IgG4‐related disease (IgG4RD) with intracranial involvement is rare. We report a 56‐year‐old male who had an excellent response to rituximab and dexamethasone after going undiagnosed for 5 years. After 3 years of rituximab maintenance, he has no evidence of disease on brain MRI., Hypertrophic pachymeningitis is a rare manifestation of IgG4‐related disease and is usually responsive to treatment with rituximab and dexamethasone. Maintenance rituximab can be considered to prevent relapse of disease after an initial response.

ALK‐positive ALCL can lead to rapid irreversible destruction of the optic pathway. Early treatment should be instituted in patients with CNS involvement or those at increased CNS risk.
