Abstract The term “co-benefits” refers to positive outcomes accruing from a policy beyond the intended outcome, often or usually in other sectors. In the urban context, policies implemented in particular sectors (such as transport, energy or waste) often generate multiple co-benefits in other areas. Such benefits may be related to the reduction of local or global environmental impacts and also extend into the area of public health. A key to identifying and realising co-benefits is the adoption of systems approaches to understand inter-sectoral linkages and, in particular, the translation of this understanding to improved sector-specific and city governance. This paper reviews a range of policies which can yield health and climate co-benefits across different urban sectors and illustrates, through a series of cases, how taking a systems approach can lead to innovations in urban governance which aid the development of healthy and sustainable cities.

Abstract There is growing interest among urban health researchers in addressing complex problems using conceptual and computation models from the field of complex systems. Agent-based modeling (ABM) is one computational modeling tool that has received a lot of interest. However, many researchers remain unfamiliar with developing and carrying out an ABM, hindering the understanding and application of it. This paper first presents a brief introductory guide to carrying out a simple agent-based model. Then, the method is illustrated by discussing a previously developed agent-based model, which explored inequalities in diet in the context of urban residential segregation.

Abstract Public Health Observatories (PHOs) were created in England in 2000 as an important adjunct to the public health system in the country. The observatories were networked together, which allowed pooling of expertise and rapid dissemination of methods and results. The network grew to include the whole of the UK and Ireland and was a very successful force for change until PHOs were subsumed into the new Government Agency Public Health Organization, Public Health England. This paper describes the lessons learnt from their existence in the public health system in England for fourteen years.

Abstract Health observatories may differ according to their mission, institutional setting, topical emphasis or geographic coverage. This paper discusses the development of a new urban-focused health observatory, and its operational research and training infrastructure under the academic umbrella of the Department of Epidemiology and the Institute of Urban Health at the Johns Hopkins Bloomberg School of Public Health (BSPH) in Baltimore, USA. Recognizing the higher education mission of the BSPH, the development of a new professional training in public health was an important first step for the development of this observatory. This new academia-based observatory is an innovative public health research and training platform offering faculty, investigators, professional epidemiology students and research partners a physical and methodological infrastructure for their operational research and training activities with both a local urban focus and a global reach. The concept of a public health observatory and its role in addressing social health inequalities in local urban settings is discussed.

Abstract The proportion of non-participation in cohort studies, if associated with both the exposure and the probability of occurrence of the event, can introduce bias in the estimates of interest. The aim of this study is to evaluate the impact of participation and its characteristics in longitudinal studies. A systematic review (MEDLINE, Scopus and Web of Science) for articles describing the proportion of participation in the baseline of cohort studies was performed. Among the 2,964 initially identified, 50 were selected. The average proportion of participation was 64.7%. Using a meta-regression model with mixed effects, only age, year of baseline contact and study region (borderline) were associated with participation. Considering the decrease in participation in recent years, and the cost of cohort studies, it is essential to gather information to assess the potential for non-participation, before committing resources. Finally, journals should require the presentation of this information in the papers.

Abstract Health inequalities are very well documented in epidemiological research: rich people live longer and have fewer diseases than poor people. Recently, a growing amount of evidence from environmental sciences confirms that poor people are also more exposed to pollution and other environmental threats. However, research in the social sciences has shown a broad lack of awareness about health inequalities. In this paper, based on data collected in Portugal, we will analyze the consciousness of both health and environmental injustices and test one hypothesis for this social blindness. The results show, even more clearly than before, that public opinion tends to see rich and poor people as being equally susceptible to health and environmental events. Furthermore, those who have this equal view of the world present lower levels of depression and anxiety. Following cognitive adaptation theory, this “belief in an equal world” can be interpreted as a protective positive illusion about social justice, particularly relevant in one of the most unequal countries in Europe.

Emergency rescue after an earthquake is complex work which requires the participation of relief and social organizations. Studying earthquake emergency coordination efficiency can not only help rescue organizations to define their own rescue missions, but also strengthens inter-organizational communication and collaboration tasks, improves the efficiency of emergency rescue, and reduces loss. In this paper, collaborative entropy is introduced to study earthquake emergency rescue operations. To study the emergency rescue coordination relationship, collaborative matrices and collaborative entropy functions are established between emergency relief work and relief organizations, and the collaborative efficiency of the emergency rescue elements is determined based on this entropy function. Finally, the Lushan earthquake is used as an example to evaluate earthquake emergency rescue coordination efficiency.

Cyberbullying is a new form of violence that is expressed through electronic media and has given rise to concern for parents, educators and researchers. In this paper, an association between cyberbullying and adolescent mental health will be assessed through a systematic review of two databases: PubMed and Virtual Health Library (BVS). The prevalence of cyberbullying ranged from 6.5% to 35.4%. Previous or current experiences of traditional bullying were associated with victims and perpetrators of cyberbullying. Daily use of three or more hours of Internet, web camera, text messages, posting personal information and harassing others online were associated with cyberbullying. Cybervictims and cyberbullies had more emotional and psychosomatic problems, social difficulties and did not feel safe and cared for in school. Cyberbullying was associated with moderate to severe depressive symptoms, substance use, ideation and suicide attempts. Health professionals should be aware of the violent nature of interactions occurring in the virtual environment and its harm to the mental health of adolescents.

This paper offers a critical overview of social science research presented at the 2014 International AIDS Conference in Melbourne, Australia. In an era of major biomedical advance, the political nature of HIV remains of fundamental importance. No new development can be rolled out successfully without taking into account its social and political context, and consequences. Four main themes ran throughout the conference track on social and political research, law, policy and human rights: first, the importance of work with socially vulnerable groups, now increasingly referred to as "key populations"; second, continued recognition that actions and programs need to be tailored locally and contextually; third, the need for an urgent response to a rapidly growing epidemic of HIV among young people; and fourth, the negative effects of the growing criminalization of minority sexualities and people living with HIV. Lack of stress on human rights and community participation is resulting in poorer policy globally. A new research agenda is needed to respond to these challenges.

- Abstract
The aim of this study was to determine the accuracy of Bayesian networks in supporting breast cancer diagnoses. Systematic review and meta-analysis were carried out, including articles and papers published between January 1990 and March 2013. We included prospective and retrospective cross-sectional studies of the accuracy of diagnoses of breast lesions (target conditions) made using Bayesian networks (index test). Four primary studies that included 1,223 breast lesions were analyzed, 89.52% (444/496) of the breast cancer cases and 6.33% (46/727) of the benign lesions were positive based on the Bayesian network analysis. The area under the curve (AUC) for the summary receiver operating characteristic curve (SROC) was 0.97, with a Q* value of 0.92. Using Bayesian networks to diagnose malignant lesions increased the pretest probability of a true positive from 40.03% to 90.05% and decreased the probability of a false negative to 6.44%. Therefore, our results demonstrated that Bayesian networks provide an accurate and non-invasive method to support breast cancer diagnosis.

- Abstract
This paper is a case study on the implementation of the Brazilian National Oral Health Policy (PNSB), known as "Smiling Brazil", in the cities of the Regional Health Department of Araraquara (DRS III) in São Paulo State. A structured questionnaire was given to the municipal oral health coordinators, an interview with oral health care professionals and managers was conducted, and the official data provided by the Brazilian Ministry of Health were coded to assess the policy's scope: (i) expansion and qualification of actions; (ii) work conditions; (iii) care; (iv) access; and (v) planning and management. The quantitative and qualitative analyses were linked by methods triangulation. In terms of PNSB implementation, the majority of the cities (52.6%) were classified as "good", with 42.1% classified as "bad". Approximately 10 years after launching the PNSB, despite strides in oral health care and access to different levels of care, the cities still experience difficulties in implementing the policy's principles.

- Abstract
This paper evaluates health as a Sustainable Development Goal (SDG) in the context of the Post-2015 Development Agenda, between 2012 and 2014. Health was part of the debate since the Millennium Summit and the MDGs (2000), and it also appears in the documents discussing the Post-2015 Agenda, from the Rio+20 to the Open Working Group (OWG), whose report was submitted to the General Assembly of the United Nations (UNGA) 2014-2015, and in the Global Consultation on Health and the High-Level Panel of Eminent Persons reports. The Authors concluded that the treatment of health in all these documents is uniform. They point out that the scope of the health-related SDG is very comprehensive, but its targets are conceptually fragmented and reduced. They advocate their change as to include not only the idea of social determinants of health, but also targets in the field of public health, which were not included in the proposal of the OWG. They also warn that the global and national governance systems need to be reformed and advocate more participation of the civil society, which can influence diplomacy, which, in turn, will be responsible for the agreement signed at the UNGA in 2015.

The paper analyzes trends in contemporary health sector reforms in three European countries with Bismarckian and Beveridgean models of national health systems within the context of strong financial pressure resulting from the economic crisis (2008-date), and proceeds to discuss the implications for universal care. The authors examine recent health system reforms in Spain, Germany, and the United Kingdom. Health systems are described using a matrix to compare state intervention in financing, regulation, organization, and services delivery. The reforms’ impacts on universal care are examined in three dimensions: breadth of population coverage, depth of the services package, and height of coverage by public financing. Models of health protection, institutionality, stakeholder constellations, and differing positions in the European economy are factors that condition the repercussions of restrictive policies that have undermined universality to different degrees in the three dimensions specified above and have extended policies for regulated competition as well as commercialization in health care systems.

- Abstract
This paper presents the factors associated with caesarean section in primiparous adolescents in Brazil using data from a national hospital-based survey conducted between 2011 and 2012. Information was obtained from postpartum women through face-to-face and telephone interviews and a theoretical model with three levels of hierarchy was used to analyze associations with the dependent variable mode of delivery (caesarean or vaginal). The results show that the caesarean section rate among primiparous teenagers is high (40%). The most significant contributing factors for caesarean section were: considering this mode of delivery safer (OR=7.0; 95%CI: 4.3-11.4); giving birth under the private health system (OR=4.3; 95%CI: 2.3-9.0); being attended by the same health care professional throughout prenatal care and delivery (OR=5.7; 95%CI: 3.3-9.0) and clinical history of risk and complications during pregnancy (OR=10.8; 95%CI: 8.5-13.7). Adolescent pregnancy continues to be an important concern on the reproductive health agenda and the rates observed by this study are worrying given the effects of early exposure to caesarean section.

- Abstract
This paper describes the sample design for the National Survey into Labor and Birth in Brazil. The hospitals with 500 or more live births in 2007 were stratified into: the five Brazilian regions; state capital or not; and type of governance. They were then selected with probability proportional to the number of live births in 2007. An inverse sampling method was used to select as many days (minimum of 7) as necessary to reach 90 interviews in the hospital. Postnatal women were sampled with equal probability from the set of eligible women, who had entered the hospital in the sampled days. Initial sample weights were computed as the reciprocals of the sample inclusion probabilities and were calibrated to ensure that total estimates of the number of live births from the survey matched the known figures obtained from the Brazilian System of Information on Live Births. For the two telephone follow-up waves (6 and 12 months later), the postnatal woman's response probability was modelled using baseline covariate information in order to adjust the sample weights for nonresponse in each follow-up wave.

- Abstract
Parental upbringing of children and adolescents has received little attention in research on the promotion of children's health and wellbeing. This paper presents the initial cross-cultural adaptation process of a Brazilian version of the s-EMBU for adolescents, which intends to capture parental rearing practices. The study takes a universalistic approach to assess the four initial phases of cross-cultural adaptation: conceptual, item, semantic, and operational equivalence. The process thus included a literature review, discussion with experts, and translation and back-translation of items. A pretest was conducted with 10 adolescents in the waiting room of a public clinic. The adolescents did not understand some items as proposed in the original constructs. After some rewording, the items were properly understood as projected by the original instrument. The findings suggest that this Brazilian version of s-EMBU for adolescents is promising and that it provides room for future psychometric studies to complete the cross-cultural adaptation process.

The probabilistic record linkage (PRL) is based on a likelihood score that measures the degree of similarity of several matching variables. Screening test results for different diseases are available for the blood donor population. In this paper, we describe the accuracy of a PRL process used to track blood donors from the Fundação Pró-Sangue (FPS) in the Mortality Information System (SIM), in order that future studies might determine the blood donor’s cause of death. The databases used for linkage were SIM and the database made up of individuals that were living (200 blood donors in 2007) and dead (196 from the Hospital das Clinicas de São Paulo that died in 2001-2005). The method consists of cleaning and linking the databases using three blocking steps comparing the variables “Name/Mother’s Name/ Date of Birth” to determine a cut-off score. For a cut-off score of 7.06, the sensitivity and specificity of the method is 94.4% (95%CI: 90.0-97.0) and 100% (95%CI: 98.0-100.0), respectively. This method can be used in studies that aim to track blood donors from the FPS database in SIM.

This paper examines the spatial pattern of ill-defined causes of death across Brazilian regions, and its relationship with the evolution of completeness of the deaths registry and changes in the mortality age profile. We make use of the Brazilian Health Informatics Department mortality database and population censuses from 1980 to 2010. We applied demographic methods to evaluate the quality of mortality data for 137 small areas and correct for under-registration of death counts when necessary. The second part of the analysis uses linear regression models to investigate the relationship between, on the one hand, changes in death counts coverage and age profile of mortality, and on the other, changes in the reporting of ill-defined causes of death. The completeness of death counts coverage increases from about 80% in 1980-1991 to over 95% in 2000-2010 at the same time the percentage of ill-defined causes of deaths reduced about 53% in the country. The analysis suggests that the government’s efforts to improve data quality are proving successful, and they will allow for a better understanding of the dynamics of health and the mortality transition.

Guidelines have been proposed for assessing the quality of clinical trials, observational studies and validation studies of diagnostic tests. More recently, the COSMIN (COnsensus-based Standards for the selection of health Measurement INstruments) initiative extended those in regards to epidemiological measurement tools in general. Among various facets proposed for assessment is the validity of an instrument’s dimensional structure (or structural validity). The purpose of this article is to extend these guidelines. A seven-step roadmap is proposed to examine (1) the hypothesized dimensional structure; (2) strength of component indicators regarding loading patterns and measurement errors; (3) measurement error correlations; (4) factor-based convergent and discriminant validity of scales; (5) item discrimination and intensity vis-à-vis the latent trait spectrum; and (6) the properties of raw scores; and (7) factorial invariance. The paper also holds that the suggested steps still require debate and are open to refinements.

This paper examined the association between individual and local level factors and the number of antenatal care visits completed by women in Colombia using data from the 2010 Colombian Demographic and Health Survey and multilevel logistic regression models. Our findings suggest that, in addition to maternal socioeconomic status, contextual factors influence whether pregnant women complete the minimum recommended number of antenatal care visits. These factors include: level of women’s autonomy in the community, regional inequalities and access barriers caused by distance (OR = 0.057), costs of services (OR = 0.035), and/or a lack of confidence in doctors (OR = 0.036). Our results highlight the existence of inequalities in access to antenatal care and the importance of considering the local context in the design of effective maternal care policies in Colombia. Furthermore, our findings regarding individual factors corroborate the evidence from other countries and offer new insights into the association between local level factors and number of antenatal care visits.

- Abstract
Categorical variables are common in the biomedical field, and many descriptive methods have been proposed for revealing intrinsic patterns in data. Correspondence Analysis is an especially useful method for categorical data analysis of large contingency tables. Although numerous studies have been published on this method, most Portuguese-language articles have failed to explore its full potential, focusing only on graphical interpretation. The current paper reviews the method, showing that graphical analysis can be enriched by the right statistics. The article presents the mathematical basis for correspondence analysis and its most frequently used statistics. The procedure has shown that such statistics enrich symmetric map evaluation, that a low relative frequency category can be represented by supplementary category points, and that inertia contributions are highly related to residual analysis of contingency tables, not easily visualized by symmetric maps. Correspondence Analysis has proven advantageous when compared to principal components analysis.

The development of new treatments has improved survival and quality of life among cancer patients. Nurses are expected to answer questions and to provide orientation regarding patients' sexuality since it is an important aspect of life. The main objective of this paper is to understand the representations of sexuality among nurses working with women who survive breast cancer after diagnosis and during treatment assuming that their representations may affect communication with the patient. This is a qualitative study using an in-depth guideline which involved interviews with 28 nurses living and working in the southeast of Brazil. The narratives were submitted to a content analysis and categories of representations were identified and are discussed here. Several representations of sexuality were found in the nurses' discourses. Some of the nurses' representations may be expected to hinder their ability to provide helpful orientation regarding the sexual lives of these patients.

- Abstract
This paper discusses the partial results of qualitative research on lifestyles and meanings attributed to health and work among settlers from the Landless Rural Workers' Movement (MST) and identifies strategies developed by workers to maintain and/or promote health. The study was conducted in a rural settlement affiliated with the MST in Campos dos Goytacazes, Rio de Janeiro State, Brazil. The ergological approach was the main theoretical and methodological reference for understanding work from the perspective of "human activity". The study techniques included document analysis, participant observation, semi-structured interviews, and focus groups, and the data were submitted to thematic content analysis. The landless rural workers attributed the meanings of freedom and satisfaction to their work, associated with self-management and autonomy, which they reported as key elements for health. Although rural work was considered tiring, the work and way of life in the settlement provided this community with possibilities for ensuring their health and resisting the hegemonic agribusiness model.

This paper describes the methods of a multidisciplinary epidemiological survey conducted in schools in Pelotas, in the Southern Region of Brazil. This cross-sectional study evaluated a representative sample (n = 1,211) of eight to 12-year-old children attending public (15) and private (5) schools. Questionnaires were applied to parents to obtain information about socioeconomic and sociocultural characteristics. Children were interviewed to provide information on demographic characteristics, oral health behavior and physical activity habits. Oral health examinations were performed on the children and anthropometric data was collected to assess nutritional status. School principals responded a questionnaire about the social and physical environment of the school. A total of 1,744 children were eligible for the study; a response rate of 69.4% was achieved, with no significant differences between schools. The method used allowed investigators to obtain data on several health outcomes as well as on possible risk factors. This strategy optimizes the use of financial resources for research and promotes interprofessional collaboration.

Transport is associated with environmental problems, economic losses, health and social inequalities. A number of European and US cities have implemented initiatives to promote multimodal modes of transport. In Latin America changes are occurring in public transport systems and a number of projects aimed at stimulating non-motorized modes of transport (walking and cycling) have already been implemented. Based on articles from peer-reviewed academic journals, this paper examines experiences in Bogotá (Colombia), Curitiba (Brazil), and Santiago (Chile), and identifies how changes to the transport system contribute to encourage active transportation. Bus rapid transit, ciclovias, bike paths/lanes, and car use restriction are initiatives that contribute to promoting active transportation in these cities. Few studies have been carried out on the relationship between transport and physical activity. Car ownership continues to increase. The public health sector needs to be a stronger activist in the transport policy decision-making process to incorporate health issues into the transport agenda in Latin America.

- Abstract
The purpose of this study was to summarize studies on adolescents' body satisfaction, focusing on the use of silhouette scales. A systematic review was carried out on MEDLINE, LILACS, SciELO, and in unpublished papers. The final analysis included 36 studies. The majority adopted the scale proposed by Stunkard et al., self-administered, presented in ascending order and on a single sheet of paper. Most studies compared characteristics on satisfaction and dissatisfaction, used the chi-square test, and did not test for confounding. Among 18 studies included in the meta-analysis, prevalence of body dissatisfaction ranged from 32.2% to 83%. The review showed wide heterogeneity between studies (p-value = 0.000; I(2) = 87.39) even after sub-group analysis and the absence of relevant information for proper comparison of studies. The article concludes by recommending greater rigor in application of the scales and presentation of study methods on body satisfaction assessed by silhouette scales, in addition to new methodological studies and those that elucidate factors related to body satisfaction.

- Abstract
The article describes epidemiological studies on the relationship between interpersonal discrimination and mental health outcomes, updating previous literature reviews on the topic. The review included 34 papers published in PubMed from 2000 to 2010, 68% of which were based on convenience samples and 82% with a cross-sectional design. Positive and statistically significant associations were observed between discrimination and adverse mental health outcomes, particularly substance use, depression, and alcohol-related disorders. Only one third of the studies explicitly adopted a specific theoretical framework to interpret the examined relationships. Mirroring previous reviews, discrimination was positively and consistently associated with adverse mental health outcomes. However, future studies should employ robust designs for causal inference, use discrimination instruments with good psychometric properties, and adopt theoretical frameworks to interpret their findings.
