6. Antihypertensive Efficacy of the Direct Renin Inhibitor Aliskiren in Patients With Diabetes: A Pooled Analysis of 10 Randomized Trials.

Abstract

Patients (pts) with diabetes (DM) and hypertension have a high risk of cardiovascular events, but. blood pressure (BP) control is hard to achieve in this group. We report here a pooled analysis of effects of monotherapy with the direct renin inhibitor aliskiren on mean sitting systolic/diastolic BP (SBP/DBP) over 8-12 weeks in the subgroup of type 1 or 2 DM pts (n=1417, 14.9%) in 10 randomized, double-blind trials of hypertensive pts (total n=9503; DBP 95-110 mmHg; 56% male, 21% ≥ 65 years, 81% Caucasian), and on clinical chemistry parameters in 10 short-term controlled trials (n=8076). BP changes and responder rates (DBP <90 mmHg and/or ≥ 10 mmHg reduction) were analysed (intent-to-treat population) by two-way ANCOVA with treatment as factor and baseline as covariate. Aliskiren 150 mg and 300 mg significantly lowered SBP/DBP vs placebo (PBO) in the overall population; reductions in BP in pts with DM were similar (Table). Responder rates with aliskiren 150 mg (62.2%) or 300 mg (74.0%) were significantly higher (p<0.05) than PBO (43.6%) in pts with DM. Aliskiren was well tolerated; overall, 1432/3584 pts (40.0%) experienced adverse events on monotherapy vs 314/781 (40.2%) receiving PBO. Aliskiren monotherapy in pts with DM was associated with a low rate of serum potassium elevations >5.5 mmol/L (2.2%) and elevations in blood urea nitrogen >14.28 mmol/L and serum creatinine >176.8 µmol/L (both 0.7%), and these did not lead to study discontinuations. In conclusion, aliskiren provides effective BP lowering, high responder rates and an excellent safety profile in pts with DM and hypertension, similar to the effects of aliskiren in the overall hypertensive patient population.
