Clinical Relevance: Clinical skills training is essential in optometry curricula to develop core graduate entry competencies, including self-directed learning to facilitate life-long learning. Efficient and efficacious approaches are required to optimise student and educator time and resources., Background: A video library of optometric clinical skills was created in 2012 to support self-directed student learning and face-to-face training. Use of videos in higher education generally increased during the COVID-19 pandemic and has remained above pre-pandemic levels. This study aimed to capture and evaluate student access patterns in the library to determine which videos were accessed most, and if this changed with training stage and following the pandemic., Methods: Data on student viewing and critique submission were extracted for 71 videos from a clinical skills video library from 2018 to 2023. The number of videos viewed by students was analysed by year, cohort, video type (gold standard, scripted errors, summary, and student generated) and video category (history, screening, refraction, anterior segment, posterior segment and tonometry)., Results: First-year students viewed the most videos, and this decreased significantly during and following the pandemic. Overall, the number of videos viewed decreased with increasing course stage. Video access, by category, aligned with the curriculum. Views were highest for gold standard and student videos. Viewing of scripted error videos and submission of critiques of procedural videos was low for all course stages and years., Conclusion: A web-based video library of optometric clinical skills was used for self-directed learning, mostly by students early in their training. Similar resources developed in the future should align with curriculum and include exemplar and student-based videos. Exploration of student and educator perspectives regarding factors that impact engagement with the online library warrants investigation to facilitate optimal integration in post-pandemic times.

Clinical Relevance: As registered health professionals, optometrists have a responsibility to apply ethical reasoning skills to ensure the best outcomes for their patients., Background: The ability to apply ethical knowledge in practical situations has emerged as a crucial skillset for health professionals. This research aimed to highlight the potential benefits and challenges associated with employing case-based scenario workshops as a method for teaching the application of ethical knowledge., Methods: An intervention study was conducted at an Australian university to establish whether a case-based ethics workshop could improve the confidence of optometry students in navigating ethical dilemmas. Prior to completing the workshop, optometry students were invited to complete a survey, and the survey was repeated at the end of each workshop., Results: Fifty nine students completed the pre-workshop survey, and 49 students completed the post-workshop survey. A Mann-Whitney U test showed no significant difference in the baseline confidence levels of students who had prior or no prior experience in tertiary education, healthcare work or eyecare work. There was a statistically significant improvement in confidence navigating ethical dilemmas (Z = -2.541, p  = 0.011) from 53% very or moderately confident pre-workshop to 82% post-workshop., Conclusion: These results suggest that case-based ethics workshops are an effective educational tool that should be considered for incorporation into optometry and other health profession courses.

The Entry-level Competency Standards for Optometry in Australia identify the required knowledge, skills and attributes for an optometrist to practise safely and competently. The 2022 competencies are the sixth generation of entry-level competency standards. To ensure the standards reflect contemporary practice and the expectations placed on optometrists, a full revision of the standards was commenced in late 2019. A steering committee was appointed by Optometry Australia to oversee the development of the Entry-level Competency Standards for Optometry. The development was informed by a review of best-practice design for health professional competency standards undertaken by Paul Hager on behalf of the Optometry Council of Australia and New Zealand (OCANZ) in 2019. To identify the changes required, workshops were held with optometrists and representatives from stakeholder organisations leading to the formation of draft Entry-level Competency Standards for Optometry in early 2020. The impacts of the COVID-19 pandemic meant development halted in March 2020, and resumed in late 2021. Consultation on the Entry-Level Competency Standards for Optometry 2022 was undertaken in two rounds with the first being targeted stakeholder consultation, followed by public consultation. Following consultation, feedback was considered by the steering committee and amendments were made to produce the final Entry-level Competency Standards for Optometry 2022. This document outlines the development process and the major changes in the updated standards. The major changes include a redesigning of the Domains, a reduction in the level of detail and a strengthening of the expectations around cultural responsiveness and culturally safe care. The revised standards capture the current entry requirements for the optometric profession as of 2022. The updated Entry-level Competency Standards for Optometry 2022 were endorsed by the national board of Optometry Australia and accepted by the Optometry Board of Australia in February 2023.

Clinical imaging provided by optical coherence tomography (OCT) and its variant, OCT-angiography (OCT-A), has revolutionised eyecare practice. The imaging techniques allow for the identification and quantification of ocular structures, supporting the diagnosis and prognosis of eye disease. In this review, an overview of the usefulness of OCT-A imaging in the diagnosis and management of a range of ocular conditions is provided when used in isolation or in combination with other imaging modalities and measures of visual function (visual field results). OCT-A imaging has the capacity to identify and quantify ocular vasculature non-invasively, thereby assisting the clinician in the diagnosis or to determine the efficacy of intervention in ocular conditions impacting retinal vasculature. Thus, additional clinically useful information can be obtained in eye diseases involving conditions such as those impacting retinal vessel occlusion, in diabetic retinopathy, inherited retinal dystrophy, age-related macular degeneration, choroidal neovascularisation and optic nerve disorders. Through a clinical case series, various ocular conditions are reviewed, and the impact of OCT-A imaging is discussed. Although OCT-A imaging has great promise and is already used in clinical management, there is a lack of set standards to characterise altered vascular features in disease and consequently for prognostication, primarily due to a lack of large-scale clinical trials and variability in OCT-A algorithms when generating quantitative parameters.

Clinical Relevance: The number, demographics, registration status and geographic distribution of optometrists in Australia who do not renew their registration is reported., Background: The size of the optometry workforce in Australia is determined by the number of new entrants to the profession and the number of optometrists leaving it. Limited attention has been paid to the latter., Methods: A dataset obtained from the Australian Health Practitioner Registration Agency about registered optometrists during the period 1 January 2011 to 31 December 2019 was analysed. It included registrants' first year of registration, gender, year of birth (in five-year bands), optometry qualification; and annual collection of registration type and postcode of principal place of practice., Results: Data for 6,595 registrants were analysed. Over the study period, 626 optometrists left the register. When those leaving the register were examined by year of birth bands, two main groups emerged - optometrists aged in their fifties or older, and optometrists who were under forty years of age and disproportionately male. Registration type had a significant effect on whether a registrant left or remained on the register ( p  < 0.05). Those holding Non-practising Registration or Limited Registration were more likely to leave the register. Registrants with an optometry qualification from an overseas institution, including from New Zealand, were more likely to leave the register ( p  < 0.05). Optometrists whose registration was not endorsed were more likely to leave the register ( p  < 0.05). No significant difference was found when the geographic location of optometrists who left the register was compared with those who remained., Conclusion: Optometrists who left the register fell into two main groups - late-career and early-career. An unanticipated finding was that younger optometrists who left the register were disproportionately male. What motivates optometrists in Australia to leave the register is worthy of future research.

Clinical Relevance: A method for determining 10-2 deployment in glaucoma with the goal of detecting additional visual field sensitivity for the purpose of functional monitoring is proposed., Background: To provide a pilot method for determining when to deploy the 10-2 visual field (VF) test grid in glaucoma by characterising the 'functional vulnerability zone'., Methods: The cross-sectional 24-2 (central 12 locations) and 10-2 VF results from 133 eyes of 133 glaucoma subjects were used to describe the central Hill of Vision using VF sensitivity. The 'volume' (defined using arbitrary units, A.U.) under the Hill was calculated. A greater A.U. on the 10-2 indicated a functional vulnerability zone (FVZ), signifying additional clinical dynamic range for potential future monitoring. The main outcome measures were calculated A.U. and 24-2 factors which were significantly related to A.U. differences between 24-2 and 10-2., Results: Over 55% of patients had an FVZ (A.U. greater using 10-2). Several 24-2 features (worse mean deviation, worse central 24-2 mean defect, and a higher proportion of defective locations) were significant in the FVZ cohort compared to non-FVZ. 24-2 mean deviation levels at which 10-2 may be favoured were low at -3.16 to -3.62 dB. Specifically, 5 or more defective central 24-2 test locations were associated with an FVZ. Subjects exhibiting a less severe defect on the 10-2 were more likely to have an FVZ, indicating its potential for future VF monitoring., Conclusions: The authors propose several clinical markers, focussing on the 24-2, which can guide clinicians on when the 10-2 may have utility in glaucoma assessment. The authors provide a pilot reference spreadsheet for clinicians to visualise the likelihood of 10-2 utility in the context of an FVZ.

Artificial Intelligence is a rapidly expanding field within computer science that encompasses the emulation of human intelligence by machines. Machine learning and deep learning - two primary data-driven pattern analysis approaches under the umbrella of artificial intelligence - has created considerable interest in the last few decades. The evolution of technology has resulted in a substantial amount of artificial intelligence research on ophthalmic and neurodegenerative disease diagnosis using retinal images. Various artificial intelligence-based techniques have been used for diagnostic purposes, including traditional machine learning, deep learning, and their combinations. Presented here is a review of the literature covering the last 10 years on this topic, discussing the use of artificial intelligence in analysing data from different modalities and their combinations for the diagnosis of glaucoma and neurodegenerative diseases. The performance of published artificial intelligence methods varies due to several factors, yet the results suggest that such methods can potentially facilitate clinical diagnosis. Generally, the accuracy of artificial intelligence-assisted diagnosis ranges from 67-98%, and the area under the sensitivity-specificity curve (AUC) ranges from 0.71-0.98, which outperforms typical human performance of 71.5% accuracy and 0.86 area under the curve. This indicates that artificial intelligence-based tools can provide clinicians with useful information that would assist in providing improved diagnosis. The review suggests that there is room for improvement of existing artificial intelligence-based models using retinal imaging modalities before they are incorporated into clinical practice.

Clinical Relevance: An assessment of the total number, demographics and geographic distribution of optometrists in Australia may inform policy to address the maldistribution of the workforce., Background: Concerns have been expressed about the growth of the optometry workforce in the context of, in the last decade, the establishment of four new optometry programs in addition to the three long-standing programs., Methods: This paper analysed data obtained from the Australian Health Practitioner Regulation Agency about registered optometrists during the period 1 January 2012 to 31 December 2019. The de-identified dataset included information about registered optometrists in Australia: their first year of registration, gender, year of birth (in five-year bands), optometry qualification, registration type including endorsement for therapeutic practice and the postcode of the principal place of practice., Results: Data for 6,596 registrants were obtained over the study period during which the number of female optometrists increased significantly. The age profile of the profession shifted, with younger age bands becoming a significantly greater proportion of the profession. The majority of the profession comprised optometrists with a qualification from one of the three long-standing Australian optometry programs. Graduates of those programs accounted for 75.1% of registered optometrists in 2019, while 14.3% held a qualification from an overseas institution. The proportion of optometrists practising in major Australian cities was greater than the proportion of the population in those locations. A concomitant low proportion of optometrists, relative to population size, was found in regional and remote areas., Conclusion: Optometrists practising in Australia can be generalised as mostly female graduates of one of the three long-standing optometry programs holding general registration with a therapeutic endorsement, and with a principal place of practice located in a major Australian city. A significant maldistribution of the workforce was found that did not change significantly during the study period.

Optometry Australia's infection control guidelines 2020 deliver a revision of the paper: Infection control guidelines for optometrists 2016. A review of recent literature was undertaken, with information collected from peer-reviewed journal articles, guidelines from professional societies, government health departments and instructions from equipment manufacturers. This information was used to provide an update on current infection control best practice. The guidelines are presented in two sections: standard precautions and transmission-based precautions. The standard (routine) precautions section covers: hand hygiene; personal protective equipment; safe handling and disposal of sharps; routine environmental cleaning; reprocessing of reusable equipment, instruments and contact lenses; respiratory hygiene and cough etiquette; aseptic and clean technique; and waste management. The transmission-based precautions section covers considerations for optometry staff and/or patients when standard precautions alone may not be sufficient to prevent the spread of an infectious agent; particularly relevant during the COVID-19 pandemic. Finally, a comprehensive list of disinfection or sterilisation techniques to use on reusable devices, instruments or equipment in optometric practice is provided in an Appendix.

Clinical Relevance: An assessment of the total number, demographics and geographic distribution of new entrants to the optometry profession in Australia can assist planning for workforce requirements., Background: Over the past decade, the number of registered optometrists in Australia has increased by 30.1 per cent, a rate that is greater than the population growth of the country (12.1 per cent). Concerns have been expressed about the size of the optometry workforce in a context of increasing numbers of graduating optometrists. This paper analyses data obtained from the Australian Health Practitioner Regulation Agency (AHPRA) about new entrants to the profession and their initial practice location during the period 1 July 2010 to 30 June 2018., Methods: A de-identified dataset was obtained from AHPRA that revealed the following characteristics of new entrants: qualification, gender, year of birth (in five-year bands), registration type, registration endorsement and principal place of practice including its Remoteness Area classification., Results: Data for 1,680 entrants were eligible for analysis; 80 per cent graduated from an Australian university, 12 per cent graduated from the optometry program in New Zealand, and seven per cent were graduates of an overseas university. The remaining two per cent registered via the Trans-Tasman Mutual Recognition Agreement, although the dataset did not include the qualification. The United Kingdom and Republic of Ireland provided the majority of overseas entrants (60 per cent). Most (75 per cent) entrants commenced practice in a major Australian city. Graduates of Australian universities tended to commence practice in the state in which they trained or an adjacent state or territory. Juxtaposed on the data outlined above is the high proportion (42 per cent) of overseas-trained optometrists commencing practice in Western Australia., Conclusion: Coincident with the newer optometry programs producing graduates is the increased number of optometrists entering the Australian workforce over the past decade, with the majority commencing practice in major cities. Australia-trained optometrists tend to commence practice in the state where their training was undertaken. New entrants to the optometry profession can be generalised as graduates of an Australian optometry program, female, aged in their early-mid 20s and qualified for therapeutic practice.

Background: To determine the extent of scleral lens fitting worldwide and to characterise the associated patient demographics., Methods: Survey forms were sent to contact lens fitters around the world, every year for 20 consecutive years (2000 to 2019). Practitioners were asked to record data relating to the first 10 contact lens fits or refits performed after receiving the survey. Data were analysed for those countries reporting ≥ 1,000 contact lens fits during this period., Results: A total of 369,209 contact lens fits were recorded from 40 eligible countries, comprising 2,309 scleral lens fits and 366,900 other (non-scleral) lens fits. Overall scleral lenses represented 0.76 per cent of all contact lens fits with significant differences between countries (p < 0.0001), ranging from no fits in six countries: Hungary, Indonesia, Iran, Korea, Malaysia and Singapore, to 5.9 per cent in Switzerland. There has been an increase in scleral lens fitting over the survey period (p < 0.0001), with the rate of fits increasing from negligible (< 0.50 per cent) before 2006 and then increasing from 2011 onward (p < 0.0001). Scleral lenses were fit to older patients (38.5 ± 15.0 years) compared to other lenses (31.4 ± 13.7 years) (p < 0.0001), this age disparity increasing over the survey period (p < 0.0001) and were more likely to be males (scleral lens fits 53 per cent, non-scleral lenses 33 per cent, p < 0.0001). The increase in scleral lens fits is commensurate with the increase in related publications in the peer-reviewed literature over the same period (p < 0.05, r 2 = 0.82)., Conclusion: Scleral lens use has increased since 2011 with these specialised lenses more likely to be fitted to males and older patients than non-scleral lenses., (© 2020 Optometry Australia.)

Background: The aim was to investigate the understanding of Australian optometrists in relation to myopia, its natural history and associations with vision-threatening ocular disease, self-reported clinical diagnosis and management approaches for childhood myopia, engagement with adult caregivers, and utilisation of information to guide practice., Methods: An online survey was distributed to Australian optometrists (n = 4,124). Respondents provided information about their demographics (for example, gender, age, practice location and modality), myopia knowledge, self-reported practice behaviours relating to childhood myopia, the information and evidence base used to guide their practice, and perceived extent of adult caregiver engagement in making management decisions for myopic children., Results: Completed surveys were returned by 239 optometrists (six per cent completed response rate). Most respondents demonstrated knowledge of the association between high myopia and retinal breaks, retinal detachment and primary open-angle glaucoma. Optometrists used a range of techniques to diagnose childhood myopia, with a preference for non-cycloplegic refractive measures. The most common approaches to management were single-vision distance and progressive addition spectacle lenses, despite most optometrists identifying orthokeratology, low-dose (0.01%) topical atropine and soft peripheral defocus contact lenses as three potentially more effective therapeutic interventions for modifying childhood myopia progression. Almost 90 per cent of respondents considered increasing time spent outdoors to be beneficial for reducing the rate of myopia progression. The main sources of information used to guide clinical practice were continuing education conferences and events, systematic reviews, and personal experience. Respondents perceived adult caregivers to generally be involved in myopia management decisions, and considered all aspects of myopia education as important., Conclusion: Current optometric practices reflect the inconclusive nature of several key aspects of the evidence for childhood myopia management. Australian optometrists appear aware of emerging evidence, but are not routinely adopting measures that have not yet received regulatory approval for modulating childhood myopia progression. Background: Some universities are looking to provide a more diverse range of clinical learning experiences through extended clinical placement programs. This approach will potentially have a significant impact on practitioners. The aim of this study was to conduct a national survey of optometrists to ascertain their perspectives on participating in extended clinical placement programs., Methods: Members of Optometry Australia were invited to participate in a survey conducted during June and July 2014., Results: A total of 268 practitioners participated (six per cent of registered Australian optometrists): 159 were predominantly employees or locums and 109 were owners or managers who identified as the key representative of a practice or organisation for the purpose of this survey. Almost two-thirds (65 per cent) of participants, who were employees or locums were supportive of extended clinical placement programs. Among this group, females were more likely to be supportive than males (p = 0.033). In comparison, just over one-third (34 per cent) of participants who were key decision-makers were supportive, with 30 per cent possibly supportive and 36 per cent not supportive. Among key decision-makers, males were more likely to be supportive (p = 0.009). The top three perceived advantages of supervising a student were: opportunity to mentor early career development, opportunity to give back to the profession and future recruitment. The top three perceived disadvantages were: burden on time, decrease in number of patients examined and burden on support staff. Suggested incentives for supervising students were credit for continuing professional development and financial remuneration., Conclusion: There appears to be moderate support for extended clinical placement programs; however, there are incentives that might engage a larger proportion of the profession in the future. These findings can inform the development of effective and sustainable clinical training programs for optometry students. Background: Differences in accommodation when reading Chinese, as compared to Latin, characters have been suggested to have a role in the higher prevalence of myopia in some Asian countries. Yeo and colleagues (Optom Vis Sci 2013; 90: 156-163) found that, in Chinese-literate children, accommodation was marginally more accurate (by less than 0.05 D), when reading Chinese text. This was attributed to the additional cognitive demand associated with interpreting the more complex Chinese symbols. The present study compared responses to single Chinese and Latin characters, while controlling for cognitive demand., Methods: The monocular accommodative response was measured in Chinese-illiterate adults (10 emmetropes, mean spherical equivalent: -0.07 ± 0.42 D, age: 29.9 ± 4.2 years; 11 myopes, mean spherical equivalent: -4.28 ± 2.84 D, age: 31.7 ± 4.6 years) with an open-field autorefractor. Four Chinese and three Latin characters (approximately 1.15 degrees subtense) were individually presented on a display screen one metre away from the subject, while their vergence was varied over the range zero to 5.00 D using spectacle trial lenses. The slope and the accommodative error index (AEI) were calculated from the accommodative stimulus/response curves (ASRC)., Results: No statistically significant differences were found between refractive groups or among characters within the same refractive group in ARSC slopes (Latin: 0.87 ± 0.14 for myopes versus 0.81 ± 0.12 for emmetropes; Chinese: 0.84 ± 0.12 for myopes versus 0.85 ± 0.12 for emmetropes). No significant differences were found between characters in accommodative error index either (Latin, 0.78 ± 0.42 D for myopes versus 1.15 ± 0.72 D for emmetropes; Chinese, 0.74 ± 0.37 D for myopes versus 1.17 ± 0.83 D for emmetropes). However, accommodative error indices and accommodative errors were significantly higher for emmetropes., Conclusion: Under controlled cognitive demand, Chinese and Latin characters elicited similar responses in both individual refractive groups. This study fails to support the hypothesis that development of myopia in some Asian populations is associated with larger lags of accommodation when reading or viewing Chinese characters., (© 2015 Optometry Australia.)
