Background Patients in Denmark undergoing total knee or hip replacement are routinely discharged within 2 days of surgery. A critical examination of traditional treatment methods, combined with focused research, has during the last 20 years increasingly optimized the treatment course in such a way that it has become possible to radically reduce the length of stay (LOS). Basics of the fast-track model The most important elements of this Fast-Track model are described. The patient motivation and transfer of partial responsibility to the patient through intensive information, optimized operation techniques, as well as modern multi-modal pain therapy with early mobilization are key issues. The relatively small and homogenous health care system of Denmark offers good research conditions and the possibility of a fast implementation of the latest results, as well as a lump-sum based re-imbursement system without minimum stay-both factors have been favorable for the development of the Fast-Track model.

Conservative treatment and surgical release of a stiff elbow requires correct pain management which should be oriented to the individual needs of the patient. Regional anesthesia in combination with opioids is necessary postoperatively to obtain sufficient pain relief. There is a need for prospective randomized studies to develop an optimal pain therapy concept following operations for elbow stiffness.

Dieser Artikel beschreibt die Geschichte der Schmerztherapie und somit auch der fruhen Anasthesie im Bereich operativer Therapieverfahren. Die Historie dieses Bereichs ist wesentlich von der Entwicklung entsprechender Medikamentengruppen gepragt, die sowohl fur die Schmerztherapie aber auch fur die Narkose von groser Bedeutung waren. Erwahnt seien hier das Lachgas, der Ather, das Morphin und die Anfange der Lokalanasthetika. Diese Meilensteine werden im Einzelnen dargestellt und diskutiert. Alle vorgenannten Entwicklungen zusammengenommen, sowie die heutige Erfahrung auf dem Gebiet der Schmerztherapie, wie beispielsweise die Gate-control-Theorie und die Psychologie haben diesen Bereich in den letzten Jahrzehnten zu einem wichtigen Bestandteil in vielen Bereichen der operativen Medizin werden lassen.

Eine suffiziente postoperative Schmerztherapie nach totaler Kniearthroplastik erlaubt die Kontrolle des Wundschmerzes, ermoglicht eine fruhestmogliche aktive wie passive Beweglichkeit und krankengymnastische Fruhbehandlung und wirkt dem Risiko der Knieeinsteifung durch schmerzbedingte Schonung und Ruhigstellung der Extremitat in Ruhe und wahrend Mobilisation entgegen. Kommt eine systemische Schmerztherapie zur Anwendung, so sollte diese die Gabe von nicht-steroidalen Antirheumatika (NSAR) beinhalten und durch eine bedarfsadaptierte, individuelle Titration (z. B. i.v.-PCA) von potenten Opioidanalgetika erganzt werden. Von besonderem Vorteil ist eine regionale kontinuierliche oder diskontinuierliche perioperative Schmerztherapie. Sie ermoglicht im Rahmen der Knieendoprothetik und deren Revisionseingriffen eine bedarfsgerechte Schmerzfreiheit und schmerzfreie Physiotherapie uber viele Tage unter weitestgehender Vermeidung von typischen Opioidnebenwirkungen. Das Gesamtergebnis des Patienten kann jedoch nur dann verbessert werden, wenn samtliche perioperativen Einzelmasnahmen in einem perioperativen multimodalen Gesamtkonzept integriert sind und sinnvoll aufeinander abgestimmt werden.

Each patient has the right of a dedicated pain therapy according to the state of the art. However an efficient pain therapy is not possible without knowing the cause of pain. In most posttraumatic pain situations peripheral nociceptors are activated and normal afferences are conducted via an intact nociceptive system. In contrast, neuropathic pain pain is caused by lesions of the nervous system itself. Mechanisms of central sensibilization and involvement of the sympathetic nervous system may lead to chronification of such pain conditions. The therapeutic regime of nociceptive and neuropathic pain is demonstrated by algorithms of treatment modalities. Apart from classic non-opioid analgesics, co-analgesics and opioids have an important status in chronic pain management as well. Prescription of these substances has to follow strictly defined standards of pain therapy. Blockades with local anaesthetics as mono-therapy of chronic pain are obsolete. In posttraumatic pain, however, a certain number of adjuvant blockades or infiltrations of triggerpoints may be helpful. The exceptional place of sympathetic blockades are in diagnosis and therapy of sympathetic maintained pain (SMP).
