Abstract

Aims: Controversy exists about the survival benefits of breast cancer screening but SDBC now constitute 60% of cancers in women aged 50-65 years. The aim of this study was to compare SDBC and symptomatic breast cancer survival in women aged 50-65 years. Methods: From 1990-98, in one unit, 1607 breast cancers (962 SDBC and 645 symptomatic) were treated in the age group 50-65 years. Clinico-pathological data were compared for risk of recurrence and cancer-specific mortality in each of the sub-groups. Median follow-up was 60 months (range 40-64). Results: Smaller turnouts (P < 0·001) and higher node negativity (P < 0·001) occurred in SDBCs, but tumour grade did not differ. Better survival occurred in SDBCs except when tumour size was > 2·5 cm. See Table below. Abstract

Aims: Meta-analysis of symptomatic breast cancer trials recommends adjuvant chemotherapy for women under 70 years, at high risk of recurrence. Identification of women needing chemotherapy is required to avoid treatment morbidity in SDBC. Methods: To determine women with SDBC undergoing surgery, at high risk of recurrence, we produced a simple index based on tumour grade scored (1, 2 or 3), size (< 1·5 cm = 1, 1·5-2·5 = 2, > 2·5 = 3) and node status (negative = 1, < 4 nodes = 2, ≥ 4 nodes = 3). Scores for each parameter were added producing a range from 3-9. Median follow-up was 60 months (range 40-64). Results: Between 1990-98, 962 SDBCs were diagnosed in one unit and overall 5 year survival was 96·3%. Regardless of grade and node status only 10/587 (1·7%) of turnouts < 1·5 cm had recurred. Overall 90% of SDBCs had scores 3-6 with 5 year survival of 98·7%. This fell to 53·3% in SDBCs with score 9. Index scores identified women (scores 7-9) at high risk of recurrence (P[sup *] < 0·001)and mortality (P[sup *] < 0·001). [sup *]χ² test for trend. 10. Does radiotherapy (DXT) affect survival after breast conserving surgery in elderly patients?

Abstract

Aims: Elderly women are less likely to be offered DXT after breast cancer conservation surgery, (BCS). To determine the effect of radiotherapy on survival after BCS we studied women ≥ 65 years who were treated on our unit between 1976-2000. Methods: Treatment of breast cancer in women ≥ 65 years of age was prospectively recorded from 1976 (2396 women) of whom 29% underwent BCS. Frequency of postoperative DXT use, recurrence and overall survival were studied. Log rank analysis of recurrence and survival was performed. Results: Out of 690 women, 198 (29%) had DXT. Use of DXT after BCS increased from 18% (29/162) in 1981-1985 to 64% (65/102) in 1996-2000 (P ≤ 0·001). DXT use decreased with increasing age (P ≤ 0·001). Failure to give DXT led to an increased recurrence and mortality (P ≤ 0·001). Breast cancer mortality after DXT was 13% (26/198) compared with 39·4% (145/492) in the patients who did not receive DXT. This effect of DXT on reducing breast cancer mortality occurred at all tumour sizes. Conclusion: Failure to give DXT after BCS (regardless of co-morbidity) affects survival frown breast cancer.
