higher in all patients with OSA (206 ml/min) than in the control group (177 ml/min); this difference might reflect the body's daytime response to the chronic apneic events experienced during sleep. The only statistically significant difference we found was in vertebral artery flow volume between the controls and the subgroup with mild or moderate OSA (p = 0.026); no difference was seen between the controls and the patients with severe OSA (p = 0.318). Likewise, no significant difference in any of the four parameters was seen when patients were subclassified by body mass index and arterial oxygen saturation level.

Piriform sinus tumors are uncommon and silent lesions. Their prognosis is poor because these tumors are usually not detected until they have reached an advanced stage. Almost all piriform sinus cancers are primary squamous cell carcinomas; other primary and metastatic tumors of the hypopharynx are exceedingly rare. One of the rare tumors in the laryngopharyngeal area is sarcomatoid carcinoma, which is an unusual type of squamous cell carcinoma. Another uncommon malignant tumor that is histologically similar to sarcomatoid carcinoma is malignant mesothelioma, which is a rare form of lung carcinoma. The macroscopic appearance and histologic characteristics of sarcomatoid carcinoma and malignant mesothelioma are so similar that differentiation is usually achieved by immunohistochemical examination. To the best of our knowledge, no case of primary or metastatic laryngohypopharyngeal malignant mesothelioma has been previously reported in the literature. In this article, we describe a case of isolated malignant mesothelioma of the piriform sinus that resembled a sarcomatoid carcinoma in a 50-year-old man with a history of lung mesothelioma.

Hyponatremia develops as a result of the inappropriate secretion of antidiuretic hormone. In rare cases, it develops as an iatrogenic complication. For example, acute iatrogenic post-tonsillectomy hyponatremia has been described in children following the infusion of hypo- or isotonic fluid. We report a case of rapidly developing post-tonsillectomy iatrogenic hyponatremia in a 5-year-old girl following an excessive infusion of hypotonic fluid. Her signs and symptoms began with nausea and vomiting and progressed to seizures and coma. We corrected the electrolyte disturbance by infusing a 3% sodium chloride solution until her neurologic manifestations disappeared, at which time her serum sodium concentration had risen back to 135 mEq/L. Otolaryngologists are not generally exposed to much information about hyponatremia, so we must be aware of its associated neurologic signs and symptoms.

We conducted a prospective study of 50 adults to investigate changes in nasal mucociliary clearance following endoscopic intranasal dacryocystorhinostomy (DCR). Each operation was performed with the use of a drill to perforate the lateral nasal wall in order to reach the lacrimal sac. Each patient's mucociliary clearance time (MCT) was determined by the saccharin test on both the operated and nonoperated sides; the preoperative times were compared with postoperative measurements made 1 and 3 months following DCR. Preoperatively, the mean MCTs were 14.48 minutes on the operated side and 14.50 minutes on the nonoperated side-not a statistically significant difference (p > 0.05). Postoperatively, the corresponding MCTs were 25.22 and 14.98 minutes at 1 month and 18.44 and 15.62 minutes at 3 months. On the operated side, both postoperative MCTs were significantly longer than the preoperative value (p < 0.001), while on the nonoperated side, neither postoperative MCT was significantly different from the preoperative value (p > 0.05). Also, the postoperative MCTs were significantly longer on the operated side than on the nonoperated side at both 1 month (p < 0.001) and 3 months (p < 0.05). Only 1 of the 50 operations was considered to be unsuccessful; the reason for this surgical failure was the patient's ongoing complaints of chronic postoperative dacryocystitis. We conclude that endoscopic DCR disturbs mucociliary clearance. Changes in MCTs may be attributable to injury to the nasal mucosa or to the continuous flow of tears through the nasal mucosa.
