ABSTRACT Purpose Compare infant suction in babies with and without ankyloglossia using a microprocessor-controlled pressure sensor coupled to a pacifier. Methods Fifty-five infants from 0 to 2 months of age underwent clinical examination for ankyloglossia, after which they were offered a silicone pacifier connected to the pressure acquisition device and suction activity was recorded. Thus, we extracted the frequency of sucks within a burst, the average suck duration, the burst duration, the number of sucks per burst, the maximum amplitude of sucks per burst and the inter-burst interval. Results The key difference in newborns with ankyloglossia in relation to control was that they perform longer bursts of suction activity. Conclusion The longer burst durations are likely a compensatory strategy and may underlie the pain reported by mothers during breastfeeding. We therefore propose a method for objectively quantifying some parameters of infant suction capacity and demonstrate its use in assisting the evaluation of ankyloglossia.

ABSTRACT Purpose To analyze the effects of lingual frenotomy on the breastfeeding of infants, based on the electrical activity of the masseter and suprahyoid muscles and assessment of the breastfeeding. Methods Observational study developed between October 2017 and June 2018 with a sample of 20 newborns and infants who attended a dental clinic and were diagnosed with ankyloglossia. Another 20 were excluded for meeting some of the following exclusion criteria: babies more than 6 months old, who were not on exclusive or mixed breastfeeding, who had other clinical impairments that interfered with breastfeeding, who had other foods introduced into their diet, who had neurological changes and/or craniofacial deformities, and/or who did not finish all the stages of the study. Breastfeeding was assessed with the UNICEF Breastfeeding Assessment and Observation Protocol, while the muscle electrical activity was assessed with the Electrical Activity Assessment Protocol for the Masseter and Suprahyoid Muscles in Newborns During Breastfeeding. The same speech-language-hearing therapist conducted the two assessments both before the conventional frenotomy and 7 days after it. Results The signs suggestive of breastfeeding difficulties changed 7 days after the surgery, with a p-value ≤ 0.002 for general observation of the mother, position of the infant, latch, and sucking. The maximum voluntary contraction of the masseter was the only integral parameter with a difference, as the electrical activity had decreased. Conclusion Behaviors favorable to breastfeeding increased 7 days after the frenotomy in all the breastfeeding assessment categories, whereas the electrical activity of the masseter decreased.

ABSTRACT Purpose: to evaluate the lingual frenulum and breastfeeding in infants from a maternal-perinatal referral center, as well as to monitor infants with ankyloglossia up to six months of age. Methods: a cohort study conducted at the Instituto Nacional Materno Perinatal - Maternidad de Lima in Lima, Peru. The consecutive intentional sample consisted of 304 newborns and their respective mothers, evaluated during December 2017 and January 2018, which were the baseline of the study. A clinical evaluation of the lingual frenulum adapted and the Clinical Evaluation of Breastfeeding Efficacy (CEBE) scale, was performed. Results: of the 304 newborns, 15 (4.9%) were considered with altered frenulum, and only 4 (26.7%) presented a low score in CEBE. The mean of the CEBE score was 9.3. (DP=1.35, Min=3, Max=10). Of the follow-up infants, only 2 (13.3%) persisted with breastfeeding difficulties for which frenotomy was indicated. Conclusion: This study demonstrates the low prevalence of ankyloglossia in infants, as it does not indicate a trend of difficulty or negative interference in breastfeeding.

RESUMO Objetivo Diagnosticar a anquiloglossia em recém-nascidos, comparando dois instrumentos de avaliação do frênulo lingual. Método Tratou-se de um estudo transversal, realizado em Recife, PE, Brasil no ano de 2018, com 147 mães/recém-nascidos com idade de até 30 dias de vida. Foram utilizados o Instrumento Bristol Tongue Assessment Tool (BTAT) e o Protocolo de Avaliação do Frênulo da Língua para Bebês (“Teste da Linguinha”). Dados sociodemográficos também foram anotados. Para a comparação entre os dois métodos de diagnóstico da anquiloglossia, foi utilizado o teste de McNemar e foram obtidos o valor da concordância de Kappa e o respectivo intervalo de confiança. Resultados A presença de anquiloglossia foi de 4,8%, quando diagnosticada por meio do BTAT, e de 17,0%, quando utilizado o “Teste da Linguinha”. Com relação ao sexo, 53,1% dos recém-nascidos eram do sexo masculino e 46,9% do sexo feminino; contudo, não houve associação entre a anquiloglossia e o sexo do recém-nascido nos dois métodos de avaliação. Conclusão O diagnóstico da anquiloglossia em recém-nascidos variou em função do instrumento de avaliação utilizado. 10. Quality indicators of hearing screening and evaluation of neonatal lingual frenulum

To analyze the quality indicators of the hearing screening program and to evaluate lingual frenulum in newborns, in terms of coverage rates, duration of hearing screening, referral indexes and prevalence of ankyloglossia in a university hospital specializing in mother and child care.This is a cross-sectional study, in which we analyzed the database of the newborn hearing screening and lingual frenulum assessment program for the period between September 2015 and August 2016.During the study period, 2,345 babies were born at the institution, 1,380 (58.8%) underwent newborn hearing screening and 1,350 (57.6%) were diagnosed with a lingual frenulum. Mean gestational age was 39 weeks (± 1.6), birth weight 3478g (± 469.2) and 69% were boys. In newborn hearing screening, 95.7% of the 1,380 babies screened were discharged with guidance, 2.4% were referred for auditory monitoring and 1.9% of babies were referred for auditory diagnosis. In lingual frenulum assessment, 123 (9.1%) of the 1,350 evaluated, had ankyloglossia, 85 were boys and 47 girls.The indicators of the time of life in which the hearing screening is performed, the referral indices and ankyloglossia prevalence are in line with those reported in the literature; however, coverage rates were lower than recommended and do not comply with Brazilian law.analisar os indicadores de qualidade do programa de triagem auditiva e avaliação do frênulo lingual em neonatos, quanto às taxas de cobertura, tempo de vida em que são realizadas as triagens auditivas, índices de encaminhamentos e prevalência da anquiloglossia, em um hospital universitário especializado em assistência materno-infantil.trata-se de um estudo transversal, no qual foi analisado o banco de dados do programa de triagem auditiva e da avaliação do frênulo lingual em neonatos, referente aos atendimentos realizados no período de setembro de 2015 a agosto de 2016.no período analisado, nasceram 2.345 neonatos na instituição, 1.380 (58,8%) realizaram a triagem auditiva neonatal e 1.350 (57,6%) a avaliação do frênulo lingual. A média de idade gestacional foi de 39 semanas (±1,6), peso de nascimento igual a 3.478 g (± 469,2) e 69% eram do sexo masculino. Na triagem auditiva neonatal, dos 1.380 neonatos, 95,7% receberam alta com orientação, 2,4% foram encaminhados para monitoramento auditivo e 1,9% foram encaminhados para diagnóstico auditivo. Na avaliação do frênulo lingual, dos 1.350 avaliados, 123 (9,1%) apresentaram anquiloglossia, 85 eram do sexo masculino e 47, feminino.os indicadores de tempo de vida em que são realizadas as triagens auditivas, índices de encaminhamentos e prevalência da anquiloglossia estão em consonância com a literatura, porém as taxas de cobertura estão inferiores ao recomendado e não seguem a legislação brasileira.

Purpose: evaluate the influence of frenotomy on the breastfeeding of newborns diagnosed with ankyloglossia. Methods: this is an intervention study performed with 50 newborns diagnosed with ankyloglossia. It was conducted in three stages: diagnosis, intervention and reassessment. In the diagnostic phase, the Protocol for the Assessment of Speech Language with Scores for Babies was applied to diagnose ankyloglossia and a questionnaire assessing the symptoms and coordination of sucking, swallowing and breathing during breastfeeding. In the intervention, frenotomy was performed, and at reassessment, the diagnostic protocol and questionnaire in order to compare the post-surgical effects. Results: of the 50 babies participating in the study, 35 (70%) were boys and 15 (30%) girls. A total of 68% of ankyloglossia cases were reported in the family, a majority (38%) involving cousins. There was a statistically significant reduction in the average protocol score in the reassessment stage, from 8.38 (7-12 points) to 0.86 (0-5 points), as well as a statistically significant improvement in all variables related to the symptoms of breastfeeding. Conclusion: surgical intervention, known as frenotomy, made it possible to improve the negative symptoms of breastfeeding in newborns diagnosed with ankyloglossia. RESUMO Objetivo: avaliar a influência da frenotomia sobre a amamentação de recém-nascidos com diagnóstico de anquiloglossia. Método: trata-se de um estudo de intervenção realizado com 50 recém-nascidos com diagnóstico de anquiloglossia. Foi realizado em três etapas: diagnóstico, intervenção e reavaliação. Na fase diagnóstica, foram aplicados o Protocolo de Avaliação do Frênulo da Língua com Escores para Bebês para o diagnóstico de anquiloglossia e um questionário de avaliação dos sintomas e coordenação de sucção, deglutição e respiração durante a amamentação. Na intervenção, foi realizada a frenotomia e, na reavaliação, foram reaplicados o protocolo de diagnóstico e o questionário para comparação dos efeitos pós-cirúrgicos. Resultados: dos 50 bebês participantes do estudo, 35 (70%) eram do gênero masculino e 15 (30%) do gênero feminino. Foram relatados 68% dos casos de anquiloglossia na família, sendo primo (a) o grau de parentesco na maioria desses casos (38%). Houve redução estatisticamente significativa na média de pontuação no protocolo de 8,38 (7-12 pontos) para 0,86 (0-5 pontos), na etapa de reavaliação, assim como melhora estatisticamente significante em todas as variáveis relacionadas aos sintomas da amamentação. Conclusão: a intervenção cirúrgica, denominada frenotomia, possibilitou a melhora dos sintomas negativos da amamentação em neonatos com diagnóstico de anquiloglossia.

Purpose To analyze the association between ankyloglossia and breastfeeding. Methods A cross-sectional study was undertaken on 130 newborn infants in exclusive breastfeeding with Apgar score ≥ 8 within the first five days of life. The research was approved by the Ethics Committee on Human Research. The data collection was performed by the researcher and by three trained speech therapists of the team. The protocols applied were the Neonatal Tongue Screening Test from the Lingual Frenulum Protocol for Infants, the UNICEF Breastfeeding Observation Aid, and the collection of maternal complaints related to the difficulty in breastfeeding was also considered. The data were submitted to statistical analysis – chi-square test and Fisher’s exact test, with a significance level of 5%. Results When correlating the data, the statistical analysis revealed an association between ankyloglossia and the items of suckling category of the Breastfeeding Observation Aid. The association between complaint of difficulty in breastfeeding and ankyloglossia was also seen. Conclusion On the first days of life, ankyloglossia is associated with the mother’s breastfeeding complaint and with the newborn’s sucking difficulty. RESUMO Objetivo Verificar associação entre anquiloglossia e amamentação. Método Estudo transversal, realizado em 130 recém-nascidos, em um Hospital Universitário. Foram incluídos recém-nascidos entre um a cinco dias de vida, com Apgar score ≥ 8, em aleitamento materno exclusivo. Participaram da pesquisa apenas recém-nascidos de termo e saudáveis. Esta pesquisa foi aprovada pelo Comitê de Pesquisa com Seres Humanos. A coleta de dados foi realizada pela pesquisadora e por três fonoaudiólogas da equipe, devidamente treinadas e calibradas. Os protocolos aplicados foram: avaliação anatomofuncional do Protocolo de avaliação do frênulo da língua em bebês, o Protocolo de Observação da Mamada da UNICEF e coleta das queixas maternas referentes a dificuldade ou não para amamentar. Os dados obtidos foram submetidos à análise estatística, sendo aplicado o teste Qui-quadrado e teste exato de Fisher, adotando nível de significância de 5%. Resultados Quando correlacionados os dados, a análise estatística demonstrou associação entre anquiloglossia e os itens da categoria de sucção do Protocolo de Observação da Mamada. Encontrou-se também associação entre queixa de dificuldade para amamentar e anquiloglossia. Conclusão Nos primeiros dias de vida, a anquiloglossia está associada com queixa da mãe para amamentar e com a dificuldade de sucção do recém-nascido.

In newborn hearing screening, 95.7% of the 1,380 babies screened were discharged with guidance, 2.4% were referred for auditory monitoring and 1.9% of babies were referred for auditory diagnosis. In lingual frenulum assessment, 123 (9.1%) of the 1,350 evaluated, had ankyloglossia, 85 were boys and 47 girls., Conclusion: The indicators of the time of life in which the hearing screening is performed, the referral indices and ankyloglossia prevalence are in line with those reported in the literature; however, coverage rates were lower than recommended and do not comply with Brazilian law.
