Introduction The purpose of this study was to define the anatomical relationships of the pterygopalatine fossa (PPF) and its operative implications in skull base surgical approaches. Methods Ten cadaveric heads were dissected at the Dianne and M Gazi Yasargil Educational Center MicrosurgicaLaboratory, in Little Rock, AK, USA. The PPF was exposed through an extended dissection with mandible and pterygoid plate removal. Results The PPF has the shape of an inverted cone. Its boundaries are the pterygomaxillary fissure; the maxilla, anteriorly; the medial plate of the pterygoid process, and greater wing of the sphenoid process, posteriorly; the palatine bone, medially; and the body of the sphenoid process, superiorly. Its contents are the maxillary division of the trigeminal nerve and its branches; the pterygopalatine ganglion; the pterygopalatine portion of the maxillary artery (MA) and its branches; and the venous network. Differential diagnosis of PPF masses includes perineural tumoral extension along the maxillary nerve, schwannomas, neurofibromas, angiofibromas, hemangiomas, and ectopic salivary gland tissue. Transmaxillary and transpalatal approaches require extensive resection of bony structures and are narrow in the deeper part of the approach, impairing the surgical vision and maneuverability. Endoscopic surgery solves this problem, bringing the light source to the center of the surgical field, allowing proper visualization of the surgical field, extreme close-ups, and different view angles. Conclusion We provide detailed information on the fossa's boundaries, intercommunications with adjacent structures, anatomy of the maxillary artery, and its variations. It is discussed in the context of clinical affections and surgical approaches of this specific region, including pterygomaxillary disjunction and skull base tumors.

Objective To describe the endoscopic and microsurgical anatomy of the cavernous sinus (CS) with focus on the surgical landmarks in microsurgical anatomy. Materials and methods Ten formalin-fixed central skull base specimens (20 CSs) with silicone-injected carotid arteries were examined through an extended endoscopic transsphenoidal approach. Fifteen formalin-fixed heads were dissected to simulate the surgical position in CS approaches. Results Endoscopic access enables identification of the anterior and posterior surgical corridors. Structures within the CS and on its lateral wall could be visualized and studied, but none of the triangular areas relevant to the transcranial microsurgical anatomy were fully visible through the endoscopic approach. Conclusion The endoscopic approach to the CS is an important surgical technique for the treatment of pathological conditions that affect this region. Correlating endoscopic findings with the conventional (transcranial) microsurgical anatomy is a useful way of applying the established knowledge into a more recent operative technique. Endoscope can provide access to the CS and to the structures it harbors.

Introduction Caustic substance ingestion is a common cause of esophageal stricture in children. The primary treatment is esophageal dilatation. Although it is known that endoscopic esophageal dilatation is a procedure associated with a high rate of bacteremia, current guidelines do not recommend routine throat swab cultures or antibiotic prophylaxis for the general children population. Case Report We describe a case of a 7-year-old boy presenting with refractory headaches who was diagnosed with cranial abscess after multiple esophageal dilatations due to stenosis caused by caustic soda ingestion. The patient was subjected to neurosurgical intervention and intravenous antibiotic treatment for 6 weeks. Streptococcus viridans culture was positive in purulent abscess content. Conclusion We highlight this condition that, although rare, needs immediate diagnosis and proper treatment. We also recommend routine testing of throat swabs and antibiotics prophylaxis to children undergoing esophageal dilatation.

Posterior circulation aneurysms represent 10–15% of intracranial aneurysms. The diagnosis is usually secondary to subarachnoid hemorrhage due to its initial asymptomatic presentation and higher risk of rupture compared with aneurysms in the anterior circulation. The surgical treatment of posterior circulation aneurysms is complex and challenging for neurosurgeons because of the particular anatomy of the posterior circulation with its close relation to the brainstem and cranial nerves and also because of the depth and narrowness of the surgical approach. Aneurysms from different locations have specific anatomical relationships and surgical approaches for better visualization and dissection. Therefore, a detailed anatomy knowledge of the posterior circulation is mandatory for an individualized preoperative planning and good neurological and angiographic outcomes. We selected the main aneurysm sites on the posterior circulation, such as: posterior inferior cerebellar artery, basilar trunk, basilar bifurcation, posterior cerebral artery (PCA) and superior cerebellar artery for a detailed description of the relevant anatomy related to aneurysm, and the main surgical approaches for its surgical treatment. Furthermore, we performed a literature review with the most recent outcomes regarding to the surgical treatment of posterior circulation aneurysms.

The sixth abducens nerve is subject to injury after rare complications of intracranial hypotension caused by procedures such as dural punctures and spinal surgeries. The purpose of this case report is to discuss the mechanism of nerve palsy in these situations. Therefore, we describe a case of onset of contralateral sixth cranial nerve palsy after intracranial aneurysm and temporal meningioma surgery. Moreover, in this case there is a singularity due to the presence of the petroclival meningioma that amplified the unfolding of the lesion.

Arachnoid cysts (ACs) are a rare condition of mass effect injury and are usually found in the Sylvian fissure. However, rarely, they can appear in the sellar area, causing symptoms of compression. Due to the mass effect, the sellar arachnoid cyst (SAC) may cause headaches, visual disturbances, hypopituitarism, precocious puberty, and the “bobble-head doll” syndrome.We present the case of JNS, 61 years old, male. The patient presented with hypotestosteronism, hypothyroidism, hypocortisolism and bitemporal hemianopsia. The magnetic resonance imaging scans revealed a mass above the pituitary gland, compatible with a SAC. A surgical excision was performed with removal of the capsule and fenestration within the subarachnoid spaces for emptying the cyst. After the procedure, the patient presented great clinical improvement. The rarity of the case calls attention to the fact that SACs should be thought of as a differential diagnosis in cases of hypopituitarism.

Computed tomographic angiography (CTA) has recently gained popularity as an initial imaging test for spontaneous subarachnoid hemorrhage (SAH). This study evaluated 59 patients presenting aneurysmal SAH who underwent microsurgical clipping based on CTA findings alone and digital subtraction angiography (DSA) at postoperative follow-up. Multiple aneurysms were identified by CTA in 27% of patients and in 10% of patients, DSA identified aneurysms in addition to those diagnosed with CTA. The time between CTA and surgical treatment ranged from 0–4 days. Postoperative DSA revealed that 24% of patients had residual neck. The use of CTA alone may not be enough to detect small unruptured aneurysms in patients with multiple lesions or aneurysm remnants adjacent to an aneurysm clip. However, the advantages of CTA compared with DSA include its rapidity, reduced invasiveness, and lower cost, which allow us to proceed to ruptured aneurysm repair entirely on the basis of good-quality CTA studies.

Introduction Postoperative hemorrhage is a well-known complication after intracranial surgeries. It usually occurs at the site of the operation; however, there are many reports of cerebellar hemorrhage following supratentorial craniotomies. Despite that, there are only three cases of multiple postoperative supratentorial hemorrhages (distant from the site of the operation) following aneurysm clipping in the current literature. Case Report A 52-year-old previously healthy woman underwent a left pterional craniotomy for the clipping of an unruptured aneurysm. The surgery was performed without any intraoperative rupture or other occurrences. On the follow-up computed tomography (CT) scan performed 24h after surgery, bilateral posterior temporal lobe hematomas were identified, and they had no relation to the surgical site. The patient developed symptomatic vasospasm, with right-sided hemiparesis. This complication was treated conservatively, and the outcome was good. Conclusions Intracerebral hemorrhage far from the site of the craniotomy is an infrequent complication after neurosurgical procedures, especially in vascular neurosurgery. The present report warns of the occurrence of this entity even in asymptomatic patients.

ResumoOs aneurismas fusiformes são pouco frequentes e geralmente não estão associados a fatores de risco específicos. Ocorrem principalmente nas artérias cerebrais, por meio de uma dilatação circunferencial na parede da artéria, garantindo seu aspecto morfológico típico: ausência de colo aneurismático. Por essa razão, não podem ser usualmente clipados ou embolizados, necessitando de métodos alternativos de tratamento que representam um desafio para os neurocirurgiões. As alternativas cirúrgicas estão associadas a um melhor prognóstico e incluem as técnicas de revascularização com o uso de bypass, a oclusão da artéria aferente (PAO, sigla em inglês) e “wrapping” ou clip-wrap. Métodos endovasculares, como o uso de stents, representam uma opção para restauração mais fisiológica do fluxo sanguíneo, embora necessitem de mais estudos para confirmar a eficácia.

ResumoA fratura de côndilos occipitais é uma afecção considerada rara, mas que pode estar sendo subdiagnosticada. Fatores como a apresentação clínica variável, o exame físico frustro e a não identificação por radiografia simples dificultam esse diagnóstico, podendo levar a complicações como paralisia de nervos cranianos caudais e até mesmo a óbito. O presente estudo tem como objetivo revisar a literatura pertinente às fraturas de côndilos occipitais, com enfoque nas considerações anatômicas da junção craniocervical e ressaltando aspectos fisiopatológicos, parâmetros clínicos e as controvérsias quanto ao tratamento. O incremento das técnicas radiológicas e a maior disponibilidade e uso de tomografia computadorizada possibilitaram o aumento do número de casos descritos dessas fraturas nas últimas décadas. A apresentação clínica é inespecífica e a tomografia da junção craniocervical é o método diagnóstico de escolha. A ausência de diagnóstico é responsável por sequelas, como déficits neurológicos, e foram descritas taxas de mortalidade de até 16% em casos de fraturas bilaterais. O mecanismo de injúria exato não é bem conhecido, mas a maioria dos autores indica a hiperextensão do pescoço associada à força vertical sobre a junção craniocervical. O tratamento é controverso, por causa da inconsistência nos resultados obtidos com o tratamento conservador baseado na classificação de Anderson e Montesano, em comparação com o escasso número de doentes tratados cirurgicamente.

Aneurysmal subarachnoid haemorrhage is one of the most deleterious acute neurological diseases. The cerebral ischemia secondary to arterial vasospasm occurring after aneurysmal subarachnoid haemorrhage is still responsible for the considerable morbidity and mortality in these patients. Besides the knowledge of basic mechanisms of cerebral vasoespasm following aneurysmal subarachnoid haemorrhage, the prophylaxis and treatment of this pathology however still remain suboptimal. There is some evidence that acute erythropoietin treatment may reduce the severity of cerebral vasospasm and eventually improve outcome in aneurysmal subarachnoid haemorrhage patients. There are underlying mechanisms extend far beyond erythropoiesis: like enhancing neurogenesis, decreasing inflammation and apoptosis inhibition. In this review the authors describe many of the biologic effects, especially experimental studies and clinical studies that reported why the erythropoietin could be beneficial to patients with aneurysmal subarachnoid haemorrhage.

ResumoA espondilodiscite é a junção de osteomielite vertebral, espondilite e discite, sendo causada por vários agentes. O Staphylococcus aureus, Brucella abortus e Mycobacterium tuberculosis são os mais comuns no Brasil, e o reconhecimento dos patógenos é de extrema importância para a definição do tratamento antimicrobiano específico. Os exames de imagem são os exames de escolha para o diagnóstico da espondilodiscite, sendo a ressonância nuclear magnética (RNM) o padrão-ouro para fechar o diagnóstico, além de poder ser utilizado como método avaliativo da eficácia terapêutica. O tratamento conservador é feito com base no uso de antibióticos, uso de órteses e repouso. Já o tratamento cirúrgico consiste na descompressão da coluna, desbridamento da área infectada e fusão vertebral. O seguinte trabalho é uma análise das publicações ligadas ao assunto com o objetivo de esclarecer e elucidar dúvidas sobre um tema ainda pouco estudado no nosso meio neurocirúrgico, evidenciada pela escassez de trabalhos brasileiros nessa área.

ResumoAo longo dos anos, a cirurgia minimamente invasiva avançou com a contribuição importante dos neurocirurgiões sobre a melhor opção para a abordagem terapêutica de tumores da região selar, partindo-se de uma cirurgia extremamente prejudicial, com alta morbidade e mortalidade, até a evolução de técnicas mais modernas, as quais dispõem de materiais mais adequados, que permitem ressecções completas com menor agressão às estruturas neurovasculares. Para praticá-las, é necessário um grupo de trabalho que envolva diferentes especialidades, como endocrinologia, oftalmologia, otorrinolaringologia, neurocirurgia e radioterapia, a fim de oferecer aos pacientes as melhores opções, garantindo atendimento individualizado, resolutivo e pouco agressivo. Há necessidade de um planejamento pré-operatório cuidadoso, com o uso de exames complementares avançados como ressonância magnética, tomografia computadorizada dos seios da face, dosagem sérica dos hormônios hipofisários e campimetria visual. Ainda, durante o procedimento cirúrgico, pode-se dispor de neuronavegação, tornando a cirurgia mais segura, especialmente nas reoperações. Por essas vantagens, a cirurgia transesfenoidal por microscopia convencional e por endoscopia é o procedimento atual de escolha para tratar tumores selares e fornecer adequada descompressão de estruturas neurais, com recuperação favorável mais rápida no pós-operatório em comparação com a abordagem transcraniana convencional.

RESUMO A tecnica de clip-wrap e utilizada quando o neurocirurgiao e confrontado com aneurismas que nao podem ser clipados pelas tecnicas tradicionais, por causa de colo largo, calcificacao, placas ateroscleroticas, aneurismas fusiformes ou quando ha vasos oriundos do corpo do aneurisma. Diversos tipos de materiais podem ser utilizados, incluindo algodao, gaze de musselina, musculo, cola de fibrina e cianoacrilato. Nesta revisao de literatura, os autores destacam os resultados obtidos por esses diferentes materiais, assim como as complicacoes e a eficacia em relacao as taxas de ressangramento e ruptura no pos- operatorio. Conclui-se, portanto, que a tecnica de clip-wrap e segura e eficaz, associada com uma baixa taxa de complicacoes pos-operatorias. O metodo pode impedir ressangramento e representa melhoria quando comparado com a historia natural da doenca. PALAVRAS-CHAVE ABSTRACT The clip-wrap technique: a literature review The clip-wrap technique is used when the neurosurgeon is confronted with aneurysms that cannot be cut by means of traditional techniques, due to a wide neck, calcification, atherosclerotic plaques, fusiforms aneurysms or when there are vessels from the aneurysm body. Various types of materials can be used, including cotton, gauze muslin, muscle, fibrin glue and cyanoacrylate. In this review, the authors highlight the results obtained by these different materials as well as the complications and efficacy in relation to the rates of rebleeding and rupture in the postoperative period. Therefore, the clip-wrap technique is safe and effective, associated with a low rate of postoperative complications. The method can prevent rebleeding and represents an improvement compared with the natural history of disease.

ResumoA ínsula representa uma estrutura cortical de anatomia complexa, permanecendo recoberta pelos opérculos e superficialmente delimitada pela fissura silviana. Apesar de inicialmente serem consideradas não ressecáveis, as patologias insulares têm, na atualidade, posição de destaque, representando desafio ao neurocirurgião experiente. Preservação funcional e ressecção macroscópica total são os objetivos do tratamento, podendo ser atingidas em até 93% dos tumores de ínsula. Sistemas de navegação, bem como eletrofisiologia transoperatória e anestesia especializada, representam importantes ferramentas cirúrgicas. A escolha adequada dos métodos de auxílio é decisão difícil, sendo, em alguns aspectos, metodologias excludentes. Realizamos revisão de literatura voltada à discussão dos fundamentos da neurocirurgia de ínsula.

ResumoEste artigo tem o objetivo de alertar e informar sobre as principais infecções do sistema nervoso central pós-procedimento da craniotomia, revisando dados epidemiológicos, profiláticos, fatores de risco, tratamento e outros pontos relevantes da infecção de ferida operatória, de meningite e de abscesso cerebral. Averígua-se a importância dos cuidados de assepsia com a lavagem do sítio cirúrgico com antisséptico degermante seguido de solução antisséptica alcoólica com princípio ativo (PVPI ou clorexidina), da equipe cirúrgica na degermação das mãos e paramentação e com os instrumentais e sala cirúrgica. Além disso, é discutido o valor da realização ou não da tricotomia, já que esta não mostra diferenças relativas nas incidências em acometimentos de patógenos. É de conclusão principal que, mesmo com as baixas incidências dessas infecções pós-craniotomias, estudos mais abrangentes e informativos sobre o assunto devem ser realizados em todo o mundo, por causa da gravidade do quadro clínico, seus prognósticos ruins, tratamentos intensivos, envolvimento de equipes multiprofissionais, extensão dos dias de internação, estadia em unidade de terapia intensiva e alta morbimortalidade.

Resumo Objetivo: Realizar uma revisão sobre os riscos aos quais pacientes neurocirúrgicos estão submetidos em relação à síndrome de Stevens-Johnson (SSJ). Métodos: Utilizaram-se as bases de dados Cochrane, Medline, Bireme/Lilacs, buscando-se artigos dos últimos seis anos que abordassem situações clínicas relacionadas à SSJ em pacientes neurocirúrgicos. Os trabalhos foram avaliados quanto a resultados, relevância clínica, desenho do estudo e nível de evidência. Resultados: A carbamazepina é comumente implicada, sendo o HLA-B*1502 fator de risco para tal afecção na população chinesa Han e associado à mortalidade. Outras populações foram testadas para tal HLA, não se encontrando resultados homogêneos. Ácido valproico, lamotrigina, fenitoína, fenobarbital e oxcarbazepina também estão relacionados. O EMPACT (Erythema Multiforme associated with Phenytoin And Cranial radiation Therapy) é observado em pacientes portadores de malignidades intracranianas submetidos à radiação, que iniciam fenitoína para prevenção ou tratamento de crises convulsivas. Em até 73% dos casos, esse eritema pode evoluir para SSJ. Conclusão: Pacientes neurocirúrgicos são frequentemente expostos a fatores causais de SSJ. Seu manejo consiste em suporte e suspensão da terapia envolvida. Procedimentos terapêuticos devem ser utilizados com cautela, levando-se em consideração sua real necessidade, HLA suscetíveis, idade e comorbidades.

Meningioma is a common CNS tumor and metastasis in these tumours is a rare occurrence. Malignant meningiomas are more prone to metastase. However, there are frequent case reports of metastases in atypical (grade II) and even benign (grade I), which demonstrates the somehow unsatisfying prognostic power of the current classification system. We describe an extraordinary case of a patient with a massive intra and extra-cranial, benign meningioma, metastasizing to both lung and spine. Following the report we discuss 1) some of the problems of classifying meningioma, 2) massive meningiomas and 3) the possibility of intra-operative iatrogenic metastasizing. This case is unique due to its combination of different sites of metastases, massiveness, invasive growth and benign histology. Thus it ranks among those rare but not uncommon complicated courses in meningioma which is otherwise a common benign tumor. Unfortunately the existing classification-criteria do not have sufficient power to predict such complicated courses.

Cerebral vasospasm is a deadly complication following the rupture of intracranial aneurysms. One new development in the experimental treatment of cerebral vasospasm is the looming target of signaling pathways. The pathogenesis of cerebral vasospasm involves multiple signaling pathways in proliferation, inflammation, cell death, smooth muscle phenotype changes, vascular remodeling, and contraction. A review of all of these areas is beyond the scope of this article, and as such, three systems that mediate these vascular responses have been selected: the tyrosine kinase-MAP kinase pathway, the sphingosine-1-Rho myosin light chain kinase pathway and protein kinase C.

Objective: To present a literature review of cognitive disorders in patients operated on anterior communicant artery aneurysm (AcomA). Methods: Literature was searched (PubMed) from 1996 to 2009 using “aneurysm”, “surgery” and “neuropsychological evaluation” as keywords. Only papers describing postoperative neuropsychological findings were included in this review. Results: A great deal of the retrieved papers noted alterations of intellectual, executive functions and memory (episodic and semantic). Some authors explain these memory deficits as secondary to the executive dysfunction or ischemia in related areas. Other papers noted secondary alterations after temporary clipping (worse alert and selective attention). Majority of deficits improve with time, however, long periods of temporary clip are associated with persistent deficits. Conclusion: The studies are scarce in the Brazilian literature that valued thoroughly neuropsychological alterations after AcomA surgery, correlating data with the resection or not of the straight gyrus, with the presence or not of vasoespasm and alterations in the quality of life. There were not found, up to the moment, studies in which these patients were valued pre and post surgery.

ResumoMais da metade dos pacientes com hematoma cerebral espontâneo evolui desfavoravelmente. O controle inadequado da pressão arterial sistêmica é uma das principais causas para esse quadro dramático. Várias pesquisas clínicas visando ao controle da expansão do hematoma têm se mostrado satisfatórias, entretanto pouco houve de avanço no tratamento cirúrgico dessas condições. Há inúmeras variáveis que devem ser avaliadas em um paciente com hematoma intraparenquimatoso espontâneo, e esta revisão visa sistematizar o tratamento cirúrgico baseado em evidências.

Resumo Contexto: O manuseio dos pacientes com infarto cerebral isquêmico inclui várias abordagens de tratamento. O tratamento cirúrgico é raramente necessário, entretanto os médicos gerais, neurologistas e neurocirurgiões devem estar atentos para essa possibilidade. Objetivo: Esta revisão visa sistematizar o tratamento cirúrgico com base em evidências nos acidentes vasculares isquêmicos. Conclusões: Apesar da crescente pesquisa envolvendo os acidentes vasculares, a taxa de mortalidade após a tentativa de tratamento cirúrgico pouco se tem alterado nas últimas décadas. Questões como quando ou a quem indicar uma craniectomia descompressiva no infarto isquêmico hemisférico são motivos de controvérsias no âmbito neurocirúrgico, e os dados da literatura são pouco esclarecedores.

ResumoAs anomalias do desenvolvimento venoso (DVA) são as malformações vasculares cerebrais mais frequentes, tendo geralmente um curso benigno, e podem estar acompanhando os cavernomas. Atualmente, são mais diagnosticadas por causa dos métodos de diagnósticos por imagem que existem, como a tomografia computadorizada e, sobretudo, a ressonância magnética nuclear. Este artigo revisa as DAV em geral e as relacionadas com os cavernomas. Os artigos foram buscados na base de dados PubMed, em publicações desde 1995 a junho de 2012, usando como filtros as expressões “anomalias do desenvolvimento venoso e malformações cavernosas” e “anomalias venosas cerebrais e malformações cavernosas”; a pesquisa foi limitada a artigos em idioma inglês e relacionados a humanos. As sessões de referência de artigos e revisões recentes foram revisadas e os artigos pertinentes foram recuperados no formato resumo; os manuscritos em texto completo foram obtidos subsequentemente para todos os artigos originais aplicados à revisão em curso.

ResumoA Sociedade Brasileira de Neurocirurgia, pelo Departamento de Trauma, com a Divisão de Clínica Neurocirúrgica da Faculdade de Medicina da Universidade de São Paulo e da Equipe Médica de Emergência do Pronto-Socorro de Neurocirurgia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, elaborou as Diretrizes do Atendimento ao Paciente com Traumatismo Cranioencefálico, buscando a homogeneidade de condutas e a redução dos índices de mortalidade e morbidade em todo país. Essas diretrizes possuem uma versão dedicada ao Atendimento Pré-hospitalar (Unidades de Resgate) e ao Atendimento Hospitalar imediato na Sala de Emergência. Com essa uniformização proposta, os estudos epidemiológicos poderão ser realizados de forma mais ampla e com dados mais seguros, permitindo identificar os pontos fracos e atuar com objetivos específicos, visando diminuir a morbidade e a mortalidade do paciente com traumatismo cranioencefálico.

We report a case of a 23-year-old man presented with a painful swelling over his right occipital region. Selective right carotid angiogram demonstrated a markedly tortuous and dilated right occipital artery feeding into a circular or cirsoid aneurysm in the right occipital scalp. Percutaneous occlusion of the cirsoid aneurysm was not attempted and the patient underwent a successful resection of the fistula with good outcome after three months followed up.
