Simple Summary: This review investigates how preprocessing parameters are related to the reproducibility and reliability of radiomic features derived from multimodality imaging techniques such as computed tomography (CT), magnetic resonance imaging (MRI), cone-beam CT (CBCT), and positron emission tomography (PET)/CT. Radiomics, which involves extracting quantitative features from medical images, shows great potential as a source of non-invasive clinical biomarkers but is hindered by variability in imaging parameters, especially during acquisition, reconstruction, and preprocessing. Standardizing and reporting preprocessing procedures is essential for consistent extraction of radiomic features, given their significant role in determining the robustness and reproducibility of these features. Background: Lately, radiomic studies featuring the development of a signature to use in prediction models in diagnosis or prognosis outcomes have been increasingly published. While the results are shown to be promising, these studies still have many pitfalls and limitations. One of the main issues of these studies is that radiomic features depend on how the images are preprocessed before their computation. Since, in widely known and used software for radiomic features calculation, it is possible to set these preprocessing parameters before the calculation of the radiomic feature, there are ongoing studies assessing the stability and repeatability of radiomic features to find the most suitable preprocessing parameters for every used imaging modality. Materials and Methods: We performed a comprehensive literature search using four electronic databases: PubMed, Cochrane Library, Embase, and Scopus. Mesh terms and free text were modeled in search strategies for databases. The inclusion criteria were studies where preprocessing parameters' influence on feature values and model predictions was addressed. Records lacking information on image acquisition parameters were excluded, and any eligible studies with full-text versions were included in the review process, while conference proceedings and monographs were disregarded. We used the QUADAS-2 (Quality Assessment of Diagnostic Accuracy Studies 2) tool to investigate the risk of bias. We synthesized our data in a table divided by the imaging modalities subgroups. Results: After applying the inclusion and exclusion criteria, we selected 43 works. This review examines the impact of preprocessing parameters on the reproducibility and reliability of radiomic features extracted from multimodality imaging (CT, MRI, CBCT, and PET/CT). Standardized preprocessing is crucial for consistent radiomic feature extraction. Key preprocessing steps include voxel resampling, normalization, and discretization, which influence feature robustness and reproducibility. In total, 44% of the included works studied the effects of an isotropic voxel resampling, and most studies opted to employ a discretization strategy. From 2021, several studies started selecting the best set of preprocessing parameters based on models' best performance. As for comparison metrics, ICC was the most used in MRI studies in 58% of the screened works. Conclusions: From our work, we highlighted the need to harmonize the use of preprocessing parameters and their values, especially in light of future studies of prospective studies, which are still lacking in the current literature. Simple Summary: The preoperative definition of the Oncotype DX recurrence score would be critical to identify breast cancer patients who could benefit from chemotherapy before surgery. In this work, we built a machine learning model applied to DCE-MRI images of a publicly available dataset to predict the Oncotype DX score in patients with breast cancer. As a result, the model achieved an accuracy of 60% in the training set and 63% (AUC = 0.66) in the test set. Our findings support the feasibility of radiomics and machine learning for the prediction of prognostic data in breast cancer, encouraging further, preferably multicenter, investigations to further improve the performance of the model and assess its generalizability. Aim: To non-invasively predict Oncotype DX recurrence scores (ODXRS) in patients with ER+ HER2- invasive breast cancer (IBC) using dynamic contrast-enhanced (DCE) MRI-derived radiomics features extracted from primary tumor lesions and a ML algorithm. Materials and Methods: Pre-operative DCE-MRI of patients with IBC, no history of neoadjuvant therapy prior to MRI, and for which the ODXRS was available, were retrospectively selected from a public dataset. ODXRS was obtained on histological tumor samples and considered as positive if greater than 16 and 26 in patients aged under and over 50 years, respectively. Tumor lesions were manually annotated by three independent operators on DCE-MRI images through 3D ROIs positioning. Radiomic features were therefore extracted and selected using multistep feature selection process. A logistic regression ML classifier was then employed for the prediction of ODXRS. Results: 248 patients were included, of which 87 with positive ODXRS. 166 (66%) patients were grouped in the training set, while 82 (33%) in the test set. A total of 1288 features was extracted. Of these, 1244 were excluded as 771, 82 and 391 were excluded as not stable (n = 771), not variant (n = 82), and highly intercorrelated (n = 391), respectively. After the use of recursive feature elimination with logistic regression estimator and polynomial transformation, 92 features were finally selected. In the training set, the logistic regression classifier obtained an overall mean accuracy of 60%. In the test set, the accuracy of the ML classifier was 63%, with a sensitivity of 80%, specificity of 43%, and AUC of 66%. Conclusions: Radiomics and ML applied to pre-operative DCE-MRI in patients with IBC showed promises for the non-invasive prediction of ODXRS, aiding in selecting patients who will benefit from NAC. Simple Summary: Nowadays, the only widely recognized method for evaluating the efficacy of neoadjuvant chemotherapy is the assessment of the pathological response through surgery. However, delivering chemotherapy to not-responders could expose them to unnecessary drug toxicity with delayed access to other potentially effective therapies. Radiomics could be useful in the early detection of resistance to chemotherapy, which is crucial for switching treatment strategy. We determined whether tumor radiomic features extracted from a highly homogeneous database of breast MRI can improve the prediction of response to chemotherapy in patients with breast cancer, in addiction to biological characteristics, potentially avoiding unnecessary treatment. Objectives: We aimed to determine whether radiomic features extracted from a highly homogeneous database of breast MRI could non-invasively predict pathological complete responses (pCR) to neoadjuvant chemotherapy (NACT) in patients with breast cancer. Methods: One hundred patients with breast cancer receiving NACT in a single center (01/2017–06/2019) and undergoing breast MRI were retrospectively evaluated. For each patient, radiomic features were extracted within the biopsy-proven tumor on T1-weighted (T1-w) contrast-enhanced MRI performed before NACT. The pCR to NACT was determined based on the final surgical specimen. The association of clinical/biological and radiomic features with response to NACT was evaluated by univariate and multivariable analysis by using random forest and logistic regression. The performances of all models were assessed using the areas under the receiver operating characteristic curves (AUC) with 95% confidence intervals (CI). Results: Eighty-three patients (mean (SD) age, 47.26 (8.6) years) were included. Patients with HER2+, basal-like molecular subtypes and Ki67 ≥ 20% presented a pCR to NACT more frequently; the clinical/biological model's AUC (95% CI) was 0.81 (0.71–0.90). Using 136 representative radiomics features selected through cluster analysis from the 1037 extracted features, a radiomic score was calculated to predict the response to NACT, with AUC (95% CI): 0.64 (0.51–0.75). After combining the clinical/biological and radiomics models, the AUC (95% CI) was 0.83 (0.73–0.92). Conclusions: MRI-based radiomic features slightly improved the pre-treatment prediction of pCR to NACT, in addiction to biological characteristics. Simple Summary: Handcrafted radiomic features (HRFs) are quantitative features extracted from medical images, and they are mined for associations with different clinical endpoints. While many studies reported on the potential of HRFs to unravel clinical endpoints, the sensitivity of HRFs to variations in scanning parameters is affecting the inclusion of radiomic signatures in clinical decision-making. In this study, we investigated the effects of variations in the in-plane resolution of scans, while all other scanning parameters were fixed. Moreover, we investigated the effects of ten different image resampling methods and ComBat harmonization on the reproducibility of HRFs. Our results show that the majority of HRFs are significantly and variably affected by the differences in in-plane resolution. The majority of image resampling methods resulted in a higher number of reproducible HRFs compared to ComBat harmonization. Our developed framework guides identifying the reproducible and harmonizable HRFs in different scenarios. While handcrafted radiomic features (HRFs) have shown promise in the field of personalized medicine, many hurdles hinder its incorporation into clinical practice, including but not limited to their sensitivity to differences in acquisition and reconstruction parameters. In this study, we evaluated the effects of differences in in-plane spatial resolution (IPR) on HRFs, using a phantom dataset (n = 14) acquired on two scanner models. Furthermore, we assessed the effects of interpolation methods (IMs), the choice of a new unified in-plane resolution (NUIR), and ComBat harmonization on the reproducibility of HRFs. The reproducibility of HRFs was significantly affected by variations in IPR, with pairwise concordant HRFs, as measured by the concordance correlation coefficient (CCC), ranging from 42% to 95%. The number of concordant HRFs (CCC > 0.9) after resampling varied depending on (i) the scanner model, (ii) the IM, and (iii) the NUIR. The number of concordant HRFs after ComBat harmonization depended on the variations between the batches harmonized. The majority of IMs resulted in a higher number of concordant HRFs compared to ComBat harmonization, and the combination of IMs and ComBat harmonization did not yield a significant benefit. Simple Summary: The increasing interest in implementing artificial intelligence in radiomic models has occurred alongside advancement in the tools used for computer-aided diagnosis. Such tools typically apply both statistical and machine learning methodologies to assess the various modalities used in medical image analysis. Specific to prostate cancer, the radiomics pipeline has multiple facets that are amenable to improvement. This review discusses the steps of a magnetic resonance imaging based radiomics pipeline. Present successes, existing opportunities for refinement, and the most pertinent pending steps leading to clinical validation are highlighted. The management of prostate cancer (PCa) is dependent on biomarkers of biological aggression. This includes an invasive biopsy to facilitate a histopathological assessment of the tumor's grade. This review explores the technical processes of applying magnetic resonance imaging based radiomic models to the evaluation of PCa. By exploring how a deep radiomics approach further optimizes the prediction of a PCa's grade group, it will be clear how this integration of artificial intelligence mitigates existing major technological challenges faced by a traditional radiomic model: image acquisition, small data sets, image processing, labeling/segmentation, informative features, predicting molecular features and incorporating predictive models. Other potential impacts of artificial intelligence on the personalized treatment of PCa will also be discussed. The role of deep radiomics analysis-a deep texture analysis, which extracts features from convolutional neural networks layers, will be highlighted. Existing clinical work and upcoming clinical trials will be reviewed, directing investigators to pertinent future directions in the field. For future progress to result in clinical translation, the field will likely require multi-institutional collaboration in producing prospectively populated and expertly labeled imaging libraries. Simple Summary: Medical images are data. They contain more information than is routinely identified by radiologists reading scans. Many scientists are investigating if extracting shape and grey-scale features from images can predict which oncology patients will respond to therapy. This approach, termed 'radiomics', must be validated before being ready for clinical use. One step is to determine measurement repeatability to ensure that radiomic features are robust, and that changes in features reflect genuine changes in tumours. In this study patients had two repeated sets of magnetic resonance imaging scans. We found that radiomic feature repeatability varied depending on scan acquisition parameters and the use of an administered contrast agent. We also compared how different repeatability assessment methods can best reveal these findings. We conclude that measuring radiomic feature repeatability is essential, but is also complex and prone to pitfalls. Overall, our study provides several insights into how radiomic feature repeatability is best assessed. Imaging biomarkers require technical, biological, and clinical validation to be translated into robust tools in research or clinical settings. This study contributes to the technical validation of radiomic features from magnetic resonance imaging (MRI) by evaluating the repeatability of features from four MR sequences: pre-contrast T 1 - and T 2 -weighted images, pre-contrast quantitative T 1 maps (qT 1 ), and contrast-enhanced T 1 -weighted images. Fifty-one patients with colorectal cancer liver metastases were scanned twice, up to 7 days apart. Repeatability was quantified using the intraclass correlation coefficient (ICC) and repeatability coefficient (RC), and the impact of non-Gaussian feature distributions and image normalisation was evaluated. Most radiomic features had non-Gaussian distributions, but Box–Cox transformations enabled ICCs and RCs to be calculated appropriately for an average of 97% of features across sequences. ICCs ranged from 0.30 to 0.99 , with volume and other shape features tending to be most repeatable; volume ICC > 0.98 for all sequences. 19% of features from non-normalised images exhibited significantly different ICCs in pair-wise sequence comparisons. Normalisation tended to increase ICCs for pre-contrast T 1 - and T 2 -weighted images, and decrease ICCs for qT 1 maps. RCs tended to vary more between sequences than ICCs, showing that evaluations of feature performance depend on the chosen metric. This work suggests that feature-specific repeatability, from specific combinations of MR sequence and pre-processing steps, should be evaluated to select robust radiomic features as biomarkers in specific studies. In addition, as different repeatability metrics can provide different insights into a specific feature, consideration of the appropriate metric should be taken in a study-specific context.
