Objective: In 2003 the New South Wales (NSW) Centre for Rural and Remote Mental Health (CRRMH) conducted an analysis of co-morbid drug and alcohol (D&A) and mental health issues for service providers and consumers in a rural NSW Area Health Service. This paper will discuss concerns raised by rural service providers and consumers regarding the care of people with co-morbid D&A and mental health disorders. Design: Current literature on co-morbidity was reviewed, and local area clinical data were examined to estimate the prevalence of D&A disorders within the mental health service. Focus groups were held with service providers and consumer support groups regarding strengths and gaps in service provision. Setting: A rural Area Health Service in NSW. Participants: Rural health and welfare service providers, consumers with co-morbid D&A and mental health disorders. Results: Data for the rural area showed that 43% of inpatient and 20% of ambulatory mental health admissions had problem drinking or drug-taking. Information gathered from the focus groups indicated a reasonable level of awareness of co-morbidity, and change underway to better meet client needs; however, the results indicated a lack of formalised care coordination, unclear treatment pathways, and a lack of specialist care and resources. Discussion: Significant gaps in the provision of appropriate care for people with co-morbid D&A and mental health disorders were identified. Allocation of service responsibly for these clients was unclear. It is recommended that D&A, mental health and primary care services collaborate to address the needs of clients so that a coordinated and systematic approach to co-morbid care can be provided. Introduction: Alcohol is the most commonly used drug within Australia. Recently, there have been indications that there is a greater incidence of high-risk drinking within rural populations as compared with their urban counterparts. High-risk drinking is associated with numerous conditions, such as diabetes, heart attack and cancer, as well as acute harms such as assault, suicide and road accidents. The objective of this article is to review the current research and relevant data pertaining to alcohol use and alcohol-related harms within rural Australia. Methods: This paper is a systematic review of 16 databases, including PubMed, PsycINFO and Google Scholar. Results: Overall, 18 studies describing alcohol consumption or alcohol-related harms were found. Approximately half of these studies were large-scale national population surveys, which were therefore limited in their representativeness of specific regional and rural towns. Most studies examining alcohol consumption used self-report data collection, meaning that interpretation of results needs to be tentative. There is a consistent pattern of higher rates of alcohol consumption and consequent harm within regional and rural Australia than in urban areas. Conclusions: There is emerging research examining alcohol consumption and alcohol-related harms within regional and rural Australia. All studies show that these populations experience disproportionate harm because of alcohol consumption. The causes and mechanism for this have not been investigated, and a program of research is required to understand how and why rural populations experience disproportionate levels of alcohol-related harm and ultimately, what interventions will be most effective in reducing alcohol-related harms. Introduction: The Cardiff model is a data sharing approach that aims to reduce the volume of intoxicated patients in emergency departments (EDs). This approach has not been tested in a rural setting. Objective: This study assessed whether this approach would reduce the number of alcohol‐associated presentations during high‐alcohol hours (HAH) in a regional ED. Design: From July 2017, people over the age of 18 attending the ED were asked by the triage nurse (1) whether they had consumed alcohol in the past 12 h, (2) their typical alcohol consumption level, (3) the location where most alcohol was purchased and (4) the location of the last drink. From April 2018, quarterly letters were sent to the top five venues reported within the ED. Deidentified, aggregated data were shared with local police, licensing authorities and local government, identifying the top five venues reported in the ED and providing a summary of alcohol‐related attendances to the ED. Interrupted time series analyses were used to estimate the influence of the intervention on monthly injury and alcohol‐related ED presentations. Findings: ITS models found that there was a significant gradual decrease in the monthly rate of injury attendances during HAH (Coefficient = −0.004, p = 0.044). No other significant results were found. Discussion: Our study found that sharing last drinks data collected in the ED with a local violence prevention committee was associated with a small, but significant reduction in the rate of injury presentations compared with all ED presentations. Objective: Psychological distress and alcohol consumption are associated but few studies have focused on the association with problematic drinking in semi‐urban and rural settings. In this study, we assessed the role of psychological distress on various measures of problematic drinking in urban, semi‐urban and rural settings. Design: Retrospective descriptive study Setting Setting: National Drug Strategy Household Survey population health survey data Participants: 151,341 Australian residents Main outcome measures: Problematic alcohol consumption categorised as either heavy‐episodic drinking (defined as drinking 5 or more standard drinks at least once in the last 12 months) or long‐term risky drinking (defined as drinking more than 730 standard drinks in the past 12 months) was the outcome variables. Psychological distress was identified from the K10 questionnaire. Participants were grouped into area of residence: urban, semi‐urban and rural. Sex differences were also explored. Results: Poor‐to‐severe psychological distress was associated with higher odds of both long‐term and heavy‐episodic drinking. Being male alone increased the risk of both long‐term and heavy‐episodic drinking in all areas. When stratified by sex, being female and severe levels of psychological distress increased the risk of both long‐term risky and heavy‐episodic drinking. These risks were greater in non‐urban environments. Conclusions: Poor mental health outcomes may exacerbate problematic drinking amongst females, especially those living in non‐urban environments. Public health campaigns could both target these at‐risk groups of females in non‐urban settings and also account for the interplay between problematic drinking and mental health. Objectives: In this study, we aimed to determine the prevalence and risk factors of alcohol use among a cohort of population in Sabah. Design: This is a prospective, cohort study involving rural community residents. Setting: Rural community resident at Bingkor, Keningau, Sabah. Participants: 363 individuals aged 13 years old and above. Intervention: Community‐based participatory research to determine the prevalence and risk factors associated with alcohol use. Main outcome measures: Measurement of alcohol use using Alcohol Use Disorders Identification Test (AUDIT) and assessment of psychiatric morbidity using Mini International Neuropsychiatric Interview (MINI) questionnaires. Results: Most alcohol drinkers aged between 36‐45 years old, followed by 26‐35 years old and 46‐55 years old. Interestingly, there are almost similar female to male ratio. Most were Kadazan‐Dusun ethnic, non‐Muslims, and married. Although only less than a third of the participants received tertiary education, the majority were working. Based on the findings, being a male, non‐Muslim and having an obsessive‐compulsive disorder (OCD) (current) posed a significantly higher risk of alcohol consumption. Conclusion: A worryingly high prevalence of hazardous alcohol consumption among the locals is reported. There is a need for population‐wide intervention towards preventive measures based on the identified risk factors for hazardous alcohol use. [ABSTRACT FROM AUTHOR]

Objectives: To advance the rural practice in working with Aboriginal communities by (a) identifying the extent of community partners' participation in and (b) operationalising the key elements of three community‐based participatory research partnerships between university‐based researchers and Australian rural Aboriginal communities. Design: A mixed‐methods study. Quantitative survey and qualitative one‐on‐one interviews with local project implementation committee members and group interviews with other community partners and project documentation. Setting: Three rural Aboriginal communities in New South Wales. Participants: Thirty‐seven community partners in three community‐based participatory research partnerships of which 22 were members of local project implementation committees and 15 were other community partners who implemented activities. Intervention: Community‐based participatory research partnerships to develop, implement and evaluate community‐based responses to alcohol‐related harms. Main outcomes measures: Community partners' extent of and experiences with participation in the community‐based participatory research partnership and their involvement in the development and implementation processes. Results: Community partners' participation varied between communities and between project phases within communities. Contributing to the community‐based participatory research partnerships were four key elements of the participatory process: unique expertise of researchers and community‐based partners, openness to learn from each other, trust and community leadership. Conclusion: To advance the research practice in rural Aboriginal communities, equitable partnerships between Aboriginal community and research partners are encouraged to embrace the unique expertise of the partners, encourage co‐learning and implement community leadership to build trust. Objective To investigate if parental disapproval of alcohol use accounts for differences in adolescent alcohol use across regional and urban communities. Design Secondary data analysis of grade-level stratified data from a random sample of schools. Setting High schools in Victoria, Australia. Participants A random sample of 10 273 adolescents from Grade 7 (mean age = 12.51 years), 9 (14.46 years) and 11 (16.42 years). Main outcome measures The key independent variables were parental disapproval of adolescent alcohol use and regionality (regional/ urban), and the dependent variable was past 30 days alcohol use. Results After adjusting for potential confounders, adolescents in regional areas were more likely to use alcohol in the past 30 days ( OR = 1.83, 1.44 and 1.37 for Grades 7, 9 and 11, respectively, P < 0.05), and their parents have a lower level of disapproval of their alcohol use ( b = −0.12, −0.15 and −0.19 for Grades 7, 9 and 11, respectively, P < 0.001). Bootstrapping analyses suggested that 8.37%, 23.30% and 39.22% of the effect of regionality on adolescent alcohol use was mediated by parental disapproval of alcohol use for Grades 7, 9 and 11 participants respectively ( P < 0.05). Conclusions Adolescents in urban areas had a lower risk of alcohol use compared with their regional counterparts, and differences in parental disapproval of alcohol use contributed to this difference.
