This report describes an unusually delayed presentation of capsular bag distension syndrome (CBDS), which was found to be associated with Propionibacterium acnes (P. acnes) endophthalmitis. Our patient presented with a gradual decrease in vision after uneventful cataract surgery done 13 years back. On examination, there was a thick turbid fluid entrapped behind the intraocular lens (IOL). Ultrasound biomicroscopy (UBM) confirmed the presumed diagnosis. The case was managed by pars plana vitrectomy (PPV) with posterior capsulotomy, and the entrapped turbid fluid was aspirated and sent for histopathology, which revealed a positive growth of P. acnes. The patient had excellent outcomes with complete resolution post-operatively.

With the introduction of the coronavirus disease 2019 (COVID-19) mRNA vaccines, the incidence of severe infection has significantly decreased. While the vaccines have been shown to be effective and safe, there have been few case reports of acute myocarditis within 3-5 days following the second dose of the vaccine. We report a case of an elderly man who presented with acute-onset chest pain after three months of receiving the second dose of the mRNA vaccine. He was found to have acute myocarditis on cardiac magnetic resonance imaging (CMRI), which was attributed to exposure to the COVID-19 vaccine in the absence of any other risk factors. Our patient demonstrated quick resolution of symptoms and was discharged within 72 hours. We review the literature and summarize published case reports on COVID-19 vaccine-associated myocarditis. The present case report provides new evidence regarding the possible subacute presentation of myocarditis post-COVID-19 vaccine, and further highlights the favorable outcome in this newly described clinical entity.

Chronic granulomatous disease (CGD) is an inherited primary immunodeficiency disease caused by a genetic defect in the nicotinamide adenine dinucleotide phosphate oxidase (NADPH) complex that affects phagocytes. This leads to recurrent severe bacterial and fungal infections manifested by recurrent pneumonia, also involving soft tissue, bones, and liver. Usually, CGD is presented and diagnosed in the first five years of life. In this case report, we describe a late presentation in an adolescent with multiple liver abscesses, the approach of diagnosis, and management.

Congenital central hypoventilation syndrome (CCHS) is a rare disorder that results in profound hypoventilation that is most prominent during periods of sleep. Caused by a genetic mutation in the PHOX2B gene, CCHS typically presents in the newborn period with symptoms of hypoventilation. However, there is a subset of patients with the same genetic mutation who present much later in life, which is termed late-onset congenital central hypoventilation syndrome (LO-CCHS). The reason for its late presentation is unclear but is often dramatic. Given its rarity, the diagnosis can be difficult to establish but can be accomplished by using a systematic approach. Here, we present a case of LO-CCHS in an 11-year-old female who presented with respiratory failure and altered mental status.

A 26-year-old male athlete presented to our hospital with bilateral leg pain after intense training. He had a history of transient numbness and pain with rigorous exercise but this time pain persisted and drop foot developed. Unfortunately, the diagnosis of acute exertional compartment syndrome was delayed due to late presentation of the patient in our department. He underwent three consecutive surgeries for decompression and debridement. At 13 months follow-up, he is ambulatory with bilateral ankle-foot orthosis. This case presents a bilateral, acute anterior exertional compartment syndrome of the leg and highlights the need for high clinical suspicion and early treatment of the acute exertional compartment syndrome.

A double‑chambered left ventricle is a rare congenital anomaly. We present the case of a 26-year-old man with such anomaly who presented with congestive heart failure. After this diagnosis was confirmed with echocardiography, surgical removal of the anomalous band and replacement of the regurgitant deformed mitral valve were performed. Postoperatively, the patient deteriorated, and no corrective response was associated with surgery. Herein we discuss what we have learned from this rare case and how it may apply to the management of similar cases in the future.

Uterine rupture is a rare disease, which usually presents in the form of a life-threatening emergency. It occurs most commonly after a vaginal birth after cesarean (VBAC) and is characterized by complete separation of the uterine incision throughout most of its length, involving all layers of the uterus. We present the case of uterine rupture in which the patient had no acute signs of rupture; instead, she presented with symptoms of generalized peritonitis. She presented 23 days after VBAC, with complaints of slight vaginal bleeding and a palpable abdominal mass. Leukocyte and platelet counts were raised, suggesting infection, which occurred due to the spread of fluid from the uterus to the abdomen through the scar defect. A total abdominal hysterectomy was performed due to widespread adhesions and a non-viable uterus. Our case was unusual as the presentation was delayed, with no acute symptoms of either uterine rupture or peritonitis. Uterine rupture can be fatal if not recognized and managed promptly.

Hypertrophic obstructive cardiomyopathy (HOCM) is a genetic condition most commonly characterized by hypertrophy of the ventricular septum, which leads to left ventricle outflow obstruction. Due to the severity of the condition, it is often diagnosed in adolescents, especially in those who exercise. We describe the case of a 53-year-old male, previously undiagnosed with hypertrophic cardiomyopathy (HCM), who became dyspneic during a bike race. He was found to have elevated troponin, pulmonary edema, and was diagnosed with HOCM. The late presentation of the case, in an active individual, makes the situation unique.

Partial anomalous pulmonary venous return (PAPVR) is a rare congenital malformation. The infracardiac variant with the right lobe of the lung draining to the inferior vena cava (IVC) is called Scimitar syndrome. The infantile subtype presents before one year of age and the adult variant is also usually diagnosed in childhood. A 70-year-old woman presented with worsening shortness of breath. An echocardiogram suggested severe pulmonary hypertension that was confirmed by right heart catheterization. A computed tomography (CT) without contrast revealed an anomalous vein from the right upper lobe suggestive of Scimitar syndrome. The patient did not have any other associated congenital heart defects (CHD) (incomplete Scimitar syndrome). A surgical treatment approach was avoided due to the incomplete nature of the Scimitar syndrome. Incomplete Scimitar syndrome may present later and with less severity than the typical Scimitar syndrome with left to right shunting occurring only in the lung and may be managed nonsurgically.
