5. Optimisation of Centrifugal Speed for PRPBAG®: A Novel Multiple Bag System for Preparing Platelet-Rich Plasma

Objective:This study aimed to identify the optimal centrifugal speed for the preparation of platelet-rich plasma (PRP) with PRPBAG®, a novel multi-bag system.Methods:The study included 120 (60 women and 60 men) clinically healthy volunteers aged 18-35 years. Participants were divided into three age-sex matched groups according to the centrifugation speeds as follows: 1600 rpm (n=40), 1800 rpm (n=40) or 2000 rpm (n=40). Whole blood (150 mL) was drawn from the volunteers, and centrifugation was performed for 10 minutes. After the first spin, the erythrocytes precipitated in the first bag, and the supernatant plasma was transferred into the second bag. Plasma centrifugation was performed at 3500 rpm for 15 minutes. The high-quality PRP product precipitated in the second bag. The supernatant platelet-poor plasma was transferred to the third bag and destroyed. Blood cell count of whole blood and PRP product was measured.Results:The mean white blood cell level of PRP in the 1800 rpm group was higher than the other groups (p=0.027). The mean platelet levels were 1005.22±105.96, 1743.7±145.2 and 1743.7±145.2 103/uL in the 1600, 1800 and 2000 rpm groups, respectively, after centrifugation (p

8. Socio-demographic and Clinical Characteristics of 113 Adolescents with Bipolar Disorder: An Inpatient Sample from Turkey

Objective:Pediatric bipolar disorder (BD) is a highly morbid disorder which is associated with impairments in social, academic, and family functioning. Despite its great impact on public health, the literature is scarce regarding the studies examining the clinical phenomenology of BD in children and adolescents.Methods:A retrospective chart review of 113 children and adolescents’ files (ages 13-18 years) who were consecutively admitted to our inpatient clinic between March 2012 and November 2014 and diagnosed as having BD type I or BD type II was made. The diagnoses were based on DSM-IV-TR criteria. A sociodemographic and clinical data form was created by the authors and was filled out for each patient by the authors themselves.Results:Totally 113 adolescents (71 male, 42 female) were included in the study. Mean age of the sample was 16.054±1.23 (range=13-18) years. Mean age of onset of BD was 15.04±1.74 (range=9-17) years. The first mood episode was manic in 60% of patients, depressive in 27.3%, mixed in 10.9% and hypomanic in 1.8% of patients. Of the patients, 23.4% had a suicide attempt history, 48.2% had a previous referral to a psychiatry clinic before the onset of BD. History of substance/alcohol use was present in 27.4% of the patients. Of the patients, 84.9% were using combination treatments.Conclusion:High rates of suicide attempt, substance use and significant functional impairment found in our study sample indicate that early recognition and intervention of BD, specialized educational programs and occupational support for these children and adolescents seem indispensable.

Objective:Immune activation plays a critical role in the immune response against cancer. Although several genetic factors are established with regard to the development of acute lymphoblastic leukemia (ALL), the role of immunogenic genes in ALL pathogenesis remains elusive. Toll-like receptor (TLR) and nucleotide-binding oligomerization domain containing protein 2 (NOD2) receptors are essential in immune response. The aim of the study was to investigate the association between TLR4 Asp299Gly, TLR4 Thr399Ile, NOD2 Arg702Trp, NOD2 Gly908Arg, and NOD2 Leu1007fsinsC polymorphisms and the risk of childhood ALL.Methods:Genotype distributions of TLR and NOD2 polymorphisms were determined by real-time polymerase chain reaction in 102 ALL patients and 110 sex- and age-matched healthy individuals.Results:We found no significant differences between the ALL and control groups in terms of TLR4 Asp299Gly, TLR4 Thr399Ile, NOD2 Arg702Trp, NOD2 Gly908Arg, and NOD2 Leu1007fsinsC polymorphisms (p>0.05). However, we found markedly negative correlation between the C–G haplotype (rs2066844-rs2066845) and the risk of ALL (p=0.055).Conclusion:The results showed that the TLR and NOD2 polymorphisms were not associated with the risk of ALL. However, these results need to be confirmed by further, larger case-control studies.

13. Investigation of the Incidence of CALR, MPL, and JAK2 Gene Mutations in Essential Thrombocytosis Cases with Laboratory Findings and Complications.

Introduction: Chronic myeloproliferative diseases are characterized by uncontrolled proliferation of clonal hematopoietic stem cells. One of the most common of these diseases is essential thrombocytosis. It is known that JAK2, CALR, and MPL gene mutations are the main causes of this disease. Our aim in this retrospective study was to investigate how effective these genes are in the diagnosis of essential thrombocytosis and its complications. Method: A total of 146 patients were included in the study. The study was designed retroprospectively. In this study, the diagnoses of the patients were made using the clinical and laboratory parameters. Genetic reports of the patients were reviewed, JAK2, MPL, and CALR gene positivity or negativity and hemorrhagia were noted. Results: Of the patients, 68.5% (n=100) were female and 31.5% (n=46) male aging from 19 to 90. No significant difference was found between the three mutation genes we examined gender, hepatomegaly, and hemorrhage. A significant difference was found between the absence of the JAK2 mutation gene and low hemoglobin and platelet counts over 1 million (p=0.012; p=0.05). Also the averag eage of those with mpl mutation was found to be significantly higher than that of those without this gene. Prefibrotic phase myelofibrosis transformation was noticed in 21 patients. Moreover, hypothyroidism was detected in 25 patients, and 52% of these patients were positive for the JAK2 gene. In our study, we observed that hemorrhage and bruising complications developed in 25 patients. Seven female patients had a miscarriage. Conclusion: The results of this study show that there is no co-relation between CALR, MPL, and clinical prognostic parameters. However, it has been shown that patients carrying the JAK2 gene are at a higher risk for complications, and we confirmed its diagnostic importance. Non-Hodgkin lymphomas constitute 1% of gastrointestinal system tumors. Lymphomas can occur from the oral cavity to the rectum. Primary gastrointestinal lymphomas are most frequent in the stomach, followed by the small and large intestines. Here, we report a 41-year-old male patient who had been under chemotherapy following a diagnosis of diffuse large B-cell lymphoma transformed from MALT lymphoma and discuss new treatment options. He presented to us with an acute abdomen. To our knowledge, this was the first case in the literature that presented with acute abdomen and was diagnosed as diffuse large B-cell lymphoma that transformed from MALT lymphoma. In addition, for the treatment, we want to draw attention to the importance of surgery, in addition to chemotherapy.

Pica is the chronic consumption of edible or nonedible materials that is thought to be caused by mental problems, pregnancy, or anemia. We wanted to report this case, which we have not seen in the literature before, of a pica patient who consumed raw rice due to iron deficiency anemia. A 26-year-old female patient was brought to the internal medicine polyclinic by her mother with complaints of consuming a bowl of raw rice (200 gr) every evening. The patient was treated with parenteral ferik hydroxide + sucrose complex (2700 mg) for 5 days. After 15 days, when she came to control her will to eat raw rice had been lost completely. As a result, the attention of doctors and family members for pica due to treatable causes, such as anemia, will prevent many probable complications.

- Abstract
Diffuse alveolar hemorrhage is a rare and life-threatening complication developed in systemic lupus erythematosus (SLE). We report a case of diffuse alveolar hemorrhage in a patient with lupus nephritis. The patient presented with acute respiratory and renal failure. Therefore, she was assessed as an activation of systemic lupus erythematosus. Despite pulse methylprednisolone, immunosuppressant drugs, and intravenous immunoglobulin (IVIG) therapy, the disease progressed. The patient participated in a plasmapheresis program, and double filtration plasmapheresis (DFPP) therapy was performed for six sessions in 6 days. Soon after the initiation of DFPP, she recovered with resolution of the radiological changes within 6 days and was successfully extubated. Thus, DFPP therapy should be considered for the treatment of diffuse alveolar hemorrhage, particularly in patients of systemic lupus erythematosus with lupus nephritis. [ABSTRACT FROM AUTHOR]

18. Impact of Follow-Up by the Child and Adolescent Psychiatrist on Emergency Department Re-Visit due to Child-Adolescent Psychiatric Causes.

Objective: Psychiatric causes of emergency department (ED) visits in childhood have been increasing more rapidly than the other causes. The rate of re-visit to the ED due to the same complaint is an important reason for ED visits. In the present study, we investigated the impact of follow-up of child-adolescent patients presenting to the ED due to psychiatric causes by the child and adolescent psychiatrist on ED re-visits due to the same complaint. Methods: This retrospective single-center study was conducted on child-adolescent patients admitted to the ED for psychiatric reasons in the last 2 years. A total of 77 patients were included in the study. The patients were divided into two groups according to whether they were followed up by the child and adolescent psychiatrist. The first group included patients who were followed up by the child and adolescent psychiatrist, whereas the second group included patients who were not seen or not followed up by the child and adolescent psychiatrist. Results: Child-adolescent patients in the first group who visited the ED due to psychiatric causes and were followed up by the child and adolescent psychiatrist had a significantly lower rate of ED re-visit due to the same complaint than those in the second group of who were not seen or followed up by the child and adolescent psychiatrist (p=0.023). Conclusion: Follow-up by child and adolescent psychiatrist plays an important role in decreasing the ED re-visit rate of child-adolescent patients presenting to the ED due to psychiatric reasons. 20. Comparison of BCL2 Positivity and Ki67 Expression Rates and Clinical Prognostic Parameters in Diffuse Large B-cell Lymphoma with Germinal Center and Activated B-Cell Immunophenotype.

Introduction: Due to its heterogeneous characteristics in prognosis and response to treatment, there are many pathological and clinical methods to categorize diffuse large B-cell lymphoma (DLBCL). We compared the clinical prognostic parameters of these 2 groups calculated by immunohistochemical BCL2 positivity and Ki67 expression rates and NCCN-IPI (National Comprehensive Cancer Network) score. Method: The study was performed with 102 patients diagnosed with DLBCL between January 2014-February 2022. Biopsy reports of the patients were reviewed, and BCL2 positivity, Ki67 expression rates, and immunophenotypes were noted. The clinical findings (age, stage, number of extranodal involvement, performance degree and LDH values) of the determined patient group were examined, and NCCN-IPI scores were calculated. Results: Of the patients, 32.4% (n=33) were female, 67.6% (n=69) aging from 17 to 93. No significant difference in BCL2 or Ki67 expression, immunophenotype, or NCCN-IPI score was found between genders. It was observed that the Ki67 expression rate was higher in which the antiapoptotic protein Bcl2 was negative (p=0.001). Also, those with BCL2 negativity were more in the GCB group as an immunophenotype (p=0.009). The difference between NCCN-IPI score and Bcl2, Ki67 and phenotype was found to be statistically insignificant (p>0.05). There was no correlation with the NCCN-IPI score in those with a Ki67 expression rate above 90, but it was detected at a higher rate in the BCL2-negative group and with the GCB phenotype (p=0.001; p=0.021). Conclusion: This study indicates that there is no co-relation between clinical prognostic parameters and BCL2, Ki67 and immunophenotype. In contrast pathological examination rutins are statistically significant among themselves. [ABSTRACT FROM AUTHOR]

22. Plasmacytomas in patients with Multiple Myeloma: Incidence, Localizations and Prognosis.

Introduction: Mulitple myeloma (MM) is a malignancy characterized by uncontrolled proliferation of clonal plasma cells infiltrating the bone marrow. Plasmacytoma is local accumulation of monoclonal plasma cells. More than 50% of the patients who are diagnosed with plasmacytoma develops MM within 2 years. With this study, we will evaluate parameters such as incidence of developing plasmacytomas, localizations, demographic factors and some lab values. Our aim is to understand this topic further and hope to find meaningful relationships with these parameters. Method: All patients diagnosed with MM who is followed up in the Hematology Department of Bezmialem Vakıf University Hospital will be included in the study. The demographic data of the patients, the stages, myeloma subtype, plasma cell numbers, whether they were diagnosed with plasmacytoma and if deemed necessary, other parameters will be examined. These data will be analyzed and the frequency of plasmacytoma with myeloma diagnosis will be determined. Result: 91 patients were included in this study. There were 52 female (57.7%) and 38 male (42.3%) patients. 28 of the patients (32.2%) developed plasmacytoma during the disease. In comparison between patients who developed plasmacytoma and patients who did not, there were statistically significant diffrenece in the values of serum lambda, beta-2 microglobulin, serum creatinine, hemoglobulin (p=0.005; p=0.042; p=0.050, p=0.002). There was no statistical significant difference in other parameters Conclusion: In conclusion 32.2% of the patients developed plasmacytoma which can be considered high. This effect prognosis and survival of the disease. Some of the lab values were found statistically significant in this research. These might be used for predicting the probability of having plasmacytoma early on stage. More studies are needed on the relationship between myeloma and plasmacytoma because the incidence of plasmacytoma has been increased with new treatments and improved survival rates. Introduction: Blood transfusion is the process of delivering blood and blood products directly into a patient's circulatory system. There have been many studies on blood and blood product transfusions until today; Generally, the results of surveys conducted to measure the knowledge of transfusion medicine among physicians, residents, and undergraduate medical students have demonstrated the need for additional training in this field. With this study, we evaluated the achievement level of the targets in the transfusion medicine field and the factors affecting the achievement of the Medical Doctors who are about to graduate from Bezmialem Vakıf University Faculty of Medicine. Method: The study was planned as a survey study with the participation of Bezmialem Vakıf University Faculty of Medicine students. 3 groups were selected as 4-5-6th grades. Our sample size is to be 25 students for each group (n1=n2=n3=25) making in 75 students. The sample has a 95% confidence level. IBM SPSS v.25 Was used for statistical analyses. Chi-square, Mann-Whitney U, and Kruskal-Wallis tests were used for parametric and nonparametric data, respectively. P<0.05 was accepted as statistically significant. Results: A statistically significant difference was found between 4th and 5th grades (p=0.001), 5th and 6th grades (p=0.001), and 4th and 6th grades (p<0.001) in the 5th and 8th questions, which have a special importance for us in transfusion. According to the analysis results of our questionnaire, a significant difference was found between the 4th, 5th and 6th grades (p<0.001). Conclusion: The result of our survey shows that from the 4th grade to the 6th grade, the knowledge of transfusion medicine increases significantly. Introduction: Encephalitis is a condition in which the brain parenchyma becomes inflamed due to an infection or an autoimmune response and causes neurological dysfunction. There are many causative agents leading to encephalitis, and the most common one is herpes simplex encephalitis (HSE). Survivors of encephalitis may be left with emotional, behavioral, and cognitive consequences such as inappropriate behavior, and difficulty learning and sleep disturbances. Method: In this study, parents of children patients aged between 6 and 17 were questioned about their quality of sleep and attention deficit through 33-question Children's Sleep Habits Questionnaire (CSHQ) and 48-question Conners' Parent Rating Scale (CPRS). This study is conducted in one group only. This group included 23 children who were previously diagnosed with Herpes Simplex Virus Encephalitis between years 2012-2021. Our only exclusion criteria have not any other known disease before the diagnosis of HSE. Results: Total of 23 pediatric patients diagnosed with HSE were included in this study. The mean CPRS score was 55.9±25. The mean CSHQ score was 69.13±12. The higher the scores get the possibility of having attention deficit and sleep problems increases. Our data strongly suggest that children previously diagnosed with HSE would later develop either attention deficit or sleep disturbances or both. There is no correlation found between CSHQ and CPRS whatsoever. Conclusion: For both CPRS and CSHQ, the higher the scores get the possibility of having attention deficit and sleep problems increases. Introduction: Meningiomas are the most common primary brain tumors in adults. The diagnosis of meningioma can be made using magnetic resonance imaging of brain. However, there are difficulties differentiating the grades of the tumor, which is important for choosing the right treatment (follow, surgical resection, Gamma Knife) and extent of tumor resection if surgery is preferred. However, there is no established method for meningioma grading. Although the expression of c-MYC, FABP7, GATA4, and MAOB have been investigated in meningioma tissues, their expression in serum has not been described. Therefore, this study aimed to evaluate the role of liquid biopsy by investigating the expression of these genes in meningioma patients. Method: Twenty patients who underwent surgical resection of intracranial meningiomas were enrolled. Tumor and serum samples were obtained during the surgery. Real time polymerase chain reaction was performed to assess the expression of FABP7, GATA-4, c-MYC, and MAOB in tumor tissue and in serum. Patients' clinical data including age, gender, radiological findings, simpson grade were retrospectively collected. Results: The expression levels of FABP7 and MAOB in serum of meningioma patients were significantly higher than healthy controls (p<0.05). The expression levels of MAOB in serum of grade 2 meningioma were significantly higher than those with grade 1 (p=0.032). MAOB, c-MYC and GATA4 genes were expressed significantly higher (p=0.031, p=0.041, p=0.003, respectively) in tumor tissues from grade 2 meningioma patients compared to grade 1. We did not observe any of these genes to be correlated with patients' clinical data and local tumor control. Conclusion: Our results suggested that expressions of FABP7 and MAOB genes in serum could be implemented as diagnostic marker for meningiomas. However, further studies are required. Objective: Freudenberger defined the concept of burnout as mental energy depletion after long-term work related stress in healthcare workers. It is experienced in almost all occupational groups especially in physicians. Without loss of work force, depression and burnout signs and symptoms should be known by the health worker and the administrator and early precautions should be taken. Method: The population of the research consisted of specialists, PhD lecturers and faculty members. Personal data form, beck depression and maslach burnout scale were used as data collection tools. MBI evaluated burnout in 3 dimensions: emotional exhaustion (EE), depersonalization(D), and personal accomplishment (PA). Results: One hundred two physicians who participated in the study were between 28 and 71 years old. 63.7% of the participants were male, 36.3% were female. Profession years ranged from 4 to 49 years while working years at this institution were between 1 and 25 years. 29.4% were professors, 19.6% were assistant professors, 20.6% were PhD lecturers and 30.4% were specialists. 38.6% worked in surgical unit, 52.5% worked in internal unit and 8.9% worked in basic medicine unit. As the age of the participants increased, depression, EE and depersonalization decreased, and personal success increased. There was a statistically significant relationship between depression and personal achievement according to age (p<0.01). Statistically significant relationship was found between total working time, depression and personal success (p<0.01). Depression score and EE decreased whereas personal success increased as professional year increased. No statistical difference was found in the comparison in terms of gender, working time, titles, and units of participants and the average of EE, D, and PA were found to be moderate (p>0.05). Conclusion: The burnout and depression levels were minimal in both gender. Burnout and depression levels decreased and personal success increased with age. Objective: Attention Deficit Hyperactivity Disorder (ADHD) is a neurodevelopmental disorder characterized by problems in attention, concentration, mobility and impulse control. Systematic investigations show that the incidence is 5-12% on average. Behavioral, cognitive, social and emotional domains are multifaceted. Although the etiology of ADHD is not known, biopsychosocial causes are blamed as in other psychiatric diseases. In the literature, there were not enough studies showing the relationship between anxiety and depression in children with ADHD and possible anxiety and depression in their parents. The aim of this study was to show the effect of comorbid anxiety and depression on maternal anxiety and depression symptoms in children with ADHD. Method: For this purpose, children and adolescents aged 8-18 years, who were followed up with the diagnosis of ADHD in Bezmialem Vakıf University child and adolescent psychiatry outpatient clinic, were screened using the childhood depression and anxiety scale. Sixty healthy children and their mothers and 60 children with ADHD and their mothers were included in the study. Results: While 62.7% of the mothers of the children with ADHD had anxiety, this rate was found to be 28.3% in the mothers of the control group. There was a significant difference between the two groups in terms of anxiety according to beck anxiety scale (p<0.0001). 46.6% of the mothers of children with ADHD and 25% of the mothers in the control group had depression. There was a significant difference between the two groups in terms of depression according to the beck depression scale (p<0.001). Conclusion: In this study, it was shown that the frequency of anxiety and depression increased in mothers of children with ADHD. It was also observed that the development of comorbid depression in children with ADHD increased the frequency of anxiety and depression in their mothers. [ABSTRACT FROM AUTHOR]

Objective: Depression is a common illness worldwide. Depression is different from usual mood fluctuations and short-lived emotional responses to challenges in everyday life. It can cause the affected person to suffer greatly and function poorly at work, at school and in the family. At its worst, depression can lead to suicide. On the other hand self-esteem is persons opinion of themselves. People with healthy self-esteem like themselves and value their achievements. While everyone lacks confidence occasionally, people with low self-esteem feel unhappy or unsatisfied with themselves most of the time. Social media host virtual communities where users can create individual public and/or private profiles. Social media is a huge force in the lives of young people with wide ranging effects on their development; given the importance of adolescence in the genesis of mental illness, social media is a factor in the mental health of young people. Overall, lack of research has been conducted in this area. Our purpose in this study is to search the link between adolescences social media use and its effect on depression and self-esteem. Method: The sample of the study were 10-17 years old children who were admitted to Bezmialem Vakif University Child and Adolescent Psychiatry Outpatient Clinic and diagnosed with depression. Healty children age and sex matched as control group were taken. The main data collection tools to be used in this research were surveys. Surveys used in this study were Socio-demographic questions regarding age, gender. Kovacs children depression inventory (CDI) measures the depression in children and consists of 27 items, each evaluating a symptom of depression or related affect. Items are presented as three statements of varying symptom severity, highest score being 54, and score higher than 19 counts pathological. The Rosenberg Self-Esteem scale (RSES), assess global self-esteem (i.e., feeling of self-worth and self-acceptance) with 10 items on a 4-point scale higher score being 6, score 0-1 counts as high, 2-4 counts as moderate, and score 5-6 counts as lower self-esteem. Weekly Social Media Use, used to assess the adolescents weekly time spent on social media, two variables going to be combined ‘The last 7 days how many days did you use the internet for social networking?’ (Categories are “never”, “1 day”, “2 days”, ... “7 days”). “In the last 30 days on an average day how many hours did you use the internet for social networking?” (Categories are “I don’t use”, “less than half an hour”, “1 hour”, “2-3 hours”, “4-5 hours” and “more than 6 hours”). The CDİ, RSES scores and Weekly Social Media Use values were compared to those control group. The data was analyzed statistically by using SPSS 22.0. Results: The study consisted of 40 depression diagnosed children which include 22 girls and 18 boys, patients ages ranges from 10 to 17 and 40 children in control group which include 22 girls and 18 boys, patients ages ranges from 10 to 17. The mean and standard deviation of the CDİ scores in patients and control group are 26.00±9.74548 and 13.25±4.99615 and p=0.001. The mean and standard deviation of the RSES scores in patients and control group are 2.1±1.79458 and 0.8±1.04268 and p=0.01. The mean and standard deviation of ‘How many days used in a week?’ question scores in patients and control group are 6.65±1.54505 and 6.55±1.55167 and p=0.773. The mean and standard devation of “How many hours a day?” question scores in patients and control group are 4.2±1.52248 and 2.3±1.05125 also p=0.001. According to RSES score for patient group, 22 (55%) patients had higher self-esteem, 11 (27.5%) patient had moderate and 7 (17.5%) had low self esteem and according to RSES score for control group, 33 (82.5%) patients had high, 7 (17.5%) had moderate self-esteem. Conclusion: The results of the present study show that amount of time spend on the social media is higher in depression diagnosed patients than control group and depression diagnosed patients had higher scores at the RSES which indicate that they had lower self-esteem. 29. The Role of Peripheral Blood Smear Information in Predicting Obstetric Results in Hypertensive Pregnants.

Objective: Arterial blood pressure ≥140/90 mmHg in pregnant women is defined as hypertension and hypertensive disorders affect approximately 12-22% of all pregnancies. Hypertension is one of the most important causes of maternal fetal morbidity. The main purpose of this study was to investigate the role of peripheral smear findings in predicting prognosis in hypertensive pregnant women. Method: Peripheral smear blood samples of hypertensive pregnants were taken together with routine hypertensive evaluation tests after 20 weeks of gestation. Peripheral smear was made by spreading the blood sample from the fingertip onto the slide. Following the treatment with romanowski dye, it was evaluated under the microscope. In this prospective study, the relationship between erythrocyte dysmorphias and pregnancy outcomes and complications were investigated. Results: Fifteen pregnant women voluntarily participated in our study. In this study, the mean age of hypertensive pregnants (n=15) was 32±5.077 years, the mean systolic blood pressure was 144.93±5.982, and the mean diastolic blood pressure was 92.33±5.3. Cesarean rate was 80% (12/15) for the whole group. Nine of 15 patients had gestational hypertension, 3 patients had chronic hypertension, and 3 patients had preeclampsia. Eight of 10 patients without complications (8/10, 80%) had no peripheral smear findings. Schistocytes were seen in 3 patients (3/5, 60%) and echinocytes were seen in 2 patients (2/5, 40%) of 5 patients who developed complications. There was a significant difference between patients in terms of complications and erythrocyte dysmorphology in peripheral smear (p<0.05). Conclusion: Peripheral smear is an easy to learn, applicable, inexpensive method. It is possible for physicians to gain sufficient experience in this subject within a short period. This study shows peripheral smear can help us to predict the prognosis of hypertensive disorders and to manage antenatal complications. Objective: Anemia is the most common hematological disorder in pregnancy. Therefore, early diagnosis and effective management have great importance to reduce and prevent complications. According to the current literature, no studies evaluate the performance of peripheral smear as an initial test in anemic pregnant women. Method: Peripheral smear was performed for pregnant women with hematocrit levels of 32% or less and without previously defined hematological disease. The pathological findings observed in the shaped elements of the blood were noted. Also, personal and obstetric histories of the volunteer were recorded on data set. Results: Twenty-three pregnants voluntarily participated in our study. The ages were between 17 and 49 years and the mean age was 30.65±7.96 years, gravida values were 2 (1-5); parity values were 0 (0-3); abortion values were 0 (0-2); the mean hematocrit value was 29.05% ±2.07, the mean hemoglobin value was 9.46±1.06 g/dL. Peripheral smear revealed findings consistent with iron deficiency anemia in 17 pregnants, whereas in 6 patients, there were no findings. In the peripheral smears, we detected echinocytes (8/23, 34,7%), increased myeloid sequence cells (6/23, 26%), band increase and toxic granulation (1/23, 4.34%), neutrophil hypersegmentation (2/23, 8.69%) and macro ovalocytes (1/23, 4.34%). In the samples, there were different erythrocyte (9/23), leukocyte (6/23) and thrombocyte anomalies (2/23). We divided the results into 4 groups; with anemia signs (17/23, 73.9%), without anemia signs (6/23, 26%), changing the main pathology (0/23, 0%), and indicating additional internal pathology (7/23, 30.4%). It was observed that 2 of the patients with echinocytes progressed to preeclampsia and they were hospitalized. Another patient who had echinocyte was hospitalized with the diagnosis of pyelonephritis. Two patients who had signs of B12 deficiency in peripheral smear were referred to the clinic for further examination. Conclusion: A peripheral smear is likely to provide extra information in the assessment of maternal anemia. 31. A Cross-sectional Study on Patients with Primary Myelofibrosis: Two Centers’ Experience.

Objective: Primary myelofibrosis (PMF) is a disease classified under the myeloproliferative neoplasms. Our aim was to utilize the patient data in two centers in İstanbul to analyze general features of PMF patients. Method: This study was conducted retrospectively using the data of PMF patients in Bezmialem Vakıf University Hospital and Haseki Training and Research Hospital. The data studied included gender, age, laktat dehidrogenaz and complete blood count (CBC) values, the presence of organomegaly, janus kinase 2 (JAK-2) gene mutation, constitutional symptoms (fatigue, weight loss, sweating and itching), and history of thrombosis. The data were provided from the patient follow-up files. Results: The study consisted of 35 patients (23 females; aged 30-78 years). When compared to the control group, CBC values of patients did not present any significant difference. Twenty-four patients (15 females) had splenomegaly (spleen size >13 cm). Eighteen patients (9 female) had constitutional symptoms, 13 (11 female) did not. Out of 17 JAK-2 gene mutation positive patients, 10 had anemia, 11 had splenomegaly, and 2 had portal hypertension. Out of 13 JAK-2 gene mutation negative patients, 9 had anemia and 10 had splenomegaly. Eleven patients (9 females) had massive splenomegaly (spleen size ≥20 cm). Two patients had splenectomy. Seventeen patients (11 female) had hepatomegaly. Concerning fibrosis scores in bone marrow samples, 1 female patient had a fibrosis score of 1, 7 patients (5 females) had a fibrosis score of 2, 9 patients (6 females) had a fibrosis score of 3, 10 patients (6 females) had a fibrosis score of 4. Nine patients had hypertension, 4 had diabetes mellitus, 2 had portal hypertension, 4 had coronary artery disease. Two female patients had thrombosis; both were positive for JAK-2 gene mutation. Conclusion: There was not any significant difference concerning the parameters analyzed either between the patient and control group, or JAK-2 gene mutation positive versus negative patients. Objective: Attention deficit and hyperactivity disorder (ADHD) is a common neuropsychiatric disorder. ADHD is known to have a complex etiology due to the interaction of genetic and environmental biological factors. Iron deficiency may play a role in the emergence of ADHD symptoms by affecting dopamine synthesis. Iron is the major cofactor of the tyrosine hydroxylase enzyme, a rate-limiting step in dopamine synthesis. Serum levels of ferritin decrease in cases of advanced iron deficiency. Our aim was to determine the relationship between iron deficiency anemia and ADHD. Method: All children (62 children, 6-15 years) with ADHD were recruited from Bezmialem Vakıf University as the case group. Meanwhile, 75 children (6-15 years) were recruited as the control group from pediatric outpatient clinics, who applied for routine control, medical report, etc. In this retrospective study, complete blood count [red blood cell (RBC), mean cell volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), hemoglobin (HB)], serum ferritin and iron parameters were compared between children with ADHD and control group. Results: There was no significant difference between the group with ADHD and the control group in terms of iron, RBC, MCV, MCH, age, ferritin, and HB [(p values respectively: (0.175), (0.973), (0.752), (0.066), (0.079), (0.299), (0.118)]. The mean MCHC value was found to be statistically significant between the groups (p=0.006). The MCHC value of the control group was 33.127±0.88, while that of the ADHD group was 33.612±1.194. Conclusion: In our study, a significant difference was detected in MCHC values between the ADHD and control groups. Increased MCHC is a parameter that can be seen in macrocytic anemia due to b12 and folic acid deficiency. In conclusion, it may be important to explain the relationship between MCHC elevation and ADHD in a larger sample of MCHC by adding b12 and folic acid parameters. [ABSTRACT FROM AUTHOR]

Objective:This study aims to evaluate the correlation between physical activity before pandemic and quarantine measures, hospitalization status and length of stay in patients infected with coronavirus disease-19 (COVID-19).Methods:A total of 638 people participated in the study. Of the participants 47.8% (n=305) were female and 52.2% (n=333) were male. Chi-square test was used to determine whether or not there was a correlation between hospitalization status and hospitalization duration and other variables.Results:It was found that there was no significant correlation between rates of hospitalization and gender (p>0.05). Objective:The recent spread of new Coronavirus disease (COVID-19) pandemic which caused worldwide concern is a public health emergency situation. The risk of getting infected as well as the concern levels of the health professionals on this issue is very high due to their close contact with the infected patients. The aim of this study is to evaluate the concern level of being infected in the health professionals, as well as their view and perception of different applications used during the treatment of the COVID-19 cases together with their whole approach during this pandemic situation.Methods:The study was conducted through an online survey that was sent to all the Health professionals of a pandemic hospital of a city in Turkey between 8 and 15 May 2020. This online survey was sent to all the professionals through social media platforms. The survey included both multiple-choice and true-false questions regarding COVID-19 diagnosis, clinical stage, treatment approach, concerns, knowledge, and awareness of the situation together with some social demographic characteristics of the health professionals. SPSS v20 program was used to evaluate the statistical analysis of the data collected. Data are shown through mean ± and the standard deviation is shown through percentage.Results:The study consisted of 250 (59.9%) doctors and 169 (41.1%) assistant healthcare professionals and the average age of the participants was 33.21±6.88 years. Seventy-six (18.1%) of 128 people who smoked during the pandemic wanted to quit smoking. 390 (93.1%) of the participants thought that they were in a high-risk group of being infected and were concerned about this matter. One hundred nine (26.01%) of the participants had positive COVID-19 tests of their relatives or friends. Sixty-four (15.3%) participants claimed to have a polymerase chain reaction (PCR) test, and 3 (0.7%) of them reported that their COVID-19 PCR tests were positive. Eighty-four (20.04%) stated that they used Hydroxychloroquine for prophylaxis.Conclusion:The healthcare professionals working in the pandemic hospital during the pandemic stated that they generally had sufficient knowledge about COVID-19 and believed that our country would be successful with its fight against this pandemic. Moreover, our study proved the importance of knowledge levels in fighting communicable diseases.

A sociodemographic and clinical data form was created by the authors and was filled out for each patient by the authors themselves.Results:Totally 113 adolescents (71 male, 42 female) were included in the study. Mean age of the sample was 16.054±1.23 (range=13-18) years. Mean age of onset of BD was 15.04±1.74 (range=9-17) years. The first mood episode was manic in 60% of patients, depressive in 27.3%, mixed in 10.9% and hypomanic in 1.8% of patients. Of the patients, 23.4% had a suicide attempt history, 48.2% had a previous referral to a psychiatry clinic before the onset of BD. History of substance/alcohol use was present in 27.4% of the patients. Of the patients, 84.9% were using combination treatments.Conclusion:High rates of suicide attempt, substance use and significant functional impairment found in our study sample indicate that early recognition and intervention of BD, specialized educational programs and occupational support for these children and adolescents seem indispensable.

Diffuse alveolar hemorrhage is a rare and life-threatening complication developed in systemic lupus erythematosus (SLE). We report a case of diffuse alveolar hemorrhage in a patient with lupus nephritis. The patient presented with acute respiratory and renal failure. Therefore, she was assessed as an activation of systemic lupus erythematosus. Despite pulse methylprednisolone, immunosuppressant drugs, and intravenous immunoglobulin (IVIG) therapy, the disease progressed. The patient participated in a plasmapheresis program, and double filtration plasmapheresis (DFPP) therapy was performed for six sessions in 6 days. Soon after the initiation of DFPP, she recovered with resolution of the radiological changes within 6 days and was successfully extubated. Thus, DFPP therapy should be considered for the treatment of diffuse alveolar hemorrhage, particularly in patients of systemic lupus erythematosus with lupus nephritis.

Objective:Psychiatric causes of emergency department (ED) visits in childhood have been increasing more rapidly than the other causes. The rate of re-visit to the ED due to the same complaint is an important reason for ED visits. In the present study, we investigated the impact of follow-up of child-adolescent patients presenting to the ED due to psychiatric causes by the child and adolescent psychiatrist on ED re-visits due to the same complaint.Methods:This retrospective single-center study was conducted on child-adolescent patients admitted to the ED for psychiatric reasons in the last 2 years. A total of 77 patients were included in the study. The patients were divided into two groups according to whether they were followed up by the child and adolescent psychiatrist. The first group included patients who were followed up by the child and adolescent psychiatrist, whereas the second group included patients who were not seen or not followed up by the child and adolescent psychiatrist.Results:Child-adolescent patients in the first group who visited the ED due to psychiatric causes and were followed up by the child and adolescent psychiatrist had a significantly lower rate of ED re-visit due to the same complaint than those in the second group of who were not seen or followed up by the child and adolescent psychiatrist (p=0.023).Conclusion:Follow-up by child and adolescent psychiatrist plays an important role in decreasing the ED re-visit rate of child-adolescent patients presenting to the ED due to psychiatric reasons.

Objective:Immune activation plays a critical role in the immune response against cancer. Although several genetic factors are established with regard to the development of acute lymphoblastic leukemia (ALL), the role of immunogenic genes in ALL pathogenesis remains elusive. Toll-like receptor (TLR) and nucleotide-binding oligomerization domain containing protein 2 (NOD2) receptors are essential in immune response. The aim of the study was to investigate the association between TLR4 Asp299Gly, TLR4 Thr399Ile, NOD2 Arg702Trp, NOD2 Gly908Arg, and NOD2 Leu1007fsinsC polymorphisms and the risk of childhood ALL.Methods:Genotype distributions of TLR and NOD2 polymorphisms were determined by real-time polymerase chain reaction in 102 ALL patients and 110 sex- and age-matched healthy individuals.Results:We found no significant differences between the ALL and control groups in terms of TLR4 Asp299Gly, TLR4 Thr399Ile, NOD2 Arg702Trp, NOD2 Gly908Arg, and NOD2 Leu1007fsinsC polymorphisms (p>0.05). However, we found markedly negative correlation between the C–G haplotype (rs2066844-rs2066845) and the risk of ALL (p=0.055).Conclusion:The results showed that the TLR and NOD2 polymorphisms were not associated with the risk of ALL. However, these results need to be confirmed by further, larger case-control studies.

Non-Hodgkin lymphomas constitute 1% of gastrointestinal system tumors. Lymphomas can occur from the oral cavity to the rectum. Primary gastrointestinal lymphomas are most frequent in the stomach, followed by the small and large intestines. Here, we report a 41-year-old male patient who had been under chemotherapy following a diagnosis of diffuse large B-cell lymphoma transformed from MALT lymphoma and discuss new treatment options. He presented to us with an acute abdomen. To our knowledge, this was the first case in the literature that presented with acute abdomen and was diagnosed as diffuse large B-cell lymphoma that transformed from MALT lymphoma. In addition, for the treatment, we want to draw attention to the importance of surgery, in addition to chemotherapy.
