Statin use is associated with reduced risk of lethal prostate cancer potentially through effects on serum cholesterol. Using data from a prospective study with measured serum cholesterol among men subsequently diagnosed with prostate cancer, we developed a tumour gene expression score to reflect the pre-diagnosis serum cholesterol environment these tumours developed in, and applied this score to a separate study to examine the relationship with prostate cancer-specific outcomes. We identified 127 men with prostate cancer in the prospective Health Professionals Follow-up Study, whose prostate tumors underwent whole genome gene expression profiling. Pre-diagnosis serum cholesterol was measured a median of 3.3 years (IQR 1.5-4.8 years) prior to cancer diagnosis. We excluded men using statins either at time of blood draw for serum cholesterol measurement or at cancer diagnosis. Using tumor biopsy whole genome transcriptomic data from 232 prostate cancer patients treated with radiotherapy in Northern Ireland (NI), we generated the serum cholesterol score by summing z-score-transformed expression for each gene. PTEN status was measured using a published 50-gene transcriptional signature. Both signatures were dichotomized using the median cut-point. Age-adjusted and multivariable Cox regression analysis was used to compute HRs and 95% CI for associations of the cholesterol gene score with biochemical recurrence and metastasis risk. We identified in HPFS a set of 30 genes associated with pre-diagnostic serum cholesterol with a fold change ≥4 which included PTEN. Introduction Dysregulated lipid metabolism is a hallmark of prostate cancer. Furthermore, use of the cholesterol lowering statin drugs has been linked to a reduced risk of lethal prostate cancer. To understand the molecular pathways implicated in this relationship, we examined the association between differential expression of genes involved in intratumoral cholesterol metabolism in prostate tumor biopsies and prostate cancer-specific outcomes. Methods Tumor biopsy gene expression data from 242 men diagnosed with localized/locally advanced prostate cancer and treated with radiotherapy and androgen deprivation therapy was analyzed. A total of 71 genes involved in cholesterol transport, biosynthesis and regulation were selected from the Reactome pathway database, with additional genes involved in cholesterol metabolism extracted from literature. For each gene, expression levels were dichotomized by median expression level. Cox proportional hazards regression was used to estimate hazard ratios (HR) and 95% confidence intervals (CI) for associations of gene expression with biochemical recurrence and metastasis. Models were adjusted for age at diagnosis and Gleason score. Multiple-testing correction was performed using the Bonferroni correction method. Results During a median follow-up of 10.4 years, 72 (30%) patients experienced biochemical recurrence and 32 (13%) developed metastatic disease. Differential expression of several genes was found to be associated with risk of biochemical recurrence and metastasis, though none remained statistically significant following Bonferroni correction. However, several genes showed similar patterns of association with risk of biochemical recurrence and of metastasis, an association which remained similar following adjustment for age and Gleason score. Low (below median) expression of FDFT1, a gene encoding squalene synthase, a key enzyme in cholesterol biosynthesis, was associated with an increased risk of biochemical recurrence (HR 1.87; 95% CI, 1.15-3.01) and metastasis (HR 3.52; 95% CI, 1.52-8.16). High (above median) expression of SLCO1B3, a gene encoding an organic anion transporting polypeptide transporting a variety of small molecules including statins, was associated with higher risk of biochemical recurrence (HR 2.14; 95% CI, 1.31-3.49) and metastasis (HR 2.70; 95% CI, 1.25-5.90). Conclusion These hypothesis-generating results point to a potential role for altered intratumoral cholesterol metabolism in prostate cancer-specific outcomes. Our findings highlighting differential expression of FDFT1 and SLCO1B3, genes involved in cholesterol biosynthesis and anion transport, respectively, in association with risk of biochemical recurrence and metastasis may provide clues as to the mechanistic pathways of most importance. Introduction: Loss of the tumor suppressor PTEN is associated with relapse following radiotherapy (RT) in men with prostate cancer (PCa). We have previously shown that PTEN-deficient tumors display increased monocyte and macrophage infiltration. Co-culture models have confirmed that monocyte-derived paracrine signaling factors can significantly alter the radiation response of PCa cells. The aim of this study is to identify monocyte-derived cytokines that can be therapeutically targeted in combination with RT to reduce the occurrence of relapse in men with PTEN-deficient PCa. Methods: Techniques employed included: cytokine arrays to detect radiation-induced proteins secreted from monocytes; IF and IHC on FFPE tumor sections; in vitro biological assays extending to RT-PCR, ELISA, immunoblotting and transwell migration assays; and analysis of relevant genes in the FASTMAN radiotherapy patient cohort. Results: Cytokine array analysis of monocytes with and without prior ionizing radiation exposure identified several upregulated secreted proteins - the most abundant of which was YKL40 (CHI3L1). This was further potentiated in co-culture with PCa cell lines. Secretion of YKL40 was confirmed as being monocyte-derived by ELISA and this was further supported by IF co-staining of YKL40 with a monocyte marker (F4/80) in PTEN-deficient tumour sections. Exposure of PCa cell lines to conditioned media (CM) from irradiated monocytes increased cell migration - an effect that was also observed by direct addition of rhYKL40. Previous studies have reported the role of syndecan-integrin complexes in mediating YKL40 signal transduction. Immunoblotting confirmed expression of these receptors (Sdc1 and αvβ3) on PCa cell lines. Using siRNA to knockdown Sdc1 expression inhibited YKL40-induced activation of the FAK pathway and subsequently attenuated YKL40-induced cell migration. In addition, YKL40 was shown to increase expression of genes involved in both actin cytoskeleton rearrangement and epithelial-mesenchymal transition. Use of a FAK inhibitor (PF-573228) successfully prevented the induction of these genes and attenuated the capacity of YKL40 to drive cell migration. The clinical relevance of our observations is supported by high expression of YKL40-receptor and signaling mediators in prostate cancer biopsy tissue and its correlation to increased biochemical recurrence following RT treatment. Conclusion: Use of a YKL40-pathway-targeted agent alongside radiotherapy may reduce disease recurrence in men with high-risk clinically-localized PTEN-deficient PCa by preventing radiation-induced cell migration and metastatic escape. Copy number alterations of the tumor-suppressor gene PTEN occur frequently in primary prostate cancer and are prognostic for relapse post-radiotherapy (RT). Preclinical modelling supports elevated CXC-chemokine signaling as a critical mediator of PTEN-depleted disease progression, and therapeutic resistance. Our objective was to establish the correlation of PTEN-deficiency with CXC-chemokine signaling and its association with clinical outcome following RT. Techniques employed included gene cluster and survival analysis of prostate cancer datasets; in vitro biologic assays extending to RT-PCR, immunoblotting, clonogenic survival assays and flow cytometry; and the use of two in vivo PTEN-deficient xenograft models. Technology advancements have enhanced our abilities to gain greater insight to the tumor environment. However, such emergent methods have brought new considerations in data storage, access and analysis. Modern large data projects and clinical trial materials could be explored to a greater degree if appropriate infrastructure could be built to support efforts. Such a scaffold would be an integrated and dynamic framework where novel hypotheses could be investigated in modern big data collections. Placing discovery back in the hands of the researcher through a reactive and supportive framework will enhance our understanding of cancer aetiology. The Cancer Integromics Research Application Framework (CIRAFm) was created to emulate current platforms that have a rigid analytical interface. We designed a robust architecture that supports a reactive user-friendly interface blended to several cross-platform coding technologies. It has been developed to accommodate an individualised framework where we can create an ‘app-store' of key software to fit research questions. In efforts to encapsulate and accelerate differing data types it sits astride of two NoSQL based database management systems, minimising data redundancy. Initial requirements have begun to create algorithms to investigate alignment-free applications on next generation sequencing (NGS) data enhancing analysis of spatial and temporal heterogeneity in cancer. Key drivers can be explored by assisting software to build correlative marker associations using Darwinian approaches such as genetic algorithms. Information requirements from external platforms are assisted through a novel domain specific language (DSL) to enable a singular interface. The modularised architecture of the platform is enabled through an Angular framework supported by interactive and dynamic data visualisation software, D3.js. Data analytics can be explored through the ‘apps' created, investigating new markers in large data and enhancing our understanding of tumor heterogeneity. Alignment-free phylogenetics of NGS data harnesses our capabilities to display fully sequence evolution in patient data. This highlights possible sub-types, markers of interest and maps to therapeutic compounds via the DSL to our externally developed drug discovery software, QUADrATiC. Future scientific endeavours further defining new tumor subtypes are paramount in efforts to help uncover treatment strategies. Unburdened by legacy pipelines and data, flexible and robust models of architecture provide an effective and efficient framework for research on our ever increasing search for precision medicine. Introduction: Next generation sequencing (NGS) has identified genomic aberrations causing DNA repair defects in sporadic metastatic castration resistant prostate cancer (mCRPC). We hypothesized that single agent olaparib would have antitumor activity in a sub-population of mCRPC patients (pts) and that exome and transcriptome studies would identify this population. Methods: TOPARP is an open-label, investigator-initiated phase II trial with a novel multi-step adaptive design (CRUK/11/029). The first part of the study (TOPARP-A) has a two-stage design evaluating the antitumor activity of single agent olaparib in unselected mCRPC pts (p0 = 0.05; p1 = 0.20; α = 0.02; β = 0.10) with a preplanned analysis to identify a biomarker defined sensitive subgroup. Primary endpoint, response rate (RR), was defined as objective response by RECIST 1.1 and/or PSA fall ≥50% and/or confirmed circulating tumor cell (CTC) count falls from ≥5 to Results: Fifty pts were enrolled from 7 UK centers; all had had prior docetaxel, 48 (96%) prior abiraterone and 29 (58%) prior cabazitaxel. Overall, 16 of 49 evaluable pts experienced a response (RR 32.7%, 95% CI: 20.0 to 47.5), with 11 and 4 pts having been on treatment for >6 and >12 months respectively at data cut-off. NGS identified homozygous deletions and/or putatively deleterious mutations in DNA repair genes in 15/49 (30.6%) evaluable pts. While a majority of these genomic aberrations occurred in BRCA2 and ATM, biallelic loss of other relevant genes, including members of the Fanconi Anemia complementation group and CHEK2, were also observed. Among these fifteen pts, 13 (86.7%) responded to olaparib. All seven pts with BRCA2 loss (somatic [4/7] or germline [3/7]) and 4/5 pts with ATM truncating mutations responded to olaparib. The specificity of the biomarker suite was 94% in this population. Conversely, PTEN loss and ERG rearrangements were not associated with response. Finally, consistent with previous studies of olaparib, anemia (10/50, 20%) and fatigue (6/50, 12%) were the most common grade>3 adverse events, with 13 (26%) pts requiring a dose reduction. Conclusions: Olaparib has durable antitumor activity in heavily pre-treated pts with sporadic mCRPC with a 32.7% overall response rate. Genomic defects in DNA repair genes associate with olaparib sensitivity in sporadic mCRPC, offering a possibility for the very first molecular treatment stratification of advanced prostate cancer.
