This study aims to compare the radiopacities of computer-aided design/computer-aided manufacture (CAD/CAM) blocks and the adhesive cements used for their bonding. 1 ± 0.2 mm thick specimens were obtained from six different CAD/CAM blocks (Incoris TZI, IPS e.max CAD, Vita Mark II, Cerasmart, Vita Enamic, and Vita Suprinity), four different adhesive resin cements (Panavia F2.0, Variolink Esthetic DC, RelyX Unicem Aplicap, G-CEM LinkAce), and a tooth. Radiographs of the specimens from each group, a tooth section, and an aluminum (Al) step-wedge were acquired. The radiopacity values of the materials were calculated as equivalents of Al thickness using the gray level values. The data were statistically analyzed using one-way ANOVA and Tukey HSD tests. All the materials except Cerasmart and Vita Enamic had significantly higher radiopacity values than dentin (p < 0.05). Of the assessed blocks, the highest radiopacity value was observed in Incoris TZI, and the lowest radiopacity value was observed in Vita Enamic. Variolink Esthetic DC and RelyX Unicem Aplicap showed significantly higher radiopacity (p < 0.05) than the other adhesive cements, including enamel and dentin. In this study, the majority of the CAD/CAM materials and all the adhesive resin cements were found to have sufficient radiopacity for prosthetic restorations according to the criteria set by the International Organization for Standardization (ISO). From a clinical and biological point of view, materials should be chosen according to their radiopacity and other properties, such as biocompatibility and esthetics. If the selected restorative CAD/CAM blocks have a radiopacity value less than or equal to dentin, cements with higher radiopacity values are recommended to facilitate radiological diagnoses for periphery and interface of restorations.

OBJECTIVES: This study compared canal transportation and centering ratio produced after instrumentation with a single heat-treated reciprocating system, WaveOne Gold (WOG; Dentsply Sirona, Tulsa, OK, USA) and a single heat-treated rotary instrument, XP-endo Shaper (XPS; FKG, La Chaux-de-Fonds, Switzerland), using micro-computed tomographic (micro-CT) imaging, and evaluated the ability of double-digital radiography (DDR) to detect canal transportation. MATERIALS AND METHODS: Mesial root canals of mandibular molars with severe curvature (25–70°) were randomly assigned to either WOG or XPS groups for preparation. Centering ratio was measured by micro-CT imaging, while canal transportation was measured by micro-CT and DDR methods at 3, 5, and 7 mm from the apex. Data were statistically compared between groups using the t test (α = 5%). RESULTS: The micro-CT method showed that XPS’s shaping ability regarding the centering ability (P = 0.030) and canal transportation (P = 0.028) was significantly better than WOG only at the 7-mm level. The DDR technique detected no difference in canal transportation between groups at any level (P > 0.05); however, a significant difference between evaluation methods was detected at the 5-mm level in the WOG group (P = 0.023). CONCLUSIONS: Micro-CT technique revealed a significantly better centering ability and less canal transportation with XPS compared to WOG. The DDR technique was not capable of detecting the significant difference between the tested groups. CLINICAL RELEVANCE: Root canal curvatures may lead to procedural errors during endodontic treatment. Thus, differences on the shaping ability of single heat-treated reciprocating and rotary systems should be known.

To compare the accuracy of computer monitor and smartphone screen for radiographic diagnosis of marginal gap. Forty teeth with mesial-occlusal-distal inlays (each tooth with a perfect fit and a 0.4-mm marginal gap restoration) were imaged with a phosphor plate system. Original digital radiographs were exported and analyzed with two different methods: computer monitor and smartphone screen; for the last method, images were shared with WhatsApp. Three examiners assessed all radiographs (n = 160) for the presence of marginal gap by using a dichotomous scale (yes/no). Diagnostic performance of each examiner and viewing method was evaluated by means of sensitivity (Se), specificity (Sp), and overall accuracy (Ac). Difference between the frequencies of gap detection of each method was analyzed using the McNemar test. Intra- and inter-examiner agreements were calculated using kappa statistics. Intra- and inter-examiner agreements were ≥ 0.80 for both methods. Similar diagnostic performance was found for computer monitor (Se = 0.87–1; Sp = 0.8–0.97; Ac = 0.84–0.99) and smartphone (Se = 0.77–1; Sp = 0.87–1; Ac = 0.88–0.95) viewing methods. No statistically significant differences in the frequency of gap detection were observed between the methods (P > 0.05). Diagnostic accuracy of smartphone screens was similar to that of computer monitor for marginal gap detection. Smartphones are becoming a common daily tool. In this sense, it might be an important new aid in Dentistry, including radiographic evaluation, which could benefit patients and dentists.

The aim of the present in vitro study was to determine the ability of Diagnocam to detect caries at an early stage and to compare it with digital radiography. One hundred twenty proximal surfaces composed equally of sound and decayed surfaces were evaluated by assessing images captured with Diagnocam (Kavo, Biberach, Germany) and digital radiography (DR). All images were assessed twice by two calibrated dentists with a minimum interval of 2 weeks between examinations. The results were compared with μCT scans. Inter-rater reliability showed nearly perfect agreement; a high intra-rater reliability was calculated. Spearman’s rank correlation coefficients showed a strong correlation of Diagnocam and μCT (0.82). DR and μCT achieved a slightly lower correlation (0.73). The surfaces were categorized into sound surfaces, enamel lesions, and dentin lesions to determine intraclass correlation coefficients (ICCs), sensitivity and specificity. ICCs between μCT and DR ranged from 0.20 to 0.63. ICCs between Diagnocam and DR were higher in all categories and ranged from 0.56 to 0.83. Sensitivity, specificity, and accuracy of Diagnocam achieved mostly higher values than DR. In detecting enamel lesions, sensitivity was 0.36 for DR and 0.59 for the DC. The areas under the receiver operating characteristic (ROC) curves of Diagnocam were larger than those of DR in all categories. Diagnocam is more capable of detecting initial proximal lesions than DR and also has a higher sensitivity for dentin lesions. However, caries progression in dentin cannot be reliably determined with Diagnocam. Diagnocam may be suitable as a supplement to X-ray diagnostics in clinical use.

To evaluate the performance of periapical radiography assessed under different radiographic brightness and contrast variations in the detection of simulated internal (IRR) and external (ERR) root resorption lesions. Additionally, observers’ preferences related to image quality for these diagnostic tasks were evaluated. Thirty single-root teeth were divided into two groups (n = 15): IRR, in which lesions were simulated using mechanical and biochemical processes; and ERR, in which cavities standardized with drills of different sizes were performed on the root surfaces. Digital radiographs were obtained and subsequently adjusted in 4 additional combinations, resulting in 5 brightness/contrast variations (V1–V5). Five radiologists evaluated the radiographs. The observers’ preference on the image quality was also recorded. For both conditions, there were no differences in the accuracy and specificity between the five brightness/contrast variations (p > 0.05), but the sensitivity for ERR was significantly lower in V4 (+ 15% brightness/−15% contrast) in the large size (p

To evaluate the effect of enhancement tools of intraoral digital radiographs on the assessment of vertical root fracture (VRF) and to quantify the resultant image noise.Thirty single-rooted human teeth (15 control and 15 fractured) were each radiographed in four intracanal conditions: no filling, gutta-percha, metal post, and fiberglass post, totaling 120 original images. Two filters were applied to the original images-Sharpen filter (SF) and Edge Enhancement filter (EE), and brightness and contrast were adjusted in four combinations (BC1 to 4), resulting in 840 images. Five oral radiologists analyzed the images for VRF detection. Pixel intensity was obtained in two regions from the radiographs. Diagnostic values were calculated and compared by two-way ANOVA, and the SD values of pixel intensity values were compared by one-way ANOVA (α = 0.05).There were no significant differences in accuracy for VRF detection between the experimental groups (p 0.05). Teeth with metal post presented the lowest sensitivity (p 0.05) for all experimental conditions, except for SF and EE (p 0.05). BC2, BC3, and BC4 had higher specificity than SF (p ≤ 0.05) for all intracanal conditions. Analysis of pixel intensity showed that all enhanced images presented statistically significant higher noise compared to those of the original images (p ≤ 0.05).Digital enhancement tools in digital radiography increase image noise; however, they can be used without compromising VRF detection.The use of digital enhancement does not impair the detection of VRF and, therefore, can be applied for this purpose according to the observer preference.

OBJECTIVES The aims of this clinical study were to investigate the diagnostic accuracy of near-infrared light transillumination (NILT) as a novel X-ray-free method for proximal dentin caries detection and to compare this method to established diagnostic methods. MATERIALS AND METHODS A total of 127 interproximal dentin caries lesions without any cavity within visible dentin in posterior teeth from 85 consecutively selected patients were included. Visual and radiographic diagnoses and laser fluorescence measurements were available. NILT images were obtained, and a dentin lesion was predicted if a demineralisation involved the enamel-dentin junction (NILT-EDJ) or a shadow in dentin was detectable (NILT-dentin). Included lesions were opened and validated (reference standard). The statistical analyses included descriptive analyses and calculations of sensitivity, specificity and Az values. RESULTS The diagnostic accuracy with respect to the reference standard was 1.6% for visual inspection, 66.7% for laser fluorescence, 96.1% for digital radiography, 29.1% for NILT-dentin and 99.2% for NILT-EDJ. Bitewings (Az 0.984) and NILT-EDJ (Az 0.992) performed equally. CONCLUSION Given the lack of true negatives in the study, the diagnostic accuracy of NILT achieved the same level as bitewings for the detection of proximal dentin caries. CLINICAL RELEVANCE This study might indicate that NILT could reduce the usage of bitewings.

The aim of this study was to compare the diagnostic accuracy of conventional films and direct digital radiographs (DDR), in the determination of the depth and type of simulated periodontal intrabony defects.Three types of periodontal intrabony defects (one, two, and three walled) were artificially created in dry mandibles. Standard radiographic images were taken with Ultraspeed, Ektaspeed, Insight films, and DDR. The radiographic images were evaluated by three oral radiologists to identify the type and depth of these defects on the radiographs.The average measured depth of the defects on the dry mandibles was 7.85 mm. The average depth of the type 1 defect on the radiographs was 7.19 mm, type 2 was 7.18 mm, and type 3 was 7.15 mm. The average depth of the defects via the Ultraspeed film was 7.15 mm, Ektaspeed film was 7.17 mm, Insight film was 7.19 mm, and DDR was 7.20 mm. Type 1, type 2, and type 3 defect depth measurements showed 8.9, 9.7, and 16.3% understated, respectively (p 0.01). The accurate estimation rates of type 1, type 2, and type 3 defects were 93.8, 53, and 25.4%, respectively.Both radiographic techniques have the same diagnostic value and display the minor destructive changes in the bone. As the number of osseous walls increases, it becomes difficult to determine the defect type and morphology. Further research is needed to monitor the intrabony defects, with less radiation exposure.The accurate identification of defect type and depth depends on the number of walls, not the imaging methods.

Recent developments in digital computer technology have enabled radiological diagnosis to be performed using a monitor screen. In medical radiography, the importance of monitors has been shown in many diseases. Digital imaging and communication in medicine (DICOM)-compatible monitors are widely used. However, the effect of monitors on the diagnosis of oral disease has not yet been clarified and remains controversial. The aims of this study are to compare the caries diagnostic ability between DICOM monitors and other monitors and to examine if monitor capability affects the diagnosis. One hundred proximal surfaces of 50 extracted human upper premolar teeth were used as specimens. Intra-oral radiographs of all specimens were taken digitally. Three types of monitors were compared in terms of caries diagnostic ability: a DICOM standard-compatible monitor, a standardized personal computer (PC) monitor, and a tablet PC. Six oral radiologists diagnosed each radiograph independently. Areas under the receiver operating characteristic (ROC) curve were generated and compared. Area under the ROC curve (AUC) of the DICOM monitor, PC monitor, and tablet PC was 0.68147, 0.67002, and 0.60189, respectively. There was no significant difference between the DICOM monitor and the PC monitor, but the tablet PC showed significantly lower accuracy. There were no significant differences among the monitors for dentin caries (p > 0.05). DICOM-compatible monitors and PC monitors have similar capabilities, but tablet PCs showed lower diagnostic accuracy, especially for superficial caries. Appropriate monitors are needed for radiographic diagnosis on monitor screens.

The objective of this study is to assess the potential of DIAGNOdent readings in detecting carious lesions under composite resin restorations in comparison with digital radiography. One hundred extracted third molars were visually selected: 50 with and 50 without carious lesions. Class I occlusal cavities were prepared. In the carious group, caries were left on the floor of cavities before composite resin restoration. The occlusal surface relevant to the caries position in the carious group and a counterfeit point in the non-carious group were marked for DIAGNOdent reading. Teeth were fixed in a plaster box. Digital radiographs were taken and examined by four observers. Definitive diagnosis of caries was based on a histological assessment. Diagnostic accuracy for each method was expressed by the area under the receiver operating characteristic (ROC) curve. Differences between the areas under the ROC curves were assessed using the McNemar test. The respective sensitivity (95 % confidence interval (%CI)) and specificity (95 %CI) of DIAGNOdent were 0.74 (0.66–0.83) and 0.84 (0.76–0.92). The respective average diagnostic performance values for digital radiographs assessed by four observers were 0.54 (0.36–0.72) and 0.77 (0.65–0.86). The respective ROC values for DIAGNOdent and digital radiographs based on four observers were 0.79 and 0.65. Cohen’s kappa statistic revealed a moderate to substantial agreement among interobserver reliabilities (k = 0.60–0.77). There were no statistically significant differences between DIAGNOdent and digital radiographs for the detection of dental caries under composite restoration (p value > 0.05). DIAGNOdent was developed to detect caries on occlusal surfaces and was tested for caries adjacent to filling materials. This study demonstrated the use of DIAGNOdent in detecting caries under old composite restorations.

This study evaluated the effect of two different tube voltages on clinicians’ ability to diagnose approximal carious lesions in digital radiographs. One hundred extracted teeth were radiographed twice at two voltage settings, 60 and 70 kV, using a standardized procedure. Seven observers evaluated the radiographs on a standard color monitor pre-calibrated according to DICOM part 14. Evaluations were made at ambient light levels below 50 lx. All observations were analyzed with receiver operating characteristic curves. A histological examination of the teeth served as the criterion standard. A paired t test compared the effects of the two voltages. The significance level was set to p < 0.05. Weighted kappa statistics estimated intra-observer agreement. No significant difference in accuracy of approximal carious lesion diagnosis was found between the two voltage settings. But five observers rated dentin lesions on radiographs exposed at 70 kV better than on radiographs exposed at 60 kV. Intra-observer agreement differed from fair to moderate. There was no significant difference in accuracy of approximal carious lesion diagnosis between digital radiographs exposed with 60 or 70 kV.

The aim of this study was to compare detection of non-cavitated approximal caries lesions in images from seven solid-state intraoral digital receptors, with particular focus on two task-specific enhancement filters. One hundred and sixty approximal non-cavitated surfaces were radiographed under standardized conditions using the following seven intraoral solid-state digital receptors: two CMOS systems, Schick CDR-APS, and Kodak RVGui; and five CCD systems: Visualix, VistaRay, Dixi2, Sidexis, and Dr. Suni Plus. The Kodak RVGui digital images were captured with two task-specific, predefined enhancement filters (“dento-enamel” and “periodontal”). Eight observers examined the digital images for the presence or absence of approximal carious lesions. The teeth were subsequently sectioned for histological analysis, which served as the “gold standard” for the radiographic examination and allowed for a calculation of sensitivity, specificity, and overall accuracy. Both RVG modalities obtained the highest sensitivity values, which were significantly higher than with Visualix, Sidexis, and VistaRay images (all P ≤ 0.03). The RVG “periodontal” images also had a higher accuracy than Dixi2, Dr. Suni Plus, and CDR-APS (P ≤ 0.05). The RVG modalities also showed the largest observer variation, and their high sensitivities were mainly due to one observer. The sensitivity for VistaRay images was the lowest of all modalities and significantly lower than almost all the other digital systems (P ≤ 0.02; except for Visualix). Besides, Visualix images had lower sensitivity than CDR-APS, Dixi2, and Dr. Suni Plus images (P ≤ 0.003). On the other hand, the two RVGui image types showed the lowest mean specificity values. For overall accuracy, the differences among the modalities were non-significant (P > 0.05).

The objective of this study is to compare the diagnostic accuracy of visual inspection (VI) and storage phosphor plate (SPP) radiography for the detection of artificial demineralization bordering different laminate veneers.Twenty human maxillary canine teeth were prepared. All-ceramic (A) and hybrid ceramic (H) laminate veneers were fabricated and luted. Veneered teeth were covered except for a circular window on the proximal surface bordering restorations. Teeth were kept in acetic acid buffer to create demineralization and imaged with a SPP system. Ten observers evaluated all teeth first visually then with SPP images for the presence/absence of demineralization. Teeth were examined using scanning electron microscopy (SEM) as well. The accuracy was expressed as the area under the ROC curves (A(z)). Pair-wise comparisons were performed using two-way ANOVA and post hoc t test (p = 0.05). Fleiss kappa (κ) was used for agreement.SPP radiography was better than the VI for both veneers (p = 0.004). The A(z)s of two veneers were different for both VI (p 0.005) and SPP (p 0.005). SEM evaluation revealed lesions confined to enamel. κ was fair for H, and fair to moderate for A. Agreement was higher for the radiographic evaluation for both veneers.Enamel demineralizations bordering hybrid and ceramic laminate veneers can be detected better with SPP radiography than VI and detectability was better for all-ceramic veneers than the hybrid ceramic ones.Early detection of enamel demineralizations bordering laminate veneers would result in time-saving and less-invasive treatment methods; therefore, SPP radiography may be recommended in clinically suspicious cases since it provides better diagnostic accuracy.

Logicon Caries Detector™ (LDDC) is the only commercially available computer-assisted diagnostic system for caries diagnosis. The object of this study is to elucidate the efficacy of LDDC when used by inexperienced dentists. Fifty extracted teeth were imaged using an RVG6000. Seven dentists who had just passed the Japanese National Dental Board Examination observed those images without LDDC (woLDDC) and assessed the probability that caries lesions were present, then re-assessed the same teeth using LDDC (wLDDC). The areas under the receiver operating characteristic curves (Az) were compared. No statistically significant difference was found between woLDDC Az values and wLDDC Az values when caries lesions of all depths were considered. When positive cases were restricted to caries lesions in the inner half of the enamel or to dentine caries lesions, however, wLDDC Az values were significantly larger than woLDDC (p = 0.043 and 0.018, respectively).

The aim of this study was to investigate changes in the trabecular architecture of the alveolar bone beneath overhanging restorations with bitewing radiographs in patients having no radiographically visible vertical bone loss. Twenty-eight digital bitewing radiographs with overhanging restorations and 28 digital bitewing radiographs without any restorations belonging to the contralateral side of the same patient were included in the study. Regions of interests (ROI) were created in the alveolar bone along the interproximal regions. These ROIs were segmented to binary images with ImageJ, and, within these binary images, the number of radiographically visible trabecular bone islands per unit area was counted; in addition, the Feret diameter and fractal dimension (FD) were measured. It was found that the mean number of objects per unit area was statistically different in alveolar bone with overhanging restorations from control sites (p < 0.0001). However, the FeD (p = 0.179) and FD (p = 0.963) did not show statistically significant differences between alveolar bone with and without overhanging restorations.
