During the first 12 months of a total population survey 249 patients were seen with viral hepatitis. A total of 215 of these were tested for hepatitis B antigen (HB Ag) by radioimmunoassay and 32 (15%) were positive.More than five times as many men (27) as women (5) were HBAg positive and 19 of the men were between the ages of 20 and 39 years. There were only four drug addicts among those tested, two of whom were positive, as were two of the four patients who were tattooed.Sixty out of 86 children (under 15 years) were tested for HBAg and none was positive.

A cost-benefit analysis of long-term maintenance haemodialysis indicates that there is a large gulf between the cost of the service and "economic" benefit. The difference may be considered to represent one estimate of the price society is prepared to pay to maintain life. Using "best estimates" from available data we found the implicit social value of maintaining a patient on haemodialysis to be approximately pounds 4720 per annum in hospital or pounds 2600 at home. The analysis would suggest that society must look carefully at alternative uses for health expenditure before extending indiscriminately to large sections of the population these treatment programmes or other similarly expensive.

Viral hepatitis in young adults in Accra, Ghana, is associated with Australia antigen (H.A.A.). Sera from 85 patients in hospital with viral hepatitis were available for determinations of H.A.A. Of the 16 patients whose serum was obtained within the first week of symptoms, 15 were positive. The only factor related to finding H.A.A. was the time between onset of symptoms and the collection of the serum sample. Persistence of H.A.A. was associated with persistence of jaundice in men but not in women. Previous epidemiological studies in Accra found no evidence for parenteral transmission of viral hepatitis and showed a shanty-town predilection pointing to faecal-oral transmission. It thus seems that H.A.A.-associated hepatitis is transmitted in West Africa either faecal-orally or by shanty-town associated arthropods. The finding that H.A.A. hepatitis is the usual hepatitis in young adults in Accra is in accord with the high prevalence of H.A.A. elsewhere in the general population in Africa and may be related to the high rate of cirrhosis and hepatoma in Africa.

Two patients with primary hepatic malignancy were treated by hepatectomy and orthotopic liver transplantation. In both cases the donor liver was infused with cold solutions and kept chilled without continuous perfusion. There was immediate satisfactory hepatic function in both transplants.The first patient died after 11 weeks from overwhelming bacterial and fungal infections probably secondary to hepatic infarction due to thrombosis of the recipient hepatic artery. The thrombus occurred at the site of the arterial clamp. In an attempt to control the growth before transplantation, the patient had been treated with large doses of chlorambucil, which resulted in extreme marrow depression and septicaemia.The second patient developed cholestatic jaundice during the second and third weeks after transplantation, with histological evidence of mild rejection, which was controlled by increasing the dose of immunosuppressive agents. He is now well, having returned to work six weeks after the operation.Though the first patient showed no evidence of rejection, it is concluded that patients receiving liver allografts should receive immunosuppressive therapy.

THREE ASPECTS OF HAEMODIALYSIS ARE OF SPECIAL CONCERN TO THE MICROBIOLOGIST: (1) the hepatitis risk, (2) shunt sepsis, and (3) the hygiene of the equipment used. It is suggested that the risks of infection and cross-infection in haemodialysis units may be diminished by several measures, including the avoidance of overcrowding the patients, setting up codes of practice for the staff, topical disinfection of the patient's skin, and sterilization of equipment. In addition pathologists should emphasize to laboratory staff dealing with specimens from patients that these carry major risks to health.
