Objective: To evaluate therapeutic footwear expectations and usability of individuals with diabetes and foot complications. Design: A prospective multicenter study was conducted on participants with a high risk of developing a diabetic foot ulcer. Setting: Participants were enrolled in 11 different specialized diabetic foot units in Spain between March 2022 and June 2023. Subjects: Patients with diabetes at moderate to high risk of foot ulceration receiving first therapeutic footwear prescription. Interventions: All the patients included in the research were prescribed with their first pair of therapeutic footwear. Main measures: Primary outcome measures were MOS-pre and MOS-post questionnaires evaluating use and usability of prescribed therapeutic footwear. Secondary outcome measures aimed to evaluate footwear clinical efficacy as ulceration rate and self-reported perceived walking distance per day. Results: The use of therapeutic footwear exceeded the patient's pre-provision prediction of their anticipated use in 94% of people (n = 126). Based on the visual analogic satisfaction scale, the median satisfaction of daily wearing their therapeutic footwear was 7 points, Interquartile Range (IQR) [5–8.25]. During the follow-up period, 39 participants (29.1%) experienced diabetic foot ulcer. Perceived walking distance participants reported an improvement in their perceived walking ability during various daily life activities. Conclusions: Diabetes patients at moderate to high risk of diabetic foot ulcer improved their perception of walking ability after therapeutic footwear prescription. Objective: To determine whether physical activity enjoyment mediated the association between motivation and physical activity in patients with heart failure. Design and setting: A cross-sectional study at the cardiology clinic in the university hospital in Valencia, Spain Subjects: A total of 134 patients with heart failure. Main measurements: Physical activity was assessed with the International Physical Activity Questionnaire, motivation was assessed with the Exercise Motivation Index and Physical Activity Enjoyment was assessed with the Physical Activity Enjoyment Scale. Analysis: Mediation analysis using Hayes' PROCESS macro (Model 4) for SPSS. Results: The mean age of the sample was 70 ± 14 years, 47 patients were female (35%), and 87 patients were in New York Heart Association I/II (67%). A positive relationship was found between exercise motivation and physical activity (t = 4.57, p <.01) and physical activity enjoyment (t = 11.52, p <.01). Physical activity enjoyment was found to positively affect physical activity (t = 3.50, p <.01). After controlling for physical activity enjoyment, the effect of exercise motivation on physical activity changed from a significant to non-significant (t = 1.33, p =.89), indicating that enjoyment completely mediated the relationship between motivation and physical activity. Overall, 25% of the variation in physical activity was explained by the mediation model. Conclusions: Physical activity enjoyment mediates the relationship between exercise motivation and physical activity in patients with heart failure. Objective: To compare the effectiveness of two different exercise-based programs through telerehabilitation in patients with coronavirus disease 2019. Design: Randomized, controlled, parallel, double-blinded, three-arm clinical trial. Setting: Patients' homes through telerehabilitation devices. Subjects: Subjects with coronavirus disease 2019 in the acute phase. Interventions: Subjects were divided into three groups: breathing exercises group, strength exercises group or no treatment/control group. Main measures: We analysed visual analogue scale for fatigue, 6-minute walking test, 30-seconds sit-to-stand test, multidimensional dyspnoea-12 questionnaire and Borg scale at baseline and 14 days later. Results: From 93 subjects recruited, 88 were enrolled, and 77 patients (mean [SD] age 39.40 [11.71]) completed the 14-days intervention and were included in the analysis: 26 in strength exercises group, 29 in breathing exercises group and 22 in control group. The intergroup analysis shows significant differences between the study groups and control group in all variables (p < 0.05); Borg scale, multidimensional dyspnoea-12 questionnaire (pre–post intervention score: strength exercises group: 7.85 [6.82] – 4.54[4.82], breathing exercises group: 11.04 [6.49] – 5.32 [3.63], control group: 10.27 [6.49] – 10.59[6.58]), visual analogue scale for fatigue, 6-minute walking test and 30-seconds sit-to-stand test (pre–post intervention score: strength exercises group: 12.19 [4.42] – 13.58 [5.37], breathing exercises group: 11.18 [3.42] – 12.79 [4.00], control group: 10.45 [2.15] – 9.86[1.88]). The greatest effect sizes were found in the variables Borg Scale (R 2 = 0.548) and multidimensional dyspnoea-12 questionnaire (R 2 = 0.475). Conclusions: Strength exercises group and breathing exercises group obtained significant improvements in fatigue, dyspnoea, perceived effort, and physical state, compared to control group, although the greatest benefits were found for dyspnoea and aerobic capacity in breathing exercises group. Objective: To analyse the feasibility of Basic Body Awareness Therapy in people with eating disorders.Design: A randomized controlled trial; the assessors were not blinded to the group allocation.Setting: The eating disorders department within a hospital setting.Subjects: Twenty-eight outpatients with eating disorders for less than five years.Intervention: All patients received standard outpatient treatment. The intervention group (n = 14) also received Basic Body Awareness Therapy for seven weeks.Main measures: Eating Disorder Inventory, Eating Attitude Test, Body Attitude Test and Quality of Life Scale SF-36.Results: Analysing the differences between both groups, significant differences were found in Eating Disorder Inventory (mean difference: 26.3; P = 0.015) and in its subscales ‘drive to thinness’ (P = 0.003), ‘body dissatisfaction’ (P = 0.025) and ‘ineffectiveness’ (P = 0.014). Also in Body Attitude Test (mean difference: 33.0; P = 0.012), Eating Attitude Test-40 (mean difference: 17.7; P = 0.039) and SF-36 in the section ‘mental health’ (mean difference: 13.0; P = 0.002).Conclusions: This study has shown some effectiveness of Basic Body Awareness Therapy in improving some symptoms in outpatients with eating disorders. Further studies should include larger samples, double-blinded and placebo methodologies, and should focus on questions such as which eating disorder diagnoses benefit most from physical therapy. Objectives: To describe the immediate effects of diacutaneous fibrolysis on pain and mobility in patients suffering from painful shoulder, and to assess the ability of the placebo technique to blind participants to group allocation.Design: A double-blind before—after randomized placebo-controlled pilot study.Setting: Two public centres of Primary Health Care of the Spanish National Health System.Participants: Fifty patients (29 women, 21 men) with a diagnosis of painful shoulder of subacromial origin were included.Intervention: Participants were randomly allocated to one of two groups. The intervention group (N = 25) was actually treated with diacutaneous fibrolysis, while the placebo group (N = 25) was treated with placebo diacutaneous fibrolysis.Measurements: Active range of motion (flexion, abduction, extension, external and internal rotation) and pain intensity in the hand-behind-back position were measured. Participants were also asked about their perception in terms of comfort of the technique and results obtained.Results: Between-groups differences were significant in flexion (mean 11.4 degrees; 95% confidence interval (CI) 5.7—17.1), abduction (mean 7.2 degrees; 95% CI 2.0—12.5) and internal rotation movements (mean 3.1 cm; 95% CI 0.1—6.1). There were no significant differences between groups in pain intensity, extension or external rotation movements. The placebo technique showed its ability to blind participants to group allocation. None of the participants suffered any adverse effects from diacutaneous fibrolysis.Conclusion: Diacutaneous fibrolysis may be an effective and safe therapeutic option to improve active range of motion in patients suffering from painful shoulder.
