Introduction: While gas narcosis is familiar to most divers conducting deep (> 30 metres) dives, its effects are often considered minuscule or subtle at 30 metres. However, previous studies have shown that narcosis may affect divers at depths usually considered safe from its influence, but little knowledge exists on the effects of gas narcosis on higher cognitive functions such as decision-making in relatively shallow water at 30 metres. Impaired decision-making could be a significant safety issue for a multitasking diver. Methods: We conducted a study exploring the effects of gas narcosis on decision-making in divers breathing compressed air underwater. The divers (n = 22) were evenly divided into 5-metre and 30-metre groups. In the water, we used underwater tablets equipped with the Iowa Gambling Task (IGT), a well-known psychological task used to evaluate impairment in decision-making. Results: The divers at 30 metres achieved a lower score (mean 1,584.5, standard deviation 436.7) in the IGT than the divers at 5 metres (mean 2,062.5, standard deviation 584.1). Age, body mass index, gender, or the number of previous dives did not affect performance in the IGT. Conclusions: Our results suggest that gas narcosis may affect decision-making in scuba divers at 30 metres depth. This supports previous studies showing that gas narcosis is present at relatively shallow depths and shows that it may affect higher cognitive functions. [ABSTRACT FROM AUTHOR]

Introduction: Air flow in full-face snorkel masks (FFSMs) should be unidirectional to prevent rebreathing of exhaled air. This study evaluated rebreathing and its consequences when using FFSMs compared to a conventional snorkel. Methods: In a dry environment 20 participants wore three types of snorkel equipment in random order: Subea Easybreath FFSM; QingSong 180-degree panoramic FFSM; and a Beuchat Spy conventional snorkel (with nose clip), in three conditions: rest in a chair; light; and moderate intensity exercise on a cycle ergometer. Peripheral oxygen saturation, partial pressure of carbon dioxide (PCO2) and oxygen (PO2) in the end tidal gas and FFSM eye-pockets, respiratory rate, minute ventilation, were measured continuously. Experiments were discontinued if oxygen saturation dropped below 85%, or if end-tidal CO2 exceeded 7.0 kPa. Results: Experimental runs with the FFSMs had to be discontinued more often after exceeding 7.0 kPa end-tidal CO2 compared to a conventional snorkel e.g., 18/40 (45%) versus 4/20 (20%) during light intensity exercise, and 9/22 (41%) versus 3/16 (19%) during moderate intensity exercise. Thirteen participants exhibited peripheral oxygen saturations below 95% (nine using FFSMs and four using the conventional snorkel) and five fell below 90% (four using FFSMs and one using the conventional snorkel). The PCO2 and PO2 in the eye-pockets of the FFSMs fluctuated and were significantly higher and lower respectively than in inspired gas, which indicated rebreathing in all FFSM wearers. Without an adequate supply of oxygen from the scuba apparatus, humans would not be able to dive. The air normally contained in a scuba tank is dry and free of toxic gases. The presence of liquid in the tank can cause corrosion and change the composition of the gas mixture. Various chemical reactions consume oxygen, making the mixture hypoxic. We report two cases of internal corrosion of a scuba cylinder rendering the respired gas profoundly hypoxic and causing immediate hypoxic loss of consciousness in divers. [ABSTRACT FROM AUTHOR]

Introduction: Venous gas emboli (VGE) are widely used as a surrogate endpoint instead of decompression sickness (DCS) in studies of decompression procedures. Peak post-dive VGE grades vary widely following repeated identical dives but little is known about how much of the variability in VGE grades is proportioned between-diver and within-diver. Methods: A retrospective analysis of 834 man-dives on six dive profiles with post-dive VGE measurements was conducted under controlled laboratory conditions. Among these data, 151 divers did repeated dives on the same profile on two to nine occasions separated by at least one week (total of 693 man-dives). Data were analysed for between- and within-diver variability in peak post-dive VGE grades using mixed-effect models with diver as the random variable and associated intraclass correlation coefficients. Results: Most divers produced a wide range of VGE grades after repeated dives on the same profile. The intraclass correlation coefficient (repeatability) was 0.33 indicating that 33% of the variability in VGE grades is between-diver variability; correspondingly, 67% of variability in VGE grades is within-diver variability. DCS cases were associated with an individual diver's highest VGE grades and not with their lower VGE grades. Conclusions: These data demonstrate large within-diver variability in VGE grades following repeated dives on the same dive profile and suggest there is substantial within-diver variability in susceptibility to DCS. Introduction: To develop the diving capacity in the Swedish armed forces the current air decompression tables are under revision. A new decompression table named SWEN21 has been created to have a projected risk level of 1% for decompression sickness (DCS) at the no stop limits. The aim of this study was to evaluate the safety of SWEN21 through the measurement of venous gas emboli (VGE) in a dive series. Methods: A total 154 dives were conducted by 47 divers in a hyperbaric wet chamber. As a proxy for DCS risk, serial VGE measurements by echocardiography were conducted and graded according to the Eftedal-Brubakk scale. Measurements were made every 15 minutes for approximately 2 hours after each dive. Peak VGE grades for the different dive profiles were used in a Bayesian approach correlating VGE grade and risk of DCS. Symptoms of DCS were continually monitored. Results: The median (interquartile range) peak VGE grade after limb flexion for a majority of the time-depth combinations, and of SWEN21 as a whole, was 3 (3-4) with the exception of two decompression profiles which resulted in a grade of 3.5 (3-4) and 4 (4-4) respectively. The estimated risk of DCS in the Bayesian model varied between 4.7-11.1%. Three dives (2%) resulted in DCS. All symptoms resolved with hyperbaric oxygen treatment. Conclusions: This evaluation of the SWEN21 decompression table, using bubble formation measured with echocardiography, suggests that the risk of DCS may be higher than the projected 1%. Introduction: In 2018, the Belgian Defence introduced a commercial off-the-shelf dive computer (Shearwater Perdix™) for use by its military divers. There were operational constraints when using its default gradient factors (GF). We aimed to provide guidelines for optimal GF selection. Methods: The Defence and Civil Institute of Environmental Medicine (DCIEM) dive tables and the United States Navy (USN) air decompression tables are considered acceptably safe by the Belgian Navy Diving Unit. The decompression model used in the Shearwater Perdix (Bühlmann ZH-L16C algorithm with GF) was programmed in Python. Using a sequential search of the parameter space, the GF settings were optimised to produce decompression schedules as close as possible to those prescribed by the USN and DCIEM tables. Results: All reference profiles are approached when GFLO is kept equal to 100 and only GFHI is reduced to a minimum of 75 to prolong shallower stop times. Using the Perdix default settings (GFLO = 30 and GFHI = 70) yields deeper initial stops, leading to increased supersaturation of the 'slower' tissues, which potentially leads to an increased DCS risk. However, Perdix software does not currently allow for the selection of our calculated optimal settings (by convention GFLO < GFHI). A sub-optimal solution would be a symmetrical GF setting between 75/75 and 95/95. Conclusions: For non-repetitive air dives, the optimal GF setting is GFLO 100, with only the GFHI parameter lowered to increase safety. No evidence was found that using the default GF setting (30/70) would lead to a safer decompression for air dives as deep as 60 metres of seawater; rather the opposite. Belgian Navy divers have been advised against using the default GF settings of the Shearwater Perdix dive computer and instead adopt symmetrical GF settings which is currently the optimal achievable approach considering the software constraints. Introduction: This study aimed to develop a standard process and checklist for technical investigation of hookah diving equipment and apply it to Tasmanian hookah fatality investigations from the last 25 years. Methods: A literature search was undertaken to identify technical reports and equipment investigations associated with diving accidents. The information was assimilated to create a process and checklist for specifically assessing the hookah apparatus. The checklist was then applied in a gap analysis of Tasmanian hookah diving fatality technical reports from 1995 to 2019. Results: As no papers specifically describing technical evaluation of hookah equipment were identified, references evaluating scuba equipment were used to create a hookah technical assessment process incorporating unique features of the hookah. Features included: owner responsibility for air quality; maintenance, function; exhaust proximity to air intake; reservoir volume; output non-return valves; line pressure; sufficiency of supply; entanglement; hose severance risk; gas supply failure and hosing attachment to the diver. Seven hookah diving deaths occurred in Tasmania (1995−2019) of which three had documented technical assessment. Gap analysis identified inconsistent structure between reports with variability in the case descriptors. Missing technical data included: overview of the hookah systems; accessories; weights; how the apparatus was worn by the diver; compressor suitability; assessment of hookah function; breathing gas output and exhaust position relative to air intake. Conclusions: The study demonstrated a need to standardise technical reporting of hookah equipment after diving accidents. The checklist generated may serve as a resource for future hookah assessments and inform strategies for preventing future hookah accidents. [ABSTRACT FROM AUTHOR]

Introduction: Breath-hold (BH) diving has known risks, for example drowning, pulmonary oedema of immersion and barotrauma. There is also the risk of decompression illness (DCI) from decompression sickness (DCS) and/or arterial gas embolism (AGE). The first report on DCS in repetitive freediving was published in 1958 and from then there have been multiple case reports and a few studies but no prior systematic review or meta-analysis. Methods: We undertook a systematic literature review to identify articles available from PubMed and Google Scholar concerning breath-hold diving and DCI up to August 2021. Results: The present study identified 17 articles (14 case reports, three experimental studies) covering 44 incidences of DCI following BH diving. Conclusions: This review found that the literature supports both DCS and AGE as potential mechanisms for DCI in BH divers; both should be considered a risk for this cohort of divers, just as for those breathing compressed gas while underwater. Introduction: Closed-circuit rebreathers (CCRs) are designed to be watertight. Ingressing water may react with carbondioxide absorbent in the CCR, which may produce alkaline soda with a pH of 12–14, popularly referred to by CCR divers as a ‘caustic cocktail’. This study aimed to explore divers’ responses to caustic cocktail events and to investigate if CCR diving experience is associated with experiencing a caustic cocktail. Methods: An online survey instrument was developed and an invitation to participate was extended to certified CCR divers aged ≥ 18 years. Relationships between number of caustic cocktail events and potential risk factors: age; hours of rebreather diving experience; and number of rebreather dives were explored. Results: Of the 413 respondents, 394 (95%) identified as male, mean age was 46 years and median length of CCR certification was six years. Fifty-seven percent (n = 237) of respondents reported having experienced a caustic cocktail. The probability of self-reporting none, one, or more caustic cocktail events increased with experience. Divers reported a variety of first aid treatments for caustic cocktails, with ~80% citing their CCR instructor as a source of information. Conclusions: The more hours or dives a CCR diver accrues, the more likely they will self-report having experienced one or more caustic cocktail events. The majority of CCR divers responded to a caustic cocktail by rinsing the oral cavity with water. A proportion of divers, however, responded by ingesting soda, dairy, juice, or a mildly acidic solution such as a mixture of vinegar and water. Pulmonary barotrauma may occur in diving and can result in a spectrum of injuries referred to as pulmonary over-inflation syndrome (POIS). Pneumomediastinum is a part of the POIS spectrum and only rarely results in respiratory symptoms. We present a case of a civilian diver who developed pneumomediastinum with respiratory symptoms which did not respond to normobaric 100% oxygen. After investigation for pneumothorax, he underwent hyperbaric oxygen treatment which resulted in significant alleviation of his symptoms. This is a novel case example of this treatment algorithm. A diver returned to diving, 15 months after an episode of neuro-spinal decompression sickness (DCS) with relapse, after which she had been found to have a moderate to large provoked shunt across a persistent (patent) foramen ovale (PFO), which was not closed. She performed a single highly conservative dive in line with the recommendations contained in the 2015 position statement on PFO and diving published jointly by the South Pacific Underwater Medicine Society and the United Kingdom Sports Diving Medical Committee. An accidental Valsalva manoeuvre shortly after surfacing may have provoked initial symptoms which later progressed to DCS. Her symptoms and signs were milder but closely mirrored her previous episode of DCS and she required multiple hyperbaric oxygen treatments over several days, with residua on discharge. Although guidance in the joint statement was mostly followed, the outcome from this case indicates that there may be a subgroup of divers with an unclosed PFO, who have had a previous episode of serious DCS, who may not be safe to dive, even within conservative limits. Introduction: This study identified characteristics and diving practices of victims of fatal surface supplied breathing apparatus (SSBA) incidents in Australia from 1965--2019 to determine underlying factors and risks associated with these activities, better educate the diving community and prevent such deaths. Methods: A hand search was made of 'Project Stickybeak' reports from 1965-2000 and SSBA fatality data were compared to the Australasian Diving Safety Foundation fatality database. The National Coronial Information System was searched to identify SSBA diving deaths for 2001-2019. Extracted data were collated and analysed using descriptive statistics and Poisson Regression. A chain of events analysis was used to determine the likely sequence of events. Results: There were 84 identified SSBA-related deaths during the study period. Most victims were relatively young, healthy males (median age 33 years). At least 50% of victims were undertaking work-related diving, and 37% were recreational diving. Equipment issues, mainly compressor-related, were the main contributor, identified as a predisposing factor in 48% of incidents and as triggers in 24%. Conclusions: Preventable surface-supplied diving deaths still occur in both occupational and recreational diving, often from poor equipment maintenance and oversight. Incorrect configuration of the SSBA and lack of training remain on-going problems in recreational users. These could be addressed by improved education, and, failing this, regulatory oversight. The increase in health-related incidents in older participants may be controlled to some extent by greater medical oversight, especially in recreational and non-certified occupational divers who should be encouraged to undergo regular diving medical assessments. Introduction: Critical flicker fusion frequency (CFFF) has been used in various studies to measure the cognitive effects of gas mixtures at depth, sometimes with conflicting or apparently paradoxical results. This study aimed to evaluate a novel automatic CFFF method and investigate whether CFFF can be used to monitor gas-induced narcosis in divers. Methods: Three hyperbaric chamber experiments were performed: 1) Automated and manual CFFF measurements during air breathing at 608 kPa (n = 16 subjects); 2) Manual CFFF measurements during air and heliox breathing at sea level (101.3 kPa) and 608 kPa (n = 12); 3) Manual CFFF measurements during oxygen breathing at sea level, 142 and 284 kPa (n = 10). All results were compared to breathing air at sea level. Results: Only breathing oxygen at sea level, and at 284 kPa, caused a significant decrease in CFFF (2.5% and 2.6% respectively compared to breathing air at sea level. None of the other conditions showed a difference with sea level air breathing. Conclusions: CFFF did not significantly change in our experiments when breathing air at 608 kPa compared to air breathing at sea level pressure using both devices. Based on our results CFFF does not seem to be a sensitive tool for measuring gas narcosis in divers in our laboratory setting. [ABSTRACT FROM AUTHOR]

The South Pacific Underwater Medicine Society (SPUMS) diving medical for recreational scuba divers was last reviewed in 2011. From 2011 to 2019, considerable advancements have occurred in cardiovascular risk assessment relevant to divers. The SPUMS 48th (2019) Annual Scientific Meeting theme was cardiovascular risk assessment in diving. The meeting had multiple presentations updating scientific information about assessing cardiovascular risk. These were distilled into a new set of guidelines at the final conference workshop. SPUMS guidelines for medical risk assessment in recreational diving have subsequently been updated and modified including a new Appendix C: Suggested evaluation of the cardiovascular system for divers. The revised evaluation of the cardiovascular system for divers covers the following topics: 1. Background information on the relevance of cardiovascular risk and diving; 2. Defining which divers with cardiovascular problems should not dive, or whom require treatment interventions before further review; 3. Recommended screening procedures (flowchart) for divers aged 45 and over; 4. Assessment of divers with known or symptomatic cardiovascular disease, including guidance on assessing divers with specific diagnoses such as hypertension, atrial fibrillation, cardiac pacemaker, immersion pulmonary oedema, takotsubo cardiomyopathy, hypertrophic cardiomyopathy and persistent (patent) foramen ovale; 5. Additional cardiovascular health questions included in the SPUMS guidelines for medical risk assessment in recreational diving; 6. Updated general cardiovascular medical risk assessment advice; 7. Introduction: This study compared the thermal balance of spinal cord injured (SCI) divers and able-bodied (AB) divers during recreational cold-water dives. Methods: Ten divers (5 AB, 5 SCI) in matched pairs dived in a shallow lake (temperature 6°C) for 30 to 36 min wearing 5 mm ‘Long John’ neoprene wetsuits. A gastrointestinal temperature radio pill recorded gastro-intestinal temperature (Tgi) prior to, immediately after and at 5, 10, 15, 30, 60, 120 min post-dive. Subjective ratings of temperature perception were recorded concomitantly using a visual analogue scale (VAS). Results: No difference between SCI and AB divers in Tgi before the dive was observed (P = 0.85). After the dive, SCI divers cooled significantly more than AB at all measured time intervals (P < 0.001). Post dive, the mean maximum fall in Tgi during the recovery phase in SCI divers was 0.85°C (SD 0.20) and in the AB group was 0.48°C (0.48). In addition, there was greater individual variation in SCI divers compared to AB divers. There were no statistically significant differences in temperature perception between the groups either before or at any time after the dives. Conclusions: In contrast to AB divers, divers with SCI were unable to maintain Tgi during short shallow dives in 6°C water and their temperatures fell further post-dive. The reduction in Tgi was not reflected in the subjective ratings of temperature perception by the SCI divers. Introduction: Gas narcosis impairs divers when diving deeper. Pupillometry is sensitive to alcohol intoxication and it has been used in anaesthesia to assess nitrous oxide narcosis. It is a potential novel method to quantify narcosis in diving. The aim of this study was to evaluate pupillometry for objective measurement of narcosis during exposure to hyperbaric air or nitrous oxide. Method: Pupil size in 16 subjects was recorded directly at surface pressure and during air breathing at 608 kPa (equivalent to 50 metres’ seawater depth) in a hyperbaric chamber. Another 12 subjects were exposed to nitrous oxide at end-tidal percentages of 20, 30 and 40% in random order at surface pressure. Pupil size and pupil light reflex were recorded at baseline and at each level of nitrous oxide exposure. Results: Pupil size did not significantly change during exposure to hyperbaric air or nitrous oxide. The pupil light reflex, evaluated using percentage constriction and minimum diameter after exposure to a light stimulus, was affected significantly only during the highest nitrous oxide exposure – an end-tidal level of 40%. Conclusion: Pupillometry is insensitive to the narcotic effect of air at 608 kPa in the dry hyperbaric environment and to the effects of low dose nitrous oxide. Introduction: In 2018 12 children and one adult were anaesthetised before being extricated through over a kilometre of flooded cave in Thailand. Full face dive masks (FFMs) putatively capable of maintaining constant positive airway pressure (CPAP) were employed. Here we describe the anaesthetic intervention and investigate the CPAP capability of the FFM. Methods: Pressure was measured inside and outside the Interspiro Divator FFM during 10 tidal and 10 vital capacity breaths in divers at the surface and submerged with the mask deployed on open-circuit scuba (10 divers); and a closed-circuit rebreather (five divers). Relative in-mask pressure was calculated as the difference between inside and outside pressures. We also measured the in-mask pressure generated by activation of the second stage regulator purge valve in open-circuit mode. Results: When submerged in open-circuit mode the mean relative in-mask pressure remained positive in normal tidal breathing (inhalation 0.6 kPa [95% CI 0.3–0.9]; exhalation 1.1 [0.8–1.4]) and vital capacity breathing (inhalation 0.8 [0.4–1.1]; exhalation 1.2 [0.9–1.4]). As expected, the relative in-mask pressure was predominantly negative when used on closed-circuit with back mounted counter-lungs due to a negative static lung load. Mean in-mask pressure during purge valve operation was 3.99 kPa (approximately equal to 40 cmH2O) (range: 2.56 to 5.3 kPa). Conclusions: The CPAP function of the Interspiro Divator FFM works well configured with open-circuit scuba. This may have contributed to the success of the Thailand cave rescue. Introduction: This study identified characteristics of victims of fatal scuba diving incidents to determine contributing factors and inform appropriate countermeasures. Methods: The National Coronial Information System (NCIS) was searched to identify scuba diving deaths for 2001–2013, inclusive. Data were extracted from witness and police reports, medical histories and autopsies. Descriptive statistics were used to analyse these data. Results: There were 126 scuba diving-related fatalities identified during the study period. The mean age was 44 years, 99 (79%) victims were male and 83 (77%) were either overweight or obese. Most deaths occurred in New South Wales and Queensland, often in a commercial setting. Twenty-three (79%) Queensland victims were overseas tourists. At least 52 (41%) were novices and 17 (13%) died during training or an introductory scuba experience. Only 35 (28%) were with a buddy when the incident occurred and at least 81 (64%) were still wearing weights when recovered. Conclusions: The age of these victims may reflect an older cohort of participants and the associated higher prevalence of chronic medical conditions. The high prevalence of obesity suggests that this may be a risk factor. The high proportion of deaths in overseas tourists highlights an on-going need for appropriate screening and monitoring in what may be a higher risk cohort. The number of deaths that occurred under instruction highlights the importance of careful assessment of the site, prevailing conditions, an appropriate instructor-student ratio and close supervision. Introduction: Divers with suspected decompression illness require high concentration oxygen (O2). There are many different O2 delivery devices, with few data comparing their performance. This study evaluated O2 delivery, using tissue O2 partial pressure (PtcO2), in healthy divers breathing O2 via three different delivery devices. Methods: Twelve divers had PtcO2 measured at six limb sites. Participants breathed O2 from: a demand valve using an intraoral mask with a nose clip (NC); a medical O2 rebreathing system (MORS) with an oronasal mask and with an intraoral mask; and a non-rebreather mask (NRB) at 15 or 10 L·min-1 O2 flow. In-line inspired O2 (FIO2) and nasopharyngeal FIO2 were measured. Participants provided subjective ratings of device comfort, ease of breathing, and overall ease of use. Results: PtcO2 values and nasopharyngeal FIO2 were similar with the demand valve with intraoral mask, MORS with both masks and the NRB at 15 L·min-1. PtcO2 and nasopharyngeal FIO2 values were significantly lower with the NRB at 10 L·min-1. The NRB was rated as the most comfortable to wear, easiest to breathe with, and overall the easiest to use. Conclusion: Of the commonly available devices promoted for O2 delivery to injured divers, similar PtcO2 and nasopharyngeal FIO2 values were obtained with the three devices tested: MORS with an oronasal or intraoral mask, demand valve with an intraoral mask and NRB at a flow rate of 15 L·min-1. PtcO2 and nasopharyngeal FIO2 values were significantly lower when the flow rate using the NRB was decreased to 10 L·min-1. [ABSTRACT FROM AUTHOR]

Introduction: Hearing loss (HL) is common in the adult working population. It is widely assumed that diving is a risk factor for HL. However, studies with sufficient follow-up comparing HL in divers to non-divers are limited. This study aimed to assess the hearing threshold (HT) of Royal Netherlands Navy divers who had been diving for more than 15 years and to compare it to the ISO standard 7029:2017 reference table. Methods: In this 25-year retrospective cohort study the Royal Netherlands Navy Diving Medical Centre audited the medical records of 1,117 Navy divers. Yearly dive medical assessments were performed according to professional standards, including audiometry. HTs were compared to the ISO 7029:2017 reference table, including Z-distribution, using paired t-tests. Results: Thirty-five divers were included who had been diving for 15 years or longer. The HT increased significantly in nine of the 16 measured frequencies, while the Z-score decreased significantly in nine of the 16 tested frequencies (eight in both ears). In the 25-year follow-up the pattern was more obvious, with one significantly increased HT, and 10 significantly decreased Z-scores. Discussion: The absolute HT increases after 15 years of military diving, but less than would be expected from normal age-related deterioration. Moreover, when comparing Z-scores, this sample of divers actually hear better than non-divers. We conclude that military diving is not an increased risk for HL compared to regular occupational hazards and suggest withdrawing the requirement for routine yearly audiometric evaluation as part of a dive medical examination. Introduction: This is the first published study on decompression illness (DCI) and its treatment in Finland. Diving conditions are demanding, as even in the summer the water temperature below 20 meters' sea/fresh water (msw/mfw) is 4-10°C. Technical diving has become more popular over the years, so the emphasis of this study was to describe DCI in technical divers and compare it with non-technical recreational divers. Methods: This study includes by estimation over 95% of all hyperbaric oxygen-treated DCI patients during the years 1999-2018 (n = 571). The cases were divided into technical divers (n = 200) and non-technical divers (n = 371). We focused on the differences between these two groups. Technical diving was defined as the usage of mixed breathing gases, closed circuit rebreather diving or planned decompression diving. Results: The mean annual number of treated DCI cases in Finland was 29 (range 16-38). The number of divers treated possibly indicate a shift towards technical diving. Technical dives were deeper and longer and were mainly performed in cold water or an overhead environment. Technical divers were more likely to utilize first aid 100% oxygen (FAO2) and sought medical attention earlier than non-technical divers. Symptom profiles were similar in both groups. Recompression was performed using USN Treatment Table Six in the majority of the cases and resulted in good final outcome. Eighty two percent were asymptomatic on completion of all recompression treatment(s). Conclusion: This 20-year observational study indicates a shift towards technical diving, and hence a more demanding and challenging style of diving among Finnish divers, with a surprisingly constant number of DCI cases over the years. Background: Physiological changes are induced by immersion, swimming and using diving equipment. Divers must be fit to dive. Using medication may impact the capacity to adapt to hyperbaric conditions. The aim of this systematic review is to assess the interaction of diving/hyperbaric conditions and medication and to provide basic heuristics to support decision making regarding fitness to dive in medicated divers. Methods: This was a systematic review of human and animal studies of medications in the hyperbaric environment. Studies were subdivided into those describing a medication/hyperbaric environment interaction and those concerned with prevention of diving disorders. Studies without a relation to diving with compressed air, and those concerning oxygen toxicity, hyperbaric oxygen therapy or the treatment of decompression sickness were excluded. Results: Forty-four studies matched the inclusion criteria. Animal studies revealed that diazepam and valproate gave limited protection against the onset of the high-pressure neurological syndrome. Lithium had a protective effect against nitrogennarcosis and losartan reduced cardiac changes in repetitive diving. Human studies showed no beneficial or dangerous pressure-related interactions. In prevention of diving disorders, pseudoephedrine reduced otic barotrauma, vitamins C and E reduced endothelial dysfunction after bounce diving and hepatic oxidative stress in saturation diving. Discussion and Conclusions: Animal studies revealed that psycho-pharmaceuticals can limit the onset of neurologic symptoms and cardiovascular protective drugs might add a potential protective effect against decompression sickness. No evidence of significant risks due to changes in pharmacologic mechanisms were revealed and most medication is not a contraindication to diving. For improving decision making in prescribing medicine for recreational and occupational divers and to enhance safety by increasing our understanding of pharmacology in hyperbaric conditions, future research should focus on controlled human studies. Background: Aircrew training often includes an hypoxic experience aimed at improving symptom recognition and selfrescue in a subsequent hypoxic event. Similar training has been advocated for rebreather divers. We investigated the effect of a prior hypoxic experience on actual and perceived cognitive function during subsequent hypoxia and measured the physiological responses to severe progressive hypoxia. Methods: Twenty-five subjects underwent two hypoxic hypoxia experiences (trials one and two) approximately five weeks apart. Subjects breathed 5.5% oxygen whilst performing a playing card recognition test. The primary endpoint was the time taken to make three consecutive errors in the card recognition test (time of useful consciousness, TUC). Secondary endpoints were the total number of errors made, accuracy of error recollection and physiological variables. Results: Mean (SD) TUC was 166 seconds (37) and 169 s (35), and subjects made 8.9 (2.4) and 7.8 (2.0) errors in trials one and two respectively. Error recall was identical between trials with participants failing to recall 6 (3) and 6 (2) errors made in trials one and two respectively. Across both trials mean nadir arterial blood and cerebral oxygen saturations were 52% and 49% respectively. The mean (SD) increase in heart rate was 42 (16) beats·min-1. Conclusion: An hypoxic experience did not improve cognitive performance or subject insight into performance in a second exposure five weeks later. Hypoxia imposes a significant physiological stress which may be hazardous in unscreened, nonmedically supervised subjects. Introduction: Few studies are available to appreciate the broad spectrum of dive-related injuries (DI), which are not limited to decompression illness (DCI) and fatalities. Studies supporting injury-management efficacy from early recognition to first-aid, final treatment and outcome are also lacking. This study aims at making an epidemiologic inventory of DI among French scuba divers. Methods: This online, retrospective, cross-sectional survey analyzed self-reported symptoms, context of occurrence, initial response and outcome. The relationships between symptoms and diver characteristics were assessed and severity scores created from the reports. Results: A total of 799 divers responded, of whose questionnaires 784 were sufficiently complete to be analyzed. Approximately one-third (35%) of respondents had never experienced a DI. DCI-like symptoms represent a small fraction of DIs, the most commonly reported being ear barotrauma. Self-reported symptom rates decreased with increasing age and male sex. The ranking dive leader was the primary care provider in 58% of reports and 32% of injured divers never sought help. Management decisions (first aid and/or hyperbaric oxygen treatment) were related to the severity score. Complete resolution was achieved in 84 (74%) of 114 DCI cases, whilst mild (n = 22, 19%) and severe (n = 8, 7%) residual symptoms were reported. One in 10 divers who did not seek treatment for symptoms believed to be related to DCI declared some residual symptoms. Introduction: Approximately 77% of professional divers leave the industry within five years of entry for reasons that are uncertain. One possibility is that attrition is due to ill-health. The health of New Zealand occupational divers is surveyed by a comprehensive medical examination every five years and by a health questionnaire in the intervening years. Divers are thereby confirmed 'fit' annually. The aim of this study was to determine if divers quit the industry due to a health problem not identified by this health surveillance system. Method: 601 divers who had left the industry within five years of entry medical examination ('quitters') were identified from a computerised database. One hundred and thirty-six who could be contacted were questioned about their principal reason for quitting. Comparison was made between the health data of all those defined as 'quitters' and a group of 436 'stayers' who have remained active in the industry for over 10 years. Results: Health was the principal reason for abandoning a diving career for only 2.9% of quitters. The overwhelming majority (97.1%) quit because of dissatisfaction with aspects of the work, such as remuneration and reliability of employment. Besides gender, the only significant difference between the health data of quitters and stayers was that smoking was four times more prevalent among quitters. Conclusions: The key determinant of early attrition from the New Zealand professional diver workforce is industry-related rather than health-related. Introduction: Persistent (patent) foramen ovale (PFO) is a recognized risk for decompression sickness (DCS) in divers, which may be mitigated by conservative diving or by PFO closure. Our study aimed to compare the effectiveness of these two risk mitigation interventions. Methods: This was a prospective study on divers who tested positive for PFO or an atrial septal defect (ASD) and either decided to continue diving without closure ('conservative group'), or to close their PFO/ASD and continue diving ('closure group'). Divers' characteristics, medical history, history of diving and history of DCS were reported at enrollment and annually after that. The outcome measures were the incidence rate of DCS, frequency and intensity of diving activities, and adverse events of closure. Results: Divers in both groups dived less and had a lower incidence rate of confirmed DCS than before the intervention. In the closure group (n = 42) the incidence rate of confirmed DCS decreased significantly. Divers with a large PFO experienced the greatest reduction in total DCS. In the conservative group (n = 23), the post-intervention decrease in confirmed DCS incidence rate was not significant. Of note, not all divers returned to diving after closure. Seven subjects reported mild adverse events associated with closure; one subject reported a serious adverse event. Conclusions: PFO closure should be considered on an individual basis. In particular, individuals who are healthy, have a significant DCS burden, a large PFO or seek to pursue advanced diving may benefit from closure. [ABSTRACT FROM AUTHOR]

Introduction: This study reviews diving deaths that occurred in Tasmanian waters over a 20-year period. Methods: Detailed analysis was undertaken of deaths that occurred from 01 January 1995 to 31 December 2014. The cases were collated from numerous sources. Utilising a chain of events analysis, factors were identified and assigned to predisposing factors, triggers, disabling agents, disabling injuries and cause of death. These were then scrutinised to ascertain regional variables, remediable factors and linkages which may benefit from targeted risk mitigation strategies. Results: Seventeen deaths were identified across this 20-year period, which included one additional case not previously recorded. All were recreational divers and 15 were male. Five were hookah divers, 12 were scuba divers. Important predisposing factors identified included equipment (condition and maintenance), pre-existing health conditions, diver experience and training. These factors can now be used to promote public health messages for divers. Conclusions: This 20-year study highlighted regional variations for Tasmanian deaths and presents opportunities for strategies to prevent diving deaths in the future. Of particular concern was the diving practice of 'hookah' diving, which has no governing regulations. Introduction: Surveillance of professional divers' hearing is routinely undertaken on an annual basis despite lack of evidence of benefit to the diver. The aim of this study was to determine the magnitude and significance of changes in auditory function over a 10-25 year period of occupational diving with the intention of informing future health surveillance policy for professional divers. Methods: All divers with adequate audiological records spanning at least 10 years were identified from the New Zealand occupational diver database. Changes in auditory function over time were compared with internationally accepted normative values. Any significant changes were tested for correlation with diving exposure, smoking history and body mass index. Results: The audiological records of 227 professional divers were analysed for periods ranging from 10 to 25 years. Initial hearing was poorer than population norms, and deterioration over the observation period was less than that predicted by normative data. Changes in hearing were not related to diving exposure, or smoking history. Conclusion: Audiological changes over 10 to 25 years of occupational diving were not found to be significantly different from age-related changes. Aim: The aim of this study was to identify the number, location and characteristics associated with fatal shark attacks on divers in Australian waters from 1960 to 2017, inclusive. Methods: Searches were made of the Divers Alert Network Asia-Pacific Diving Mortality Database; the International Shark Attack File website; the Australian Shark Attack File and the Global Shark Attack File to identify cases of fatal shark attacks on divers in Australia. In addition, a systematic search of published medical and sporting literature was conducted to identify relevant reports. The data collected were scrutinised for relevance and duplication. Results: There were 187 recorded attacks on divers, comprising 112 snorkellers, 62 scuba divers and 13 on divers using surface-supplied breathing apparatus. These included 28 verified deaths: 13 involving snorkellers, eight involving scuba divers, and seven divers using surface-supply. The victims' ages ranged from 13-50 years (mean 31 years). All but three were males. The vast majority of attacks were by Carcharodon carcharias (Great White Shark). Conclusion: Spearfishing and other seafood collection, as well as diving near fishing activities and/or seals, were identified as major risk factors. However, shark attacks on divers are relatively rare and represent only 3% of diving-related fatalities in Australia over this 57-year period. [ABSTRACT FROM AUTHOR]

Occult diaphragmatic injury was uncovered in a patient who returned to scuba diving after a traumatic injury. Diaphragmatic injury can be a difficult diagnosis in the setting of trauma and a significant number of injuries are missed on the initial presentation. This has potential implications to those wishing to return to diving after trauma, and diving doctors must maintain a high degree of suspicion for such injuries. [ABSTRACT FROM AUTHOR]

Objective: To systematically search the literature for studies evaluating the typical presentation and testing that is performed for divers with inner ear symptoms and then to create a tool for clinicians when evaluating a diver with inner ear symptoms. Methods: Nine databases, including PubMed/MEDLINE were systematically searched through 31 January 2018. The PRISMA statement was followed. Results: Three-hundred and two manuscripts were screened, 69 were downloaded and 21 met criteria to be included in this review. The articles were evaluated for symptomatic trends and initial evaluation work-up primarily focusing on inner-ear barotrauma (IEBt) and inner ear decompression sickness (inner ear DCS). The trends for IEBt were compared to typical inner ear DCS presentation based on large study inner ear DCS results consistent with the plethora of research available. Finally, the HOOYAH Tool was developed to assist the receiving provider to better determine the most likely diagnosis and thus initiate appropriate treatment. The HOOYAH Tool is comprised of the following: 1) H: hard to clear; 2) O: onset of symptoms; 3) O: otoscopic exam; 4) Y: your dive profile; 5) A: additional symptoms and 6) H: hearing. For each of these components, the typical presentation is described allowing the provider better to discern the correct diagnosis. Conclusion: The diagnosis of IEBt remains difficult to define short of visualization through surgical exploration. Early treatment is defined by conservative management with a subsequent observational period to determine symptomatic resolution and need for surgery. However, a similar differential diagnosis is inner ear DCS which requires early recompression. The HOOYAH tool provides a method for assisting the provider in forming a more confident decision regarding the underlying pathology and facilitation of the appropriate treatment. Background: Pressure changes can influence dental restorations especially among divers. The aim of the current study was to evaluate the fracture resistance and microleakage of mesio-occluso-distal (MOD) amalgam and composite restorations undergoing pressure changes via diving and aviation simulation. Methods: For the fracture resistance test, 60 sound maxillary premolar teeth were randomly allocated to two groups. Each group was then divided into three subgroups (n = 10) for simulating scuba-diving (pressure cycle to 203 kPa, 2 bar), flight (50.6 kPa, 0.5 bar), and control (atmospheric pressure). The teeth underwent pressure cycles for one month and then the fracture resistance test was conducted on them using the Instron testing machine. Microleakage scores were afterwards recorded by using a 2% methylene blue dye for 24 hours. Results: Composite restorations showed significantly higher fracture resistance values compared to the amalgam group (P < 0.05). The control group had significantly higher fracture resistance values compared to the dive group, whereas there was no significant difference between the control group and the flight group (P = 0.083). No significant difference in the level of microleakage was observed between restoration materials or pressure cycles (P > 0.05). Conclusions: Composite restorations showed promising fracture resistance compared to the amalgam group. Diving pressure cycles demonstrated adverse effects on the fracture resistance values of the two restorative materials. [ABSTRACT FROM AUTHOR]

Divers suspected of suffering decompression illness (DCI) in locations remote from a recompression chamber are sometimes treated with in-water recompression (IWR). There are no data that establish the benefits of IWR compared to conventional first aid with surface oxygen and transport to the nearest chamber. However, the theoretical benefit of IWR is that it can be initiated with a very short delay to recompression after onset of manifestations of DCI. Retrospective analyses of the effect on outcome of increasing delay generally do not capture this very short delay achievable with IWR. However, in military training and experimental diving, delay to recompression is typically less than two hours and more than 90% of cases have complete resolution of manifestations during the first treatment, often within minutes of recompression. A major risk of IWR is that of an oxygen convulsion resulting in drowning. As a result, typical IWR oxygen-breathing protocols use shallower maximum depths (9 metres' sea water (msw), 191 kPa) and are shorter (1-3 hours) than standard recompression protocols for the initial treatment of DCI (e.g., US Navy Treatment Tables 5 and 6). There has been no experimentation with initial treatment of DCI at pressures less than 60 feet' sea water (fsw; 18 msw; 286 kPa; * see footnote) a since the original development of these treatment tables, when no differences in outcomes were seen between maximum pressures of 33 fsw (203 kPa; 10 msw) and 60 fsw or deeper. These data and case series suggest that recompression treatment comprising pressures and durations similar to IWR protocols can be effective. The risk of IWR is not justified for treatment of mild symptoms likely to resolve spontaneously or for divers so functionally compromised that they would not be safe in the water. However, IWR conducted by properly trained and equipped divers may be justified for manifestations that are life or limb threatening where timely recompression is unavailable. [ABSTRACT FROM AUTHOR]

Guidelines for the pre-hospital management of decompression illness (DCI) had not been formally revised since the 2004 Divers Alert Network/Undersea and Hyperbaric Medical Society workshop held in Sydney, entitled "Management of mild or marginal decompression illness in remote locations". A contemporary review was initiated by the Diver's Alert Network and undertaken by a multinational committee with members from Australasia, the USA and Europe. The process began with literature reviews by designated committee members on: the diagnosis of DCI; first aid strategies for DCI; remote triage of possible DCI victims by diving medicine experts; evacuation of DCI victims; effect of delay to recompression in DCI; pitfalls in management when DCI victims present at hospitals without diving medicine expertise and in-water recompression. This was followed by presentation of those reviews at a dedicated workshop at the 2017 UHMS Annual Meeting, discussion by registrants at that workshop and finally several committee meetings to formulate statements addressing points considered of prime importance to the management of DCI in the field. The committee placed particular emphasis on resolving controversies around the definition of "mild DCI" arising over 12 years of practical application of the 2004 workshop's findings, and on the controversial issue of in-water recompression. In healthy divers, the occurrence of immersion pulmonary oedema (IPE) is commonly caused by contributory factors including strenuous exercise, cold water and negative-pressure breathing. Contrary to this established paradigm, this case reports on a 26-year-old, well-trained combat swimmer who succumbed to acute IPE during static immersion in temperate (21°C) water, while using a front-mounted counterlung rebreather. The incident occurred during repeated depth-controlled ascent practice at the French military diving school. It was discovered that the diver had attempted to stop any gas leakage into the system by over-tightening the automatic diluent valve (ADV) (25th notch of 27) during the dive, thus causing a high resistance to inspiratory flow. The ventilatory constraints imposed by this ADV setting were assessed as a 3.2 Joules·L-1 inspiratory work of breathing and -5 kPa (-50 mbar) transpulmonary pressure. This report confirms the key role of negative pressure breathing in the development of interstitial pulmonary oedema. Such a breathing pattern can cause a lowering of thoracic, airway and interstitial lung pressure, leading to high capillary pressure during each inspiration. Repetition of the diving drills resulted in an accumulation of interstitial lung water extravasation, causing pathological decompensation and proven symptoms. [ABSTRACT FROM AUTHOR]

Introduction: Pulmonary function testing (PFT) is an important part of dive medical examinations. Depending on the standard used to assess fitness to dive, different reference sets and fixed cut-off points are used. Reference values are part of an ongoing debate regarding the validity and accuracy related to different age groups, sex and ethnic backgrounds. The Global Lung Initiative (GLI) has provided an all-age reference set which corrects for sex and ethnicity (GLI-2012); this has had substantial impact on pulmonary medicine. Method: We present an algorithm that can be used to standardise analysis of PFT in divers using the GLI-2012 reference set. Differences in the analysis of PFT between the ECSC/ERS-1993 and the GLI-2012 reference values are illustrated by means of three case reports. Conclusion: Using a valid database of reference values increases accuracy and might prevent additional medical investigations and/or incorrect assessment of fitness to dive. Although our algorithm needs further evaluation to ensure its validity, the preliminary results are promising. Whatever algorithm is used, we urge dive medical physicians to consider using valid reference sets when analysing PFT for assessment of fitness to dive. [ABSTRACT FROM AUTHOR]

Aim: To determine whether long-term engagement in occupational diving causes significant changes in spirometric measurements. Method: All divers with adequate spirometric records spanning at least 10 years were identified from the New Zealand occupational diver database. Changes in lung function over time were compared with normative values derived using published prediction equations. Any significant changes were tested for correlation with age, duration of occupational diving, gender, smoking history and body mass index (BMI). Results: Spirometry data spanning periods of 10 to 25 years were analysed for 232 divers. Forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) declined with increasing duration of diving, but slightly less than predicted with increasing age, while peak expiratory flow (PEF) declined more than expected for age in longer-term divers. The changes in PEF were statistically significant, and correlated with duration of diving exposure, initial age and final BMI. Nevertheless, the changes were small and probably clinically insignificant. Conclusion: We compared changes in spirometric parameters over long periods of occupational diving with normative data and found no clinically significant differences that could be attributed to diving. Background: The underwater environment presents physiological challenges for the cardiovascular, renal and pulmonary systems. Increases in external hydrostatic pressure reduce the capacity of the venous compartment and cause blood to move toward the lung. The aim of this study was to evaluate retrospectively electrocardiographic (ECG) changes in a cohort of professional divers. Methods: Between January 2009 and January 2012, 225 randomly selected professional divers, 204 male (91%) and 21 female (9%) attended our clinic for their biannual diving medical assessment. Their ECG records were evaluated retrospectively. Results: The most common ECG abnormality observed was incomplete right bundle branch block (IRBBB) in 30 divers (13.3%). Eleven divers (4.9%) showed right QRS axis deviation (seven with IRBBB). Six divers had a sinus tachycardia; in four divers there was early repolarization; three divers had ventricular extrasystoles; one diver had ST elevation in lead V3; there was one with sinus arrhythmia and another with T-wave inversion in leads V2, V3 and aVF. These ECG changes were evaluated retrospectively by a cardiologist who made various recommendations for further review including bubble-contrast echocardiography for IRBBB. Conclusions: No serious ECG abnormalities were identified, but IRBBB should be further investigated because o f its association with persistent (patent) foramen ovale. Rapid cardiological review of ECGs could be achieved using modern communications technology, such as telecardiography, and further clinical investigations directed by specialist recommendation arranged promptly if indicated. Introduction: An individual case review of diving-related deaths reported as occurring in Australia in 2011 was conducted as part of the DAN Asia-Pacific dive fatality reporting project. Method: The case studies were compiled using reports from witnesses, the police and coroners. In each case, the particular circumstances of the accident and, where available, details from the post-mortem examination are provided. A chain of events analysis was conducted for each case. Results: In total, there were 30 reported fatalities (10 more than in 2010). These included 15 snorkel/breath-hold divers, 14 scuba divers and one diver using surface-supplied breathing apparatus. Twenty-four victims were males. The mean age of snorkelling victims was 49.6 (range 23-75) years and compressed gas divers 42.2 (range 23-55) years. Cardiac-related issues were thought to have been the disabling injury in the deaths of at least seven snorkel divers and five scuba divers. Immersion pulmonary oedema was implicated in at least one death; and three fatalities resulted from attacks by marine animals. Two novices died while under instruction/supervision after separation from their instructor in poor visibility. Conclusions: Pre-existing medical conditions, separation and inadequate supervision and seafood collection in areas frequented by marine predators were once again features in several deaths in this series. Introduction: There are few data available on which to estimate the risk of death for Australian divers. This report estimates the risk of a scuba diving fatality for Australian residents, international tourists diving in Queensland, and clients of a large Victorian dive operator. Methodology: Numerators for the estimates were obtained from the Divers Alert Network Asia-Pacific dive fatality database. Denominators were derived from three sources: Participation in Exercise, Recreation and Sport Surveys, 2001-2010 (Australian resident diving activity data); Tourism Research Australia surveys of international visitors to Queensland 2006-2014 and a dive operator in Victoria 2007-2014. Annual fatality rates (AFR) and 95% confidence intervals (95% CI) were calculated using an exact binomial test. Results: Estimated AFRs were: 0.48 (0.37-0.59) deaths per 100,000 dives, or 8.73 (6.85-10.96) deaths per 100,000 divers for Australian residents; 0.12 (0.05-0.25) deaths per 100,000 dives, or 0.46 (0.20-0.91) deaths per 100,000 divers for international visitors to Queensland; and 1.64 (0.20-5.93) deaths per 100,000 dives for the dive operator in Victoria . On a per diver basis, Australian residents are estimated to be almost twenty times more likely to die whilst scuba diving than are international visitors to Queensland, or to lower than fourfold on a per dive basis. On a per dive basis, divers in Victoria are fourteen times more likely to die than are Queensland international tourists. Conclusions: Although some of the estimates are based on potentially unreliable denominator data extrapolated from surveys, the diving fatality rates in Australia appear to vary by State, being considerably lower in Queensland than in Victoria. These estimates are similar to or lower than comparable overseas estimates, although reliability of all such measurements varies with study size and accuracy of the data available. Aim: The purpose was to explore causative tendencies among diving fatalities to prevent similar injuries in the future. Methods: We report 33 fatal diving injuries that occurred among Danish divers during the period 1999-2012 in Scandinavian waters. The study was performed as a retrospective overview. The empiric data consists of police reports, forensic autopsy reports and examination of the diving equipment. Data were assembled and analyzed using Pivot and Excel. Frequencies and means (+/- SD) were used to describe categorical and continuous variables respectively. Results: The mean age was 38.9 years and drowning was considered the cause of death in 24 of 28 divers for whom a diagnosis was possible. Elevated body mass index (18 of 22 divers had a BMI > 25) was overrepresented in our group compared to the background population. A drysuit was worn by 17 divers. Diving independently of a dive centre and mishandling of buoyancy aids were common risk factors. Only two divers released their weights. Three-quarters of those who did not would have increased their chance of survival by doing so; nevertheless, in a quarter of cases the weights were not readily releasable or not releasable at all. Conclusion: Unfamiliarity with drysuit diving, lack of a diving buddy and mismanagement of weights were important contributors to diving injuries. [ABSTRACT FROM AUTHOR]

Background: People with asthma are an under-represented group amongst scuba divers. Many may avoid or are advised against diving due to the potential risks, including bronchoconstriction, pulmonary barotrauma and arterial gas embolism. The aim of this study was to establish whether divers with asthma were more likely to experience reversible airways obstruction following typical scuba diving than divers without asthma. Method: All divers with a history of asthma attending Operation Wallacea in Honduras were identified and peak expiratory flow rates (PEF) were measured pre and immediately post dive. All dives were boat dives in tropical sea water. Scuba dives were defined as those lasting between 40 and 55 minutes to a depth of between 10 and 18 metres. Of the 356 divers attending, 22 were identified as having asthma, of whom 19 were suitable for testing. They were classified by treatment regimen: five on no treatment, 11 on salbutamol only and three on regular preventative treatment. Twenty-four divers without a history of asthma acted as a control group. Results: Open-water scuba diving caused a small decrease in PEF in all populations (median decrease 4.4%, P < 0.001). Percentage decrease in PEF was significantly more in divers with asthma on regular preventative medication than in the control group (mean 9.3%, median decrease 6% vs. mean 3.1%, median 4.3%; P = 0.039). Conclusion: These findings support the view that asthmatics are more susceptible to airway changes following scuba diving. Divers are exposed to circulatory stress that directly affects the endothelial lining of blood vessels, and even asymptomatic dives are associated with inflammatory responses, microparticle release and endothelial dysfunction. As humans age, there is a relative increase in the risk of cardiovascular disease, attributed in part to declining endothelial function. Whether extensive diving in the older diver increases the risk of disease as a result of accumulated circulatory stress, or provides protection through processes of acclimatization remains an open question. We provide a brief review of current knowledge about the separate effects of diving and aging on the vascular endothelium in humans and rodents, and discuss the available data on their combined effects. The aim is to elucidate possible outcomes of the interplay between exogenous and endogenous stress factors for endothelial function and to question potential implications for cardiovascular health in the aging diver.
