Abstract

Extracorporeal membrane oxygenation (ECMO) provides continuous cardiopulmonary support on a long-term basis. It has been speculated that patients undergoing ECMO via femoral arterial cannulation are more likely to develop peripheral vascular complications. The purpose of this study was to evaluate the incidence of peripheral vascular complications in this group of patients and outline the modalities used for treatment.

Abstract

•Type of Research:Analysis of a national survey of Vascular Surgery Program directors (PDs) and Associate Program directors (APDs).•Key Findings:There are numerous extracurricular educational courses available for Vascular Surgery Residents and Fellows throughout the US. Most of these courses lack uniform, standardized curricula questioning their educational merit. Our survey of the Vascular Surgery PD/APDs shows that majority of the PD/APDs deem these courses to be useful. Mock oral examinations and cadaver dissections are the most commonly sought out features for these courses. Majority of the PD/APDs do not receive trainee feedback from these courses. They also would like to have these courses held at regional meetings and sponsored by industry.•Take home Message:Standardized curricula for vascular surgery educational courses with collaborative oversight by educational/simulation key opinion leaders, PD/APD's, course directors and industry partners are needed. Feedback identifying vascular trainees’ strengths and areas for improvement to PD/APDs would be of great educational value and is currently a missed opportunity.

No. of bitstreams: 0 Previous issue date: 2021-10-01 Introduction: Aortic graft infection remains a considerable clinical challenge, and it is unclear which variables are associated with adverse outcomes among patients undergoing partial resection. Methods: A retrospective, multi-institutional study of patients who underwent partial resection of infected aortic grafts from 2002 to 2014 was performed using a standard database. Baseline demographics, comorbidities, operative, and postoperative variables were recorded. The primary outcome was mortality. Descriptive statistics, Kaplan-Meier (KM) survival analysis, and Cox regression analysis were performed. Results: One hundred fourteen patients at 22 medical centers in 6 countries underwent partial resection of an infected aortic graft. Seventy percent were men with median age 70 years. Ninety-seven percent had a history of open aortic bypass graft: 88 (77%) patients had infected aortobifemoral bypass, 18 (16%) had infected aortobiiliac bypass, and 1 (0.8%) had an infected thoracic graft. Infection was diagnosed at a median 4.3 years post-implant. All patients underwent partial resection followed by either extra-anatomic (47%) or in situ (53%) vascular reconstruction. Median follow-up period was 17 months (IQR 1, 50 months). Thirty-day mortality was 17.5%. The KM-estimated median survival from time of partial resection was 3.6 years. There was no significant survival difference between those undergoing in situ reconstruction or extra-anatomic bypass (P = 0.6). During follow up, 72% of repairs remained patent and 11% of patients underwent major amputation. On univariate Cox regression analysis, Candida infection was associated with increased risk of mortality (HR 2.4; P = 0.01) as well as aortoenteric fistula (HR 1.9, P = 0.03). Resection of a single graft limb only to resection of abdominal (graft main body) infection was associated with decreased risk of mortality (HR 0.57, P = 0.04), as well as those with American Society of Anesthesiologists classification less than 3 (HR 0.35, P = 0.04). Multivariate analysis did not reveal any factors significantly associated with mortality. Persistent early infection was noted in 26% of patients within 30 days postoperatively, and 39% of patients were found to have any post-repair infection during the follow-up period. Two patients (1.8%) were found to have a late reinfection without early persistent postoperative infection. Patients with any post-repair infection were older (67 vs. 60 years, P = 0.01) and less likely to have patent repairs during follow up (59% vs. 32%, P = 0.01). Patients with aortoenteric fistula had a higher rate of any post-repair infection (63% vs. 29%, P < 0.01) Conclusion: This large multi-center study suggests that patients who have undergone partial resection of infected aortic grafts may be at high risk of death or post-repair infection, especially older patients with abdominal infection not isolated to a single graft limb, or with Candida infection or aortoenteric fistula. Late reinfection correlated strongly with early persistent postoperative infection, raising concern for occult retained infected graft material. 46. Acute Limb Ischemia In Patients On Veno-Arterial Extracorporeal Membrane Oxygenation (VA-ECMO) Support: A Ten-Year Single-Center Experience.

Abstract

Objectives: VA-ECMO has been increasingly utilized as a life-saving modality in patients with cardiopulmonary compromise. Acute limb ischemia (ALI) has been reported when the femoral artery was accessed, and it was associated with higher mortality in patients on ECMO. However, prior studies were limited by small sample size and the lack of long-term follow-up. We aimed to investigate the predictors of ALI in ECMO and the effect of ALI on long-term mortality., Methods: Retrospective institutional chart review was performed. Patients who underwent VA-ECMO (Jan/2008-Jan/2018) were identified. Primary outcomes were ALI and 4-year mortality. Logistic regression analysis was used to investigate the predictors of ALI. Survival analysis methods were used to examine 4-year mortality., Results: A total of 377 patients were included. Vascular complications took place in 149 (39.5%) patients. The majority was ALI (107, 75.4%). Eleven patients had limb loss. ALI patients were younger (mean age: 50.2±15.9 vs 54.1±15.5; p=.03), were more likely to have history of PAD (7.6%vs 3.0%; p=.047), were less likely to be on aspirin (40.2% vs 54.4%; p=.013) and were more likely to be on vasopressors (97.1% vs 88.0%; p=.007). Arterial cannula size was not associated with ALI so as the concurrent use of ventricular offloading devices (intra-aortic balloon pump or Impella (Abiomed Inc, Dancers, MA)). The use of distal perfusion catheter (DPC) was not protective against ALI in this cohort. However, DPC was associated with less likelihood to require vascular intervention (20.1% vs 32.0%; p=.009). In adjusted analysis, aspirin use was protective against ALI [adjusted odds ratios (aOR) 95%CI: 0.52(0.30-0.90); p=.018]. The only predictor of ALI was the use of vasopressors [aOR(95%CI): 6.8 (1.5-30.4); p=.012]. For those who were successfully decannulated, four-year survival was 65.1% in patients without ALI vs 46.8% in ALI (p=0.044). After adjusting for potential risk factors, 4-year mortality hazard was significantly higher in patients with ALI [HR: 1.80(1.04-3.12); p=.035]., Conclusions: Patients requiring ECMO are critically ill. The development of acute limb ischemia is detrimental to this population. This effect extends beyond the acute period. ALI increased 4-year mortality risk by 80%. 47. Factors Associated with Increased Fluoroscopy Time During Elective Endovascular Abdominal Aortic Aneurysm Repair and its Utilization as an Indicator of Intraoperative and Postoperative Outcomes.

Abstract

Objectives: This study explores the impact of prolonged fluoroscopy time (FT) on outcomes in endovascular aortic repair (EVAR) for abdominal aortic aneurysms (AAA). While total operative time includes multiple variables, FT precisely captures the technical precision of the EVAR procedure. By examining the factors that extend FT, we aim to establish FT as a critical quality metric for evaluating surgical performance and predicting postoperative outcomes., Methods: A retrospective review of the Society for Vascular Surgery (SVS) Vascular Quality Initiative (VQI) was conducted (2003-2021). The FT was studied based on a median dichotomy of ≤18 mins (Group I) and >18 mins (Group II). Primary outcomes of in-hospital mortality and discharge status were studied, along with numerous secondary outcomes pertaining to systemic complications. Factors associated with more extended FT were also measured. All the variables examined in multivariate analyses were estimated in odds ratios, and a p-value of <0.05 was deemed significant for all the analyses performed., Results: 41,841 patients were studied, of which 20,339 were categorized in Group I and 21,502 in Group II. The average fluoroscopy time in the selected patients was reported to be 23.2 minutes. Patients in Group II generally had overall poorer health status with multiple comorbidities and on various medications. Aortic aneurysm parameters can influence the FT, including the greater aorta-neck angle, neck angle, neck diameter, and neck length. Patients treated by high-volume surgeons were observed to have less likelihood of prolonged FT. On trends analysis, it was observed that the FT has been consistent over the study period., Conclusion: Various factors can influence the FT in patients undergoing EVAR, including the patient characteristics and the complexity of the aneurysm. 48. Favorable Trends in Mortality among Patients with Concomitant Peripheral Arterial Disease and Diabetes Mellitus over Past Two Decades.

Abstract

Background: This study examines the temporal trends in diabetes and peripheral artery disease (PAD)-related mortality in the United States, considering sociodemographic and regional factors, using data from death certificates in a national public database., Methods: Data were extracted from the Centers for Disease Control and Prevention Wide Ranging Online Data for Epidemiologic Research database from 1999 to 2020. Age-adjusted mortality rates (AAMRs) per 100,000 individuals and annual percent changes (APCs) with 95% confidence intervals (CI) were calculated., Results: Between 1999 and 2020, there were 2,252,252 PAD-related and 5,413,811 diabetes-related deaths, with 469,699 deaths involving both conditions. The overall AAMR for PAD and diabetes-related mortality (aged ≥25 years) decreased from 7.97 in 1999 to 6.37 in 2020, with a notable decline from 2001 to 2010 (APC: -6.16, 95% CI: -7.07, -5.23). The AAMR for PAD-related mortality fell from 40.25 to 30.56, while the AAMR for diabetes-related deaths rose from 76.71 to 93.63. Males and non-Hispanic (NH) Black individuals had higher AAMRs than females and other racial groups. The highest crude mortality rate was in the 80-84 age group. Nonmetropolitan areas consistently reported higher AAMRs than metropolitan areas, and states like Ohio, Vermont, District of Columbia, and West Virginia had significantly higher rates., Conclusions: Over 2 decades, PAD- and diabetes-related mortality trends show a positive overall reduction in AAMR. However, disparities persist, with higher rates among males, NH Black individuals, and residents of nonmetropolitan areas.
