Newborn screening (NBS) for inherited and congenital disorders is a form of secondary disease prevention and a public health responsibility. The development of NBS programs is one of the most important achievements in health care. While the scope of the screening targets has expanded and methods have evolved, the screening process has remained essentially unchanged.In 2006, the Canadian province of Ontario implemented a new provincial NBS program that differs from conventional programs. One of the key differences is the structured and fully funded partnership between the ministry of health, the NBS laboratory, and the treatment centers in the province. Each one of these partners has defined roles and accountability. Another difference is the move away from the conventional recall process to an immediate referral system to selected treatment centers.In this article, we report our experience with the new screening structure and discuss it as a model for future NBS programs., (© 2023. An evidence-based approach is considered a central requirement in the public health system. Five general principles apply in evidence-based medicine as well as in evidence-based public health (EBPH). These comprise a systematic approach, transparency in relation to uncertainty, integration and participation, management of conflicts of interest, and a structured, reflective process. This article aims to promote a common understanding of what taking an evidence-based approach entails in the field of public health.We explored the international understanding of EBPH through systematic literature searches of definitions, concepts, and models regarding an evidence-based approach in public health. In this manner, 20 English sources and one German source were identified and examined using content analysis. Beyond the above-described general principles, characteristics of these perspectives are concerned with a population orientation, the underlying concept of evidence, disciplines, and stakeholder groups to be involved in the process, as well as relevant criteria and necessary competencies for implementing an evidence-based approach.Based on this, and taking into account methodological developments to address complexity, four specific implementation factors of EBPH can be derived. These include theory, interdisciplinarity, context-sensitivity, and complexity as well as general societal aspects.The practical implementation of EBPH requires human and financial resources as well as competencies - among others, for conducting systematic reviews of the effectiveness of measures, examining other relevant questions in a scientific manner, and establishing transparent processes to formulate recommendations.

In 1986, the Ottawa charter marked a paradigm shift for public health, putting the focus on strengthening community action and on creating supportive environments for health. A key to this is "capacity building" (CB), which we understand as the development and sustainable implementation of structural capacities, e.g. coordinated data collection, collaboration processes across sectors and reliable provision of basic resources in all areas of local health promotion.Many efforts and three and a half decades later we still envisage infrastructure deficits, scattered public health landscapes and restraints to intersectoral cooperation much too often. While agreement on the theoretical insights on what is needed appears to be broad, translating these insights into practice remains a challenge. In this situation, digital public health (DPH) can contribute to overcoming barriers and making knowledge for action more visible and more accessible. With DPH, data can be integrated, structured and disseminated in novel ways.We discuss why CB at the local level could benefit from technological advances and what DPH might do for the provision of information services on public health capacity. Our focus is on the web-based, interactive representation of public health data for use in information, governance or benchmarking processes. As an example from public health practice, the Finnish tool TEAviisari (National Institute for Health and Welfare, Finland) is presented.The 2020 EU Council Presidency of Germany - with the topics of digitalisation and the common European health data space - offers opportunities to decisively advance the development of CB in health promotion in this country.

The nexus between urban planning and public health acknowledges the importance of creating cities that contribute to their residents' physical, mental, and social well-being. The Healthy Cities movement underlines that community participation and intersectoral work are important to create sustainable, equitable, and healthy cities.Several theoretical and practical participatory approaches form the foundation for participation in public health and urban planning. Growing digitalization has significantly transformed how participation is conducted in various fields. Digital technologies not only play a large role in daily life, but they have opened more opportunities for individuals to interact, share, and collaborate in the planning and design of cities.This article explores how digital technologies enable participation among residents and stakeholders in order to support the health-oriented planning of cities and neighborhoods. From the selective case studies presented in the paper, it can be ascertained that digital technologies can support various forms of participation by enabling different levels of engagement as well as both one-way and two-way interactions. Some forms of engagement can be supported entirely within digital platforms. However, in the case of higher engagement, which requires deeper insights into the problems and the codevelopment of solutions, other nondigital formats and traditional methods such as follow-up workshops and focus group discussions are necessary to complement the digital form of participation., (© 2024. Population health monitoring and reporting provides regular and up-to-date information on health outcomes and its determinants. The outputs of population health monitoring aim to inform policymakers and other stakeholders to develop fact-based decisions. Population health monitoring is located at the national, but also federal state and county level. This paper describes the legal basis for population health monitoring in the federal states' public health acts as well as current challenges and possibilities for further development from a federal state and county perspective on population health monitoring.A legal basis for population health monitoring on the federal state and county level exists for almost all federal states in Germany. The level of detail of these laws varies in terms of responsibility, periodicity of population health monitoring and reporting, content, and designated use. Population health monitoring needs to respond to challenges in the areas of population health monitoring resources, data sources, (intersectoral) reporting, and impact. Practical examples illustrate how the challenges can be handled and further development can take place.Based on its solid legal basis, being a routine task, and its close link to the living conditions of the population, population health monitoring on the federal state and county level has the possibility for cocreating public health at the grassroots level and to be a pioneer for health equity.

Background: Population health monitoring, the regular and institutionalized production and dissemination of information and knowledge about the health status of a population, is an essential element of public health. Nevertheless, while epidemiology and biostatistics, for example, are well-recognized disciplines, this does not (yet) apply to population health monitoring. Over the past decade, however, it has matured as a distinct field of expertise., Objectives: This paper presents a comprehensive model for population health monitoring and describes its current status as a field of expertise. It concludes with an overview of the most important developments that are likely to shape the health information systems and population health monitoring practices of the future., Results and Conclusions: Combining the information pyramid (an application of the data-information-knowledge-wisdom hierarchy), describing outputs, and a so-called monitoring chain, describing activities, results in a comprehensive model for population health monitoring. The steps of the activity chain can be viewed as a stairway by which the information pyramid is climbed, reaching evidence-informed policymaking at the top. Population health monitoring has several inherent strengths, such as its high societal relevance; its integrative, comprehensive, and structured approach; and the fact that it makes use of routinely collected data. In practice, however, secondary use of routine data is often hampered by technical, motivational, economic, political, ethical, and legal barriers. Important developments that will shape health information systems and population health monitoring practices of the future include digitalization and data-driven technology, citizen science, and the growing need for intersectoral approaches. Population health monitoring practice will need to adapt in order to counteract the risks and reap the benefits that these developments hold.

The rapid development and proliferation of digital health technologies have not only changed the medical professions, but offer great potential for public health, particularly in health promotion and disease prevention.At the same time, this emerging field is also characterized by conceptual and terminological fuzziness, a marked lack of high-quality evidence, and an absence of an honest discussion of unintended consequences and side effects. Further challenges for digital public health lie in the fact that the development of new health technologies is mainly driven by technological progress and less by evidence-based needs and research in public health.In this overview paper, we aim at conceptually denoting the field of digital public health, using principal public health functions as guiding principles. We discuss some current applications of digital health technologies in fulfilling public health functions and propose a needs-based development of digital health technologies.We will further address specific challenges to digital public health, in particular socio-economic differences in the usage of and profiting from digital health technologies, data protection and privacy issues, as well as ethical issues.

Digital public health promises not only more comprehensive medical care, but also individual health promotion and support for positive lifestyle changes. Mobile digital health devices and services, also called mobile health (mHealth), play a key role in this. They include health-specific hardware and software applications such as smartphone apps and wearable technology for recording, monitoring, and evaluating specific health parameters. Although there is scientific evidence for the effectiveness of individual applications, most often applications are used for a relatively short amount of time. In order to achieve a higher acceptance and utilization rate, evidence is needed that is more practice oriented.This paper explains how participatory development approaches take into account the individual needs and preferences of users and can improve the quality and effectiveness of mHealth services. The sociodemographic characteristics of the target group as well as individual, social, linguistic, and cultural barriers should be considered. The wishes of users, for example personalization, transmission of real-time information, and transparency in terms of privacy should also be considered. In the co-design approach, users are therefore included directly in the product concept. However, the study situation is still limited and there are no methodical approaches.In order to increase the use of mHealth services in the future, participation processes should be systematized. In addition, a framework for classification and certification as well as procedures for promoting effective applications should be developed.

The discussion of digital health technologies, in particular medical and health apps, is currently dominated by a focus on their potential to reach large parts of the population for the dissemination of evidence-based health promotion and prevention content. However, potentially unintended consequences, side effects, and negative effects of digital health technologies are rarely discussed in public health.In this paper, via a narrative literature review, we propose a perspective on unintended consequences and side-effects of digital health technologies on multiple hierarchical levels of a socio-ecological model of health. Unintended consequences and side-effects of digital health technologies can be identified on an individual level, a level of social relationships, and a health services level.We propose a broader conceptualization of unintended consequences and side-effects of digital health technology together with a more thorough documentation of such effects using multiple levels in a socio-ecological approach. This would build a cumulative evidence base of unintended effects and shift the focus from development-centered discussion of risks and challenges to a comprehensive conception of side effects and undesirable effects of digital health technologies. The proposed division into three effect levels may be helpful here.

For materials and articles intended to come in contact with food - particularly for substances from plastics (e.g., monomers, additives) - there are comprehensive European regulations based on toxicological evaluations. Other food contact materials, e.g., paper and rubber, are covered by resolutions established by the European Council without additional evaluations, while in Germany substances from these materials are toxicologically evaluated by the Federal Institute for Risk Assessment (BfR) and are included in the "BfR recommendations". Some further areas of food contact materials are not regulated by European or by national regulations (e.g., substances from printing inks). Recently, new European regulations for recycled plastics intended for food contact and for active and intelligent materials and articles have been released. Future challenges are the risk assessment of contaminants and reaction products from the production of food contact substances, which may migrate into the food in only tiny amounts, and of complex mixtures of substances, e.g., of hormone-like substances (endocrine disruptors).

In 2008 the forum gesundheitsziele.de selected "Healthy aging" as a new health target. Besides "Healthy aging" three other targets were considered: chronic back pain, myocardial infarction, and vaccination. This paper describes the selection process for all four health targets which followed several criteria: mortality, morbidity, prevalence, burden of disease, economic impact, potential for improvement, equity in health, empowerment of and priorities in the population. The analysis particularly focused on assessment of feasibility and readiness of stakeholders to participate in the development and implementation of the health target.

Ethical issues have always been part of the debate around work and its relation to people. In the context of workplace health management, however, ethical considerations have received little attention to date. This paper discusses some examples of the ethical questions arising in the pursuit of workplace health management, such as issues around the organization of work procedures, double loyalties, the significance of occupational screening examinations or how people in precarious working conditions are being dealt with. Subsequently, two ethical codes commonly used in the field of work and health in German-speaking countries are introduced. They originate from the field of occupational medicine, but have meanwhile been opened to other professions in the field of work and health. Finally, some perspectives for further discussion are put forward.

Until now there are only a few approaches in the German-speaking realm to establish an explicit ethical framework for moral issues of public health--although a need for public health ethics in times of SARS and avian flu is obvious. One deficit of the discussion so far is that there is no systematic separation of medical ethics and public health ethics. Thus, the core and interdisciplinary focus of public health is often not met. However, to frame discussions of moral issues within a specific public health ethics framework seems to be fruitful. This paper deals with the conceptual differences of medical ethics and public health ethics. The discussion helps both applied ethical discourses to sharpen their focus and strengthen their appeal. The author develops and presents a conceptual and normative frame for public health ethics and offers a concise set of ethical principles for the discussion of moral challenges in public health.

Almost unnoticed by the general public, but also to some extent by public health professionals, psychiatric care in Germany has recently undergone considerable change. This development could be considered as more revolutionary than changes seen in somatic medicine. These changes can also be found in progress in "psychiatry as a science"in areas as diverse as prevention, diagnosis and treatment of those with mental illness, and their rehabilitation, recovery and after-care. Public health professionals have long witnessed these changes which have fluctuated in intention and intensity. This paper reviews the development of present-day responsibilities of the Public Health Service in psychiatric care and underlines the need for reform presenting an implementation plan and an outline of possible structural changes.

Clinical trials only allow for a limited assessment of adverse drug reactions (ADRs) due to their restricted number of patients, their short duration, and their narrow inclusion criteria. To assess potential ADRs in the population actually treated after drug approval, different systems for signal generation such as spontaneous reporting systems have been established and are required by drug laws in many countries. As of today epidemiological methods for signal confirmation for rare adverse events or events with long induction periods use record linkage databases such as the United Health Care (Ingenix) in the USA or the practice databases such as the General Practice Research Database in Great Britain. However, this approach also has its limitations. This type of database will be presented in this paper. In addition, databases operated by German public health insurance will be described. The topic of linking different data sources in Germany to support pharmaco-vigilance activities will be addressed.

Since January 2003, the new German Drinking Water Ordinance (DWO) has become operative. This paper briefly reviews some major consequences for hospitals. One of the main topics is the increased responsibility of operators of drinking water installation systems in hospitals concerning both the maintenance and control of good drinking water quality. Besides harmful chemical parameters, proliferation of pathogens can occur such as Pseudomonas aeruginosa, Legionella spec., Acinetobacter, and others which are mainly bound to biofilms and thus less affected by disinfectants. Recent epidemiological investigations point out that the relevance of waterborne pathogens is still considerably underestimated, particularly in hospitals. Local public health authorities play a central role in clinic inspection, supervision of water installations, surveillance, and risk assessment in cases of noncompliance with DWO requirements. For this reason, every irregularity detected must be reported to the local public health authority.
