Objective: To describe the Evidence-Based Practice (EBP) competency level in Primary Care (PC) nurses in Spain and to determine the associated factors., Design: Cross-sectional, national survey design, carried out between January and March 2020., Setting: PC in Spain., Participants: Seven hundred eighty PC active nurses in the National Health Service with at least one year of professional experience., Main Measurements: (1) Sociodemographic, professional and access to scientific information variables; (2) outcome variable: EBP competency (attitude, knowledge, skills and utilization) assessed through the EBP-COQ Prof© questionnaire. Bivariate and multiple lineal regression analyses were carried out., Results: The mean score for the EBP competency of the PC nurses was 131.5 (standard deviation [SD] 17.0), according to dimensions: attitude 36.8 (SD 3.6); knowledge 38.2 (SD 8.9); skills 23.0 (SD 3.5); and utilization 33.3 (SD 6.1). The number of articles read in the last month has showed the most influence on all the EBP-COQ Prof© dimensions, followed by EBP training (more than 150h) and nursing students mentoring. The education level (master, specialist and doctorate) is associated with knowledge and skills dimensions, meanwhile belonging to a BPSO® center is associated with the EBP utilization., Conclusions: These findings can guide PC service managers to plan strategies that improve the EBP competency level of the nurses, aimed mainly at achieving real application in clinical practice. Objectives: To analyse the reliability and validity of a scale of social evaluation of the elderly., Design: Descriptive, cross-sectional study., Setting: Primary care., Patients: Sample of 1062 people from the over-65 population., Interventions: The scale evaluated has five items (family situation, economic situation, housing, relationships and social support), and an overall score is obtained. Its reliability was evaluated by an interview with two observers, and validity by contrasting the score obtained on the scale with a reference criterion of an independent, blind assessment by a social work expert., Measurements and Main Results: The intraclass correlation coefficient (inter-observer reliability) was 0.957. The Cronbach alpha coefficient was 0.4467, which denoted moderate to low internal consistency. Sensitivity and specificity were calculated for the validity of the criterion. Nevertheless, to detect social problems in care practice, probability proportions for different levels on the scale were more useful. These ranged from 1 to 23, while in the detection of social risk they ranged from 1 to infinity., Conclusions: The scale studied by us as a measuring instrument enables risk situations and social problems to be detected with good reliability and acceptable validity. It should be introduced into the care practice of professionals working in the social or health care of the elderly.

Objectives: To find how much alcohol, tobacco and drugs adolescents consume and identify some linked socio-family variables., Design: A crossover and descriptive study, with randomised sampling stratified by classrooms., Setting: Secondary schools in the town of Lugo., Patients and Other Participants: 805 students of both sexes from the secondary schools., Measurements and Results: We used a self-filled anonymous semiclosed questionnaire. 34.6% habitually consumed alcohol, of whom 43.3% had got drunk at least once in the previous 6 months and 7.1% on more than 13 occasions. 25.7% were habitual smokers. 12.3% had consumed cannabis; 10.1% tranquilisers; 7.5% amphetamines; 4.6% sleeping pills; 2.1% cocaine; 1.9% LSD; 1.5% heroin. Variables linked to consumption were: age, repeating the school year, considering him/herself a bad student, fighting, consumption by both parents, not doing sports, having more money and not having a good relationship with parents., Conclusions: Adolescent alcohol, tobacco and drug consumption is high and similar to other regions of Spain. Prevention strategies are needed, in the family, at school and in the media.

Objectives: To analyse family dynamics among health service users. To establish a connection between perception of family dysfunction and different variables., Design: A descriptive crossover study, in line with a closed question survey model: a) family Apgar, b) 16 item questionnaire., Setting: Jumilla Health Centre, Murcia., Patients: 397 patients between 18 and 65 were selected by systematic sampling., Interventions: The Chi-squared statistical method and remainder analysis were used., Measurements and Results: a) Analysis of family perception: 315 normally functioning (79.35%) and 82 dysfunctional (20.65%). b) There were statistically significant connections between family dysfunction and the number of brothers and sisters and the place occupied by them, marital status, number of children, educational level, mental health and reason for the consultation., Conclusions: The family Apgar enables patients with family dysfunction to be identified. There is a clear connection between family dysfunction and the reasons for seeking health care.

Objective: To discover the attitude of tutors to tutoring and describe how they apply the teaching programme during the period interns spend at the Health Centre., Design: Crossover study. Stratified random sampling by Teaching Units., Setting: Primary Care Centres with teaching accreditation., Participants: 258 tutors from the state-wide Teaching Programme., Measurements and Main Results: Tutors' attitudes and the activities undertaken with the interns were studied by means of a self-filled questionnaire. The attitudes scale revealed a good attitude toward tutoring. Above all ability to motivate (73%) was required of the good tutor. Shared quotas was the commonest model of tutoring (85%). Clinical care was the activity the tutors developed best (70%), with the doctor-patient relationship in second place (46%). The majority discussed the Clinical History with their intern (75%). Few supervised the carrying out of procedures (36%). 68% of interns performed at least one piece of research, although tutors thought they should do more investigative work (41%). During the three intern years, the relationship was hardly ever maintained (13%)., Conclusions: Tutors' attitude to tutoring is adequate. There is agreement with the basic features of the Programme, which is best developed in the area of care. The general model is that of a shared quota. Teaching must be improved in the non-care aspects, with specific training being made available to tutors.

Objectives: To estimate the vaccination coverage of two-years-old children in the health district of Gijón (Spain), the number of incomplete vaccinations and their causes. To evaluate the vaccination registry coverage., Design: Descriptive cross-sectional survey. SITE. Primary health care., Patients: From the cohort born in 1989, living in the health district, a sample of 219 children was taken. Of these, criteria for inclusion in the study were met by 199 (90.8%), and the vaccination status was ascertained in 191 (87.2%)., Main Measurements and Results: The variables related with the vaccination coverage were studied through registrations or specific questionnaires. The full vaccination coverage with 3 doses of DTP, 1 dose of DT, 4 doses of VPO and one dose of MMR in the district was 92.5%. Estimating vaccination coverage using only our public registration systems, would lower it to 84.9. This result suggest that the public sector vaccination registry in Gijón under-registers immunizations. Vaccination coverage was 99.5% for first dose (3 months: DTP and VPO) and 93.5% for last doses (18 months: DT and VPO). For MMR it was 94.5%., Conclusions: Our data suggests there is a high vaccination coverage in the Gijón area. We consider population's questionnaires a satisfactory way to estimate vaccination coverage, and besides to evaluate the validity of the usual registers to use them as information resource.
