The International Agency for Research on Cancer (IARC) proposed this international historical cohort study trying to solve the controversy about the increased risk of cancer in the workers of the Pulp and Paper Industry. One of the most important aspects presented by this study in Brazil was the strategies used to overcome the methodological challenges, such as: data access, data accuracy, data availability, multiple data sources, and the large follow-up period. Through multiple strategies it was possible to build a Brazilian cohort of 3,622 workers, to follow them with a 93 percent success rate and to identify in 99 percent of the cases the cause of death. This paper, has evaluated the data access, data accuracy and the effectiveness of the strategies used and the different sources of data.

Este artigo objetiva identificar as associações das morbidades comuns e dos acidentes de trabalho com setor, descrevendo as cargas de trabalho e realizando um amplo controle de fatores de confusão. Através de delineamento transversal, estudou-se a totalidade dos trabalhadores de uma indústria de celulose e papel (n=671). Realizaram-se entrevistas nesta indústria, caracterizando a percepção dos trabalhadores sobre as exposições ocupacionais e a morbidade. A área industrial caracterizou-se pelo excesso de problemas auditivos (RO>2,5), respiratórios (RO>2,7) e acidentes (RO>4,7), possivelmente relacionados com ruído, poeira, mudanças bruscas de temperatura e exposições a substâncias químicas, além do trabalho físico pesado e exposições a situações de risco. A administração apresentou um aumento de problemas nos olhos, dor nas costas, irritação e nervosismo (RO>1,7), que parecem ter relação com a falta de autonomia e criatividade no trabalho, problemas ergonômicos e esforço visual. Confirmaram-se não só as altas prevalências de morbidades comuns, mas também sua relação com as particularidades do processo de trabalho sintetizadas pela categoria setor.The purpose of this study was to identify associations between common morbidity and work accidents and the respective departments in a pulp and paper industry, describing work loads and performing broad control of potential confounding factors. We studied all the workers from a specific pulp and paper industry, using a cross-sectional design and interviews at the workplace (n=671) with a standardized questionnaire. We characterized workers' perceptions of their occupational exposures and health problems. The industrial area had an excess in auditory problems (OR>2.5) and respiratory problems (OR>2.7) as well as accidents (OR>4.7). These diseases were probably related to the high prevalence of noise, dust, sudden temperature changes, exposure to various chemicals, and excessive effort and high-risk situations. Moreover, the management group showed an excess in eye problems, backache, irritation, and nervousness (OR>1.7), apparently related to lack of autonomy and creativeness at work, ergonomic problems, and strained eyesight. The study confirmed not only a high prevalence of common diseases, but also their relationship to specific features of the work process indicated by each department category.

Um dos aspectos mais relevantes deste estudo no Brasil refere-se às estratégias utilizadas para superar desafios metodológicos, tais como: acesso aos dados, acurácia e disponibilidade dos dados, múltiplas fontes de dados e longo período de acompanhamento. Através de múltiplas estratégias, foi possível construir uma coorte brasileira de 3.622 trabalhadores, acompanhar 93% dos casos e identificar a causa de 99% dos óbitos. Este artigo avalia o acesso aos dados, sua acurácia e a efetividade das estratégias utilizadas para o acompanhamento da coorte.

5. [The historical and epistemological development of epidemiology and the concept of risk].

- Abstract
Statistical reasoning in health practice was disseminated by epidemiology and is widely used in various health fields. This paper revisits the historical development of epidemiology and the concept of risk, responsible for the use of statistical chance as part of causal thinking in health sciences. The study covered the period from 1872 to 1965, and the documental base consisted of scientific articles related to the development of epidemiology as a field, particularly in the American Journal of Hygiene, as well as scientific books. Three phases were identified in the development of epidemiology: constitution, exposure, and risk. The article proceeds to discuss epistemological and social and health aspects required for the historical understanding of each phase. It concludes by stressing the relevance of critical reflection on epidemiological science and its relationship to health practice, especially in public health, in order to optimize its current use and foster its on-going creative reconstruction.

6. [Health promotion, social epidemiology, and social capital: associations and perspectives for public health].

- Abstract
The idea of health promotion predates the use of the actual term. However, the incorporation of this idea and the practice of health promotion were influenced by the Canadian health reform movement, which echoed the voices of many others who were concerned with the influence of the physical and social environment on health. This provided the basis for the World Health Organization to launch a series of conferences beginning with the Alma Ata Conference in 1977 and followed by the Ottawa Conference, from which resulted the first international document on health promotion, known as the Ottawa Charter. Although health promotion has been the subject of a wide range of studies, the concept is still not well understood and its explicit practice is limited. Health conferences have been important for keeping the notion of equity in health alive, while the gap between the rhetoric of these conferences and practice remains to be bridged. However, the rise of social epidemiology and the development of the concept of social capital for health could bring new insights into traditional epidemiology in order to narrow this gap. The purpose of this paper is to elucidate these concepts and to describe the roles they play in public health in order to stimulate further debate.

7. [Historical issues in case-control studies].

- Abstract
This paper presents the main issues in the evolution of case-control studies (CCS) as of the late 1980s. The comparison of two groups regarding exposure to a risk factor has been known since the 18th century. The latter half of the 19th century witnessed a decline in the "Epidemiology of Populations", and the first CCS were only done in the 1920s. Improvement of the method occurred in the latter half of the 20th century, related to increased mortality from chronic diseases, especially in research on lung cancer and smoking. The main scientific contributions to the method were: use of the odds ratio as an estimate of relative risk; definition of statistical data analysis for retrospective studies; calculation of attributed risk and etiologic fraction; and discussion of the essence of CCS. Critics pointed to the method's possible weakness and susceptibility to bias. Application of CCS increased in the final decades of the 20th century in various areas of Epidemiology and has been considered an important tool to improve Public Health.

- Abstract
This paper describes the evolution of disease frequency statistics and techniques used to present them. Beginning in the 17th century were the first specific references to causes of death and proceeding practically until the mid-20th century as mortality statistics by causes, the instrument for a methodical description of diseases was classification of causes of death. From the latter half of the 20th century on, in addition to mortality statistics by cause, morbidity statistics became routine, with the adoption of disease classification and no longer only classification by cause of death. More recently, classification has been adapted to medical specialties, resulting in what has been termed "disease family classification", including classification of the consequences of diseases, with statistics on disability. Disease classification, both for mortality and morbidity, fostered the development of various types of epidemiological studies (both descriptive and analytical) that expanded knowledge on health.

- Abstract
Epidemiology's conceptual and methodological shortcomings have placed constraints on the study of health phenomena related to human communities, thus posing a challenge to the field. This paper presents some basic principles resulting from the observation of the field of geography in defining its object - space - and the application of this object to medicine and epidemiology. Such principles state the pertinence of a conceptual and methodological strategy focusing on an approach to geographic space as expressing the population's living conditions. Application of this concept to epidemiological studies is still limited, although such proposals have already been developed in other areas of knowledge. Ecological studies, whose unit of analysis is the group, and the ecological model, based on the idea of an inter-relationship of factors, if improved, could become a promising alternative in this direction. The authors emphasize that researchers should have wholeness as their scientific reference in order to guarantee the non-separation of the complex interactive processes determining health phenomena in the population.

In recent years increasingly powerful and versatile statistical tools have been developed in this application area. This paper discusses the general classes of problem in geographical epidemiology and reviews the key statistical methods now being employed in each of the application areas identified. The paper does not attempt to exhaustively cover all possible methods and models, but extensive references are provided to further details and to additional approaches. The overall aim is to provide a picture of the "current state of the art" in the use of spatial statistical methods in epidemiological and public health research. Following the review of methods, the main software environments which are available to implement such methods are discussed. The paper concludes with some brief general reflections on the epidemiological and public health implications of the use of spatial statistical methods in health and on associated benefits and problems.O estudo da distribuição geográfica da incidência de doenças e da sua relação com fatores de risco potenciais (chamada aqui de "epidemiologia geográfica") vem constituindo um terreno fértil para a aplicação e desenvolvimento de métodos e modelos estatísticos. Nos últimos anos, foram desenvolvidas ferramentas cada vez mais poderosas e versáteis nesta área de aplicação. O artigo discute as classes gerais de problemas na epidemiologia geográfica e faz uma revisão dos principais métodos estatísticos utilizados atualmente em cada uma das áreas de aplicação identificadas. O artigo não procura cobrir exaustivamente todos os medos e modelos possíveis, mas fornece referências bibliográficas para outros detalhes e abordagens. O objetivo geral é dar um panorama do "estado da arte" no uso de métodos estatísticos espaciais na pesquisa em epidemiologia e saúde pública. Após a revisão metodológica, o autor discute os principais ambientes de software atualmente disponíveis para implementar tais métodos. O artigo conclui com algumas reflexões gerais sobre as implicações, para a epidemiologia e a saúde pública, do uso de métodos estatísticos espaciais em saúde, além dos benefícios e problemas associados.

As limitações evidenciadas no emprego de variáveis empírico-indutivas nas alternativas hegemônicas da Epidemiologia para o tratamento das variações sócio-econômicas em coletivos humanos levaram à necessidade de novas propostas. Assim, foi introduzido o emprego da categoria classe social, operacionalizada de acordo com a inserção produtiva dos indivíduos estudados, apresentando potencialidades já evidenciadas em alguns estudos. O presente trabalho buscou identificar problemas e limites existentes na utilização do conceito de classe social na investigação epidemiológica. São abordados neste artigo alguns aspectos, entre eles: o tratamento das classes sociais enquanto agregados articulados no processo de investigação, a necessidade de adequar o esquema de operacionalização às particularidades de cada formação sócio-econômica, a simplificação efetivada no processo de redução do conceito de classe a uma série de variáveis articuladas empregadas, a necessidade de amostra relativamente grande, a existência de controvérsias na definição da inserção de classe de segmentos afastados da produção e de indivíduos com duas ou mais inserções produtivas distintas.The application of inductive empirical variables in hegemonic alternatives of Epidemiology in order to treat socioeconomic variations in human groups has displayed certain limitations raising the need for new proposals. The use of social class was thus introduced as an analytical category operated according to place in the productive process for the subjects under study. This choice has already shown potentialities in several studies. This paper was intended to identify some limits and problems occurring when using the social class concept in epidemiological research. Besides the need for an adequate theoretical framework, construction of categories consistent with both their theories and the levels of abstraction employed, and the concern for more carefully analyzing the consumer and working modes for each social class (questions that have been discused elsewhere), this paper approaches several issues, as follows: the treatment of social classes as articulated clusters in the research process, the need to adjust the operational scheme to the particularities of each socioeconomic formation, the simplification resulting from the process of reducing the concept of social class to several linked variables, the need for a relatively large sample, the existence of debate over the definition of the position of class groups not participating in production, and subjects with two or more different positions in production.

Este trabalho é parte de um estudo em que se aborda, interdisciplinarmente, o processo de violência/saúde no município de Duque de Caxias, processo este entendido como uma cadeia onde se expressam as violências estrutural, da resistência e da delinqüência, cujos reflexos se fazem presentes na mortalidade do município. Constitui-se em uma análise de dados de registro relativos à mortalidade por causas externas para o período de 1979 a 1987, e em uma análise crítica das informações coletadas das declarações de óbito para o ano de 1987. Apresenta-se a mortalidade por causas externas da área investigada, sua magnitude e importância em relação às principais causas de morte no município, em números absolutos, proporções, coeficientes brutos e padronizados, taxas e anos de vida potencialmente perdidos, estratificados por grupos de causas externas específicas, sexo e idade. Na análise, constrói-se um perfil sócio-econômico-demográfico destas vítimas. Discute-se os resultados à luz do conhecimento sociológico e histórico da dinâmica social de Duque de Caxias. Conclui-se sugerindo algumas medidas que consideram-se de extrema necessidade para o apoio às vítimas da violência.This paper is part of an interdisciplinary study dealing with violence and the health process in the city of Duque de Caxias, State of Rio de Janeiro. This process expresses structural violence in addition to that related to resistance and deliquency, the outcomes of which are present in the area's overall mortality. This is an analysis of recorded data on mortality form external causes in the period 1979-1987, as well as a critical analysis of information gathered from death certificates for the year 1987. Mortality from external causes in the area studied is shown according to its magnitude and importance in relation to the major causes of death in the city, using absolute numbers, ratios, crude and standardized coefficients, rates, and years of life potentially lost, which are classified by groups of specific external causes, sex, and age. The analysis gives a socio-economic and demographic profile of the victims. The findings are discussed in the light of the sociological and historical knowledge on the social dynamics of Duque de Caxias. As a conclusion, this paper suggests a few actions that are badly needed for providing support to the victims of violence.

Este artigo trata a problemática da violência social através do quadro de mortalidade por causas externas no Brasil, com ênfase na situação do município do Rio de Janeiro. São utilizados dados de mortalidade do Ministério da Saúde, consolidados pelo Departamento de Epidemiologia e Métodos Quantitativos em Saúde, da Escola Nacional de Saúde Pública, no período de 1980 a 1988. São apresentadas a mortalidade proporcional e as taxas de mortalidade por causas externas segundo sexo, idade, grupos específicos de causas externas, tipos de homicídios e acidentes de trânsito. Apesar de se tratar, basicamente, de um trabalho descritivo, faz-se aqui também uma reflexão sobre os homicídios, considerando-os como o fenômeno gerador de morte mais significativo na configuração da violência brasileira hoje.This paper investigates the problem of social violence based on the situation for mortality from external causes in Brazil, particularly in the city of Rio de Janeiro. Mortality data from the Brazilian Ministry of Health (consolidated by the Department of Epidemiology of the National School of Public Health in the period 1980-1988) are used in this study. Proportional mortality and mortality rates from external causes are given according to sex, age, specific groups of external causes, types of homicides, and traffic accidents. Despite being basically a descriptive study, this paper also studies homicides as the most significant cause of death in the configuration of violence in Brazil today.

Se analizan los cambios operados en las relaciones laborales, la legislación sobre enfermedades y accidentes de trabajo,así como el impacto en la salud de los trabajadores (1989-1995). Se comprueba un incremento de los índices de desocupación, subempleo, cuentapropismo y nuevas formas de contratación. Se exponen los mecanismos de traspaso de la responsabilidad en la prevención que realizó el estado en las aseguradoras de riesgos en el trabajo. La discusión teórica de la causalidad adquiere dimensión político-práctica. Se verifica un incremento de los accidentes de trabajo a partir de la reforma laboral. Las investigaciones desarrolladas por universidades, ONGs y sindicatos ponen de manifiesto los perfiles de alteraciones en la salud de los trabajadores asalariados, cuentapropistas y subempleados. Los tres Encuentros Nacionales de Trabajo y Salud (1992-1994), crearon un espacio de confluencia entre el saber académico y el de los trabajadores, con una producción que registra 128 ponencias, investigaciones y trabajos, constituyéndose en una instancia superadora, como nuevos modelos de vigilancia epidemiológica e intervención. Se analizan las investigaciones nacionales de la Confederación de Trabajadores de la Educación y la Asociación de Trabajadores del Estado.This paper analyzes changes in labor relations, legislation pertaining to work-related diseases and accidents, and the impact on workers' health in Argentina (1989-1995). The data indicate an increase in unemployment, underemployment, self-employment, and new hiring practices. The study shows the mechanisms by which the state has delegated responsibility for prevention of work-related risks to the private insurance sector. The theoretical discussion of causality thus acquires a political/practical dimension. The data show an increase in work-related accidents following labor reform. Research by universities, NGOs, and labor unions reveal changes in health profiles for salaried, self-employed, and underemployed workers. The three National Meetings on Employment and Health (1992-1994) established an area of exchange between academic and workers' knowledge, with some 128 presentations, research projects, and studies, pointing to an enhanced field for epidemiological surveillance and intervention. The paper reviews the surveys performed by the National Confederation of Workers in Education and the Public Employees' Association.

This paper presents an overview of the development of HIV/AIDS in South Africa, taking into consideration the social context and analyzing the factors most likely to have influenced its spread as well as the societal response to it. The authors argue that macro factors such as social and political structures, in addition to behavioral ones, have combined to shape the course of the epidemic. Since various factors linked to social inequalities have combined to shape the pattern and growth of the HIV/AIDS epidemic in South Africa, it is inappropriate to focus on only one dimension in an attempt to combat the epidemic. Following the psycho-socio- environmental model, all potential contributing elements need to be addressed simultaneously. This calls for a true interdisciplinary and multi-sectorial approach. It also requires great commitment as well as strong political will.O artigo apresenta um panorama da epidemia de HIV/AIDS na África do Sul, levando em conta o contexto social e analisando os principais fatores que influenciaram sua disseminação, assim como, a resposta pela sociedade sul-africana. As autoras argumentam que fatores macro, tais como estruturas sociais e políticas, além de fatores comportamentais, agiram em conjunto para configurar o curso da epidemia. Uma vez que vários fatores, associados a desigualdades sociais, influenciaram o padrão e o crescimento da epidemia do HIV/AIDS na África do Sul, é inapropriado focalizar apenas uma dimensão da epidemia na tentativa de combatê-la. Todos os elementos que possam influenciar o curso da epidemia devem ser abordados simultaneamente, de acordo com o modelo psico-sócio-ambiental. Tal abordagem deve ser verdadeiramente interdisciplinar e multi-setorial, o que também requer grande compromisso e vontade política.

Neste trabalho foram analisados, sob a ótica da vigilância epidemiológica dos acidentes por animais peçonhentos, quatro sistemas nacionais de informação, o SINAN (Sistema de Informações de Agravos de Notificação), o SINITOX (Sistema Nacional de Informações Tóxico-Farmacológicas), o SIH-SUS (Sistema de Informações Hospitalares do Sistema Único de Saúde) e o SIM (Sistema de Informações sobre Mortalidade). Concluiu-se que esses sistemas possuem características próprias, foram criados para atender demandas diferentes e apesar de produzirem um grande volume de dados, não conseguem, ainda que analisados em conjunto, dar conta da dimensão real desses acidentes.This paper highlights the epidemiological surveillance of venomous bites and stings according to four national information systems: SINAN (National Databank of Major Causes of Morbidity), SINITOX (National Information System on Poisoning), SIH-SUS (Hospital Information System of the Unified Health System), and SIM (Mortality Information System). The authors conclude that each information system has specific characteristics and addresses different demands. Although they contain large amounts of data, even if combined they fail to reflect the real magnitude of disorders caused by venomous bites and stings in the country.

Este artigo visa compreender a contribuição de fatores do ambiente externo ao sistema de assistência na ocorrência de óbitos perinatais em maternidades, que em 1994, compunham a rede de atendimento obstétrico em um município da região metropolitana do Rio de Janeiro. Dividiram-se os elementos em quatro grupos de variáveis, para entender a relação entre falhas no atendimento e deficiências no ambiente externo e institucionalizado: repasse de recursos para custeio, fatores geográficos e temporais, características organizacionais e administrativas e ação/participação de grupos de interesse. Entrevistas semi-estruturadas foram realizadas. Os resultados indicaram as seguintes falhas: repasses de recursos para custeio insuficientes para manter cuidados de qualidade, sobretudo nos casos das maternidades privadas; nenhuma regionalização ou hierarquização formal ou informal dos cuidados obstétricos no município; desconhecimento das normas do Ministério da Saúde nas maternidades estudadas e as adotadas em três das quatro maternidades não faziam referência nem aos procedimentos para a admissão, nem ao seguimento do trabalho de parto, nem ao seguimento fetal e o nível de participação não era o efetivamente implementado.This paper focuses on the role of environmental factors external to the health care system in the occurrence of perinatal deaths in maternity hospitals belonging to the local health system in a city in Greater Metropolitan Rio de Janeiro in 1994. Elements from the political and administrative context that contribute to an understanding of the relationship between failures in health care and structural deficiencies in these maternity hospitals were divided into four groups of variables: distribution of resources, spatial and temporal factors, organizational and managerial features, and action by interest groups. Semi-structured interviews were conducted. The study concluded that poor performance in four groups of variables may have contributed to perinatal mortality: distribution of resources was insufficient to provide quality in health care, especially in private maternity hospitals; there was no formal or informal regional or hierarchical organization of obstetric care in the city; Ministry of Health guidelines were ignored in all four maternity hospitals, while in three of the hospitals there were no admissions procedures and delivery and fetal follow-up listed in their own rules; and the level of actual participation was low.

This paper presents an overview of the development of HIV/AIDS in South Africa, taking into consideration the social context and analyzing the factors most likely to have influenced its spread as well as the societal response to it. The authors argue that macro factors such as social and political structures, in addition to behavioral ones, have combined to shape the course of the epidemic. Since various factors linked to social inequalities have combined to shape the pattern and growth of the HIV/AIDS epidemic in South Africa, it is inappropriate to focus on only one dimension in an attempt to combat the epidemic. Following the psycho-socio- environmental model, all potential contributing elements need to be addressed simultaneously. This calls for a true interdisciplinary and multi-sectorial approach. It also requires great commitment as well as strong political will.

Neste artigo, apresentamos princípios sistematizados por Geoffrey Rose, raramente explicitados no debate contemporâneo sobre promoção da saúde. São enfatizadas as seguintes noções: muitos parâmetros biológicos apresentam-se ao longo de um continuum e não seguem as dicotomias expostos/não expostos ou doentes/não doentes; a relação entre determinadas exposições e seus efeitos ocorre linearmente, ao longo de toda a gama dos valores da exposição; portanto, muitos indivíduos expostos a um risco baixo podem gerar um número maior de casos do que poucos indivíduos expostos a um risco alto de adoecer. Além disso, há forte relação entre os comportamentos populacionais médios e os desviantes, como resultado da dinâmica entre forças biológicas e sociais, que favorecem ou limitam a diversidade de características individuais. Assim, o risco de alguns agravos, em populações concretas, torna-se alto ou baixo em virtude do deslocamento em bloco da distribuição populacional, e não em função do número de indivíduos na faixa de alto risco. Em conjunto, esses conceitos favorecem estratégias voltadas para o conjunto da população, em vez das que apenas visam indivíduos considerados de alto risco.In this paper, we discuss some of the ideas developed by Geoffrey Rose, which are seldom explicitly expressed in the contemporary debate on health promotion. The following notions are emphasized: many health exposures and outcomes are expressed in a continuum, and do not follow the dichotomy exposed/non-exposed or diseased/ non-diseased; there is a linear relationship between certain exposures and their effects along the range of exposure levels; thus, many individuals exposed to low risk may generate more cases of a disease than few individuals exposed to high risk. In addition, there is a strong relationship between average behaviors and the occurrence of deviance, as a result of the balance between biological and social forces favoring diversity or uniformity. Thus, risk differences between defined populations involve differences in the population distributions as a whole, rather than in the proportion of individuals with high risk. Overall, these concepts favor emphasis on strategies aiming the general population, and not only the individuals considered to be at high risk of disease.

Foram analisadas 352 fichas de registro de acidentes por escorpião ocorridos no Distrito Sanitário Noroeste no Município de Belo Horizonte, no período de 1993 a 1996. Este distrito congrega, tradicionalmente, uma das maiores incidências de casos, tendo-se encontrado 10,37 acidentes por 10.000 hab., no período estudado. A avaliação segundo o sexo não revelou risco diferenciado e a faixa etária com maior incidência foi a de 50 anos ou mais. Não se verificou sazonalidade na distribuição temporal dos acidentes. A distribuição espacial dos acidentes revelou áreas com maior concentração de casos, tendo o aparecimento de escorpiões seguido, aproximadamente, padrão de distribuição semelhante. Esta primeira avaliação descritiva é importante para nortear as atividades e áreas a serem priorizadas no planejamento das ações de combate ao escorpionismo de acordo com os diferentes perfis epidemiológicos das populações atendidas pelos serviços de saúde pública, mormente o do Distrito Sanitário Noroeste.This paper analyzes a total of 352 scorpion sting reports in the Northwest quarter of Belo Horizonte, Minas Gerais. Traditionally, this area has shown one of the highest case incidence rates, and the present study pointed to 10.37 stings per 10,000 inhabitants. No gender preference was observed, but most victims were 50 years of age or older. Likewise, no relationship was observed between seasonality and sting rates. According to geographic distribution analyses, the highest incidence rates were in areas with the largest scorpion populations. This preliminary descriptive evaluation is important for guidelines to prevent scorpion stings, which should obviously consider local epidemiological features.

O presente artigo descreve o perfil nutricional da população idosa brasileira (idade igual ou superior a sessenta anos), com base nos dados da Pesquisa Nacional sobre Saúde e Nutrição, estudo com amostra probabilística realizado em 1989. Utilizaram-se para tal análise: o índice de massa corporal - IMC (kg/m²), como indicador do estado nutricional segundo recomendação da OMS (WHO, 1995), e as variáveis idade, gênero, região e situação do domicílio no país (urbana/rural), renda, escolaridade e qualidade de moradia. As prevalências gerais de magreza (IMC < 18,5) e sobrepeso (IMC > ou = 25) foram, respectivamente, 7,8% e 30,4% em homens e 8,4% e 50,2% em mulheres, mais elevadas que na população adulta jovem. A magreza foi mais freqüente em mulheres nas faixas etárias avançadas; em áreas rurais das regiões Centro-Oeste/ Nordeste (mulheres) e Sudeste/Centro-Oeste (homens); nas classes de menor renda, menor escolaridade e pior qualidade de moradia. O sobrepeso foi prevalente em mulheres, em áreas urbanas das regiões Sul e Sudeste (ambos os sexos); nos grupos de maior renda, maior escolaridade e melhor qualidade de moradia. Os resultados indicam um quadro preocupante de alterações nutricionais em idosos no Brasil, principalmente em mulheres.This paper presents the nutritional status of the elderly Brazilian population (60 years and older) based on a probabilistic sample survey in 1989. Nutritional assessment used the BMI (kg/m2), with cut-off points recommended by the WHO (1995). Socio-demographic variables included age, gender, region and place of residence in the country (urban/rural area), income, education, and housing conditions. Prevalence rates for thinness (BMI < 18.5) and overweight (BMI > or = 25) were 7.8% and 30.4% in men and 8.4% and 50.2% in women, higher than in the young adult population. Thinness was more frequent in older women, in rural areas from the Midwest/Northeast regions (women) and Southeast/Midwest (men), in groups with lower income, less schooling, and worse housing conditions. Overweight was more frequent in women from urban areas in the South and Southeast and groups with higher income, more schooling, and better housing conditions. The results indicate problematic nutritional status in the Brazilian elderly, particularly women.

O objetivo deste trabalho é estudar a validade das informações sobre ocupação habitual e causa básica em declarações de óbito (DO) de moradores de Botucatu falecidos nesta cidade. Analisou-se a concordância dessas informações com seus padrões-ouro, estabelecidos por intermédio de entrevistas com familiares de 552 falecidos em 1997 e análise de documentação médica. O coeficiente Kappa para a concordância entre a informação sobre ocupação e o padrão-ouro foi 0,31 (IC 95% 0,29-0,34). Para a concordância entre a causa básica declarada e o padrão-ouro, o coeficiente Kappa foi de 0,76 (IC 95% 0,75-0,77). Conclui-se que, embora a validade da informação sobre a causa básica do óbito seja boa, a validade da informação sobre a ocupação habitual é bastante precária. Isto impossibilita a utilização dessas informações em estudos epidemiológicos locais. Tal constatação alerta para a necessidade de previamente verificar a fidedignidade de informações ocupacionais provenientes de declarações de óbito em estudos de saúde do trabalhador. Outra implicação desse achado é enfatizar a necessidade de investir-se na melhoria da qualidade dessas informações.The aim of this paper was to evaluate the accuracy of data on death certificates for occupation and main cause of death. Measure of agreement was assessed comparing data from death certificates with those from both medical records and next-of-kin interviews, analyzing information for 552 residents of Botucatu, Southeast Brazil, who died in 1997. Kappa coefficients of 0.31 (95% C.I. 0.29-0.34) and 0.76 (95% C.I. 0.75-0.76) were obtained for data on occupation and main cause of death, coded by a Brazilian two-digit classification and the three-digit ICD-10 classification, respectively. One can conclude that, although quality of the main cause of death is acceptable for pilot studies, data on occupation taken only from death certificates is not accurate enough to be used in epidemiological research.

As atuais fragilidades conceituais e metodológicas da epidemiologia são fatores que têm restringido o estudo dos fenômenos de saúde das coletividades humanas. Neste artigo alguns princípios básicos são apresentados como resultado da observação do percurso da geografia na definição do seu objeto o espaço e da sua aplicação na medicina e na epidemiologia. Esses princípios fundamentam a pertinência da estratégia conceitual e metodológica que tem como perspectiva a abordagem do espaço geográfico-social, já que este é entendido como expressão das condições de vida da população. A aplicação desse conceito na prática da investigação epidemiológica ainda é limitada, embora outras áreas do conhecimento já tenham desenvolvido propostas de superação. Os estudos de agregados, cuja unidade de análise é o grupo, mais freqüentemente os agregados espaciais, e o modelo ecológico que se baseia na idéia de inter-relação de fatores, se aperfeiçoados, podem vir a ser uma alternativa promissora nesta direção. Destaca-se que a totalidade deve ser a referência científica, visando garantir o não afastamento dos complexos processos interativos determinantes dos fenômenos de saúde na população.Epidemiology's conceptual and methodological shortcomings have placed constraints on the study of health phenomena related to human communities, thus posing a challenge to the field. This paper presents some basic principles resulting from the observation of the field of geography in defining its object space and the application of this object to medicine and epidemiology. Such principles state the pertinence of a conceptual and methodological strategy focusing on an approach to geographic space as expressing the population's living conditions. Application of this concept to epidemiological studies is still limited, although such proposals have already been developed in other areas of knowledge. Ecological studies, whose unit of analysis is the group, and the ecological model, based on the idea of an inter-relationship of factors, if improved, could become a promising alternative in this direction. The authors emphasize that researchers should have wholeness as their scientific reference in order to guarantee the non-separation of the complex interactive processes determining health phenomena in the population.

O presente trabalho tem como objetivo caracterizar a prevalência de baixo peso e obesidade em grupamentos (estratos) sociais da Área Metropolitana de São Paulo, Brasil. O estado nutricional foi definido pelo Índice de Massa Corporal (IMC). As prevalências de desnutrição energético-protéica foi de 3,9% nos homens e 6,2% nas mulheres. O sobrepeso variou de 27,5% a 34,0 %, nos homens e de 25,8% a 43,6%, nas mulheres. A obesidade, nos homens, variou de 2,5% a 11,1%.; nas mulheres de 7,1% a 28,5%. Nestas o excesso de peso apresentou aumento brusco após os 40 anos de idade, com diferenciais estatisticamente significantes (pThis paper focuses on prevalence of nutritional status (Body Mass Index, BMI) in social groups in Greater Metropolitan São Paulo, in Southeastern Brazil. The population was stratified in four socioeconomic groups. Prevalence of malnutrition (BMI < 18.5kg/m2), low body mass (BMI < 20.0kg/m2), and overweight (BMI > 25.0 kg/m2) and obesity (BMI > or = 30.0kg/m2) were calculated. Prevalence of malnutrition was 3.9% in men and 6.2% in women. Prevalence of overweight ranged from 27.5% to 34.1% in men and from 25.8% to 43.6% in women. Obesity ranged from 2.5% to 11.1% in men and from 7.1% to 20.5 in women. Prevalence of overweight and obesity was greater among women than men (p 25.0 kgm/2), the study showed that prevalence in men was 43, 51, 35, and 30% for strata I, II, III, and IV, respectively. Among women, prevalence was 12, 61, 55 and 46% for strata I, II, III, and IV, respectively. In women there were an abrupt increase in excess weight just before 40 years of age. A high percentage of overweight and obesity was observed in all population groups.

On the eve of the new millennium, it has become 'natural' to admit the emergence of tendencies to perform evaluations and inventories of the past and attempts to forecast future scenarios. While recognizing the ensuing uncertainties, the current paper takes this point of view as the point of departure for proposing a discussion on the future directions and prospects of epidemiology. Based on the pertinent analyses performed by the Sussers (father and son), I approach and discuss the scope and limits of new aspects assumed by the field, especially to the extent that it has included techniques and instruments from bioinformatics and molecular biology. In the latter areas (amongst many others), the notion of information has gained enormous importance. I then proceed to analyze the conceptual origins and shifts in this notion, in addition to possible repercussions and effects on the field of biological sciences in general and their research practices in particular.inclinações para procederem-se a avaliações e balanços do passado e a tentativas de prever-se cenários futuros. Este texto parte deste ponto de vista reconhecendo suas incertezas para propor uma discussão dos rumos e perspectivas da epidemiologia. A partir das análises nesta direção pelos Sussers (pai e filho), são abordadas e discutidas o alcance e os limites de novos aspectos assumidos pela disciplina, especialmente, ao incluir técnicas e instrumentos da bioinformática e da biologia molecular. Nestas áreas (entre muitas outras), é notável o fato da noção de informação possuir enorme importância. São, então, analisadas origens e deslocamentos conceituais desta noção e possíveis repercussões e efeitos no campo das ciências biológicas, em geral, e de suas práticas de pesquisa, em particular.

Four epidemiological areas with different prevalences were identified and stratified.

28. Alcohol consumption and gastric cancer in Mexico

This paper presents an assessment of alcohol consumption, including the popular Mexican liquor tequila, in relation to the incidence of gastric cancer. We conducted a population-based case-control study in Mexico City, with 220 gastric cancer cases and 752 population-based controls. A food frequency questionnaire was used to measure consumption of alcohol and other dietary items. Grams of ethanol were estimated by the Food Intake Analysis System 3.0 software. After adjustment for known risk factors, wine consumption was positively associated with the risk of developing gastric cancer (OR = 2.93; CI 95% 1.27-6.75) in the highest category of wine consumption, corresponding to at least 10 glasses of wine per month, with a significant trend (p = 0.005). This association remained among intestinal (OR = 2.16; CI 95% 0.68-6.92, p-value for trend = 0.031) and diffuse (OR = 4.48; CI 95% 1.44-13.94, p-value for trend = 0.018) gastric cancer cases. A borderline significant trend between GC risk and total ethanol intake was observed (p = 0.068). Consumption of beer and distilled alcoholic beverages including brandy, rum, and tequila was not associated with GC risk. The results indicate the need to focus on the study of the potential effects of different types of wine, with emphasis on components other than ethanol regarding the incidence of gastric cancer, even among populations with moderate to low levels of alcohol consumption.

Evidence exists that exposure to poultry oncogenic viruses may produce elevated cancer mortality in human populations, particularly excesses of cancer of lung and excesses of cancer of lymphopoietic tissues. To date, this potential risk is unknown in populations from the developing countries. This paper suggests the need to assess cancer risk in populations of developing countries with reported environmental exposure to chicken meat products and eggs; the need to assess risk of cancer in populations inoculated with vaccines from infected chicken embryos; and the need to assess risk of cancer in occupational populations highly exposed to poultry oncogenic viruses, and with potential concurrent exposure to chemical agents known or suspected to be carcinogens.

On the eve of the new millennium, it has become 'natural' to admit the emergence of tendencies to perform evaluations and inventories of the past and attempts to forecast future scenarios. While recognizing the ensuing uncertainties, the current paper takes this point of view as the point of departure for proposing a discussion on the future directions and prospects of epidemiology. Based on the pertinent analyses performed by the Sussers (father and son), I approach and discuss the scope and limits of new aspects assumed by the field, especially to the extent that it has included techniques and instruments from bioinformatics and molecular biology. In the latter areas (amongst many others), the notion of information has gained enormous importance. I then proceed to analyze the conceptual origins and shifts in this notion, in addition to possible repercussions and effects on the field of biological sciences in general and their research practices in particular.

Neste trabalho, descreve-se a evolução da mortalidade infantil no Brasil na década de 80. Diante da elevada proporção de óbitos não registrados no País, a caracterização do risco de morrer entre as crianças menores de um ano de idade é realizada por um conjunto de indicadores de saúde elaborados com base na distribuição por causa de morte e por componente etário. Por meio de uma análise estatística por componentes principais, desenvolve-se um índice sintetizador que permite expressar quantitativamente as diferentes situações de saúde nas Unidades Federadas. Através desta escala de valores, dividindo-se o território nacional em três grandes grupos, demonstra-se que a parte mais pobre persiste com padrão similar ao da Índia. Adicionalmente, a análise da mortalidade neonatal nos estados mais desenvolvidos mostra redução pouco expressiva nos coeficientes na primeira semana de vida, demonstrando-se que o padrão observado, mesmo nos estados em situação privilegiada em relação aos demais, sequer se aproxima do que ocorre no mundo desenvolvido. De maneira geral, o confronto com a experiência internacional leva a constatar que o declínio da mortalidade infantil na década de 80 foi pobre, ficando evidente que o rumo a ser seguido envolve intervenções específicas sobre ambos os componentes, o neonatal e o tardio.In this paper, we analyze the infant mortality in Brazil in the 1980 decade. Since the completeness of death registration is very low in some regions of the country, the analysis is based on five health indicators constructed from the proportional distribution by causes of death and by age-group. Using a principal component analysis, we develop an index that expresses quantitatively the diversified conditions of mortality. Through this index, classifying the Brazilian states in three groups, it is shown that the poorest one continues to exhibit a pattern similar to India and other low income populations. Additionally, analyzing the neonatal mortality in the wealthiest group, it is found a small reduction in the first week mortality, indicating that even the favored group still remains very far from the developed world. In general, when compared to other countries, Brazil has shown a poor performance. It becomes evident the need of reestablishing strategies to improvement on both components, the neonatal and the post-neonatal mortality.

32. Implications of Helicobacter pylori infection for stomach cancer prevention

Accumulating evidence has implicated Helicobacter pylori, an established cause of chronic gastritis and peptic ulcer, in the etiology of gastric cancer. Control of this infection would reduce the occurrence of chronic gastritis and peptic ulcer and might substantially lower the risk of stomach cancer as well. The public health impact of this infectious agent warrants efforts to identify preventive measures. This paper reviews the evidence linking H. pylori infection to gastric cancer and evaluates the potential for control in high-risk populations. Current obstacles to H. pylori control are discussed, including the link to poor socioeconomic conditions, difficulty in identifying incident cases, lack of natural immunity to reinfection, limited effectiveness of antibiotic therapy in high-prevalence populations, and incomplete knowledge regarding the reservoir of infection, mode of transmission, host susceptibility factors, and the potential for developing an effective vaccine. Worthwhile avenues of research include studies designed to identify modifiable risk factors for acquisition of the infection, modifiable host factors that may increase resistance to chronic infection, more effective antibiotic therapies, and effective vaccines.

We carried out a case-control study to investigate risk factors for childhood pneumonia in two groups of 650 children aged under two years in the city of Fortaleza, Ceará, Brazil. The cases were children recruited at the main pediatric hospital with a radiological diagnosis of pneumonia, and controls were children of the same age group recruited from the neighbourhood of the cases. In this paper we focus on variables related to childcare practices. Working mothers, proportion of time the mother had worked since the child was born, and use of day care centers emerged as important risk factors with estimated relative risks of 1.58, 1.76 and 5.22, respectively. Also important were the number of children living in the house and presence of grandparents. However, the presence of siblings under two years and the birth order were not associated with pneumonia. All analysis included adjustment for confounding by income, parents' education, and other risk factors as appropriate. This is the first study from a developing country to identify attendance at day care centers as a risk factor for increased childhood morbidity, in this case pneumonia. This finding is of significant public health importance for countries such as Brazil with growing urban populations and an increasing need by mothers to find work outside the home.Estudou-se o risco de pneumonia nas crianças menores de dois anos na região metropolitana de Fortaleza entre junho/89 e maio/90. Foram investigadas as práticas relacionadas aos cuidados da criança como possíveis fatores de risco de pneumonia. Fatores maternos, ambientais e sócio-econômicos foram também estudados devido a sua possível relação com as práticas no cuidado da criança. Foi utilizada metodologia de caso-controle, sendo casos as crianças com diagnóstico clínico e radiológico de pneumonia, e controles aquelas crianças com diferença de idade inferior a dois meses que não apresentassem sintomas de infecção respiratória, e que residissem na vizinhança do caso selecionado. Razão de "odds" (RO) foi utilizada para estimar os riscos relativos, através de regressão logística condicional. Os principais fatores de risco encontrados foram a freqüência a creche (RO=5,2), trabalho da mãe (RO=1,6) e presença dos avós no domicílio (RO=1,4). A idade da mãe, o número de gestações e a aglomeração também estiveram associados com pneumonia.

Este artigo apresenta os resultados de um estudo sobre mortalidade neonatal no Estado do Rio de Janeiro (1979-1993), segundo principais grupos de causa de morte, de acordo com idade e região de residência. Foi proposto um critério de agrupamento das causas de morte, levando-se em consideração a freqüência das mesmas e a evitabilidade do óbito. Para os óbitos ocorridos no primeiro dia de vida, a imaturidade e o crescimento fetal retardado foram as únicas causas que mostraram declínio significativo. Houve acentuada elevação do número de mortes por afecções respiratórias do feto e do recém-nascido. As causas de óbito ditas reduzíveis por "adequado controle da gravidez e adequada atenção ao parto" e "diagnóstico e tratamento precoce" permaneceram inalteradas. Constatou-se que o decréscimo nas taxas ocorreu a partir do grupo etário de 1 a 6 dias. A comparação com outros países chama a atenção para a elevada proporção de óbitos relacionadas a "problemas respiratórios do recém-nascido", demonstrado falta de especificidade na definição da causa básica de óbito.This paper reports the results of a study on neonatal mortality in the State of Rio de Janeiro (1979-1993), according to major causes of death, by age and region of residence. The authors suggest criteria for grouping neonatal deaths according to causes, taking into consideration frequency and preventability. For deaths which occurred in the first day of life, immaturity and fetal growth retardation were the only causes of death that showed significant decline. An increase in deaths due to respiratory affections of the fetus and newborns was observed. Causes of death said to be reducible by "adequate control of pregnancy and attention at delivery" and "early diagnosis and treatment" remained unaltered. The authors noted that a decrease in death rates took place starting with the 1-6 day age group. The comparison with other countries called attention to the high rates of neonatal death in Rio de Janeiro. It also pointed out an increase in the proportion of deaths classified as "respiratory problems of the newborn", demonstrating lack of specificity in defining the basic cause of death.

Este segundo artigo (de uma série de dois) tem como finalidade apresentar apenas os resultados básicos de um estudo, em amostra representativa de população, sobre Hipertensão Arterial na Ilha do Governador (região administrativa do município do Rio de Janeiro), em três estratos de acordo com a renda média domiciliar (baixa, média e alta) de setores censitários. No conjunto da Ilha do Governador a prevalência de hipertensão não controlada foi de 16,1 % (I.C. de 95%: 13,4 a 18,9%), e incluindo-se os hipertensos controlados, este índice subiu para 24,9% (I.C. de 95%: 21,7 a 28,1 %). Por ambos os critérios (incluindo ou não os hipertensos controlados), as prevalências mais elevadas foram as do estrato de renda baixa e as menores do estrato de renda alta. Porém, só foi possível rejeitar a homogeneidade de prevalências de hipertensão (incluindo os controlados) entre os estratos, e ainda assim para uma significância de 10%, quando se ajustou o efeito de confusão da idade. As mulheres hipertensas parecem controlar em maior proporção seus níveis de pressão arterial em qualquer faixa etária dos adultos, do que os homens.The aim of this paper (the second of two) is to present the basic results of a crosssectional study on arterial hypertension in adults in Ilha do Governador (a district of the city of Rio de Janeiro, Brazil), with a representative sample of the population, stratified by the census tracts' mean household incorre (low, median, and high). Overall, prevalence of uncontrolled hypertension in Ilha do Governador was 16.1% (C.I. of 95%: 13.4 to 18.9%), and with the inclusion of controlled hypertensive individuals this figure rose to 24.9% (C I. of 95%: 21.7 to 28.7%). Under both criteria (whether including controlled hypertensive individuais or not), higher prevalences were from low‑incorre strata, while lower ones were from the high‑incorre strata. However, it was only possible to reject the hypothesis of strata homogeneity of prevalences (including controlled hypertensives), and even then at a 10% level of significance, when the confounding effect of age was adjusted. Proportionally, women with hypertension appear to control their blood pressure levels better than men in all age brackets.

São descritos com detalhes o delineamento amostral (amostragem de aglomerados em estratos, segundo renda média domiciliar) e os instrumentos de coleta de dados. São apresentados, ainda, indicadores de desempenho no trabalho de campo e do controle de qualidade.The aim of this paper (first of a series of two) is to present the main methodological aspects used in a cross-sectional study whose proposals were to establish the prevalence of arterial hypertension in adults in Ilha do Governador (a district of the city of Rio de Janeiro, Brazil), and to examine its relationship to other risk factors, including those for cardiovascular diseases, so as to establish its control status. Sampling design (of clusters in strata, according to mean household income) is described in detail, in addition to measurement methods, and training of observers. Indicators of performance in field work and quality control are also presented as results of the application of the metodology of study.

O objetivo desse trabalho foi discutir a questão de validade, tomando como referencial um fragmento de uma pesquisa desenvolvida em área urbana na Região Metropolitana do Recife, com a finalidade de investigar a associação entre fatores socioeconômicos e esquistossomose. Discute-se as possíveis distorções na razão dos produtos cruzados (odds ratio) que podem ser introduzidas, quer pela forma como os indivíduos são selecionados para o estudo, quer por problemas ligados a obtenção das informações. Ressalta-se alguns dos mecanismos utilizados, em diferentes momentos da investigação, no sentido de minimizar a possibilidade do aparecimento desses tipos de bias ou de avaliar a sua influência na interpretação dos resultados.The purpose of this paper was to make some comments on validity based on an investigation developed in an urban area in the Greater Metropolitan Area of Recife, Brazil, the objective of which was to study the association between socioeconomic factors and schistosomiasis. It discusses the distortion in the odds ratio that may be introduced either by the way individuals are selected for the study or by the way the information is collected. The article describes the mechanisms used by the authors to avoid bias when planning or conducting the investigation and considerations were made on how these types of bias could have influenced the results.

Em 1988, um inquérito epidemiológico foi realizado para se estimar a prevalência de alcoolismo crônico e uso de álcool na população da XX Região Administrativa da cidade do Rio de Janeiro. O instrumento incluiu perguntas sobre o consumo de psicofármacos, café e cigarros. Este trabalho analisa os dados referentes ao consumo de psicofármacos nos 30 dias anteriores à pesquisa. A amostra representativa da população acima de 13 anos foi composta por 1.459 pessoas, Os resultados mostram uma prevalência de consumo global de 5,2% (3,1% para homens e 6,7% para mulheres). As mulheres, as pessoas mais velhas (especialmente pessoas entre 60 e 69 anos), os separados e viúvos, e as pessoas de renda mais baixa apresentaram prevalências mais elevadas. Avaliou-se a associação de cada variável demográfica através do cálculo deodds ratio ajustado por regressão logística. Os tranqüilizantes derivados dos benzodiazepínicos predominaram entre os tipos de psicofármacos referidos (85,23%), seguidos pelos antiepilépticos (5,68%) e pelos hipnóticos e sedativos (4,54%). Os clínicos não-especializados em neurologia ou psiquiatria lideraram as prescrições (65,8%). Oitenta por cento dos medicamentos foram obtidos na rede privada, enquanto 13,16% foram adquiridos na farmácia pública, quase todos mediante a apresentação do receituário controlado. Os resultados são discutidos e novas linhas de pesquisa são apontadas.In 1988, a cross-sectional survey was conducted to estimate the prevalence of alcoholism and alcohol consumption in the 20th Administrative Region of the city of Rio de Janeiro. The interview protocol included questions about consumption of psychotropics, coffee, and cigarettes. This paper presents data from psychotropic consumption in the month. The sample of the population older than 13 years old included 1,459 subjects. Overall consumption of psychotropic drugs was 5.2%. Rate of psychotropic use was higher for females (6.7%) than for males (3.1%). Users were predomonantly women, elderly, (the highest rate was for those between 60-69 years), divorced or widows, and with lower family income. The association of each socio-demographic variable was evaluated by the odds ratio adjusted by logistic regression. Anxiolytics (benzodiazepines) predominated (85.23%) among the reported psychotropics, followed by antiepileptics (5.68%) and hypnotics and sedatives (4.54%). Physicians not specializing in neurology or psychiatry lead prescriptions (65.8%). 80.26% of the drugs were obtained in pharmacies, while 13.16% were obtained in governmental institutions, most of them by controlled prescriptions. The results are discussed and new lines of research are suggested.

Transplacental transmission of Trypanosoma cruzi has been the focus of much attention in highly endemic areas in South America. Frequency of congenital transmission and factors associated with risk of it are still not well understood. Parasite strains may account for part of the geographical variation observed. Methodological differences between the studies do not permit a combined interpretation of results. This paper examines the epidemiological data available from Brazil, Bolivia, and Argentina and discusses possible epidemiological study design to investigate risk factors for transmission.

Abordam-se questões como a definição de alcoolista e os obstáculos para a sua identificação através de instrumentos utilizados em estudos populacionais. Discutem-se ainda as vantagens e desvantagens dos diferentes deliamentos de pesquisa para estimar a prevalência do alcoolismo e dos problemas associados ao álcool.This paper presents some methodological difficulties in studying the prevalence of alcoholism and alcohol abuse. Diante das reiteradas propostas de elaboração de um Plano Diretor para o Desenvolvimento da Epidemiologia, o A. analisa as dificuldades inerentes à construção do seu objeto. O arcabouço teórico-metodológico da "nova" Epidemiologia é contrastado com as necessidades de ampliação do uso da disciplina a nível dos serviços de saúde, geralmente como arma do planejamento das ações. Passando por considerações a respeito dos pólos científico e técnico, no caso peculiar de uma disciplina que tem socialmente determinados os problemas com que se defronta e a urgência das respostas exigidas, o A. considera o meio acadêmico lugar privilegiado, mas não exclusivo, onde se elaboram as respostas. Ao considerar a estratégia de divulgação dos avanços obtidos, o A. lembra que, além da produção acadêmica consolidada em livros e artigos de revista, há que levar em conta a literatura fugidia, tão comum nesta área do conhecimento. Comenta a incipiência da divulgação sistematizada da informação relevante produzida em teses e dissertações. Conclui atribuindo à linha editorial da Abrasco papel saliente, no Brasil, no direcionamento da divulgação da produção epidemiológica. Propõe a consolidação desta linha editorial, sugerindo algumas alterações.Despite several proposals concerned with the plans of development of Epidemiology, there is, already a great discussion over the object of this subject. The recent development of a "new" Epidemiology has often been questioned as a tool adequate enough to give bases to the projection and operation of contemporaneous health centers. Epidemiology as a scientific subject has a peculiar character: the social determinations attached to problems and urgency of answers. The academic environment, though a privileged place, is not the exclusive producer of Epidemiologic knowledge. Apart from papers and books produced by the academic work, we must consider the "marginal" writings so common in such area of knowledge. Also we have an incipient systematic divulgence of relevant epidemiological information originated from the health centers. Even the academic work such as thesis and dissertations are not well divulgated. The Brazilian editorial policy of ABRASCO (Brazilian Association of Colective Health) has the most important role in the divulgence and definition of the lines of Epidemiology.

O Brasil tem uma alta prevalência de laqueadura tubária. Alguns setores da sociedade acreditam que essa alta prevalência estaria indicando um controle da natalidade dissimulado, que visa diminuir a fecundidade nas camadas mais pobres da população. O objetivo deste trabalho é contribuir para a compreensão das possíveis diferenças sócio-econômicas quanto à prática da laqueadura. Para este fim, analisamos uma base de dados com 1.335 mulheres com idade de 15 a 49 anos, obtida entre mulheres de nível sócio-econômico médio-baixo ou baixo, em duas regiões do Estado de São Paulo, no ano de 1991. Os resultados mostraram uma aparentemente maior prevalência de laqueadura entre as mulheres com menor nível educacional e também entre as que moravam em habitação de boa qualidade. Entretanto, ao controlar por idade, essas associações desapareceram, sugerindo que se deviam apenas a menor escolaridade e maiores recursos das mulheres de maior idade, sendo este último o fator mais fortemente associado à prevalência de laqueadura. Discute-se a complexidade das relações entre nível sócio-econômico e ligadura tubária, incluindo a diferença sócio-econômica na prevalência de cesárea, intimamente ligada à esterilização feminina.Brazil has a high prevalence of tubal ligation. The hypothesis that it indicates undercover birth control efforts specifically addressed to the poorest sector of Brazilian society has been raised. The purpose of this paper is to evaluate whether there are differences in rates of tubal ligation depending on socioeconomic status. Data were gathered from interviews with 1335 women 15-49 years of age, of low or lower-middle socioeconomic status, carried out in 1991 in two regions of the State of São Paulo. The results showed an apparently higher prevalence of tubal ligation among women with less schooling,, but also among those living in better housing. However, after controlling for women's age, these associations disappeared, suggesting that they were explained by lower educational levels and greater economic resources of women of older age, where the latter factor is more heavily associated with a higher prevalence of tubal ligation. The complexity of the associations between socioeconomic status and tubal ligation are discussed, including the correlations between socioeconomic status and C-sections, which are in turn closely related to tubal ligation.

Evidence exists that exposure to poultry oncogenic viruses may produce elevated cancer mortality in human populations, particularly excesses of cancer of lung and excesses of cancer of lymphopoietic tissues. To date, this potential risk is unknown in populations from the developing countries. This paper suggests the need to assess cancer risk in populations of developing countries with reported environmental exposure to chicken meat products and eggs; the need to assess risk of cancer in populations inoculated with vaccines from infected chicken embryos; and the need to assess risk of cancer in occupational populations highly exposed to poultry oncogenic viruses, and with potential concurrent exposure to chemical agents known or suspected to be carcinogens.Existe evidência de que exposição a vírus oncogênicos de aves possa produzir elevada mortalidade de câncer em populações humanas, havendo particularmente excesso de câncer de pulmão e dos sistemas linfáticos e hemopoiéticos. This paper presents an assessment of alcohol consumption, including the popular Mexican liquor tequila, in relation to the incidence of gastric cancer. We conducted a population-based case-control study in Mexico City, with 220 gastric cancer cases and 752 population-based controls. A food frequency questionnaire was used to measure consumption of alcohol and other dietary items. Grams of ethanol were estimated by the Food Intake Analysis System 3.0 software. After adjustment for known risk factors, wine consumption was positively associated with the risk of developing gastric cancer (OR = 2.93; CI 95% 1.27-6.75) in the highest category of wine consumption, corresponding to at least 10 glasses of wine per month, with a significant trend (p = 0.005). This association remained among intestinal (OR = 2.16; CI 95% 0.68-6.92, p-value for trend = 0.031) and diffuse (OR = 4.48; CI 95% 1.44-13.94, p-value for trend = 0.018) gastric cancer cases. A borderline significant trend between GC risk and total ethanol intake was observed (p = 0.068). Consumption of beer and distilled alcoholic beverages including brandy, rum, and tequila was not associated with GC risk. The results indicate the need to focus on the study of the potential effects of different types of wine, with emphasis on components other than ethanol regarding the incidence of gastric cancer, even among populations with moderate to low levels of alcohol consumption.Para determinar o papel do consumo de bebidas alcoólicas, incluindo tequila, sobre incidência de câncer gástrico (CG), foi realizado um estudo caso-controle de base populacional na Cidade do México, com 220 casos de CG e 752 controles. Accumulating evidence has implicated Helicobacter pylori, an established cause of chronic gastritis and peptic ulcer, in the etiology of gastric cancer. Control of this infection would reduce the occurrence of chronic gastritis and peptic ulcer and might substantially lower the risk of stomach cancer as well. The public health impact of this infectious agent warrants efforts to identify preventive measures. This paper reviews the evidence linking H. pylori infection to gastric cancer and evaluates the potential for control in high-risk populations. Current obstacles to H. pylori control are discussed, including the link to poor socioeconomic conditions, difficulty in identifying incident cases, lack of natural immunity to reinfection, limited effectiveness of antibiotic therapy in high-prevalence populations, and incomplete knowledge regarding the reservoir of infection, mode of transmission, host susceptibility factors, and the potential for developing an effective vaccine. Worthwhile avenues of research include studies designed to identify modifiable risk factors for acquisition of the infection, modifiable host factors that may increase resistance to chronic infection, more effective antibiotic therapies, and effective vaccines.Um acúmulo de evidências aponta para Helicobacter pylori, uma causa comprovada de gastrite e úlcera péptica, na etiologia do câncer gástrico. O controle dessa infecção poderia reduzir a ocorrência de gastrite crônica e úlcera péptica, além de diminuir o risco de câncer de estômago. O impacto desse agente infeccioso em nível de saúde pública justifica os esforços no sentido de identificar medidas preventivas. Este artigo revê as evidências ligando o H. pylori ao câncer de estômago e avalia o potencial para controle em populações de alto risco. A autora discute os obstáculos atuais ao controle do H. pylori, inclusive sua associação com condições suir o risco de câncer de estômago. O impacto desse agente infecciodentes, a falta de imunidade natural à reinfecção, a eficácia limitada da antibioticoterapia em populações com alta prevalência e o conhecimento incompleto sobre o reservatão com condições suir o risco de câncer de estômago. O impacto desse agente infecciopedeiro e possibilidade de desenvolver uma vacina eficaz. Linhas de pesquisa prioritárias incluem estudos projetados para identificar fatores de risco modificáveis para a aquisição da infecção, fatores modificáveis no hospedeiro que possam aumentar a resistência à infecção, antibioticoterapias e vacinas mais eficazes.

A amamentação é fundamental para o crescimento e desenvolvimento da criança. No Brasil, a duração mediana da amamentação é muito baixa; por isso, no início da década de 80, foram implementadas campanhas nacionais de estímulo ao aleitamento materno. O presente artigo teve como objetivo avaliar as mudanças na duração da amamentação na cidade de Pelotas, Rio Grande do Sul, entre os anos de 1982 e 1993. Foram avaliados todos os nascimentos hospitalares ocorridos em ambos os anos, e amostras sistemáticas dessas crianças foram visitadas com cerca de 12 meses de idade. Observou-se um aumento na duração mediana da amamentação de 3,1 meses em 1982 para 4,0 meses em 1993. Este aumento suporta um possível impacto das campanhas nacionais ocorridas durante a década. Em ambas as coortes, houve interação entre a renda familiar e os percentuais de crianças amamentadas em diferentes idades. Nos primeiros meses, estes percentuais foram superiores nas famílias de maior renda, mas, a partir de nove meses, a amamentação foi mais freqüente no grupo de baixa renda. Crianças com baixo peso ao nascer apresentaram durações mais curtas da amamentação. Apesar dos progressos observados, a duração do aleitamento permanece bastante inferior ao recomendado internacionalmente, salientando a necessidade de campanhas futuras que priorizem crianças com baixo peso ao nascer e pertencentes às famílias de baixa renda.Breastfeeding is fundamental for infant health. However, its median duration in Brazil is very short, and national campaigns since the mid-1980s have attempted to revert this situation. In the present paper, data on breastfeeding are compared for two population-based cohorts of children born in 1982 and 1993 in the city of Pelotas in Southern Brazil. All hospital births in both years were studied and samples of these children were visited at home around 12 months of age. Median duration of breastfeeding increased from 3.1 to 4.0 months in this period. This rise suggests an impact by the national campaigns. In both cohorts, there were interactions between family income and the percentages of children breastfed at different ages. In the early months, breastfeeding was more prevalent in high-income families, but from nine months onwards it was more common among the poor. Low birthweight babies enjoyed shorter duration of breastfeeding. Despite the progress observed during the decade, duration of breastfeeding is still far short of international recommendations, justifying further campaigns prioritizing low birthweight babies and those from low-income families.

Mudanças ocorridas na última década em termos econômicos e assistenciais podem haver afetado a saúde materno-infantil. Dois estudos foram realizados em Pelotas, Rio Grande do Sul. As coortes de mães e recém-nascidos nos anos de 1982 e 1993 foram estudadas desde o nascimento. As mães foram identificadas nos hospitais-maternidade e responderam a um questionário padronizado, sendo seus filhos examinados. Mais de 99% dos recém-nascidos foram incluídos nas coortes, totalizando 5.914 nascidos vivos em 1982 e 5.249 em 1993. A mortalidade das crianças foi monitorizada, e em 1993 as hospitalizações também o foram. Em 1982, tentou-se localizar cerca de 25% das crianças aos 12 meses de idade e todas com a idade média de vinte meses. Foi possível encontrar cerca de 82% das crianças aos 12 meses e, graças a uma mudança de estratégia, 87% aos vinte meses. Em 1993, tentou-se acompanhar 20% das crianças e mais todos os recém-nascidos de baixo peso aos 12 meses de idade, sendo 95% localizados. Este artigo descreve os principais aspectos metodológicos de ambos os estudos, cujos principais resultados estão incluídos nos próximos artigos desse suplemento.Important changes have taken place in both the economy and health care in Brazil since 1980 and may have affected maternal and child health. Two studies in the city of Pelotas in Southern Brazil provide a unique opportunity for assessing these issues. Cohorts of mothers and infants from 1982 and 1993 were studied from the time of delivery. In both years, all mothers identified in the city's maternity hospitals answered a standardized questionnaire, and their infants were examined. Over 99% of all children born in the city in each of the two years were included in the cohorts, totalling 5,914 live births in 1982 and 5,249 in 1993. Deaths among these children were monitored prospectively, as were all hospital admissions in the 1993 cohort. In 1982, attempts were made to locate a 25% sample of the children at the mean age of 12 months, as well as all of the cohort children at the mean age of 20 months; 82% of the former were located, and thanks to a change in strategy, 87% were traced at 20 months. In 1993, a search was made at 12 months of age for 20% of all children plus all low birthweight infants, and 95% were successfully traced. This paper presents the main methodological aspects of both cohort studies, the principal results of which are presented in the subsequent articles in this supplement.

Este artigo propõe uma estratégia de definição de indicadores para avaliar a qualidade do processo de assistência, baseada na identificação dos objetivos dessa assistência e na verificação da existência de normas técnicas que a norteiem. Simultaneamente, aplica esta metodologia ao processo de assistência ao nascimento e parto, adotando enfoque de risco, tanto para a saúde individual, como para a coletiva. Utiliza para tal um banco de dados de 4.558 Histórias Clínicas Perinatais Simplificadas de 12 hospitais. Os indicadores selecionados foram: sorologia para sífilis, tipagem sanguínea Rh, vacinação antitetânica, taxa de cesáreas, recepção do bebê por pediatra na sala de partos, boletim de Apgar, avaliação de idade gestacional por exame físico, alojamento conjunto, aleitamento materno exclusivo na alta, encaminhamento para consulta de puerpério. Para classificação dos hospitais, foram utilizadas a soma simples, padrão-ouro, e hierarquização. A metodologia proposta permitiu identificar um hospital com qualidade excelente, cinco com qualidade boa, dois com qualidade regular e quatro com desempenho completamente insatisfatório.This paper proposes a strategy for defining indicators to assess the quality of the process of health care, based on the identification of the objectives of the care being delivered and on the verification of whether the technical guidelines for the procedures are being enforced. The article goes on to apply the proposed methodology to the process of health care during childbirth, based on a perspective of comprehensive care and adopting both an individual and collective risk approach. It uses a data bank of 4,558 Simplified Perinatal Clinical Histories (CLAP/PAHO/WHO) from 12 hospitals.The indicators selected were: V.D.R.L., Rh blood typing, antitetanus vaccination, rate of cesarean sections, reception of the baby by a pediatrician at birth, Apgar score, evaluation of the gestational age by physical exam, rooming-in, exclusive breastfeeding upon discharge, and referral for a puerperal consultation. Methods for classification of hospitals were the sum of their scores for each indicator, gold standard, and ranking. This methodology warranted identification of one hospital with excellent quality of care, five with good care, two with fair care, and four whose performances were completely unsatisfactory. Finally, some remarks on the assessment of quality of health care are made and future developments are proposed.
