9. Grating visual acuity impairment assessed by sweep visually evoked potentials in children with optic pathway tumors unable to perform optotype acuity tests.

Purpose: To determine visual impairment due to optic pathway tumors in children unable to perform recognition acuity tests., Methods: Grating visual acuity scores, in logMAR, were obtained by sweep visually evoked potentials (SVEP) in children with optic pathway tumors. The binocular grating visual acuity deficit was calculated by comparison with age-based norms and then assigned to categories of visual impairment as mild (from 0.10 to 0.39 logMAR), moderate (from 0.40 to 0.79 logMAR), or severe (≥0.80 logMAR). Interocular differences were calculated by subtraction and considered increased if >0.10 logMAR., Results: The participants were 25 children (13 boys; mean ± SD age, 35.1 ± 25.9 months; median age, 32.0 months) with optic pathway tumors (24 gliomas and 1 embryonal tumor), mostly located at the hypothalamic-chiasmatic transition (n=21; 84.0%) with visual abnormalities reported by parents (n=17; 68.0%). The mean grating acuity deficit was 0.60 ± 0.36 logMAR (median, 0.56 logMAR). Visual impairment was detected in all cases and was classified as mild in 10 (40.0%), moderate in 8 (32.0%), and severe in 7 (28.0%) children, along with increased interocular differences (>0.1 logMAR) (n=16; 64.0%). The remarkable ophthalmological abnormalities were nystagmus (n=17; 68.0%), optic disc cupping and/or pallor (n=13; 52.0%), strabismus (n=12; 48.0%), and poor visual behavior (n=9; 36.0%)., Conclusion: In children with optic pathway tumors who were unable to perform recognition acuity tests, it was possible to quantify visual impairment by sweep-visually evoked potentials and to evaluate interocular differences in acuity. The severity of age-based grating visual acuity deficit and interocular differences was in accordance with ophthalmological abnormalities and neuroimaging results. Grating visual acuity deficit is useful for characterizing visual status in children with optic pathway tumors and for supporting neuro-oncologic management.

10. Visual outcomes of audio-luminous biofeedback training for a child with idiopathic nystagmus.

- Abstract
Microperimetry biofeedback training is a vision rehabilitation method that involves the training of attention and oculomotor control, and the rehabilitation of poorly located and non-functional preferred retinal loci. It can significantly improve distance and near visual acuity in age-related macular degeneration. Previous studies have shown that biofeedback training using electrical nystagmography can reduce nystagmus amplitude and increase foveation time. However, these improvements have not been sustained following training sessions. We hereby report a pediatric case of idiopathic nystagmus in an 11-year old patient treated with microperimetric biofeedback to improve visual acuity and fixation stability. The training had a beneficial impact, positively affecting fixation stability as well as distance and near reading vision. Subjectively, improvement in quality of life was also reported. Conversely to previous studies, the positive effects in this case were maintained for as long as twelve months following therapy. To the best of our knowledge, this is the first case with long-term benefits to be reported in the literature.

OBJETIVOS: Avaliar a confiabilidade interexaminadores, fazer o teste-reteste e verificar a consistência interna da avaliação da visão funcional para crianças com baixa visão de dois a seis anos de idade (AVIF-2 a 6 anos). MÉTODOS: Foram testados sete domínios: fixação visual, seguimento visual, campo visual de confrontação, coordenação olho-mão, visão de contraste, deslocamento no ambiente e visão de cores, em 40 crianças, 20 com baixa visão e 20 sem alterações visuais. A consistência interna foi examinada para o teste de todas as crianças e a confiabilidade interexaminadores e teste-reteste para 12 delas. RESULTADOS: Seis domínios apresentaram ao teste-reteste coeficiente de correlação intraclasse com valores de 0,5361 a 1,000. Para o domínio deslocamento no ambiente esse coeficiente foi de 0,37. Na confiabilidade interexaminadores, somente o campo visual de confrontação apresentou valor abaixo do esperado (0,3901). O coeficiente kappa ponderado obteve resultados que variaram de -0,087 a 1,000. O valor do alfa de Cronbach variou de 0,584 (no deslocamento no ambiente) a 0,973 (na visão de contraste). CONCLUSÃO: A AVIF-2 a 6 anos apresentou bons índices de confiabilidade, mas a confiabilidade dos domínios, quando analisados isoladamente, precisa ser aprimorada. PURPOSE: The functional vision assessment (AVIF-2 to 6 years) for low vision children with ages from 2 to 6 years old was analyzed. The inter-rater, test-retest and the internal consistency were verified for reliability. METHODS: The test, composed by seven domains (visual fixation, visual following, visual field confrontation, eye-hand coordination and surrounding locomotion, contrast vision and color vision discrimination), was applied to 40 children, 20 with low vision and 20 without low vision. The inter-rater and test-retest reliability were analyzed with 12 children and the internal consistency with 40 children. RESULTS: Concerning test-retest reliability, six domains exhibited intra-class correlation coefficients ranging from 0.5361 to 1.000. For the movement within surroundings category, this coefficient was 0.37. For inter-rater reliability, only the category of confrontation visual field was bellow expectations (0.3901). The weighted kappa coefficient exhibited values ranged from -0.087 to 1.000. Cronbach's alfa ranged from 0.584 (movement within surroundings) to 0.973 (contrast sensitivity). CONCLUSION: AVIF-2 to 6 years presented good reliability indexes, however a few items within the domains need minor adjustments in order to improve scores' consistency of some items.

OBJETIVO: Analisar a qualidade de vida em crianças com deficiência visual atendidas em Ambulatório de Estimulação Visual Precoce da Unifesp em duas situações, antes e após intervenções reabilitacionais pela equipe multiprofissional. MÉTODOS: Aplicou-se o questionário de qualidade de vida QFVI. O instrumento possui uma versão para crianças menores de três anos e outra para crianças maiores de três anos (três a sete anos) e é dividido em seis domínios: Saúde geral; Saúde geral da visão; Competência; Personalidade; Impacto familiar e Tratamento. RESULTADOS: A correlação entre as subescalas nos dois momentos mostrou-se significativa. Houve diferença estatisticamente significativa em saúde geral da visão (p=0,029) e outras diferenças importantes foram obtidas em saúde geral, impacto familiar e qualidade de vida geral. DISCUSSÃO: Este instrumento mostrou-se eficiente na aferição da qualidade de vida relacionada à visão nas famílias que frequentam esse ambulatório. As intervenções multidisciplinares promoveram melhora da visão funcional da criança e qualidade de vida da família como um todo. CONCLUSÃO: A qualidade de vida relacionada à visão de crianças atendidas no Ambulatório de Estimulação Visual Precoce da Unifesp apresentou melhora significativa na saúde geral da visão. PURPOSE: To evaluate the quality of life in visually impaired children followed in the Early Visual Stimulation Ambulatory of Unifesp in two moments, before and after rehabilitational intervention of multiprofessional team. METHODS: A CVFQ quality of life questionnaire was used. This instrument has a version for less than three years old children and another one for children older than three years (three to seven years) divided in six subscales: General health, General vision health, Competence, Personality, Family impact and Treatment. RESULTS: The correlation between the subscales on two moments was significant. There was a statistically significant difference in general vision health (p=0,029) and other important differences obtained in general health, family impact and quality of life general score. DISCUSSION: The questionnaire showed to be effective in order to measure the quality of life related to vision on families followed on this ambulatory. The multidisciplinary interventions provided visual function and familiar quality of life improvement. CONCLUSION: The quality of life related to vision in children followed in Early Visual Stimulation Ambulatory of Unifesp showed a significant improvement on general vision health.

Objetivo: Traçar o perfil dos pacientes portadores de baixa visão atendidos no serviço de visão subnormal do Instituto Brasileiro de Oftalmologia e Prevenção da Cegueira (IBOPC). Métodos: Realizou-se um estudo transversal no qual foram revisados 82 prontuários de pacientes com baixa visão, atendidos pelo Serviço Único de Saúde (SUS) no primeiro ano do departamento de Visão Subnormal do Instituto Brasileiro de Oftalmologia e Prevenção da Cegueira em 2004. Os dados analisados foram: sexo, faixa etária, etiologia, acuidade visual inicial e final (pós-refração) para longe e perto, com e sem auxílio óptico e recurso óptico indicado. Resultados: Dos 82 pacientes avaliados, 11 (13,4%) foram excluídos do trabalho por não apresentarem visão subnormal. Dos 71 pacientes restantes, 32 (45%) eram menores de 20 anos. Quanto ao sexo, não houve diferença estatisticamente significante (51% eram mulheres e 49% eram homens). A etiologia mais frequente em crianças e adolescentes foi o glaucoma congênito (15,6%). Em pacientes de 20 a 39 anos prevaleceu a toxoplasmose ocular (21,1%). Entre os pacientes de 40 a 59 anos, a retinose pigmentar foi a patologia mais frequente (19%). Nos idosos, o glaucoma foi a patologia mais encontrada (40%). Trinta e três pacientes (40,2%) tinham acuidade visual entre 20/60 e 20/160. O sistema telescópico foi o único auxílio óptico indicado para longe (44%) e os óculos foram os mais indicados para perto (54,5%). Conclusão: Existe uma alta prevalência de baixa visão em crianças e adolescentes em países em desenvolvimento como o Brasil. Desta forma o oftalmologista precisa criar programas preventivos, melhorar as condições de atendimento e atentar para o diagnóstico precoce e o tratamento destes pacientes.Purpose: To outline low vision (LV) patients profile of the low vision department of the Instituto Brasileiro de Oftalmologia e Prevenção da Cegueira (IBOPC). Methods: Transversal study of 82 medical charts from the low vision service at the Instituto Brasileiro de Oftalmologia e Prevenção da Cegueira of the first year of the department - 2004, considering age, gender, etiology, distance and near visual acuity with and without optical aids and prescription assistance. Results: Of the 82 available patients, 11 (13.4%) were excluded of the study because they did not present low vision. Of the 71 patients, 32 (45%) were younger than 20 years old. Regarding the gender, there was not a significant difference (51% female and 49% male). The most frequent etiology in infants and adolescents was congenital glaucoma and in patients aged between 20-39 years was ocular toxoplasmosis (21.1%). In patients with 40-59 years old, pigmentary retinosis was the most frequently pathology (19%). In elderly people it was glaucoma (49%). Thirty-three patients (40.2%) had visual acuity between 20/60 and 20/160. The telescopic system was the only optical aid indicated for distance (44%) and glasses were the most indicated for near (54.5%). Conclusion: There is a high prevalence of low vision in infants and adolescents that live in developing countries like Brazil. Therefore, ophthalmologists need to be aware to promote early diagnosis and treatment to these patients, creating preventive programs and better assistance conditions.

OBJETIVOS: Traçar o perfil do usuário da Sala de Recursos para Deficiente Visual na cidade de Assis - SP, avaliar a porcentagem de deficientes visuais empregados e comparar o emprego com idade, gênero, tipo de deficiência e doença causadora da deficiência, entre dois períodos: de 1984 a 1996 e de 1997 a 2009. MÉTODOS: Foi realizado estudo retrospectivo dos prontuários médicos e da ficha escolar dos portadores de deficiência visual que frequentaram a Sala de Recursos para Deficiente Visual, no período de 1984 a 2009 na cidade de Assis - SP, divididos em dois períodos de 1984 a 1996 (G1) e de 1997 a 2009 (G2). Analisaram-se dados demográficos, a doença que provocou a baixa visão, o tipo de deficiência visual (cegueira ou baixa visão), escolaridade, recursos ópticos, frequência à Sala de Recursos para Deficiente Visual e taxa de emprego. Foi feita associação da taxa de emprego com: idade, gênero, raça/cor, tipo de deficiência e doença nos dois diferentes períodos. RESULTADOS: Foram encontrados 149 deficientes visuais sendo: 61,07% homens, 38,9% mulheres, 82,5% brancos e 17,4% não brancos e a mediana da idade foi de 18 anos. Dos 149 deficientes visuais, 63,75% eram portadores de baixa visão e 36,24% portadores de cegueira. As principais doenças que levaram à deficiência visual dos 149 pacientes foram em ordem decrescente: retinocoroidite por toxoplasmose (17,40%), atrofia óptica congênita (12,10%), alta hipermetropia (8,72%), retinose pigmentar e alta miopia (6,71% cada uma) e glaucoma congênito e catarata congênita (6,04% cada uma). A frequência à Sala de Recursos para Deficiente Visual foi boa em mais de 50% dos pacientes. Estavam trabalhando regularmente 44,7% e 12,3% dos maiores de 14 anos respectivamente nos períodos de 1984 - 1996 e de 1997 - 2009. Não houve diferença entre os dois períodos quanto às características demográficas, tipo de deficiência e doenças, havendo correlação da taxa de emprego apenas com a idade (pacientes com média de idade maior apresentavam maior porcentagem de emprego). CONCLUSÕES: Os deficientes visuais eram na maioria homens, brancos, portadores de baixa visão e portadores de toxoplasmose ocular. Quase metade (44,7%) dos pacientes com mais de 14 anos encontrava-se trabalhando regularmente até 1996 sugerindo que esforços conjuntos de oftalmologistas e educadores auxilia na inclusão social destes pacientes. A taxa de emprego caiu no período de 1997 a 2009 e a de aposentados aumentou.PURPOSES: To establish the visually impaired resource class users profile in the city of Assis - SP, to estimate the visually impaired employment percentage and to compare employment rate with age, gender, deficiency type and causing disease between two periods:1984 - 1996 and 1997 - 2009. METHODS: Retrospective review of both medical and school records from visually impaired individuals attending visually impaired resource class in Assis - SP between 1984 and 2009. This time period was divided into two intervals: 1984 - 1996 (G1) and 1997 - 2009 (G2). Collected data included demographic features and diseases, type of visual impairment (blindness or low vision), education, optic aids, attendance to visually impaired resource class and employment rate. Employment rate was associated with age, gender, type of visual impaiment and causing disease between both study time intervals. RESULTS: A total of 149 visually impaired individuals were identified - 61.07% male, 38.9% female, 82.5% white, and 17.4% non-white. Mean age was 18 years. Of the 149 visually impaired, 63.75% had low vision and 36.24% were blind. In decreasing order, the more frequent conditions leading to visual impairment were toxoplasmosis retinochoroiditis (17.40%), congenital optic atrophy (9.39%), high hyperopia (8.72%), pigmentary retinosis and high myopia (with 6.71% each) and congenital glaucoma and congenital cataract (with 6.04% each). Attendance to visually impaired resource class was good by over 50% of the patients. Of the individuals >14 years-old, 44.7% and 12.3% were regularly employed during 1984 - 1996 and 1997 - 2009 periods, respectively. Employment rate only correlated with age (individuals with higher mean age were most frequently employed) and there was not correlation with demographic characteristics, deficiency type and causing diseases. CONCLUSIONS: The main characteristics of the individuals attending the visually impaired resource class are: male, white, with low vision and ocular toxoplasmosis. Nearly half of the individuals older than 14 years-old (44.7%) were regularly employed up to 1996, suggesting that the joint effort of ophthalmologists and teachers helps the social inclusion of these patients. Employment rate dropped over 1997 - 2009 period, and the rate of retirees increased.

A case of identical male twins with Cohen syndrome who present multiple ophthalmic findings is reported. The patients were identical 16 year-old twin boys who showed down slanting eyelids, mild ptosis, high-grade myopia, small cortical lens opacities, posterior subcapsular cataracts, myotic and corectopic pupils with poor dilation due to focal iris atrophy and retinochoroidal dystrophy. Ophthalmologists must be aware of the ocular and systemic findings of Cohen syndrome in the evaluation of young patients with mental retardation and visual impairment.Relata-se caso de gêmeos idênticos com síndrome de Cohen que apresentam múltiplos achados oftalmológicos. Os pacientes eram gêmeos idênticos, do sexo masculino, que apresentavam pálpebras em forma de onda, ptose moderada, alta miopia, opacidades cristalinianas corticais discretas, catarata subcapsular posterior, pupilas mióticas e corectópicas com pobre dilatação devido à atrofia focal de íris, além de distrofia retinocoroidiana. Os oftalmologistas devem estar atentos quanto aos achados oftalmológicos e sistêmicos da síndrome de Cohen na avaliação de pacientes jovens com retardo mental e baixa visão.

OBJETIVOS: Identificar os sentimentos e reações de mães perante ao diagnóstico de baixa visão do filho e a sua compreensão do diagnóstico. MÉTODOS: Este estudo caracteriza-se como uma pesquisa qualitativa. Foram entrevistadas 11 mães de crianças com baixa visão atendidas pelo Serviço de Estimulação Visual do Setor de Oftalmologia Hospital de Clínicas da Unicamp. Para a coleta dos dados foram utilizadas entrevistas semiestruturadas. Os tópicos principais da entrevista foram: sentimentos diante do diagnóstico e a compreensão do diagnóstico da baixa visão. RESULTADOS: As mães apresentaram medo de que o filho fique cego e compreensão não clara da baixa visão, interpretando o diagnóstico médico de acordo com a sua subjetividade. CONCLUSÕES: A maneira como o diagnóstico é transmitido para as famílias é de extrema importância. As condições culturais, econômicas e emocionais dos pais, na maioria das vezes podem impedir a compreensão clara do diagnóstico.PURPOSE: To identify mothers' feeling and reactions when facing the diagnosis of low vision and their comprehension of this diagnosis. METHODS: This study features a qualitative research. Eleven low vision children's mothers attended at the Visual Stimulation Service of Ophtalmology Sector of Unicamp Hospital were interviewed. Semi-strutured interviews were used to collect data. The main issues of the interview were: feelings when faced with the diagnosis and comprehension of this low vision diagnosis. RESULTS: The results revealed mothers' fear of blindness and a low comprehension of the meaning of low vision, and their interpretation of medical diagnosis according to their subjectivity. CONCLUSIONS: The way the diagnosis is transmitted to the families is extremely important. Parents cultural, economical and emocional background may often impair the diagnosis comprehension.

Paciente de 22 anos, sexo masculino, notou mancha central no olho esquerdo durante um episódio da cefaléia de forte intensidade há aproximadamente 6 meses. Referia diagnóstico de enxaqueca com aura, apresentando crises recorrentes desde os 15 anos de idade. O exame oftalmológico revelou perda da camada de fibras nervosas da retina na região do feixe papilomacular do olho esquerdo. A tomografia de coerência óptica demonstrou importante redução da espessura macular, acompanhada de redução localizada da camada de fibras nervosas da retina no setor temporal do disco óptico no olho esquerdo. Avaliação sistêmica, exames de imagem e avaliação cardiovascular nada revelaram e a causa da perda da perda visual foi atribuída a possível enxaqueca retiniana, embora um evento isquêmico de outra natureza não pudesse ser descartado. Nosso caso é interessante porque demonstra que a tomografia de coerência óptica pode ser útil na detecção de perda neural localizada não apenas pela medida da camada de fibras nervosas da retina, mas também pela avaliação da espessura macular.A 22-year-old man developed a central scotoma in the left eye during an episode of severe headache 6 months previously. He had a diagnosis of migraine headaches with aura since age 15. Ophthalmic examination showed retinal nerve fiber layer thickness reduction on the papilomacular bundle in the left eye and unremarkable in the right. Optical coherence tomography documented severe macular thickness reduction and a mild and localized retinal nerve fiber layer loss on the temporal side of the optic disc in the left eye. Systemic investigation, neuroimaging and cardiovascular studies were non-revealing. The cause of his visual loss was possibly related to retinal migraine, although an ischemic episode of another origin could not be excluded. Our case is interesting because it draws attention to the fact that optical coherence tomography may be able to identify localized neural loss not only by measuring peripapillary retinal nerve fiber layer but also by means of macular thickness measurements.

OBJETIVO: Desenvolver textos para medida da velocidade de leitura comparáveis com outros quatro idiomas europeus. MÉTODOS: Dez textos com similar grau de dificuldade, número de caracteres, número de palavras e sintaxe (segundo a teoria de Gibson) foram desenvolvidos por um especialista em lingüística, em português (BR), fazendo-se a tradução de textos previamente padronizados em quatro idiomas: inglês, francês, finlandês e alemão. A velocidade de leitura foi medida em 25 indivíduos saudáveis de idade entre 19 e 35 anos (mediana=24 anos) com os dez textos. RESULTADOS: A velocidade de leitura nos textos em português foi em média 1100 ± 167 (desvio padrão) caracteres por minuto. Pequenas diferenças foram encontradas entre as velocidades de leitura medidas com os dez textos, e essas diferenças não foram estatisticamente significantes em grupos de no mínimo seis textos. A velocidade de leitura dos voluntários da mesma faixa etária nos outros idiomas foi: alemão: 1126 ± 105; finlandês: 1263 ± 142; francês: 1214 ± 152 e inglês: 1234 ± 147. CONCLUSÃO: Os autores desenvolveram um conjunto de textos em português (BR) padronizados e homogêneos para medida da velocidade de leitura, que são comparáveis com textos em outros quatro idiomas europeus. Esses textos podem ser usados para estudos multicêntricos internacionais envolvendo leitura e visão subnormal.PURPOSE: To develop standardized texts in Brazilian Portuguese to assess reading speed and compare performances among four European languages. METHODS: 10 texts were designed by a linguistic expert at the level of a sixth grade reading material (reading ages 10-12 years) and were matched for length and syntactic complexity, according to the syntactic prediction locality theory of Gibson. Normally sighted native speaking volunteers aged 18-35 years read each text aloud at random. RESULTS: The reading speed was on average 1100 ± 167 (standard deviation) characters per minute. Only small differences were found between the measured reading speeds of the 10 texts, and these differences were not statistically significant in groups of at least 6 texts. The mean reading speed in age-matched volunteers in Finnish was: 1263 ± 142; French: 1214 ± 152; English: 1234 ± 147; and German: 1126 ± 105. CONCLUSIONS: The authors developed a set of standardized, homogeneous, and comparable texts in Brazilian Portuguese. These texts will be a valuable tool to measure reading speed for repeated measurements and in international studies in the field of reading and low vision research.

OBJETIVOS: Pesquisa experimental, com laser de diodo infravermelho, para estimar a segurança, a reprodutibilidade e a permeabilidade da parede ocular à sua atuação clínica, quando aplicado via transescleral, em condições de baixa visibilidade. MÉTODOS: Submetemos olhos de coelhos pigmentados da raça Nova Zelândia à fotocoagulação retiniana por laser de diodo infravermelho. No olho direito, realizamos fotocoagulação via transescleral sob parâmetros de potência e tempo pré-determinados clinicamente. No olho esquerdo, foram repetidos os mesmos valores da potência e tempo usados no olho direito, desta vez, via transpupilar. Imediatamente e após 2 meses, estudos clínicos baseados na retinografia e histopatológicos foram realizados. RESULTADOS: A permeabilidade da parede ocular, quando da aplicação do laser de diodo infravermelho via transescleral, variou entre 58,95 e 63,87%. A média da permeabilidade da parede ocular a 300 mW (63,14%) mostrou-se significativamente superior àquela da permeabilidade da parede ocular encontrada a 500 mW (59,11%), (PPURPOSE: Retinal photocoagulation under poor visualization condition is often required. Transscleral infrared laser can be used as an alternative to regular transpupillary treatment. Based upon retinographic measurements, we proposed to estimate the reproducibility as well as ocular wall permeability rate for this treatment. Our primary goal was to evaluate whether this technique can deliver adequate photocoagulation at predetermined parameters without direct retinal visualization. METHODS: In New Zealand pigmented rabbits, optimal transscleral infrared diode laser settings were administered to the right eye. With the same parameters, transpupillary photocoagulation was repeated in the left eye. Retinographic and clinical examinations were performed immediately and two months later. RESULTS: Ocular wall permeability rate varied between 58.95 and 63.87%. OBJETIVO: Avaliação preliminar na Clínica Oftalmológica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (USP) de equipamento inovador para baixa visão desenvolvido na USP que consiste em prancha de leitura acoplada a lente de aumento que mantém fixos o foco e a linha de leitura. MÉTODOS: Foram avaliados 9 pacientes com visão subnormal usando o supracitado equipamento prancha de leitura acoplada a lente para auxílio em visão subnormal desenvolvida na USP em comparação com uso de uma lupa teste manual de dioptria semelhante, considerando-se os seguintes parâmetros de avaliação: causa da doença, acuidade visual corrigida no melhor olho para longe, impressão pessoal do paciente comparando prancha de leitura e lupa teste, impressão dos autores observando o uso dos 2 auxílios acima citados. RESULTADOS: A preferência pelos recursos foi: prancha de leitura acoplada a lupa - 5 pacientes; lupa manual - 2 pacientes; sem preferência por nenhum dos recursos - 1 paciente; inadequados para avaliação dos recursos preferidos - 1 paciente. CONCLUSÃO: Neste estudo preliminar, a maioria dos pacientes avaliados preferiu o recurso prancha de leitura acoplada à lupa, o que mostra que este produto inovador facilita a leitura; o médico avaliador interpreta a opinião do paciente como um especialista e contribui para o aprimoramento do produto para que ele possa ser futuramente submetido a novas avaliações.ABSTRACT PURPOSE: To perform a preliminary evaluation at the "Clínica Oftalmológica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (USP)" of an innovative equipment for low-vision developed at USP containing a reading stand and a magnifier that maintains in a stable position the reading line and focus. METHODS: 9 low-vision patients were evaluated using the above mentioned reading stand and a magnifier developed at USP comparing it with a hand magnifier of similar power taking into account the following evaluation parameters: etiology of low-vision, best corrected visual acuity for distance, patient's opinion comparing both low-vision aid resources, authors' opinion observing the patient using both low-vision aids. RESULTS: The numerical preference for the low-vision aids was: 5 patients for the reading stand and a magnifier that maintains at a stable position the reading line and focus; 2 patients for the hand magnifier; 1 patient indifferent regarding any of the resources; 1 patient inadequate for the evaluation of the low-vision aids. CONCLUSION: This preliminary study shows the preference of the majority of the patients for the reading stand and a magnifier that maintains at a stable position the reading line and focus showing that this innovative product makes reading easier; the evaluator doctor translates the patient's opinion as an expert and contributes to the improvement of the product so that it can be evaluated again in the future.

PURPOSE: To develop in an interdisciplinary approach between ophthalmology and design areas an ergonomic +22-diopter 50-mm aspheric hand magnifier for low vision. METHODS: An aluminum cylinder was cut, processed using a lathe and carved to produce a ring that accommodated a 50-mm aspheric lens, with an external depression not to slide from the holder's fingers. A cylindrical steel bar was cut, processed using a lathe and carved in order to form an externally turned ring to be screwed into the internal thread of the aluminum ring, to maintain the lens in a steady position. Both rings were submitted to electrostatic painting with a dull black electrostatic Epoxi ink, except the lower border of the external ring, to indicate the correct side of the magnifier to face the material to be read. RESULTS: A 22-diopter 50-mm diameter aspheric lens magnifier with a black ring to be hold at its external circular depression was obtained in order to safely search the adequate reading focus with an inferior aluminum colored ring to face the object to be read and allow a less distorted reading. This is the first Brazilian high-magnification great-diameter magnifier for low vision that permits basically the focusing on an entire word, not only syllables, in order to allow a faster and more comfortable reading. CONCLUSIONS: By an interdisciplinary approach a 22-diopter 50-mm aspheric lens magnifier was developed with image and ergonomic characteristics such as to permit comfortable and adequate reading performance in cases of low vision.OBJETIVO: Desenvolver de modo interdisciplinar entre as áreas de oftalmologia e design uma lupa de mão ergonômica de +22 dioptrias de 50 mm de diâmetro asférica para baixa visão. MÉTODOS: Um cilindro de alumínio foi cortado, torneado e teve feita internamente uma rosca a fim de produzir um anel para acomodar uma lente asférica de 50 mm de diâmetro com uma depressão externa para não escorregar dos dedos do portador. Uma barra de aço cilíndrica foi cortada, torneada e teve uma rosca feita para formar um anel com fio de rosca externo para ser rosqueado na parte interna do anel de alumínio, para manter a lente em posição estável. Ambos anéis foram submetidos a pintura eletrostática preta fosca exceto a borda inferior do anel externo para sinalizar o lado correto da lupa a ficar voltado para o material a ser lido. RESULTADOS: Foi obtida uma lupa de 22 dioptrias de 50 mm de diâmetro com um anel preto para ser segurado pela sua depressão circular externa a fim de procurar com segurança o foco adequado de leitura e com um anel inferior aluminizado para apontar para o objeto de leitura e permitir uma leitura menos distorcida. Esta é a primeira lupa brasileira de grande diâmetro e grande aumento para baixa visão, asférica, permitindo a focalização de uma palavra inteira, não apenas sílabas, proporcionando leitura mais rápida e confortável. CONCLUSÃO: Foi desenvolvida de modo interdisciplinar uma lupa de +22 dioptrias, de 50 mm de diâmetro, asférica, com tal qualidade de imagem e características ergonômicas para permitir leitura de desempenho adequado e confortável em casos de baixa visão.

OBJETIVO: Avaliar as causas de baixa visão e cegueira em indivíduos facectomizados, de amostra da população de cidades da região centrooeste do estado de São Paulo. Métodos: Estudo transversal, observacional, feito em cinco cidades da região centro-oeste do estado de São Paulo, em amostra domiciliar e baseada nos dados do último Censo Demográfico (IBGE, 1995), com escolha sistemática dos domicílios. Foi considerada para o presente estudo uma subamostra de indivíduos facectomizados, dos quais foram obtidos dados de identificação e exame oftalmológico completo. Os dados foram avaliados por estatísticas descritivas, análise de freqüência de ocorrência e proporção de concordância, com intervalo de confiança de 95%. RESULTADOS: Dos indivíduos amostrados, 2,37% haviam sido submetidos à facectomia. Dos 201 olhos operados, 26,9% apresentavam acuidade visual compatível com cegueira ou deficiência visual. Com a melhor correção óptica, a acuidade visual permaneceu PURPOSE: To determine the outcomes and causes of visual impairment and blindness in cataract-operated patients who are living in the central-western zone of São Paulo state. METHODS: A transversal, observational, systematic study was done involving patients inhabitants of five cities from a central-western zone of São Paulo state. The random sample was based on the Demographic Census Data (IBGE, 1995). All patients underwent to visual screening and complete eye examination. The individuals who had cataract-operated eyes were separated for the present study. The results were statistically analyzed by descriptive methods, frequency of occurrence and concordance proportion with 95% confidence intervals. RESULTS: At examination 2.37% of the subjects had cataract-operated eyes. Of the 201 operated eyes 26.9% had visual acuity compatible with blindness or visual impairment. The visual acuity persisted

OBJETIVO: A Fundação Altino Ventura realizou o projeto "Enxergando através das mãos", com o objetivo de investigar a situação atual da múltipla deficiência no estado de Pernambuco, verificar a demanda reprimida nesta área e analisar relevância para implantação de um centro especializado de referência para o diagnóstico e tratamento desses pacientes. MÉTODOS: Foi avaliado o perfil de um grupo de pacientes atendidos em cinco serviços de referência do grande Recife. Os casos foram avaliados quanto a ocorrências perinatais, inclusão social e dificuldades de acesso aos serviços especializados. Promoveu-se o atendimento oftalmológico especializado e o tratamento indicado para cada caso. O projeto foi desenvolvido de forma prospectiva, seguindo protocolo e organograma previamente elaborados. Os atendimentos foram realizados em seis dias não consecutivos, durante os quais todos os casos passavam pelas etapas de entrevista dos genitores e exame oftalmológico completo. Durante o período de espera entre entrevista e exame oftalmológico os pacientes realizavam atividades lúdicas com artistas plásticas. Contou-se com equipe multidisciplinar de 31 profissionais. RESULTADOS: Foram atendidos 309 pacientes. Estavam incluídos na sociedade 83,1% dos pacientes, apenas 45,4% freqüentavam escola (normal ou especial). A dificuldade de acesso aos serviços na área de oftalmologia foi relatada por 51,1% dos casos. Foram identificadas 274 crianças com múltiplas deficiências, destas, 154 (56,2%) nunca haviam sido examinadas pelo oftalmologista e 33% tinham baixa de visão (inferior à 20/60 na tabela de Snellen). Os diagnósticos oftalmológicos mais freqüentes foram: erro refracional, estrabismo, ambliopia, alterações do nervo óptico (glaucoma e atrofia), e catarata. Foram prescritos e doados 84 óculos. CONCLUSÃO: O projeto foi pioneiro na região, permitindo a obtenção de importantes dados quanto à problemática da deficiência visual e/ou múltipla, possibilitando o acesso ao oftalmologista e tratamento especializado dos casos.PURPOSE: The Project "Vision through the hands" was performed by the Altino Ventura Foundation, with the purpose of investigating the present child situation with multiple disabilities in the state of Pernambuco, Brazil. METHODS: Birth and social conditions, social problems, and difficulty in accessing the specialized services were evaluated, aiming to collect relevant data with the goal of posterior implantation of a specialized diagnosis and treatment center. Ophthalmologic care and proper treatment were offered. RESULTS: Three hundred and nine children from five reference institutions in disabilities were attended, on six non consecutive days. All cases underwent ophthalmologic examinations and parent interviews. During the waiting time between the ophthalmologic visits and parent interviews the patients performed activities with artists. A multidisciplinary team of 31 professionals was available. According to parent information 83.1% of the patients were considered included in the society and only 45.4% attended school (normal or special). The difficulty to get access to ophthalmologic service was reported by 51.1%. Two hundred and seventy children with multiple disabilities were indentified, 154 (56.2%) of whom underwent an ophthalmologic examination for the first time, and 33.0% had low visual acuity (inferior to 20/60 Snellen). The most frequent diagnoses were: refraction error, strabismus, ambliopia, alterations of optic nerve (glaucoma and atrophy), and cataract. Eighty-four glasses were prescribed. CONCLUSION: This project was pioneer in the region, allowing to obtain the information about important problems regarding multiple disabilities, and making possible ophthalmologic care and specialized treatment to the children.

OBJETIVOS: Determinar quais auxílios de visão subnormal podem ser úteis na reabilitação de pacientes com retinose pigmentária e os benefícios adquiridos com o programa de reabilitação visual baseado em medidas de acuidade visual nas tarefas da vida diária. MÉTODOS: Participaram deste estudo 30 pacientes com retinose pigmentária com idade variando de 7 a 73 anos. Foram realizados testes de acuidade visual e de função visual (perimetria manual, eletrorretinograma de campo total) e testes de adaptação de auxílios de visão subnormal. Um histórico visual foi pesquisado e perguntas específicas da utilização da visão foram feitas. Foi efetuado programa de treinamento do auxílio óptico a ser adaptado e de manuseio do mesmo antes de sua prescrição final. RESULTADOS: A acuidade visual no melhor olho variou de MM (movimentos de mão) a 20/40 para longe e visão melhor que 16M a 0,5M para perto. Do total de pacientes, 90% receberam prescrição de auxílio óptico ou eletrônico: treze para perto, nove para longe, dois auxílios eletrônicos e três lentes filtrantes. Três pacientes com grave constrição de campo visual e profunda baixa de acuidade visual foram encaminhados para orientação e mobilidade. CONCLUSÕES: Os auxílios ópticos: os telescópios, as lupas manuais e de apoio, as lentes esferoprismáticas, os auxílios não ópticos, os auxílios eletrônicos e iluminação adequada foram úteis na reabilitação visual de pacientes com retinose pigmentária, podendo trazer benefícios como a melhora da acuidade visual e da eficiência visual. A prescrição dos auxílios ópticos possibilitou a realização de atividades da vida diária, antes prejudicada pela deficiência visual, sendo relatado alto nível de satisfação por parte dos pacientes, com o programa de reabilitação visual implementado.PURPOSE: To determine which low-vision aids could be useful to patients with retinitis pigmentosa and also the benefits that the rehabilitation program could provide based on visual acuity and/or daily visual tasks. METHODS: A group of 30 patients with retinitis pigmentosa aged from 7 to 73 years were enrolled in this study. Visual acuity and visual function tests (visual field, full-field electroretinogram) was performed and low-vision aids tested. Information about the use of the remaining vision was obtained. After choosing the best optical or electronic devices and before their prescription, a low-vision training program was carried out. RESULTS: The best corrected visual acuity varied from HM (hand movements) to 20/40 for distance and visual acuity better than 16M to 0.5M for near. 90% of the patients had optical devices prescribed: 13 for near, 9 for distance, 2 electronic devices and 3 filters. Three patients with extremely narrow visual field and very low visual acuity were referred to orientation and mobility. CONCLUSIONS: The low-vision aids were useful for the retinitis pigmentosa patients: telescopes, hand-held magnifiers, stand magnifiers, half-eye base-in prism lenses, electronic devices and illumination control were beneficial to enhance visual acuity and visual efficiency. The prescription of low-vision aids was helpful in daily-life activities and a high level of satisfaction with the implemented visual rehabilitation program was reported.

PURPOSE: To identify conceptions, social and cultural factors regarding the use of optical aids by visually impaired students and to present information to health and educational professionals. METHODS: Qualitative research using spontaneous theater (interactive theater modality based on improvisation) as research instrument. To analyze data, an adapted form of the collective subject discourse technique - procedures for organization of verbal data - was applied. Scenes, gestures, expressions, silences and behaviors were added to the original proposal. The study population included all visually impaired students from elementary public schools, aged 10 to 14 years who attended a resource room in a São Paulo state city. The students were examined at a university low vision service. RESULTS: Little knowledge about the impairment and difficult adaptation to use of optical aids were identified. The students' behavior showed denial of own problems, discomfort on public use of aids and lack of participation in own health decisions. CONCLUSION: Analysis through spontaneous theater session allows the professional to gather information which is not possible to acquire in the health assistance atmosphere. Needs, difficulties and barriers the users found before the prescribed treatment were identified.OBJETIVO: Identificar concepções e verificar aspectos socioculturais a respeito do uso de auxílios ópticos por escolares deficientes visuais e oferecer informações a profissionais das áreas de saúde e educacional. MÉTODOS: Utilizou-se pesquisa qualitativa mediante aplicação da técnica do teatro espontâneo - modalidade de teatro interativo, de improviso, construído a partir de histórias contadas pelos participantes. Para análise das informações colhidas, foi adaptada a técnica de análise do discurso do sujeito coletivo, conjunto de procedimentos de organização de dados discursivos de natureza verbal. Por se tratar de técnica psicodramática foram consideradas não somente as informações verbais, mas também as cênicas, que incluem gestos, expressões, silêncios e comportamentos. A população estudada constituiu-se da totalidade de escolares deficientes visuais, com idades entre 10 e 14 anos completos, inseridos no sistema público de ensino, que recebiam apoio pedagógico em sala de recursos para deficientes visuais da cidade de Santa Bárbara d'Oeste (SP), no período da pesquisa. Os sujeitos foram previamente examinados em Serviço de Visão Subnormal de Hospital Universitário. RESULTADOS: Foram identificados conhecimentos insuficientes sobre a própria deficiência e dificuldade de adaptação ao uso dos auxílios ópticos. Na observação de comportamentos, verificou-se desconforto no uso dos auxílios ópticos em público, negação da deficiência visual e falta de participação nas decisões sobre tratamentos para sua própria saúde. CONCLUSÃO: A análise de sessões de teatro espontâneo permitiu identificar necessidades, dificuldades e barreiras encontradas pelos sujeitos na efetivação dos tratamentos a eles prescritos, assim como concepções e comportamentos que dificilmente se revelam no ambiente de assistência à saúde.

OBJETIVO: Verificar percepções e conduta de escolares portadores de deficiência visual, em relação aos recursos ópticos e equipamentos utilizados no processo de escolarização. MÉTODOS: Estudo descritivo transversal em população de escolares de 12 anos e mais, portadores de deficiência visual, congênita ou adquirida, em processo de escolarização, inseridos no sistema público de ensino no município de Campinas no ano de 2000. Aplicou-se questionário por entrevista, elaborado com base em estudo exploratório. RESULTADOS: Foi obtida população de 26 alunos, sendo 46,2% portadores de visão subnormal e 53,8% portadores de cegueira. A maioria cursava o ensino fundamental (65,4%) em escolas com salas de recursos (73,1%). Entre os recursos utilizados em atividades de leitura e escrita 94,1% dos escolares declararam utilizar a máquina Braille e 81,8% relataram que o colega dita a matéria. A maioria dos escolares com visão subnormal utilizavam óculos (91,7%) e 33,3% utilizavam lupa como recursos ópticos. Entre os recursos não ópticos destacaram-se os ambientais - aproximação da lousa (75,0%) e da janela (66,7%%) para maior iluminação. CONCLUSÕES: Foi evidenciado o fato de que escolares portadores de visão subnormal utilizavam recursos destinados a portadores de cegueira como a aplicação do sistema Braille. Verificou-se reduzido número de portadores de visão subnormal utilizando recursos ópticos e não ópticos referentes à sua problemática, o que indica provável desconhecimento de seu potencial visual e de recursos apropriados para melhorar sua eficiência.PURPOSE: To verify perceptions and conduct of students with visual impairment regarding devices and equipment utilized in schooling process. METHODS: A transversal descriptive study on a population of 12-year-old or older students in schooling process, affected by congenital or acquired visual impairment, inserted in the government teaching system of Campinas during the year 2000. An interview quiz, created based on an exploratory study was applied. RESULTS: A group of 26 students, 46% of them with low vision and 53.8% affected by blindness was obtained. Most of the students were from fundamental teaching courses (65.4%), studying in schools with classrooms provided with devices (73.1%). Among the resources used in reading and writing activities, 94.1% of the students reported they used the Braille system and 81.8% reported that the reading subject was dictated by a colleague. Most of the students with low vision wore glasses (91.7%), and 33.3% utilized a magnifying glass as optical devices. Among the non-optical devices, the most common were the environmental ones, getting closer to the blackboard (75.0%) and to the window (66.7%) for better lighting. CONCLUSIONS: It became evident that students with low vision eye-sight made use of devices meant for bearers of blindness, such as applying the Braille system. A reduced number of low vision students making use of optical and non-optical devices applicable to their problems were observed, indicating a probable unawareness of their visual potential and the appropriate devices to improve efficiency.

OBJETIVOS: Construir e validar a tabela "Minnesota Low Vision Reading Test" - MNREAD na versão do idioma Português. MÉTODOS: A tabela de acuidade e leitura do "Minnesota Low Vision Reading Test" contém 19 sentenças (logMAR 0,5 a 1,3) com 60 caracteres impressos em três linhas. Todas as sentenças devem possuir o mesmo comprimento e vocabulário simples. Foi construído um total de 110 sentenças. As sentenças foram testadas em 36 sujeitos (20 adultos e 16 crianças); o tempo de leitura e os erros cometidos foram apurados e a média de velocidade de leitura e os erros foram analisados. Trinta e oito sentenças foram selecionadas para a construção do protótipo (MNREAD-P). Foram excluídas sentenças com alta e baixa média de velocidade de leitura, alto desvio padrão e sentenças com erros repetidos cometidos pelos sujeitos. Validação: Vinte sujeitos com visão normal (logMAR 0 ou mais, com a melhor correção) foram testados na tabela MNREAD-P e leram um texto de leitura do dia-a-dia. A velocidade de leitura em palavras/minuto foi medida na tabela e no texto. RESULTADOS: As sentenças da tabela MNREAD-P são suficientemente consistentes para medidas confiáveis da habilidade de leitura. A velocidade de leitura do texto (logMAR = 0,6) foi 197,8 palavras/minuto e a velocidade máxima na tabela MNREAD -P foi 200,1 palavras/minuto. A correlação entre as duas medidas foi r = 0,82. CONCLUSÕES: A tabela MNREAD-P foi testada em sujeitos com visão normal e os resultados foram os mesmos da "Minnesota Low Vision Reading Test" original e a velocidade de leitura foi estatisticamente equivalente à da velocidade de leitura do texto.PURPOSE: To create and to validate a version of the Minnesota Low Vision Reading Test (MNREAD) acuity chart for the Portuguese language. METHODS: The Minnesota Low Vision Reading Test acuity chart contain 19 sentences (logMAR 0.5 to 1.3) with 60 characters printed on three lines. All the sentences must have the same length with simple vocabulary. A total of 110 sentences were generated. The sentences were presented to 36 subjects (20 adults and 16 children) and mistakes and reading time were marked. 38 sentences were selected for a prototype (MNREAD-P). Sentences with extreme high and low mean reading time, large standard deviation, and with persistent mistakes by subjects were excluded. Validation: Twenty subjects with normal vision (logMAR 0 or 20/20 or better, with best refractive correction) were tested with the MNREAD-P and read a passage of text, representing normal, day-to-day reading. Reading speeds in words per minute were recorded for both the MNREAD and the text passage. RESULTS: Sentences in the MNREAD Portuguese chart are sufficiently consistent to provide reliable measures of reading abilities. Reading speeds for the passage (logMar = 0.6) were 197.8 words/minute and the maximum reading speeds calculated by the MNREAD-P were 200.1 words/minute. The correlation between the two measures was r = 0.82. CONCLUSION: The MNREAD-P was tested on normal vision subjects and the results were the same from the original Minnesota Low Vision Reading Test. The reading speed measured on the MNREAD-P was statistically equivalent to the reading speed of the passage.

OBJETIVO: Avaliar a efetividade do setor de Visão Subnormal no Instituto Benjamin Constant (IBC) e comprovar a real necessidade de um Instituto nos moldes do Instituto Benjamin Constant na atual política de inclusão. MÉTODOS: Estudo ecológico, realizado de 1º de outubro de 1990 a 20 de dezembro de 2002, analisando três períodos de atendimento no setor de Visão Subnormal no Instituto Benjamin Constant: a) 1991 - início do atendimento; b) 1995 - integração médico-pedagógica; c) 2002 - estágio atual. Nesta análise foram estudados os seguintes indicadores: I - número de atendimentos no setor de Visão Subnormal; II - condições do setor de Visão Subnormal no Instituto Benjamin Constant e III - associados. RESULTADOS: Observou-se aumento do atendimento, que passou a ser mais abrangente após a integração médico-pedagógica. Outros indicadores, como a capacitação de profissionais médicos, a participação em cursos de capacitação no Instituto Benjamin Constant, o aumento do número de convênios e encaminhamentos para o Instituto Benjamin Constant, além da Reabilitação, também confirmam a efetividade do setor de Visão Subnormal no Instituto Benjamin Constant. CONCLUSÕES: O setor de Visão Subnormal mostrou ser a interface entre o serviço médico e o serviço pedagógico, passando a interagir posteriormente com a Reabilitação e a Coordenação de Educação Física. Isto acarretou mudança de postura de diferentes setores do Instituto Benjamin Constant com relação ao paciente portador de visão subnormal, desde o educando até o paciente da comunidade. Desse modo, o Instituto Benjamin Constant mostrou a sua utilidade no tocante à política de inclusão.PURPOSE: To evaluate effectiveness of the Low Vision the Benjamin Constant Institute (BCI) and confirm the real necessity of an Institute like BCI in the present inclusion policy. METHODS: Ecological study, analyzing 3 periods of Low Vision Assistance at the Benjamin Constant Institute from October 1, 1990 to december 20, 2002: a) 1991 - starting assistance; b) 1995 - medical pedagogic integration; c) 2002 - present-day situation. We considered in this analysis as indicators: I - Low Vision Assistance, II - Low Vision sector in the Benjamin Constant Institute, III - Associates. RESULTS: This study demonstrated an increase in assistance, reaching a wider spectrum of patients after medical-pedagogic integration. Other indicators, such as physician capacitation, participation in Benjamin Constant Capacitation Courses, increase in orienta tion to institutions, schools and others and referrals to the Benjamin Constant Institute, and Rehabilitation also attest the effectiveness of the Low Vision sector of the Benjamin Constant Institute. CONCLUSIONS: The Low Vision sector proved to be the interface between the Medical and Pedagogic Departments, and later on the Rehabilitation and Physical Education Coordination sectors. This has implied alterations in the way to manage the low-vision patient, not only regarding the regular Benjamin Constant Institute student as well as any other patient in the community. The Benjamin Constant Institute proved its importance as regards inclusion policy.

Neste artigo, os autores descrevem dois casos de síndrome de Charles Bonnet, definida como a percepção de alucinações visuais complexas em pacientes com déficit visual, tendo os pacientes a consciência da natureza irreal do fenômeno. Grande número de casos não é diagnosticado pela ausência do questionamento direto do médico. Em vista do transtorno emocional causado por esta doença, o reconhecimento dos seus sintomas é essencial no manejo destes pacientes.In this article the authors report two cases of Charles Bonnet syndrome, defined as complex visual hallucinations in patients with low vision, and the patient is aware of the unreal nature of the phenomenon. A great number of cases is misdiagnosed due to lack of direct questioning by the physician. Since the emotional distress caused by this disease, the knowledge of its symptoms is essential in the management of these patients.

OBJETIVO: Avaliar e comparar com a literatura o perfil dos pacientes portadores de colobomas oculares, bem como verificar a acuidade visual, alterações oculares associadas e alterações sistêmicas apresentadas pelos mesmos. MÉTODOS: Foram estudados retrospectivamente 18 pacientes com diagnóstico de coloboma ocular matriculados no serviço de Visão Subnormal do Hospital São Geraldo da Universidade Federal de Minas Gerais (HSG - HCUFMG). Foram avaliados: sexo, idade, raça, história familiar de anomalias oculares, história gestacional, tipo de coloboma, localização, bilateralidade, presença de microftalmia ou anoftalmia, alterações oculares concomitantes, e associação com doenças sistêmicas e acuidade visual. RESULTADO: Dez (55,6%) pacientes eram do sexo feminino, 15 (83,3%) eram leucodérmicos. A idade variou de 4 a 57 anos, com média de 9,5 anos. Apenas 3 (16,7%) apresentavam história familiar para coloboma. Todos apresentavam história gestacional negativa para qualquer intercorrência. Houve acometimento bilateral em 100% dos casos, com microftalmia presente em 6 (33,3%) casos. O coloboma foi típico em 14 (77,8%) pacientes, sendo o coloboma mais comum o retinocoroidal ocorrendo em 16 (88,9%) pacientes. 50% dos pacientes apresentavam o coloboma como achado isolado, e a outra metade associado a alguma doença sistêmica. Em 14 (77,8%) pacientes foi encontrado algum tipo de alteração ocular além do coloboma. CONCLUSÃO: O exame oftalmológico completo é muito importante tanto no diagnóstico quanto no prognóstico de pacientes com coloboma, visto que este pode estar associado a uma baixa visual importante, além de poder estar relacionados a diferentes anomalias ou síndromes. PURPOSE: To verify and compare with the literature the profile of patients with ocular coloboma, and also to verify visual acuity, ocular and systemic abnormalities. METHODS: Retrospective analysis of 18 patients with a definite diagnosis of ocular coloboma at the Low Vision Department of the São Geraldo Eye Hospital of the Federal University of Minas Gerais. Patients underwent a complete ophthalmologic evaluation: sex, age, race, family history, prenatal and birth history, complete or incomplete coloboma, location, microphthalmos or anophthalmos, other ocular and systemic abnormalities, and visual acuity. RESULTS: 10 (55.6%) patients were women. The age was from 4 to 57, and the mean was 9.5 years. Only three (16.7%) patients had a family history of coloboma. All patients had negative prenatal and birth history. The coloboma was bilateral in 100% of the patients, and associated with microphthalmos in 6 (33.3%) cases. The coloboma was considered typical in 14 (77.8%) patients, with retinochoroidal coloboma being the most frequent, 16 (88.9%) patients. 50% of the patients had coloboma as an isolated ocular anomaly, while the other 50% presented association with systemic anomalies. In 14 (77.8%) patients other ocular abnormalities were found. CONCLUSION: The complete ophthalmologic examination is extremely important for both the diagnosis and prognosis of the patients with coloboma, since the coloboma may be associated with important low visual and also systemic anomalies and syndromes.

MÉTODOS: Foram estudados 29 pacientes, com idade variando entre 10 e 78 anos, encaminhados ao Serviço de Visão Subnormal do Hospital São Geraldo da Faculdade de Medicina da UFMG, com diagnóstico prévio de doenças com escotoma central. Foram utilizados os métodos carta de Asmler, observação da face e observação da tabela de leitura para perto. RESULTADOS: Dentre estes métodos, o que obteve o melhor desempenho, ou seja, apresentou maior percentual de pacientes nos quais se constatou a presença de escotomas, foi a carta de Asmler (63,6%), seguida pela observação da face (47,3%) e observação da tabela de leitura para perto (30,9%) (pOBJECTIVE: The aim of this study was to compare the sensitivity of three methods of central scotoma localization. O aprendizado do correto manuseio do auxílio óptico foi treinado: localização, focalização e seguimento com o telescópio de 8x de magnificação. Após essa etapa iniciou-se o aprendizado de cópia da lousa com o auxílio adaptado. Totalizaram-se 17 sessões para a criança receber a prescrição final devido às grandes dificuldades apresentadas pela baixa acentuada das funções visuais.Training of the proper handling of optical devices was performed: location, targeting and tracking with the 8x magnification telescope. After this step, learning to a copy from a blackboard with the adapted optical aid was initiated. Uveitis is a group of intraocular inflammatory diseases that affects mainly patients of working age. Vogt-Koyanagi-Harada disease is a chronic, granulomatous, systemic and autoimmune disease with manifestations in the eye, skin, and hearing and nervous systems. Ocular complications include cataract, glaucoma, choroidal neovascular membrane formation, and subretinal fibrosis, which can lead to phthisis bulbi with profound low vision or even blindness. The purpose of this article is to present the visual rehabilitation of a young patient with pre phthisis bulbi as a complication of the uveitis. During the active stage of the disease the patient quit her studies in a crucial phase of her life. After clinical treatment and surgery, training program of optical device adaptation and residual visual optimization were performed. Due to the great patient's motivation, it was possible to reintegrate her to social life.Uveíte é o termo utilizado para descrever um grupo de doenças inflamatórias oculares que ocorrem predominantemente na idade produtiva. Vogt-Koyanagi-Harada é uma doença sistêmica, autoimune, granulomatosa, crônica com manifestações oculares, neurológicas, auditivas e cutâneas. As principais complicações incluem catarata, glaucoma, membrana neovascular e fibrose sub-retiniana, que podem evoluir para atrofia ocular, baixa visão profunda e cegueira. O propósito deste artigo é apresentar um caso de reabilitação visual em paciente jovem com pré phthisis bulbi consequente às complicações da uveíte. Após tratamento clínico e cirurgia, um programa de treinamento e adaptação de recursos ópticos foi realizado para otimizar o uso do resíduo visual, motivando a paciente para reintegrar-se socialmente e voltar aos estudos.

Purpose: To evaluate the use of optical and nonoptical aids during reading and writing activities in individuals with acquired low vision. Methods: This study was performed using descriptive and cross-sectional surveys. The data collection instrument was created with structured questions that were developed from an exploratory study and a previous test based on interviews, and it evaluated the following variables: personal characteristics, use of optical and nonoptical aids, and activities that required the use of optical and nonoptical aids. Results: The study population included 30 subjects with acquired low vision and visual acuities of 20/200-20/400. Most subjects reported the use of some optical aids (60.0%). Of these 60.0%, the majority (83.3%) cited spectacles as the most widely used optical aid. The majority (63.3%) of subjects also reported the use of nonoptical aids, the most frequent ones being letter magnification (68.4%), followed by bringing the objects closer to the eyes (57.8%). Subjects often used more than one nonoptical aid. Conclusions: The majority of participants reported the use of optical and nonoptical aids during reading activities, highlighting the use of spectacles, magnifying glasses, and letter magnification; however, even after the use of these aids, we found that the subjects often needed to read the text more than once to understand it. During writing activities, all subjects reported the use of optical aids, while most stated that they did not use nonoptical aids for such activities. Objetivo: Avaliar o uso de auxílios ópticos e não ópticos na leitura e na escrita de pessoas com baixa visão adquirida. Métodos: Realizou-se estudo descritivo, tipo transversal. O instrumento de coleta de dados continha questões estruturadas, desenvolvidas em estudo exploratório e teste prévio e foi aplicado por entrevista, onde foram investigadas as seguintes variáveis: características pessoais, uso de auxílios ópticos e não ópticos, atividades para as quais utiliza auxílios ópticos e não ópticos. Resultados: A população foi composta por 30 sujeitos com baixa visão adquirida, com acuidade visual entre 20/200 a 20/400. A maioria dos sujeitos declarou fazer uso de algum tipo de auxílio óptico (60,0%). Desses 60,0%, a maioria (83,3%) citou os óculos como o auxílio óptico mais usado. A maioria (63,3%) dos sujeitos relatou fazer uso de auxílios não ópticos e os mais citados foram a ampliação de letras (68,4%), seguidos da aproximação dos objetos aos olhos (57,8%). Os sujeitos ressaltaram o uso de mais de um auxílio não óptico. Conclusão: A maioria informou utilizar os auxílios ópticos e não ópticos nas atividades de leitura, evidenciando o uso de óculos e da lupa manual, assim como da ampliação de letras, mas mesmo utilizando tais auxílios verificou-se que os sujeitos necessitavam ler o texto mais de uma vez para terem a compreensão do mesmo. Para a realização das atividades de escrita, a totalidade dos sujeitos declarou utilizar os auxílios ópticos, enquanto que a maioria declarou não utilizar auxílios não ópticos nessas atividades.

To evaluate the quality of life in visually impaired children followed in the Early Visual Stimulation Ambulatory of Unifesp in two moments, before and after rehabilitational intervention of multiprofessional team.A CVFQ quality of life questionnaire was used. This instrument has a version for less than three years old children and another one for children older than three years (three to seven years) divided in six subscales: General health, General vision health, Competence, Personality, Family impact and Treatment.The correlation between the subscales on two moments was significant. There was a statistically significant difference in general vision health (p=0,029) and other important differences obtained in general health, family impact and quality of life general score.The questionnaire showed to be effective in order to measure the quality of life related to vision on families followed on this ambulatory. The multidisciplinary interventions provided visual function and familiar quality of life improvement.The quality of life related to vision in children followed in Early Visual Stimulation Ambulatory of Unifesp showed a significant improvement on general vision health.

The Functional Vision Assessment (AVIF-2 to 6 years)'s capacity to differentiate visual ability levels in children with low vision was analyzed. The AVIF-2 to 6 years was created at the Infantile Low Vision Sector from São Geraldo Hospital, Brazil.After a debate among professionals from different areas regarding the appropriateness of the test items, AVIF-2 to 6 years was applied to 40 children aged from 2 to 6 years; twenty children with low vision (Group 1) and twenty without it (Group 2) were assessed. Group 1 was recruited from the Infantile Low Vision Sector from São Geraldo Hospital. Group 2 comprised children from two different public day care centers and children were matched by age, gender and social level with group 1. Seven domains were studied: visual fixation, visual following, visual field confrontation, eye-hand coordination and surrounding locomotion, contrast vision and color vision discrimination. Group 1 children were submitted to a complete ophthalmologic exam and group 2 to ophthalmologic screening. Children with low vision and neurologic disease were excluded. Comparative analyzes were performed for both groups and for distinct subgroups classified by age (24 to 35 months, 36 to 59 months and 60 to 78 months) and by visual acuity subgroups (1.0 logMAR and ≥1.0 logMAR).The scores at total AVIF-2 to 6 years and its domains were statistically significant (p0.05), except for the contrast vision and color vision discrimination domains. The total AVIF- 2 to 6 years median was lower for group 1 at the three interval ages.AVIF- 2 to 6 years can discriminate different levels of functional vision of low vision children, however the authors emphasize that although the results are encouraging, further studies shall be done until the test is ready for clinical use.

A 22-year-old man developed a central scotoma in the left eye during an episode of severe headache 6 months previously. He had a diagnosis of migraine headaches with aura since age 15. Ophthalmic examination showed retinal nerve fiber layer thickness reduction on the papilomacular bundle in the left eye and unremarkable in the right. Optical coherence tomography documented severe macular thickness reduction and a mild and localized retinal nerve fiber layer loss on the temporal side of the optic disc in the left eye. Systemic investigation, neuroimaging and cardiovascular studies were non-revealing. The cause of his visual loss was possibly related to retinal migraine, although an ischemic episode of another origin could not be excluded. Our case is interesting because it draws attention to the fact that optical coherence tomography may be able to identify localized neural loss not only by measuring peripapillary retinal nerve fiber layer but also by means of macular thickness measurements.

To identify mothers' feeling and reactions when facing the diagnosis of low vision and their comprehension of this diagnosis.This study features a qualitative research. Eleven low vision children's mothers attended at the Visual Stimulation Service of Ophthalmology Sector of Unicamp Hospital were interviewed. Semi-structured interviews were used to collect data. The main issues of the interview were: feelings when faced with the diagnosis and comprehension of this low vision diagnosis.The results revealed mothers' fear of blindness and a low comprehension of the meaning of low vision, and their interpretation of medical diagnosis according to their subjectivity.The way the diagnosis is transmitted to the families is extremely important. Parents cultural, economical and emotional background may often impair the diagnosis comprehension.

A case of identical male twins with Cohen syndrome who present multiple ophthalmic findings is reported. The patients were identical 16 year-old twin boys who showed down slanting eyelids, mild ptosis, high-grade myopia, small cortical lens opacities, posterior subcapsular cataracts, myotic and corectopic pupils with poor dilation due to focal iris atrophy and retinochoroidal dystrophy. Ophthalmologists must be aware of the ocular and systemic findings of Cohen syndrome in the evaluation of young patients with mental retardation and visual impairment.

To establish the visually impaired resource class users profile in the city of Assis - SP, to estimate the visually impaired employment percentage and to compare employment rate with age, gender, deficiency type and causing disease between two periods:1984 - 1996 and 1997 - 2009.Retrospective review of both medical and school records from visually impaired individuals attending visually impaired resource class in Assis - SP between 1984 and 2009. This time period was divided into two intervals: 1984 - 1996 (G1) and 1997 - 2009 (G2). Collected data included demographic features and diseases, type of visual impairment (blindness or low vision), education, optic aids, attendance to visually impaired resource class and employment rate. Employment rate was associated with age, gender, type of visual impairment and causing disease between both study time intervals.A total of 149 visually impaired individuals were identified - 61.07% male, 38.9% female, 82.5% white, and 17.4% non-white. Mean age was 18 years. Of the 149 visually impaired, 63.75% had low vision and 36.24% were blind. In decreasing order, the more frequent conditions leading to visual impairment were toxoplasmosis retinochoroiditis (17.40%), congenital optic atrophy (9.39%), high hyperopia (8.72%), pigmentary retinosis and high myopia (with 6.71% each) and congenital glaucoma and congenital cataract (with 6.04% each). Attendance to visually impaired resource class was good by over 50% of the patients. Of the individuals14 years-old, 44.7% and 12.3% were regularly employed during 1984 - 1996 and 1997 - 2009 periods, respectively. Employment rate only correlated with age (individuals with higher mean age were most frequently employed) and there was not correlation with demographic characteristics, deficiency type and causing diseases.The main characteristics of the individuals attending the visually impaired resource class are: male, white, with low vision and ocular toxoplasmosis. Nearly half of the individuals older than 14 years-old (44.7%) were regularly employed up to 1996, suggesting that the joint effort of ophthalmologists and teachers helps the social inclusion of these patients. Employment rate dropped over 1997 - 2009 period, and the rate of retirees increased.

OBJETIVO: A Fundação Altino Ventura realizou o projeto "Enxergando através das mãos", com o objetivo de investigar a situação atual da múltipla deficiência no estado de Pernambuco, verificar a demanda reprimida nesta área e analisar relevância para implantação de um centro especializado de referência para o diagnóstico e tratamento desses pacientes. MÉTODOS: Foi avaliado o perfil de um grupo de pacientes atendidos em cinco serviços de referência do grande Recife. Os casos foram avaliados quanto a ocorrências perinatais, inclusão social e dificuldades de acesso aos serviços especializados. Promoveu-se o atendimento oftalmológico especializado e o tratamento indicado para cada caso. O projeto foi desenvolvido de forma prospectiva, seguindo protocolo e organograma previamente elaborados. Os atendimentos foram realizados em seis dias não consecutivos, durante os quais todos os casos passavam pelas etapas de entrevista dos genitores e exame oftalmológico completo. Durante o período de espera entre entrevista e exame oftalmológico os pacientes realizavam atividades lúdicas com artistas plásticas. Contou-se com equipe multidisciplinar de 31 profissionais. RESULTADOS: Foram atendidos 309 pacientes. Estavam incluídos na sociedade 83,1% dos pacientes, apenas 45,4% freqüentavam escola (normal ou especial). A dificuldade de acesso aos serviços na área de oftalmologia foi relatada por 51,1% dos casos. Foram identificadas 274 crianças com múltiplas deficiências, destas, 154 (56,2%) nunca haviam sido examinadas pelo oftalmologista e 33% tinham baixa de visão (inferior à 20/60 na tabela de Snellen). Os diagnósticos oftalmológicos mais freqüentes foram: erro refracional, estrabismo, ambliopia, alterações do nervo óptico (glaucoma e atrofia), e catarata. Foram prescritos e doados 84 óculos. CONCLUSÃO: O projeto foi pioneiro na região, permitindo a obtenção de importantes dados quanto à problemática da deficiência visual e/ou múltipla, possibilitando o acesso ao oftalmologista e tratamento especializado dos casos. PURPOSE: The Project "Vision through the hands" was performed by the Altino Ventura Foundation, with the purpose of investigating the present child situation with multiple disabilities in the state of Pernambuco, Brazil. METHODS: Birth and social conditions, social problems, and difficulty in accessing the specialized services were evaluated, aiming to collect relevant data with the goal of posterior implantation of a specialized diagnosis and treatment center. Ophthalmologic care and proper treatment were offered. RESULTS: Three hundred and nine children from five reference institutions in disabilities were attended, on six non consecutive days. All cases underwent ophthalmologic examinations and parent interviews. During the waiting time between the ophthalmologic visits and parent interviews the patients performed activities with artists. A multidisciplinary team of 31 professionals was available. According to parent information 83.1% of the patients were considered included in the society and only 45.4% attended school (normal or special). The difficulty to get access to ophthalmologic service was reported by 51.1%. Two hundred and seventy children with multiple disabilities were indentified, 154 (56.2%) of whom underwent an ophthalmologic examination for the first time, and 33.0% had low visual acuity (inferior to 20/60 Snellen). The most frequent diagnoses were: refraction error, strabismus, ambliopia, alterations of optic nerve (glaucoma and atrophy), and cataract. Eighty-four glasses were prescribed. CONCLUSION: This project was pioneer in the region, allowing to obtain the information about important problems regarding multiple disabilities, and making possible ophthalmologic care and specialized treatment to the children.

To develop the design of a near low vision aid that has esthetic and functional qualities intending to give its user a better life quality.To verify low vision people's main needs concerning the current devices for near distance; to identify possible design interventions to make a project that fulfills their main needs.A prototype having a reading stand tied to the end of a magnifier that moves horizontally and vertically permitting the horizontal reading of a text line was developed.The developed device fulfills low vision people's visual and handling needs for near distance, contributing to the easiness to read and write.

To verify perceptions and conduct of students with visual impairment regarding devices and equipment utilized in schooling process.A transversal descriptive study on a population of 12-year-old or older students in schooling process, affected by congenital or acquired visual impairment, inserted in the government teaching system of Campinas during the year 2000. An interview quiz, created based on an exploratory study was applied.A group of 26 students, 46% of them with low vision and 53.8% affected by blindness was obtained. Most of the students were from fundamental teaching courses (65.4%), studying in schools with classrooms provided with devices (73.1%). Among the resources used in reading and writing activities, 94.1% of the students reported they used the Braille system and 81.8% reported that the reading subject was dictated by a colleague. Most of the students with low vision wore glasses (91.7%), and 33.3% utilized a magnifying glass as optical devices. Among the non-optical devices, the most common were the environmental ones, getting closer to the blackboard (75.0%) and to the window (66.7%) for better lighting.It became evident that students with low vision eye-sight made use of devices meant for bearers of blindness, such as applying the Braille system. A reduced number of low vision students making use of optical and non-optical devices applicable to their problems were observed, indicating a probable unawareness of their visual potential and the appropriate devices to improve efficiency.

To outline low vision (LV) patients profile of the low vision department of the Instituto Brasileiro de Oftalmologia e Prevenção da Cegueira (IBOPC).Transversal study of 82 medical charts from the low vision service at the Instituto Brasileiro de Oftalmologia e Prevenção da Cegueira of the first year of the department--2004, considering age, gender, etiology, distance and near visual acuity with and without optical aids and prescription assistance.Of the 82 available patients, 11 (13.4%) were excluded of the study because they did not present low vision. Of the 71 patients, 32 (45%) were younger than 20 years old. Regarding the gender, there was not a significant difference (51% female and 49% male). The most frequent etiology in infants and adolescents was congenital glaucoma and in patients aged between 20-39 years was ocular toxoplasmosis (21.1%). In patients with 40-59 years old, pigmentary retinosis was the most frequently pathology (19%). In elderly people it was glaucoma (49%). Thirty-three patients (40.2%) had visual acuity between 20/60 and 20/160. The telescopic system was the only optical aid indicated for distance (44%) and glasses were the most indicated for near (54.5%).There is a high prevalence of low vision in infants and adolescents that live in developing countries like Brazil. Therefore, ophthalmologists need to be aware to promote early diagnosis and treatment to these patients, creating preventive programs and better assistance conditions.

Congenital retinal macrovessel is a rare vascular anomaly in which a large vessel and its tributaries cross the macula. We describe a case of retinal macrovessel in a patient complaining of decrease in visual acuity.

Ophthalmic manifestations of neuroendocrine carcinoma. This case report describes the clinical presentation, diagnosis and treatment of a case of neuroendocrine carcinoma. A 43-year-old man presented with ocular manifestation due to orbital and brain metastasis of neuroendocrine carcinoma. The histopathologic and immunohistochemical analysis suggested the diagnosis. Partial and temporary remission of the symptoms occurred after the first chemotherapy cycle. We discuss the importance of creating treatment guidelines for this type of neoplasm, that can be very aggressive and fatal.

50. [Ophthalmologic findings in multiple handicapped patients]

To report the visual findings in patients with multiple handicaps.Two hundred and seventy-four patients cared for at the Pediatric and Strabismus Ophthalmology Department of the "Fundação Altino Ventura" - Brazilian National Health System, were examined from June to September 2004. Age varied from 0.1 to 20 years with a median of 5.The majority of the patients (61.3%) presented good visual acuity; however low visual acuity (20/80) was observed in 38.7% of the patients. Heterotropias were observed in 66 patients (24.0%); astigmatism (53.2%) and hyperopia (29.0%) were more frequent.Children with multiple handicaps need an early ophthalmologic diagnosis and treatment for better global development. The integration of a multidisciplinary team with pediatricians, pediatric ophthalmologists and specialists in low vision, may assure a better visual rehabilitation.
