Objective: To identify techniques commonly used in the management of periradicular disease by general dental practitioners and to ascertain views on continuing professional education in endodontics.Design: Data was collected via a postal questionnaire distributed to 617 general dental practitioners in Scotland (33% of practitioners registered with the Dental Practice Board).Results: 417 (69%) questionnaires were completed and returned. Only 24.9% of respondents used rubber dam routinely. The majority of respondents used hand instruments for preparation with either sodium hypochlorite or local anaesthetic being used most frequently for irrigation. The mean time for treatment of a single rooted tooth was 71 minutes. The demand for continuing education courses was high with 340 and 197 respondents requesting endodontic and rubber dam courses, respectively.Conclusions: Traditional methods for preparing the root canal using hand instruments were favoured by most dentists. The major disincentive to the use of rotary instruments and new techniques for obturating with thermally softened gutta-percha was expense. Objective: To identify factors influencing the diagnosis and management of periradicular disease by general dental practitioners.Design: The study was conducted in two stages. 1. Analysis of recent returns to the Scottish Dental Practice Board. 2. Data collection via a postal questionnaire distributed to 617 general dental practitioners in Scotland (33% of practitioners registered with the Dental Practice Board).Results: 417 (69%) questionnaires were completed and returned. The majority of respondents undertook root canal treatment and the number of cases treated had increased in the last five years. The vast majority of the respondents (89.3%) were confident in their diagnosis of periradicular disease and 77.1% were confident of their treatment of the disease. A referral system for treatment was used by 31% of respondents of whom the majority used a specialist in a hospital. Over 50% of the respondents undertook surgical root canal treatment. Only 40% of respondents followed up their completed cases for longer than six months. Constraints on the provision of treatment included the time available and the low level of fees.Conclusions: Current arrangements for the treatment of periradicular disease in general dental practice are less than optimal. The fiscal arrangements for the provision of these treatments must be developed to encourage a high standard of treatment to be performed thereby maximising the likelihood of success. Cosmetic dentistry has become increasingly popular, largely as a result of social trends and increased media coverage. This understandable desire for the alleged 'perfect smile' needs to be tempered with an appropriate awareness of the significant risks associated with invasive cosmetic procedures such as veneers and crowns. Patients need to be properly informed that elective removal of healthy enamel and dentine can result in pulpal injury and poorer periodontal health in the longer term, particularly if they are young. The duty of candour means that they ought to be informed that aggressive reduction of sound tooth tissue is not biologically neutral and results in structural weakening of their teeth. Less invasive procedures such as bleaching on its own or for example, combined with direct resin composite bonding, can satisfy many patient's demands, while still being kinder to teeth and having much better fall-back positions for their future requirements. It is the opinion of the British Endodontic Society, British Society for Restorative Dentistry, Restorative Dentistry UK, Dental Trauma UK, British Society of Prosthodontics and the British Society of Paediatric Dentistry that elective invasive cosmetic dental treatments can result in great benefit to patients, but that some aggressive treatments used to achieve them can produce significant morbidities in teeth which were previously healthy. This is a worrying and growing problem with many ethical, legal and biologic aspects, but many adverse outcomes for patients who request cosmetic dental improvements are preventable by using biologically safer initial approaches to treatment planning and its provision. When considering endodontically treated teeth, the quality of the restoration is important from the outset. It sheds light into possible causes of pulp necrosis or failure of endodontic treatment and influences the outcome of future endodontic treatment. A tooth undergoing endodontic treatment requires an effective coronal seal during and following completion of endodontic treatment. The role of periodontitis as a risk factor for multiple systemic diseases is widely accepted and there is growing evidence of an association between periodontitis and sporadic late onset Alzheimer's disease (SLOAD). Recent epidemiologic, microbiologic and inflammatory findings strengthen this association, indicating that periodontal pathogens are possible contributors to neural inflammation and SLOAD. One of the most challenging treatment planning decisions in restorative dentistry is the retention of a root filled tooth or its extraction and replacement with an implant. Making an informed judgement relies upon not only clinical experience and expertise but also interpretation of the published clinical evidence. In the last few years, more robust data and closer scrutiny of the evidence has helped to understand the advantages and disadvantages of each of these treatment modalities. The purpose of this paper is to provide a narrative review of some of the factors that may influence treatment planning for root canal treated teeth and indications for whether these teeth would be better replaced by an implant. From the evidence presented it is clear that both treatments have a place in contemporary restorative dental treatment and that survival for both root filled teeth and implants are similar. The speciality of endodontics encompasses numerous elements, but the use of a variety of instruments to shape root canal systems probably challenges the clinical time of practitioners more than any other. The restoration of endodontically treated teeth has undergone significant changes in the last 20 years. Most of these changes are associated with the preservation of tooth structure, this has been achieved first of all with the increasing use of operative microscopes, nickel titanium instruments and more recently cone beam computed tomography; these instruments have allowed the clinicians to reduce significantly the amount of coronal and radicular hard tooth tissue removed in the process of cutting access cavities. The use of composites has also allowed the clinicians to restore with adhesive techniques teeth that would otherwise require extensive and destructive mechanical retentions. The use of partial crowns is becoming increasingly popular and this also helps prevent tooth structure loss. This article will focus on the choices available to restore both anterior and posterior teeth and will focus more on these contemporary adhesive techniques. [ABSTRACT FROM AUTHOR]

This article will provide an overview of the rationale behind taking a contemporary approach to obturation of the root canal system. It will outline a broad range of obturation materials including those traditionally used and newer, advanced ones that are now available that have active physical and biological properties. Obturation techniques will be described in detail, including advice for those difficult cases such as managing obturation of a canal with a wide open apex. Provision of indirect restorations is associated with more complaints and litigation than any other area of dentistry. In the third part of this series we identify possible causes of such complaints. We explore each of the stages from clinical assessment to treatment, highlighting areas which may trigger litigation when they are not addressed carefully – particularly in relation to cosmetic dentistry. The article is illustrated with clinical examples drawn from the authors' personal experience. Aim The purpose of this study was to gather information from across the EU about requirements of dental undergraduates in the area of root canal treatment (RCT).Methodology One hundred and ninety-two EU dental schools were invited to complete an online survey. The survey inquired about assessment in RCT, whether students complete a minimum number of RCTs, whether this includes both anterior and posterior teeth, and if root canal retreatment and endodontic surgery are compulsory. The final question asked how competence in RCT can be described.Results Forty-eight dental schools in 20 countries responded. Results showed variation in assessment methods. Ninety-four percent insist on students performing RCT in anterior and posterior teeth. Eighty-one percent ensure students perform a minimum number and there is considerable variation in what that minimum number is. Seventy-nine percent require undergraduates to perform retreatment and 4% require them to perform endodontic surgery. Descriptions of what might constitute competence in RCT varied greatly.Conclusions Despite convergence of the undergraduate curriculum being endorsed by many and movement of dentists within the EU escalating, there is still inconsistency in the output of dental schools with regard to RCT. Aim This paper reports a prospective evaluation of 59 dentine-bonded crowns placed in a dental school environment for patients, a majority of whom were suffering from tooth substance loss.Results Forty-eight crowns (83%) were available for examination, with their mean age since placement being 3.9 years. The mean age of the patients in whom the crowns were placed was 37.5 years. Three crowns had failed due to porcelain fractures, an overall failure rate of 6%. Two of the failures were minimal cracks of which the patient was unaware, and one, in an upper premolar tooth, because of crown fracture. No secondary caries was noted, incidence of pulp symptoms or pulp death was nil, and margins were rated as 'excellent' in 42 crowns (86%).Conclusion The dentine bonded crowns assessed in this study showed excellent retention and low incidence of fracture at four years. This technique would appear to be suitable for a variety of clinical indications, including treatment of tooth substance loss, although the results presented in this study are relatively short term in relation to the anticipated life of restorations. The Dahl appliance was described nearly 30 years ago. This removable metal bite platform was used to create inter-occlusal space, in a localised part of the mouth, to facilitate the placement of restorations on worn anterior teeth. The Dahl concept is traditionally associated with the management of worn teeth. However, the same principles can be successfully and safely applied to a variety of clinical situations. This has simplified the management of historically complex problems. The advantages are the preservation of tooth tissue and the long-term benefits that brings. This paper reviews the literature related to the Dahl concept and how the concept has developed. There is a discussion regarding possible future applications and research. [ABSTRACT FROM AUTHOR]

Aim The aim of this in vitro study was to compare the marginal adaptation achieved by two obturation techniques (lateral condensation and Thermafil) using human teeth prepared by continuous rotation with the HERO 642® system.Method The percentages of gaps and sealer on the root canal surface were determined by analysing the images of 12 sections per tooth. Tubule sealer penetration was assessed by backscattered scanning electron microscopy and energy dispersive spectrometry microanalysis.Results The Thermafil obturation technique resulted in virtually no gaps and very low amounts of sealer on the root surface, unlike the lateral condensation technique. Tubule sealer penetration occurred with both techniques, but was deeper, especially in the mid and apical zones, with the lateral condensation technique. [ABSTRACT FROM AUTHOR]

Failure of root canal treatment and/or post crowns can be avoided in many cases if appropriate tooth preparation is carried out. This paper discusses the rationale for the timing of post placement following root canal treatment and appropriate methods for removal of gutta-percha prior to post space preparation. The basic principles of post space preparation are described, which should reduce the risk of weakening the root unnecessarily, causing damage to the periodontium and post perforation. This is the first in a series of four papers related to the management of root canal treated teeth. When teeth compromised by extensive restorations become non-vital, suggestions have been given as to how root canal treatment can be carried out with the greatest chance of success. Once root canal treated, either by a previous dentist or by the current dentist, a review of the assessment process that should be carried out prior to placing costly indirect definitive restorations is given. It will be clear that post-retained restorations are mainly reserved for anterior or single-rooted teeth, posterior teeth rarely requiring a post for core retention. The second paper in this series describes the basic tooth preparation that should be carried out prior to placing a post. Depending on the type of post system used, further modifications to tooth preparation may be required and the cementation techniques may also have to be modified. The third paper therefore discusses the various post types, when and how they should be used for optimum results. The final paper addresses reinforcement and restoration of compromised root canals, such as those with immature, open apices, or those that have been over-prepared for previous post-retained restorations. Objectives: To investigate, by questionnaire, the use and selection of materials and techniques for indirect restorations and fixed prosthodontics by dental practitioners in the North West of England and Scotland.Methods: A questionnaire was sent to 1,000 general dental practitioners selected at random from dentists in Scotland and the North West of England. Non-responders were sent another questionnaire after a period of 4 weeks had elapsed.Results: A total of 701 usable questionnaires were returned, giving a response rate of 70%. When selecting a material for the core build-up of vital teeth, practitioners used the following materials (%): amalgam (60), dual and light-cured resin composite (54), glass-ionomer cements (47), compomer (29) and resin-modified glass-ionomer cements (24). Where the use of a post was indicated indirect posts of both precious (67) and non-precious (37) alloys were preferred to prefabricated posts by the majority of practitioners for the restoration of root filled teeth. Direct titanium (14) and stainless steel (14) posts were not used extensively. Impression materials used by the practitioners were as follows: addition-cured silicone (70), condensation-cured silicone (20), polyether (9) and polysulphide (2). Traditional glass-ionomer cements (59) were used for the luting of single and multiple porcelain fused to metal units, with zinc phosphate and polycarboxylate cements (33) being the preferred alternatives.Conclusions: The majority of the practitioners surveyed in this study used: amalgam for core build-ups; indirect posts; addition-cured silicone for impressions; and glass-ionomer cements for luting procedures. The purpose of the obturation phase of a root filling is two-fold; to prevent microorganisms from re-entering the root canal system, and to isolate any microorganisms that may remain within the tooth from nutrients in tissue fluids. The seal at the apical end of the root canal is achieved by a well-fitting gutta-percha master point, and accessory points, although heated techniques may result in a better seal. The seal at the coronal end is achieved by the application of a layer of resin-modified glass ionomer cement as accessory canals may lead from the floor of the pulp chamber to the furcation area. Aim To determine the five-year success rates, site or sites of failure, prognostic indicators and lower lip morbidity associated with molar apicectomy using amalgam root-end filling.Design Multicentre, prospective study.Setting The departments of oral and maxillo-facial surgery in two district general hospitals.Method One thousand and seven molar apicectomy procedures, combined with amalgam root-end filling were expedited during the period 1974-1995. A five-year review of each operated tooth was carried out or attempted between 1979-2000.Results Of the 790 (78%) operated molars successfully reviewed at 5 years or later 451 (57%) exhibited 'complete healing' and 39 (5%) 'uncertain healing'. Three hundred (38%) were classified as 'unsatisfactory healing' (failures), and these included 12 which were assumed to be of periodontal origin. Whilst longitudinal root fracture, perforation and/or infection in the furcation, periodontal disease or a non-restorable crown accounted for treatment failure and often the need to remove teeth subsequently, the study probably pointed to the apical ends of the roots rather than the furcation as being the major sites at which 'unsatisfactory healing' occurred. Mandibular first molars attracted the highest 'complete healing' rate (60%) and mandibular second molars the lowest (46%). 'Good' root canal treatment (RCT) at the outset improved the prognosis of a root-end filling (REF) whilst the absence of RCT compromised it. Cystic change pointed to a better prognosis than apical granulomatous change as did a deep compared with a shallow 'bone cuff'. Disease at the furcation suggested a worse prognosis. Teeth which showed 'complete healing' at 1 year had a 75% probability of maintaining this outcome at 5 years. Sensory disturbance of variable duration occurred in the lower lip following 20-21% of mandibular molar procedures. In the majority of cases (79-80%) this had remitted within 3 months. A permanent deficit occurred in 8 patients (1%) where the apicectomy could definitely be incriminated as causative. Four were associated with first molar apicectomy and four with second molar apicectomy.Conclusions Molar apicectomy with amalgam root-end filling attracts an overall 'complete healing' rate at 5 years of 57%, the results being best with mandibular first molars and worst with mandibular second molars. The prognosis is also better where there is 'good' initial orthograde root filling, an associated radicular cyst as compared with granulomatous change and where the buccal sulcus is deep rather than shallow. It is worse when orthograde root filling is absent and when there is disease in the furcation. 'Complete healing' at 1 year can be expected to be maintained at 5 years in 75% of cases. The commonest site of subsequent periradicular rarefaction seems to be 'apical' whilst failure at the furcation is probably comparatively rare. There is a threefold increase in the occurrence of permanent lower lip sensory impairment following second molar surgery in comparison with first molar surgery, the overall incidence being 1%. Objective: To describe the knowledge and practice of general denta practitioners (GDPs) working in Liverpool (where there is no milk fluoridation programme) and St Helens and Knowsley, and the Wirral (where children have fluoridated milk in schools and pre-schools) relating to the advice given for child patients regarding the use of fluoridated toothpaste.Design: Data were collected via a postal questionnaire sent to all 329 GDPs working within the three areas. GDPs working in more than one of the areas and those working in specialist orthodontic or oral surgery practices were excluded.Results: Two hundred and thirty-four (71%) questionnaires were completed and returned. Only 3% of dentists said that no-one in their practice gave advice on the concentration of fluoride toothpaste to be used. For caries free children under 7 years of age only 64% of GDPs gave advice concerning the concentration of toothpaste which coincided with the available clinical guidelines. Twenty eight per cent of GDPs also contradicted the guidelines by advising children under 7 with high caries to use a low fluoride toothpaste. Although 59% of GDPs in the fluoridated milk areas asked the child whether they had fluoridated milk at school, they did not appear to alter the advice given regarding the use of fluoridated toothpaste.Conclusion: The study showed that a significant number of GDPs did not adhere to clinical guidelines relating to the use of fluoride toothpaste when giving advice to their child patients. For evidence-based dentistry to become a reality in this area, ways must be found to disseminate the available guidelines more fully and increase their acceptance and use by practitioners. A pleasing dental appearance is the subjective appreciation of the shade, shape and arrangement of the teeth and their relationship to the gingiva, lips and facial features. Achieving such a pleasing appearance in our patients is not always easy but is critical, not least because our work is effectively on display and this has implications for patients' perceptions of our practice. To be successful, thorough assessment, careful planning and precise clinical execution is required. Every bit as important though, is good communication, both with the dental laboratory and particularly with the patient. Considerations when planning treatment is the third in the series of crowns and other extra-coronal restorations. Articles or chapters on treatment planning in restorative dentistry can make pretty dry reading, often built around a list of factors that might influence your decision-making. In truth though, planning and placing crowns or other extra-coronal restorations cannot be distilled into a series of lists. Objective: To investigate variations in tooth preparations for resin-bonded all-ceramic crowns (RBCs) in general dental practice (GDP).Design: Laboratory-based retrospective analysis of dies for RBCs.Setting: General dental practices in the UK and Ireland (2000).Methods: A sample (n = 132) of laboratory models containing 180 tooth preparations for RBCs, featuring work from different general dental practitioners was obtained from four commercial dental laboratories. Aspects of the preparations were quantified and compared with accepted criteria defined following a review of the literature.Results: The teeth found to be most frequently prepared for RBCs were maxillary incisors (41%). Margin positions were variably positioned with 29% of the preparations on the buccal aspect having subgingival margins. There were many tooth preparation dies for low fusing RBCs (47%) and Chameleon Fortress RBCs (62%) demonstrating overpreparation in the mesiodistal plane. The majority of the margins (84% buccally and 79% lingually) of the dies examined exhibited appropriate shoulder or chamfer finishes. Of the Chameleon Fortress preparations analysed, 86% had been underprepared occlusally. 42% of the teeth had been prepared with no regard to tooth morphology and demonstrated just one plane of reduction. The majority (93%) of the clinicians failed to provide any information regarding the shade of the prepared tooth stump.Conclusions: On the evidence of this survey of this sample of general dental practitioners' work, it was found that relevant guidelines for the preparations of RBCs are not being fully adhered to. Objective: The objective of this study was to investigate the outcome of conventional root canal treatment in a general practice setting within the Royal Air Force dental service. Design Retrospective review.Methods: Teeth that had been root-filled for 12 months or more by Royal Air Force dental practitioners in patients attending a large Royal Air Force dental centre were included in the study. Following clinical and radiographic review the root fillings were classified as 'definitely successful', 'probably successful' or 'failed' The effect on success of several variables on the outcome was investigated.Results: Out of a total of 406 teeth, 59% were maxillary teeth and 41% were mandibular teeth. Sixty-nine per cent of the total sample had pre-existing periapical radiolucencies. Cold lateral condensation of gutta-percha was the most widely used filling technique (64% of all cases). Fifty per cent of the teeth had root fillings within 2 mm of the radiographic apex, 32% were greater than 2 mm from the radiographic apex and 18% were overfilled. Cold lateral condensation was the most successful (92% overall) filling technique. Maxillary anterior teeth had a better success rate (96%) than other tooth types. Teeth with pre-existing periapical radiolucencies had a higher success rate (87%) than those cases where there was no pre-existing periapical radiolucency (80%). Root fillings that were less than 2 mm from the radiographic apex of the tooth had a higher success rate (88% overall) than those that were greater than 2 mm from the radiographic apex (77% overall). Of the 406 cases, 57% (n=231) were classified as definitely successful, 28% (n=114) were classified as probably successful and 15% (n=62) were classified as failures. Thus, the overall success rate combining definitely successful and probably successful root fillings was 85% (n=344).Conclusions: Root fillings placed using cold lateral condensation of gutta-percha to within 2 mm of the radiographic apex of the tooth were associated with the best outcome. As we move into the twenty-first century, patterns of dental disease in adults are changing. Surveys of adult dental health indicate that more people are keeping their teeth for longer in life. In many cases, the ravages of dental disease and the cumulative effect of a lifetime of restorative dentistry lead to gradual tooth loss. For many of these patients, restoration of a complete dentition may not be feasible nor desirable. In recent years, functionally oriented treatment planning has become acceptable in light of recent research findings. Using this approach, treatment efforts and resources are directed principally at retaining the 'strategic' part of the dentition in the long term, ie, the anterior and premolar teeth. This paper describes, with the aid of treated cases, a means of combining a shortened dental arch strategy with resin bonded bridgework. Aims: Concerns have been expressed about the technical quality of NHS endodontic treatment. Bringing performance into line with guidelines for good practice needs to be underpinned by an understanding of barriers to compliance. To this end, our research involved an exploratory investigation of the factors influencing the behaviour of general dental practitioners in their practice of endodontics.Materials and Methods: Subjects 12 dental practitioners, representative of varying levels of professional experience and status, and of compliance with good practice guidelines. Data collection In-depth interviews, following a topic guide. Analysis Identification, abstraction and charting of major themes.Findings: Informants' responses suggested that general dental practitioners' endodontic practice is influenced by a complex web of factors. A key barrier to high quality treatment is the NHS remuneration scheme. Undergraduate and postgraduate education and training are also highly influential on practice. Dentists reported employing a range of strategies to manage the time-cost tensions imposed by the remuneration system. Perceived deficiencies in the content and delivery of postgraduate training were highlighted by our informants.Conclusions: There was a perception among our informants that the NHS fee structure needs to be revised. Their views suggest that a system which rewards quality rather than volume may be more appropriate, but, we believe, such a system would need to take into account efficiency as well as effectiveness. Modification of the current system of postgraduate training in endodontics is also indicated by the views expressed in the interviews. From the diversity of views and from a critical review of the literature, we conclude that flexibility is the key note in changing practice, with no single strategy likely to be universally appropriate. Unlabelled: The gagging reflex is a physiological reaction which safeguards the airway from foreign bodies. In some people this response is exaggerated to the extent that the acceptance/provision of dental treatment is not possible. The aim of this paper is to review the role of acupuncture in controlling gagging as a safe, cheap, quick and relatively non-invasive technique.Method: Ten people agreed to try ear acupuncture to control gagging during dental treatment. Prior to treatment the severity of gagging was assessed. Acupuncture needles were inserted into a specific anti-gagging point on each ear, manipulated briefly and left in situ. Dental treatment was then carried out and the effectiveness of the acupuncture in preventing gagging was assessed. After treatment, the needles were removed and the patient discharged. All acupuncture was carried out by a dentist trained in its use.Results: Four people had a severe gag reflex which made treatment impossible and six had a very severe reflex which made treatment impossible and affected their dental attendance. Ear acupuncture completely controlled the gag reflex in eight cases (23 treatment episodes) and partially controlled the reflex in two cases (two treatment episodes). Dental treatment could be carried out in all cases and at all visits. The cost of materials was 0.2 pounds per person per visit. Additional clinical time was in the order of 2-3 minutes. There were no adverse reactions to the technique and, on all occasions, patients were fit to leave the surgery and travel home unaccompanied.Conclusions: Ear acupuncture was successful in controlling the gag reflex. It is a safe, quick, inexpensive and relatively noninvasive technique. A controlled clinical trial is required to investigate any placebo effect. [ABSTRACT FROM AUTHOR]

Objective: The purpose of this study was to determine the effect that two guides to tooth preparation had on an operator's ability to appropriately and consistently prepare teeth for porcelain laminate veneers.Study Design: In-vitro study.Materials and Methods: Thirty typodont central incisor teeth were randomly allocated into three groups and a general dental practitioner was asked to prepare the teeth for porcelain laminate veneers. Group A were prepared freehand while Groups B and C were prepared with the assistance of a silicone index and depth preparation bur respectively. Images of the prepared teeth were used to calculate the mean labial depth of preparation and incisal reduction of teeth in each group.Results: The mean labial reduction for Groups A, B and C was 0.37 mm (SD 0.13), 0.62 mm (SD 0.17) and 0.61 mm (SD 0.15) and the mean incisal reduction for Groups A, B and C was 1.0 mm (SD 0.28), 1.0 mm (SD 0.38) and 1.03 mm (SD 0.26) respectively.Conclusion: It is suggested that consideration be given to the use of a silicone index or depth gauge bur when teeth are prepared for porcelain laminate veneers. Important developments for the dental profession from a number of clinical guidelines will be presented, commencing with the Guidelines for surgical endodontics. Aim To assess the prevalence of apical periodontitis (AP) and the technical quality of root canal treatment in an adult sub-population in London.Methodology Panoramic radiographs of 136 patients who attended a dental hospital were collected. The periapical health of teeth present was assessed radiographically and the presence or absence of AP noted. The technical quality of the root canal treatment was scored as adequate or inadequate, based on the European Society of Endodontology (2006) guidelines.Results A total of 3,396 teeth were assessed; AP was detected in 4.1% of the teeth. Forty-nine percent of patients had at least one tooth with radiographic evidence of AP. The percentage of root filled teeth with AP was 38.3%. AP was significantly more frequently found in root treated, compared with non-root treated, teeth (p <0.001). In 44.3% of the cases, the technical quality of the root canal filling was inadequate. AP was detected in 14% of adequately, compared with 68.6% of inadequately, root filled teeth. There is a significant negative correlation between the technical quality of root canal treatment and the presence of AP (p <0.001).Conclusions There was a high prevalence of AP and poor technical quality root canal treatment; a strong association between AP and root filled teeth, and between the periapical health and the technical quality of the root canal treatment. The results are consistent with previous studies using similar methodology and re-confirmed that high technical quality root canal treatment is crucial to ensure a favourable treatment outcome. Aim To consider the survival of restorations in root filled and non-root filled teeth.Methods A data set was established consisting of patients, 18 years or older. For each patient on the database with a tooth restored with a direct or indirect restoration with or without a root filling, the subsequent history of intervention on that tooth was consulted, and the next date of intervention, if any could be found in the data set, was obtained. Thus a data set was created of restored teeth and whether they have also received root fillings, with the dates of restoration and root filling placement and the dates, if any, of re-intervention. Modified Kaplan-Meier statistical analysis was used to quantify the distribution of time to intervention.Results Data for over 80,000 different adult patients were analysed, of whom 46% were male and 54% female. A total of 538,967 restoration placements were obtained from the data over a period of 11 years, of which 30,073 were root fillings.Conclusions Examination of the survival of restorations in teeth with and without root canal fillings indicated that those with root canal fillings have shorter intervals before re-intervention than teeth without root fillings. Objective To evaluate ten-year survival and clinical performance of resin-based composite restorations placed at increased vertical dimension as a 'Dahl' type appliance to manage localised anterior tooth wear.Design A prospective survival analysis of restorations provided at a single centre.Setting UK NHS hospital and postgraduate institute.Methods The clinical performance of 283 composite resin restorations on 26 patients with localised anterior tooth wear was reviewed after a ten year follow-up period. The study used modified United States Public Health Service (USPHS) criteria for assessing the restorations. Survival of the restorations was analysed using Kaplan-Meier survival curves, the log-rank test, and the Cox proportional hazards regression analysis.Results The results indicated that the median survival time for composite resin restorations was 5.8 years and 4.75 years for replacement restorations when all types of failure were considered. The restorations commonly failed as a result of wear, fracture and marginal discoloration. The factors that significantly influenced the survival of these restorations were the incisal relationship, aetiology, material used, and the nature of opposing dentition. The biological complications associated with this treatment regime were rare. Patient satisfaction remained high despite the long term deterioration of the restorations.Conclusion With some degree of maintenance, repeated use of composite resin restorations to treat localised anterior tooth wear at an increased occlusal vertical dimension is a viable treatment option over a ten-year period. Objective To investigate current endodontic clinical practice in the north west of England and evaluate practitioner's training needs.Method A questionnaire was posted to 702 primary care dentists in the north west of England in January 2006. The questionnaire investigated aspects of dentists' endodontic clinical practice, the provision of endodontics within their practice, their recent postgraduate training in endodontics and their training needs.Results Responses to the questionnaire were received from 498 (70.9%) primary care dentists, of which 449 questionnaires contained useful information. Only 30% of respondents use rubber dam for isolation in all endodontic cases. The majority used radiographs either as the only method for establishing the working length (57.3%) or in conjunction with an apex locator (34.5%). Most dentists used sodium hypochlorite solution as an irrigant during the root canal treatment. Almost 55% used a combination of conventional hand files and rotary files to prepare root canals. Fifty-eight percent used the crown down technique while 35% used the step back technique for canal preparation. Almost two-thirds routinely used non-setting calcium hydroxide as an intra-canal medicament. Lateral condensation technique was the popular obturation method and the vast majority of respondents routinely took postoperative radiographs. Almost 25% of respondents had not received any teaching or training in endodontics in the past two years.Conclusions The results of this study suggest that primary care dentists in the north west of England have embraced modern techniques and follow most aspects of accepted endodontic practice. Objective To compare and contrast the longevity of conventionally placed dental amalgam restorations with those placed using bonding techniques.Design Retrospective survival analysis (Kaplan Meier) of dental amalgam restorations placed by a single operator in a private general dental practice.Subjects and methods The records relating to dental amalgam restorations placed between 1 August 1996 and 31 July 2006 were sourced. The details of these were placed into a database that permitted flexible interrogation. Survival data on conventionally placed amalgams (C) and those bonded with either Panavia Ex (PE) or Rely X ARC (RX) were exported into a statistical package to permit survival analysis by the method of Kaplan and Meier.Results The number of restorations available for analysis were C = 3,854, PE = 51 and RX = 1,797. Percentage survival at one year was C = 96.29, PE = 95.65, and RX = 97.58. Percentage survival at five years was C = 86.21, PE = 76.35 and RX = 82.59. A Log Rank test demonstrated no statistically significant difference (p >0.05) in survival between the restoration types. Amalgam restorations bonded with PE or RX exhibited an acceleration of failure rate around 1,000 days post-placement. Further survival analyses of the method of restoration versus type of restored teeth (molar/premolar) and cavity preparation (Class I/II) showed no significant difference in the survival curves in respect of type of restored tooth. In the comparison of Class I and II cavities, the survival curves for the restorations differed significantly (p <0.0001), however when the curves for the Class I restorations alone were compared, no significant difference was found (p = 0.2634). This was also the case for the Class II restorations (p = 0.2260).Conclusions Within the limitations of the study, bonding amalgams, compared to placing them conventionally, afforded no significant benefit upon restoration longevity. This, coupled with the emerging trend of an accelerating decline in longevity of bonded amalgams from 1,000 days onwards and with the greater cost, challenges the justification for routine bonding of amalgams. Aim To evaluate the reproducibility of the Restorative Index of Treatment Need (RIOTN) system for grading the complexity of root canal treatment.Methodology The RIOTN system of grading the complexity of root canal treatment was applied to all endodontic referrals to a department of restorative dentistry in a district general hospital within a period of one year. Grading was repeated in 60 randomly selected teeth and weighted kappa analysis was used to test for intra-observer as well as inter-observer agreement with a consultant in restorative dentistry and a vocational trainee (VT).Results In all, 152 patients were referred for root canal treatment of 186 teeth within the period of study. Weighted kappa for intra-observer agreement when categorising complexity was 0.636. Weighted kappa for inter-observer agreement with the consultant was 0.570 and that for agreement with the VT was 0.223.Conclusion The RIOTN system of grading the complexity of root canal treatment was incomplete; with moderate to poor reproducibility.
